1.Effect of lidocaine and tramadol on remifentanil combined general anesthesia

Objective To evaluate the effect of lidocaine and tramadol on remifentanil combined general anesthesia. Methods Eighty patients undergoing abdominal hysterectomy were divided into group LT,L,T and C by random digits table,each with 20 cases .In group LT,during induction,the patients were intravenously. In group L,only used lidocaine as introduced in group LT. In group T,only used tramadol as shown in group LT. In group C,used the same volume of saline solution. The changes of mean arterial pressure (M A P) and heart rate during the operation, dysphoria grade (RS), Ramsay score (RSS), pain score (VAS),extubation time(ET),medication and dose,complications were recorded. Results In group LT,the dosage of propofol [(450 ± 178) mg] was less (P < 0.05), ET [(15.0 ± 4.8) min] was shorter (P < 0.05),during recovery time, RS was lower, RSS was higher,the cases needad fentanyl (1 case), 48 h after operation, only 1 case needed pethidine to relieve pain was less than those in other groups (P < 0.05). In group L there was a tend that its RS and the cases needed fentanyl and fentanyl dosage were more, RSS was less than those in group T during recovery time,but 48 h after operation,the cases needed pethidine was less than that in group T. Conclusions Using lidocaine during operation may relieve pain after operation, reduce the dosage of analgesic;giving tramadol may alleviate the acute pain and lower dysphoria incidence rate after remifentanil anesthesia. Combining them will get a better effect.

Objective To evaluate the analytical performance of ARCHITECT-i2000SR chemiluminescence analyzer in detec-tion of HBsAg,HIV-antibody,TP-antibody and HCV-antibody.Methods Validated the precision,carryover,ference interval of alpha-fetoprotein and linearity performance of ARCHITECT-i2000SR.The same specimens were tested by both CMIA and ELISA,and the results were compared and analyzed.Results The precision,carryover and ference interval of alpha-feto-protein of HBsAg,HIV-antibody,TP-antibody and HCV-antibody were within the range provided by the manufacturer for ARCHITECT-i2000SR.The theoretical and measured values of HBsAg were:Y = 1.102X - 6.678 6 (r = 0.995 5,P <0.05),the range of linearity 1.08~208.6.The sesult was very good for the four blood index by both methods.Conclusion The basic performances of ARCHITECT-i2000SR were consistent with the data provided by the manufacturer,so it canbe used to inspect the clinical samples and the sasults were credible.
4.Mediation effects of neuroticism between parental rearing patterns and interpersonal sensitivity

Objective To explore the role of neurotic in parental rearing patterns and interpersonal sensitive.Methods Using stratified sampling method to test 702 middle school students,symptom checklist 90 (SCL90),Egma minnen av bardndosnaupp forstran(EMBU) and Eycenck Personality Questionnaire(EPQ) were used to investigate and take one and a half years of follow-up observation on the sample of 245 students from first grade of junior and senior middle school.Correlated variables were made the correlation analysis,regression analysis,constructing the structure equation model and using tracking data to confirm.Results (1) Neuroticism,father's affective warmth and understanding,father's excessive protection,mother's rejection and deny had a direct effect,and the variation of total symptoms could be explained 40.8% ; father's affective warmth and understanding,father's excessive protection,mother's over-interference and over-protection,mother's rejection and deny had a indirect effect on interpersonal sensitive,and explained 24.0%.(2) Effect analysis showed father's affective warmth and understanding,father's excessive protection,mother's rejection and deny impact on interpersonal sensitive by neuroticism,and father's excessive protection was the largest which accounted for 48.0% of the total effect.The second was father's affective warmth and understanding,accounting for 47.3 %.The last was mother's rejection and deny accounted for 42.7%,and its indirect effect increased with the age(69.1%).All of mother's over-interference and overprotection through the neurotic influence interpersonal sensitive,and the direct effect was 50.3% between neurotic and interpersonal sensitivity.Conclusion Indirect effect of neurotic existed in relationships of parents rearing patterns and interpersonal sensitivity.
5.Finite element analysis of knee flexion and extension movement

BACKGROUND:Three dimensional finite element analysis is widely used in orthopedics, but research in the knee flexion movement is not much. OBJECTIVE:To analyze biomechanical properties of knee flexion using finite element analysis.METHODS:Three dimensional finite element models and models of knee flexion at 30 degrees, 60 degrees and 120 degrees were established. Femoral forward movement, femur inward movement, tibia internal rotation and tibia vara were analyzed at different flexion angles. RESULTS AND CONCLUSION:(1) When knee flexion was at 30 degrees, 60 degrees and 120 degrees, the femur had backward movement. The backward movement value was larger at 60 degrees than at 30 and 120 degrees (P<0.05). The backward movement value was larger at 120 degrees than at 30 degrees (P>0.05). (2) The femur had outward movement at 30 degrees, and inward movement at 120 degrees. The inward movement value was significantly larger at 120 degrees than at 30 and 60 degrees (P<0.05). No significant difference in outward movement was determined between the 30 and 60 degrees (P>0.05). (3) Tibia internal rotation was not significantly different at 30, 60 and 120 degrees of flexion (P>0.05). (4) Tibia vara was found at 30, 60 and 120 degrees of flexion, but the tibia vara was not obvious at 120 degrees. The tibia vara was maximum at 60 degrees, and significantly higher than that at 120 degrees (P<0.05). The tibia vara was larger at 30 degrees than at 120 degrees (P>0.05). (5) These findings verify that backward movement was most obvious at 60 degrees. Femur inward movement was most large at 120 degrees. Tibia internal rotation was noticeable at 30 and 120 degrees. Tibia vara was remarkable at 60 degrees.
6.Cohort mortality study in three ceramic factories in Jingdezhen in China.

To investigate the health impacts of crystalline silica mixed dust and other potential occupational hazards on workers in ceramic factories, a cohort study of 4851 workers registered in the employment records in 3 ceramic factories in Jingdezhen city of China between 1972 and 1974 was identified. The cohort mortality was traced throughout 2003 with an accumulation of 128970.2 person-years, revealed 1542 deaths. Standardized mortality ratios (SMRs) were calculated for the main causes of death by using Chinese national mortality rates as reference. The mortality from all causes in three ceramic factories was 12.0 per thousand and the cumulative mortality was 31.8%. Malignant neoplasm, cardiovascular diseases, respiratory diseases, infectious diseases were the first four illnesses that threaten workers' life, and they accounted for 73.2% of all deaths. The results of this study showed that the standardized mortality ratio for all subjects was 1.02, which is very close to that expected on the basis of the China national mortality rates. Statistically significant mortality excesses for respiratory disease (SMR=1.36), pneumoconiosis (SMR=37.34), infectious disease (SMR=5.70) and pulmonary tuberculosis (SMR=3.88) were observed. The mortality of 2938 dust-exposed workers was higher than that of 1913 non dust-exposed workers. Except for pneumoconiosis, the mortality from lung cancer, non-malignant respiratory diseases and pulmonary tuberculosis in dust-exposed workers were significantly increased as compared with that in non-exposed workers, and the relative risks (RRs) were 1.86 (1.16-2.99), 2.50 (1.84-3.40), 1.81 (1.34-2.45). The exposure-response relationships between cumulative dust exposure level and mortality from all causes, colorectal cancer, lung cancer, respiratory diseases, and pulmonary tuberculosis were also identified. The findings indicated that silica mixed dust in ceramic factories has harmful impact on the workers' health and life span in ceramic factory.
7.Smoking status among people with disabilities in Shanghai Jing′an District

Objective To survey on the smoking status among people with disabilities in Shanghai Jing′an District.Methods A cross-sectional questionnaire survey on smoking status among people with disabilities was conducted from July to December 2015 in 5 communities of Shanghai Jing′an District. Results Total 1 815 valid questionnaires were collected.Among all participants,278 were smokers with an overall smoking rate of 15.3%(278/1 815); the smoking rate in males was 28.1%(265/944)and in females was 1.5%(13/871).The disables people aged 45 -60 years had the highest smoking rate (23.4%,141/602).The nicotine dependence rate in regular smokers accounted for 74.6%(182/244). The nicotine dependence was mainly mild to moderate, and the average of Fagerstrom Test of Nicotine Dependence(FTND)score was 3.75 ±1.97.Most participants(90.8%,238/262)were aware of the harm of smoking to health,but the awareness rate of health hazards of second-hand smoke was only 59.5%(156/262).The awareness rates of tobacco-related diseases were all lower than 70%.The rate of quitting was 33.5%(93/278),but only 17.2%(16/93)succeeded;66.0%(173/262)of the smokers did not intend to quit,only 15.3%(40/262)started to quit or to reduce the smoking;and 83.6%(219/262)of current smokers thought that it was impossible to quit smoking.Conclusion The smoking rate in people with disabilities is lower than that in the general population,the awareness of the harm of second-hand smoke is low and there is lack of knowledge of tobacco related diseases;the intend of quitting is low but the success rate of smoking cessation is high among people with disabilities in Shanghai Jing ′an District.
8.Treatment of geriatric osteoporotic femoral intertrochanteric fractures with proximal femoral nail antiro-tation and bone cement enhancement

Objective:To investigate the efficacy of proximal femoral nail anti-rotation (PFNA) plus bone cement enhancement in the treatment of geriatric osteoporotic femoral intertrochanteric fractures.Methods:A retrospective analysis was conducted of the 52 elderly patients with osteoporotic femoral intertrochanteric fracture who had been treated at Department of Bone and Joint Surgery, The Fourth People's Hospital of Guiyang from September 2012 to October 2018 by PFNA internal fixation plus bone cement enhancement. They were 8 males and 44 females, aged from 65 to 91 years (average, 78 years). The time from injury to operation averaged 4.4 d. By the AO/OTA classification, there were one case of type 31-A1.2, 2 cases of type 31-A2.1, 8 cases of type 31-A2.2, 34 cases of type 31-A2.3, one case of type 31-A3.1, and 6 cases of type 31-A3.3. The patients' operation time, intraoperative bleeding, hospital stay, fracture reduction, postoperative weight-bearing time, fracture union time, function of the affected hip and complications were recorded.Results:The operation time for this cohort averaged 79.4 min, intraoperative bleeding 123.6 mL, and hospital stay 11.9 d. By the Francisco evaluation, the reduction of intertrochanteric fracture was assessed as good in 43 cases and as fair in 9 cases. Of this cohort, 45 were followed up for 5 to 12 months (average, 6 months) and 7 lost to follow-up. By the Harris hip scores at the last follow-up, 14 cases were rated as excellent, 25 as good and 6 as fair, giving an excellent to good rate of 88.9% (39/45). No complications like loosening, cut-out or breakage of implants or femoral head necrosis occurred during follow-up.Conclusion:In the treatment of geriatric osteoporotic femoral intertrochanteric fractures, PFNA plus bone cement enhancement can achieve satisfactory clinical efficacy.
9.Analysis on the status and influencing factors of health care intention in transitional children and adolescents with diabetes

Objective:To investigate the intention to participate in health care in transitional children and adolescents with diabetes and analyze its influencing factors.Methods:This was a cross-sectional survey study. From March to October 2021, 185 children and adolescents with diabetes were selected as research subjects by convenience sampling method from People′s Hospital Affiliated to Jiangsu University. General data questionnaire, Participation in Health Care Intention Questionnaire, Health Literacy Scale and Diabetes Self-management Scale were used to investigate, and multiple linear regression was used to analyze the influencing factors of the population′s intention to participate in health care.Results:The transitional children and adolescents with diabetes participation in health care intention total score was (124.87 ± 16.31) points, the health literacy total score was (33.70 ± 4.38) points, diabetes self-management total score was (35.11 ± 5.19) points. The regression analysis found that age, course of the disease, diabetes type, family structure modes, health literacy, and self-management ability were the main factors influencing adolescents involved in health care intention ( t values were -1.99-2.66, all P<0.05), including health literacy ability and disease management ability was positively correlated with the disease ( r = 0.250, 0.232, both P<0.01). Conclusions:The transitional children and adolenscents with diabetes have a medium level of intention to participate in health care. The transitional children and adolenscents with older age, longer disease course and nuclear family structure had higher levels of health care intention. The higher the level of health literacy and self-management, the higher the level of intention to participate in health care, medical staff should take targeted measures to improve the intention to participate in health care, promote this group to improve disease management ability and quality of life.
