1.Determination and clinical significance of amino acid transmitters in plasma of the patients with ischemic vascular disorders

Objective To observe the determination and clinical significance of amino acid transmitters in plasmas of the patients with ischemic vascular disorders.Methods Excitatory and inhibitory amino acids in plasmas of 57 old peoples with ischemic vascular disorders and 33 normal old peoples were determined with high performance liquid chromatography(HPLC).Results Significant increases were found in plasma concentration of glutamate(26.48%)、aspartate(39.70%), ?-aminobutyric acid(GABA,74.57%), glycine(36.79%) and alanine(21.33%) within fist three days after cerebral ischemia compared to normal controls. The content of glutamate and aspartate began to increase significantly in 24 hours after ischemia. The plasma levels of glutamate, aspartate and GABA have returned to normal in recovery patients.Conclusions These results showed that the concentration changes of excitatory and inhibitory amino acids in plasmas may suggest the early happening and severity of cerebral ischemia.
2.The expression of membrane type 1 matrix metalloproteinase in the esophageal carcinoma tissues and its clinical significance

Objective To investigate the expression of membrane type 1 matrix metalloproteinase(MT1-MMP) in the esophageal carcinoma tissues and in normal periearcinomatous tissues and its clinical significance. Methods Immunohistochemistry was used to study the expression of MT1-MMP in esophageal carcinoma tissues and normal tissues in distance for 5-7 em from tumor. Results Among 54esophageal carcinoma specimens, the positive expression rate of MT1-MMP was 87.0%, while all of themwere expressed a small quantity in normal esophageal epithelial tissues, the expression in the esophageal carcinoma tissues was significantly higher than those in normal esophageal epithelial tissues (P < 0.01 ).The degree of MT1-MMP expressions was associated with the depth of tumor invasion (P < 0.01 ), the invasion of the lymph nodes(P < 0.01 ) and the relapse and/or metastasis of the tumors for three years after surgery(P <0.01 ), and was also related to differentiation of tumor cells (P < 0.01 ). Conclusions The expression of MT1-MMP in the esophageal carcinoma tissues is well associated with the depth of tumor invasion, the invasion of the lymph nodes, the differentiation of tumor cells and relapse and/or metastasis of the tumors for three years after surgery. MT1-MMP plays an important rote in the invasion and metastasis in the patients with esophageal carcinoma.
3.Application of TEE to monitor intracardiac gas embolism during hysteroscopy

Objective To observe the incidence and the rate of embolism in patients undergoing hysteroscopy procedures by using the transesophageal echocardiography (TEE), as well as the changes of respiratoric and haemodynamic variables. Methods Fourty ASA Ⅰ~Ⅱ patients undergoing hysteroscopy procedures under general anesthesia received intraoperative TEE monitoring. The systolic pulmonary artery pressure, the incidence of the venous gas embolism (VGE) and the eegmental wall motion abnormality (SWMAs) were observed. Results In the 40 patients, 38 patients received intravenous anesthesia hysteroscopic surgery, with the intraoperative TEE to monitor the intracardiac VGE. The degree of gas embolism was related with the perfusion and the usage of monopolar or bipolar diathermia (P<0.05). The systolic pulmonary artery pressure promoted and SWMAs were also observed. Conclusions The continue TEE monitor during hysteroscopy could detect the intracardiac gas embolism in time , contributing to early diagnosis and avoiding the occurrence of malignant arrhythmias or myocardial ischemic events.
4.Prognostic value of interim and post-therapy 18F-FDG PET/CT in patients with diffuse large B-cell lymphoma

Objective To investigate the prognostic value of interim and post-therapy 18F-FDG PET/CT in patients with DLBCL.Methods A retrospective analysis was conducted in 110 patients (62males,48 females; median age 52 years) with newly diagnosed DLBCL.Forty-two patients underwent interim PET/CT after 4 cycles chemotherapy,44 patients underwent post-therapy PET/CT after 6-8 cycles of chemotherapy,and 24 patients underwent both interim PET/CT and post-therapy PET/CT.Interim and post-therapy PET/CT status (i.e.,positive or negative) were visually interpreted according to criteria of the International Harmonization Project.Three-year progression-free survival (PFS) and 3-year overall survival (OS) was compared between the subjects with positive or negative PET/CT results.x2 test and KaplanMeier analysis were used for data analysis.Results Interim 18F-FDG PET/CT results were positive in 28cases and negative in 38 cases.The median PFS and OS were 20 and 28 months in patients with positive scan results,those were 37 and 39 months in patients with negative results,respectively.Three-year PFS and 3-year OS rates were 17.9%(5/28) and 35.7%(10/28) in patients with positive interim PET/CT results,those were 52.6% (20/38) and 55.3% (21/38) in patients with negative results (x2 =8.285,P <0.01,x2=2.473,P>0.05,respectively).Post-therapy 18F-FDG PET/CT results were positive in 20 cases and negative in 48 cases.Median PFS and OS were 21 and 26 months in patients with positive results,those were 54 and 57 months in patients with negative results.Three-year PFS and 3-year OS rates were 20.0%(4/20) and 25.0% (5/20) in patients with positive results at post-therapy PET/CT,those were 77.1%(37/48) and 83.3% (40/48) in patients with negative results (x2=19.215,21.462,both P<0.01).Conclusions Post-therapy 18F-FDG PET/CT is useful to predict prognosis of patients with DLBCL.However,the value of interim PET/CT may be limited in terms of prognosis prediction.
5.Prevention and treatment of HBV reinfection following liver transplantation.

OBJECTIVETo study the prevention and treatment of HBV reinfection after liver transplantation.
METHODSTotal 19 cases of chronic fulminant hepatitis B, the end-stage of liver cirrhosis and liver carcinoma complicated with HBV cirrhosis. Were performed liver transplantation and given anti-viral drugs pre and post transplantation. Famciclovir was administered in 4 cases, lamivudine in 13 cases and lamivudine+HBIG in 2 cases. The serum HBVM and liver biopsy immunohistochemistry were performed.
RESULTSFour cases given famciclovir developed reinfection. Serum HBsAg, HBeAg and HBV DNA were positive in 3 cases. Liver biopsy immunohistochemistry showed HBsAg and HBeAg phenotype. Classical viral hepatitis in 1 case occurred, three cases died. In the lamivudin group 7 cases showed positive for HBsAg, 2 cases positive for HBV DNA, 4 cases HBsAg or HBcAg phenotype. One case showed positive for serum anti-HBc the other negative for HBVM, and liver biopsy immunohistochemistry was negative too.
CONCLUSIONSThese date suggest that anti-virus prevention and treatment in pre and post liver transplantation with HBV infected correlative disease is necessary, feasible and effective.

6.Combination of hysteroscopy and laparoscopy in gynecologic operation:an analysis of 65 cases

Objective To introduce the procedures, preliminary experience, and advantages of operation using combination of hysteroscopy and laparoscopy.Methods Sixty-five women underwent transcervical resection of endometrium (11), myoma (38), polyp (2), septa (9), adhesion (1) or foreign body (4). All had the indications for simultaneous laparoscopic monitoring and operation. They were first diagnosed laparoscopically to decide the type of laparoscopic operation. Then they were treated hysteroscopically and finally received laparoscopic operation. Results Laparoscopic examinations confirmed the original diagnoses, revealed the causes of pelvic pain in 8 patients, and detected ovarian cyst, endometriosis, and pelvic adhesion in 5. Two cases of uterine perforation were found by laparoscopic monitoring. Thirty laparoscopic operations included removal or puncture of ovarian cyst, lysis of adhesion, coagulation of endometriosis, Rubin's test, and partial salpingectomy. The postoperative course was smooth and the operative result was satisfactory. Conclusions Operations using combination of hysteroscopy and laparoscopy are safe and effective. They also reduce the pain, and save the time and money of patients. Laparoscopic monitoring may detect uterine perforation in time.
7.Long-term outcome of lung cancer patients treated with surgical resection: A report of 210 cases.

BACKGROUNDTo study the prognostic factors in patients with lung cancer after curative resection.
METHODSA retrospective study was conducted on 210 cases of clinicopathological survival data of lung cancer patients who underwent surgical resection from January 1987 to December 1999. Nine conventional prognostic factors were analyzed by COX model.
RESULTSThe overall 3-, 5- and 10-year survival rates were 37.4%, 30.1% and 23.5% respectively. Univariate analysis showed that regional lymph nodes status (N), primary tumor status (T), histological type of lung cancer, the type of operation and curability of surgical resection were significantly related to disease specific survival. Multivariate analysis showed that regional lymph nodes status, primary tumor status and curability of surgical resection were the three independent predictors of long term outcome. The hazard ratio of death was 2.42 for patients with N2-3 vs N0-1( P =0.000 1), 3.50 for patients with T2-4 vs T1( P =0.033 0) and 1.77 for patients with incomplete resection vs complete resection ( P =0.022 4).
CONCLUSIONSPrimary tumor status, regional lymph nodes status and curability of surgical resection are the three important prognostic factors of lung cancer. In order to improve long-term survival of lung cancer patients, it is very important to operate in the earlier stage of tumor, to extensively dissect intra-pulmonary and ipsilateral mediastinal lymph nodes and to avoid incomplete resection.
8.The design, preparation and compressive strength testing of interbody fusion cages made from a composite of multi-amino acid copolymer/tri-calcium phosphate.

This research was to design and prepare interbody fusion cages using composite materials of multi-amino acid copolymer/tri-calcium phosphate (MAACP/TCP) and to test compressive strength of the cages. 16 specimens of C3-4 segments from female adult goats were scanned by X-ray to exclude disease of cervical spine, and then anatomical data were measured, i. e. disc space height of C3-4 segment (DSH), sagittal diameter of C3 lower endplate (SDLE3), sagittal diameter of C4 upper endplate (SDUE4), coronary diameter of C3 lower endplate (CDLE3), and coronary diameter of C4 upper endplate (CDUE4). According to the anatomical data, we designed and prepared the interbody fusion cage using the composite of MAACP/TCP and titanium with the same sizes. The MAACP/TCP Cages were made with the method of injection molding and finish machining, and titanium Cages were made with machining. In the testing of compressive strength of Cages, the specimens were divided into three groups, tricortical iliac crest bone group (isolated from goats), MAACP/TCP Cage group and titanium Cage group. There were 8 specimens in every group with the same sizes, the length of 12 mm, the width of 10 mm, and the height of 6 mm. The compressive strength of all specimens was tested on a universal testing machine. The values of DSH, SDLE3, SDUE4, CDLE3 and CDUE4 were (4.78 +/- 0.17) mm, (15.06 +/- 0.53) mm, (12.46 +/- 0.44) mm, (14.47 +/- 0.51) mm and (12.15 +/- 0.65) mm, respectively. MAACP/TCP Cage was successfully designed and made with a compressive strength of 76.34 MPa, which was much higher than that of tricortical iliac crest bone (18.41 MPa). The maximal loading of universal testing machine was 50 000 N, so the compressive strength of titanium Cages, whose value should be more than 541.35 MPa, could not be tested precisely. It is feasible to make cages with MAACP/TCP composite, and the compressive strength of MAACP/TCP Cages was much higher than that of tricortical iliac crest bone isolated from goats.

9.Correlation Study Between Blood Level of Big Endothelin-1 and Cardiac Remodeling in Patients With Hypertrophic Obstructive Cardiomyopathy

Objective:To explore the relationship between big endothelin-1 (big ET-1) and cardiac remodeling in patients with hypertrophic obstructive cardiomyopathy (HOCM).
Methods:A total of 167 consecutive HOCM patients admitted in our hospital from 2015-01 to 2016-05 were enrolled. Blood levels of big ET-1 and NT-proBNP were measured;electrocardiogram(ECG), dynamicECG,echocardiography (UCG) and cardiovascular magnetic resonance (CMR) were examined for relevant statistical analysis.
Results:In all 167 HOCM patients, blood level of Big ET-1 was positively related to NT pro-BNP (r=0.35, P=0.000), left atrial (LA) diameter (r=0.169, P=0.019) and heart rate (r=0.141, P=0.037);negatively related to hemoglobin (r=-0.173, P=0.013) and the ratio of interventricular septum (IVS)/posterior wall of left ventricle (r=-0.165, P=0.017). Based on the finding of positive correlation between Big ET-1 and LA diameter, the patients were divided into 2 groups:Normal LA group, n=74 and Enlarged LA group, n=93. Compared with Normal LA group,Enlarged LA group had the higher blood level of Big ET-1 (P=0.001);increased left ventricular end-diastolic diameter (LVEDD) (P=0.024), thicker IVS (P=0.003), lower LVEF (P=0.001);enlarged LVED volume (P=0.002) and IVS (P=0.002);elevated ratio of atrial fibrillation(AF);more patients with moderate to severe mitral regurgitation (MR) and NYHA functional classIII/IV.
Conclusion: Blood level of Big ET-1 was positively related to NT pro-BNP and LA diameter in HOCM patients;the patients with enlarged LA had increased Big ET-1, more obvious cardiac remodeling and the higher incidence of AF occurrence which implied that Big ET-1 might play the role in cardiac remodeling in HOCM patients.
10.The clinical significance of CD4+CD69+T lymphocytes in patients with autoimmune hemolytic anemia/Evans syndrome

Objective To investigate the significant of peripheral CD4+CD69+T lymphocytes in patients with autoimmune hemolytic anemia (AIHA)/Evans syndrome (ES).Methods In this study peripheral blood samples from 32 patients with AIHA/ES (15 hemolytic episode patients,17 remission patients) and 13 healthy controls were collected.Patients with AIHA/ES were recruited in Tianjin Medical University General Hospital from October 2015 to May 2016.The percentages of CD69+ T lymphocytes were analyzed by flow cytometry.The expression of CD69 mRNA in CD4+T lymphocytes which was sorted from peripheral blood by magnetic activated cell sorting (MACS) was detected using real-time PCR.Soluable CD69 was measured by ELISA.Results In hemolytic episode patients,the ratio of CD3+CD69+/CD3+T lymphocytes [(3.08 ± 1.48)%] was significantly higher than that in healthy controls [(1.28 ± 0.83)%,P<0.01] and in remission group[(1.96± 1.33)%,P<0.05].The absolute count of CD3+CD69+T lymphocytes in hemolytic episode group [(2.94± 1.81)× 107/L] was higher than that in healthy controls [(1.48± 1.42)× 107/L,P<0.05].The ratio of CD3+CD4+CD69+/CD3+CD4+T cells in hemolytic episode group [(2.16± 1.56)%] was significantly higher than that in remission group [(1.16±0.62)%,P<0.05] and healthy controls[(0.94±0.78)%,P<0.05].The quantity of CD3+CD4+CD69+T lymphocytes in hemolytic episode group[(1.04±0.98)× 107/L] was higher than in healthy controls [(0.44± 0.38) × 107 / L,P<0.05].The ratio of CD3+CD8+CD69+/CD3+ CD8+T lymphocyte in hemolytic episode group [(4.87±2.56)%] was significantly higher than that in healthy controls[(1.83± 1.27)%,P<0.01].The quantity of CD3+CDs+CD69+T lymphocytes in three groups did not show significant difference.The ratio of CD3+CD4+CD69+/CD3+CD4+T lymphocytes in hemolytic episode group was negatively correlated with hemoglobin (Hb) (P<0.01),positively correlated with the percentage of reticulocytes (Ret%)(P=0.01)total bilirubin(TBil),indirect bilirubin(IBil) (P<0.01) and not correlated with absolute reticulocytes count,lactic dehydrogenase (LDH),complement 3(C3),complement 4 (C4).The ratio of CD3+CD4+CD69+/CD3+CD4+T lymphocytes in remission group was negatively correlated with Hb (P<0.05).In hemolytic episode patients CD69 mRNA (32.26±35.11) was significantly higher than that in remission group(6.05±5.87)(P<0.05)and healthy controls (1.76± 1.85)(P<0.01).CD69 mRNA in remission group was significantly higher than healthy controls (P<0.05).Serum CD69 in hemolytic episode patients [(494.21 ± 16.06) ng/L] was significantly higher than that in healthy controls [(441.39± 104.6) ng/L,P<0.05].Conclusion Our findings suggest that the proportion of CD4+CD69+ T lymphocytes increase in AIHA/ES patients,which is correlated with the severity of disease.
