1.Antioxidative effect of sodium ozagrel on myocardial ischemia in rats

Objective To disccuss the antioxidative effects of sodium ozagrel(OZG) in myocardial ischemia in rats and illuminate its effect of decreasing oxidative damage.Methods Forty healthy Wistar rats were randomly devided into five groups:control,model,pretreatment,treatment and therapeutic alliance groups.OZG was used for 2 weeks in pretreatment group,then isoprenaline hydrochloride (ISO) was applied to make myocardial ischemia model.5 min after the establishment of model,OZG and OZG combined with glyceryl trinitrate(GT) were used in treatment group and therapeutic alliance group,respectively,ECG was recorded at the same time.1 h later,the activities of serum myocardial creatases(AST,LDH,CK),malonaldehyde(MDA) level and superoxide dismutase(SOD) activity were detected.Results ECG:When the model was successfully set up,ST segment in every group all raised≥0.1 mV,the minimum value of raise was in pretreatment group.When the model was succeeded 5 min later,ST segment in model group slightly decreased but still ≥0.1 mV and it was significant in pretreatment group.In treatment and therapeutic alliance groups,compared with model succeeding'ST segment,ST segment decreased significantly at 5 min later(P
2.Lemierre syndrome caused by Staphylococcus aureus:one case report

Objectives To analyze the clinical features, diagnosis and treatment of Lemierre syndrome. Methods The clinical data of one case of Lemierre syndrome were retrospectively analyzed. The relevant literatures were reviewed. Results The primary infection of the patient was oral infection, and jugular vein thrombosis and metastatic lung abscess were followed. The blood culture showed that Staphylococcus aureus was positive. Lemierre syndrome was diagnosed. After anticoagulation and anti-infection treatment, the symptoms were improved. Conclusion Lemierre syndrome should be considered in present of jugular vein thrombosis and pulmonary abscess caused by infection.
3.Reliability and validity of the Chinese version of the Functional Assessment of Cancer Therapy-Cognitive Function in Chinese women with breast cancer

Objective To examine the reliability and validity of the Chinese version of the Functional Assessment of Cancer Therapy-Cognitive Function (FACT-Cog) in Chinese women with breast cancer.Methods The Chinese version of the FACT-Cog was obtained by FACT.A total of 202 patients with breast cancer were investigated by convenient sampling survey.The reliability and validity of the scale were analyzed by the principal component analysis and Pearson correlation analysis.Results The Cronbach α of the Chinese version of the FACT-Cog was 0.96, and those of four dimensions were 0.87-0.96.The test-retest correlation coefficients for the Chinese version of the FACT-Cog were 0.45-0.73, P<0.01.The four-factor structure of the Chinese version of the FACT-Cog was confirmed and accounted for 66.63% of the total variance.The criterion-related validity of the Chinese version of the FACT-Cog were significantly related with those of anxiety, depression and fatigue,-(0.36~0.48),-(0.42~0.52), 0.48-0.59,P<0.01.Conclusion The Chinese version of the FACT-Cog is valid, reliable, and appropriate for clinical and research use in Chinese women with breast cancer.
4.Development and application of transparent restraint protection gloves

Objective To prevent unplanned extubation,reduce complications caused by restraint, improve satisfaction degree of patients′relatives,and to lower nurses′work stress.Methods Transparent restraint protection gloves were developed,within which fingers could move freely and with the help of which unplanned extubation could be prevented.A total of 1 00 ICU patients were selected and divided into intervention group and control group randomly,each with 50 cases.Patients in the intervention group used the new type restraint gloves,while patients in the control group used traditional ones.Restraint effects of the two groups were compared.Results Unplanned extubation happened to non patient in the intervention group,while it happened to 6 cases in the control group,Fisher′s exact test was used in comparison between the two groups (P <0.05). Scores of satisfaction degree in the intervention group were (3.1 4 ±0.639)points,while the scores in the control group were (2.20 ±0.756)points (t =6.71 4,P <0.01 );scores of nurses′work stress on accident extubation of patients in the intervention group were (2.1 2 ±0.761 )points,lower than that in the control group with (t =-8.696,P <0.01 ).Conclusions Transparent restraint protection gloves can effectively prevent unplanned extubation,reduce complications caused by restraint,improve satisfaction degree of patients′relatives,and lower nurses′work stress.
5.Application of continuous low pressure suctioning combined with intermittent high pressure suctioning on drainage of subglottic secretion

Objective To observe the effect of the continuous low pressure suctioning unite intermittent high pressure suctioning on drainage of subglottic secretions in the patients with artificial airway.Methods Using random number table,a total of 44 patients were evenly and randomly divided into observation group and control group.In the control group,continuous low pressure suctioning was performed to clear away the subglottic secretions.In the observation group,continuous low pressure suctioning combined with intermittent high pressure suctioning were performed.The average daily drainage volume of subglottic secretion,the incidence of early onset VAP (Ventilatorassociated pneumonia),the frequence of cough,the incidence of the airway mucosal damage,the incidence of the clogging of the drainage tube were investigated and compared between two groups.Results The average daily drainage volume of subglottic secretion in the observation group (52.80 ±4.181)ml was significantly more than in the control group (33.00 ± 2.675) ml (t =16.656,P < 0.05).The incidence of early onset VAP in the observation group (10.0%) was significantly less than in the control group (40.0%) (x2 =4.800,P < 0.05).There was no statistical difference in the frequence of cough between the observation group (1.93 ± 1.023) times and the control group (1.75 ± 1.006) times (t =0.771,P > 0.05).There was no statistical difference in the incidence of the mucosal damage of airway between the observation group (15%) and the control group(10%) (x2 =0.229,P >0.05).The incidence of the clogging of the drainage tube in the observation group (0.0%) was significantly less than in the control group (20.0%) (x2 =4.444,P <0.05).Conclusions Continuous low pressure suctioning combined with intermittent high pressure suctioning can significantly improve the effect of drainage of the subglottic secretions by increasing the drainage volume,reducing the clogging of the drainage tube and the early onset of VAP.
6.Multivariate analysis and countermeasures of attracted subglottic secretion volume

Objective To explore the factors related to the amount of suction under glottis, look for effective nursing counter measures in order to improve the suction effects. Methods A retrospective analysis of the clinical data of 71 patients with subglottic secretion drainage from 1st January 2013 to 1st May 2014 was performed. The secretion viscosity, posture, the endotracheal tube cuff pressure, patients restlessness, suction pressure, and acute physiology and chronic health evaluationⅡscore were set as independent variables. A multiple linear regression analysis was performed to screen factors that may lead to reduction of suction volume of subglottic secretion and nursing counter measures were planned and implemented accordingly. Results Secretion viscosity, supine, endotracheal tube cuff pressure and patient restlessness were the main reasons that influence the amount of suction under glottis(P<0. 05). Multiple linear regression equation:amount of suction of subglottic secretion/2 h(ml) =4. 868 +2. 470 × restlessness-1. 624 × secretion viscosity+0. 874 × endotracheal tube cuff pressure-0. 933 × posture. Conclusions Quantity of suction of subglottic secretion were positively correlated to patients′restlessness, secretion viscosity and endotracheal tube cuff pressure. Subglottic attract amount for the most when take left lateral position. Strengthening the control and intervention of related factors plays a very important role in the care of suction under glottis.
7.The changes of EEG signals during resting state in adolescents with smoking addiction

Objective To observe the changes of electroencephalogram (EEG) signal during the resting state in adolescents with smoking addiction.Methods Spectral analysis during the resting state was used to observe and analyze the EEG signals between adolescents with smoking addiction (n=21) and nosmoking control group (n=21) by using Brain Vision Analyzer software.Results Compared with the nosmoking control group,adolescents with smoking addiction showed increased alpha band signals in 3 brain regions (including left-anterior,right-anterior and midline-anterior regions,P<0.05),and decreased delta band signals in 3 brain regions (including right-posterior,midline-anterior and midline-posterior regions,P<0.05).Moreover,the signals in these brain regions were positively correlated with pack-years (alpha band:left-anterior(r=0.648,P=0.003),right-anterior (r=0.629,P=0.004),midline-anterior (r=0.456,P=0.043);delta band:right-posterior (r=0.602,P =0.005),midline-anterior (r =0.533,P=0.019),midline-posterior (r=0.692,P=0.001))in adolescents with smoking addiction.Conclusion The changes of EEG signals during the resting state in adolescents with smoking addiction may be associated with cumulative effects of smoking.
8.Study of event-related potentials based on Flanker task in adolescent addicted to smoking

Objective To investigate the change of the error positivity(Pe)of event-related poten-tial(ERP)under Flanker task in adolescents with smoking addiction. Method The signals of ERP during the Flanker task in fifteen adolescents with smoking addiction were compared with that of 15 non-smoking ad-olescents. Results The Pe amplitude in adolescents with smoking addiction((3.575±2.299)μV)was sig-nificantly lower than that of non-smokers((5.865 ± 4.290)μV)(F=5.324,P=0.029),and the reaction time of the Flanker task in smoking addiction adolescent group((440.754± 36.051)ms)was significantly higher than that in the control group((422.850±20.194)ms)(F=0.620,P<0.05).The Pe amplitude in ado-lescents with smoking addiction was positively correlated with smoking index(pack-years)(r=0.599,P=0.018). Conclusion The adolescents with smoking addiction may exit worse ability of error monitoring dur-ing the Flanker task.
9.Relationship between coronary flow reserve by CZT SPECT and invasive coronary fractional flow reserve and its potential clinical value

Objective:To evaluate the relationship between regional coronary flow reserve (CFR) obtained from cadmium-zinc-telluride SPECT(CZT SPECT) myocardial functional perfusion imaging (MFPI) and invasive fractional flow reserve (FFR) measured during coronary angiography (CAG) and its clinical value in guiding coronary interventions.Methods:Forty-two patients (30 males, 12 females, age (63.3±9.8) years) who completed CZT SPECT MFPI in the First Affiliated Hospital of Nanjing Medical University from June 2022 to September 2022 and underwent CAG within 3 months were included retrospectively. The concordance of CFR and FFR for diagnosing myocardial ischemia (CFR<2.0 and FFR<0.8) was calculated at the vascular level. The diagnostic efficacy of coronary stenosis≥70% for decreased myocardial blood flow (CFR<2.0) was calculated. Kappa test was used to analyze the data. Results:A total of 126 major coronary arteries were identified in 42 patients, of which 30(23.8%) had a CFR<2.0 by CZT SPECT and 33(26.2%) had stenosis≥70% in CAG. A total of 32 coronary vessels were performed with MFPI CFR and FFR measurements, of which 6 were both decreased and 21 were both normal, so the concordance rate was 84.4%(27/32)( Kappa=0.612, P<0.001). Among 33 coronary vessels with stenosis≥70%, 13 were with CFR≥2.0. Among 30 coronary vessels with CFR<2.0, 10 were with stenosis<70%. When using stenosis≥70% to diagnose CFR decreasing, the sensitivity was 66.7%(20/30), specificity was 86.5%(83/96), positive predictive value was 60.6%(20/33), negative predictive value was 89.2%(83/93), and accuracy was 81.7%(103/126). Conclusions:The concordance between CFR and FFR for the diagnosis of myocardial ischemia is good. Nearly 1/3 of the coronary arteries with decreased CFR have stenosis<70%, whereas nearly 40% of the coronary arteries with stenosis≥70% are not result in myocardial ischemia. Regional CFR determined by CZT SPECT may have potentially significant clinical value in the diagnosis of coronary artery disease and decision-making of coronary intervention.
