1.Correlation of Immune Infertility with Chronic Genital Tract Inflammation and Intervention of Chinese Medicine

【Objective】To explore the correlation of antisperm immune infertility(AII) in females with chronic genital tract inflammation and to observe the therapeutic effect of Chinese medicine of Qingshi Huayu Mixture(QHM) and Yishen Huoxie Prescription Ⅰ(YHPⅠ) for AII.【Methods】Fifty-eight AII patients were screened from 200 infertility patients,and were randomized into three groups.Group A(N=18) was treated with YHPⅠ,group B(N=20) with QHM and group C(N=20) with western medicine.Three months constituted one treatment course.The incidence of positive antisperm antibody(AsAb+)combined with chronic genital tract inflammation,therapeutic effect,pregnancy rate,the content of serum AsAb,as well as the changes of lymphocyte subtypes,natural killer(NK) and interleukin 6(IL-6) were observed.The correlation of the above indexes with serum AsAb content was also analyzed.【Results】(1) The incidence of positive antisperm antibody(AsAb+)combined with chronic genital tract inflammation: There were 96(48%)with AsAb+ in 200 patients,and 62(64.58%) with AsAb+ in 122 combined with chronic genital tract inflammation;there were 34(35.42%)AsAb+ patients in 78 without inflammation,the difference of AsAb+ incidence was significant between the inflammation patients and the non-inflammation patients(P0.05).There were 2 in 5 cured patients of group A,2 in 7 of group B and 1 in 7 of group C being pregnancy.(3) Content of serum AsAb: serum AsAb content was decreased in the three groups after treatment,the difference being significant as compared with that before treatment(P0.05).(4) The changes of lymphocyte subtypes,NK and IL-6: CD8+ was increased,CD4+/CD8+ and NK were decreased in groups A and B,the differences being significant as compared with those before treatment and those in group C after treatment(P

【Objective】To observe the therapeutic effect of laparoscopic ovarian electrocauterization(LOE) combined with cycle treatment of traditional Chinese medicine(TCM-CT) for type phlegm-damp refractory polycystic ovary syndrome(PCOS).【Methods】Forty PCOS patients admitted from August,2004 to March,2006 were randomized into groups A,B and C.Group A(N=20) received LOE and TCM-CT,group B(N=10) received LOE and western medicine for ovulation,and group C received LOE only.The endocrine hormones levels,ovulation rate and pregnancy rate were observed.【Results】The differences of serum luteinizing hormone(LH) and testosterone(T) levels before and after treatment were significant in group A(P0.05).After treatment,LH and T levels in group A differed from those in groups B and C(P0.05).The pregnancy rate in group A differed from those in groups B and C(P0.05).【Conclusion】LOE combined with cycle treatment of traditional Chinese medicine exerts a certain effect on improving reproductive health of patients with type phlegm-damp refractory polycystic ovary syndrome.
3.Clinical Study on Tianma Gouteng Decoction Combined with Routine Western Therapy in the Treatment of Early Onset Severe Pre-eclampsia

Objective To observe clinical efficacy of Tianma Gouteng Decoction combined with Western medicine for early onset severe pre-eclampsia. Methods Sixty patients were randomly divided into treatment group with 30 cases and control group with 30 cases. Two groups were both given the routine Western therapy, while the treatment group was added with Tianma Gouteng Decoction. Indicators were observed and compared between two groups including blood pressure, 24 h UAE, D-dimer, fibrinogen and their effects on pregnancy. Results Compared with the control group, the blood pressure, 24 h UAE, fibrinogen, D-dimer, hypoproteinemia, and liver and kidney function were reduced obviously in the treatment group (P<0.05);fibrinogen and D-dimer of control group were not changed significantly;gestational weeks of treatment group were extended (15.2 ± 4.8)d, and neonatal asphyxia was 7 cases;gestational weeks of control group were extended (9.3 ± 2.2)d, and neonatal asphyxia was 11 cases. There was no significant difference about cesarean section rate between the two groups. Conclusion Tianma Gouteng Decoction combined with Western medicine for treatment of early onset severe pre-eclampsia have better clinical efficacy.

【Objective】To observe the effect of Zhuyun Prescription Ⅲ(ZPⅢ),which is composed of kidney-tonifying and spleen-strengthening herbal medicine,on T lymphocyte antigen recognition receptors(TCR?&T) in luteal insufficiency abortion rats with deficiency of kidney.【Methods】Forty SD rats were equally randomized into 5 groups: blank control group,model group,high-and low-dose ZPⅢ groups(in the doses of 7.08 and 3.54 g?kg-1?d-1,respectively) and allylestrenol tablets(1.5 mg?kg-1?d-1) group.Luteal insufficiency abortion rats model with deficiency of kidney was established by hydroxyurea(450 mg?kg-1)and mifepristone(3.75 mg?kg-1).The uterus weight,the amount of embryo and the abortion rate were observed,and the peripheral blood levels of TCR?&T and CD3+T lymphocytes were detected with flow cytometer.【Results】The uterus weight and the amount of embryo were decreased,and the abortion rate was increased in the model group(P

[Objective] To evaluate the significance of plasma endothelin in tubal pregnancy (TP) and to observe the regulatory effect of kidney-reinforcing, blood-activating and blood-stasis-removing Chinese herbal medicine on endothelin (ET) level. [Methods] Plasma ET level and ET-positive rate were detected in 40 cases of TP, 20 of intrauterinc pregnancy (IP) and 20 of chronic pelvic inflammation (CPF) and the relationship between the two above indexes in the patients were analyzed. TP patients were randomized to three groups; group A (n = 15) was treated with No. 1 Yishen Huoxue Pills (6g, tid) and Penyankang mixture (25mL, tid) for two treatment courses; group B (n = 15) was treated with aspirin (257ng, qd) for two treatment courses; group C ( n - 10) was not given any medicines. The case is the cornerstone of case teaching. The construction of case database can support case teaching and its orderly development. The clinical cases of gynecology of Chinese medicine were collected, sorted and processed in combination with the teaching syllabus and teaching objective. Operating platform was based on the Excelltable. The column was divided into overview, menstrual disorders, leukorrhoeal diseases, pregnancy disease, postpartum disease and miscellaneous diseases of gynecology with hierarchical set of 15 modules per column, including basic information, complaints, history, symptoms, physical examination, diagnosis, application purpose and context and so on. And the corresponding search term was also selected. Cases can be divided into introduction cases and improvement ones according to their easiness and difficulty , into typical cases and atypical ones according to their feature types. Case database content also needs to be constantly revised by teaching activities to make it more suitable for clinical teaching.

Background/Aims:
The accuracy of endosonographers in diagnosing gastric subepithelial lesions (SELs) using endoscopic ultrasonography (EUS) is influenced by experience and subjectivity. Artificial intelligence (AI) has achieved remarkable development in this field. This study aimed to develop an AI-based EUS diagnostic model for the diagnosis of SELs, and evaluated its efficacy with external validation.
Methods:
We developed the EUS-AI model with ResNeSt50 using EUS images from two hospitals to predict the histopathology of the gastric SELs originating from muscularis propria. The diagnostic performance of the model was also validated using EUS images obtained from four other hospitals.
Results:
A total of 2,057 images from 367 patients (375 SELs) were chosen to build the models, and 914 images from 106 patients (108 SELs) were chosen for external validation. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of the model for differentiating gastrointestinal stromal tumors (GISTs) and non-GISTs in the external validation sets by images were 82.01%, 68.22%, 86.77%, 59.86%, and 78.12%, respectively. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy in the external validation set by tumors were 83.75%, 71.43%, 89.33%, 60.61%, and 80.56%, respectively. The EUS-AI model showed better performance (especially specificity) than some endosonographers.The model helped improve the sensitivity, specificity, and accuracy of certain endosonographers.
Conclusions
We developed an EUS-AI model to classify gastric SELs originating from muscularis propria into GISTs and non-GISTs with good accuracy. The model may help improve the diagnostic performance of endosonographers. Further work is required to develop a multi-modal EUS-AI system.
