Esophageal carcinoma represents high incidence and mortality rate in China.Irinotecan (CPT-11) has shown promising activity in a number of gastrointestinal cancers,including esophageal cancer.This article reviews recent clinical trials of irinotecan-based chemotherapy and radiochemotherapy in esophageal carcinoma.

Chronic exposure to risk factors at the beginning stage of atherosclerosis development can damage endotheli?um and ultimately initiat endothelial dysfunction. Endothelial dysfunction is one of the major mechanisms in atherosclerosis development. High-density lipoprotein (HDL) is a lipoprotein with complicated function and is composed of a variety of pro?teins and lipids. Epidemiological studies showed an inverse correlation between HDL and atherosclerosis. The protective role of HDL on endothelium might related to its function in reversing cholesterol transport, reducing peripheral cholesterol accu?mulation, preventing foam cells formation, dilating vessels, antioxidant and anti-inflammation, all of which antagonize devel?opment of atherosclerosis. But HDL obtained from patients with cardiovascular disease which has known dysfunctional HDL may induce endothelial dysfunction. This review addresses the relationships between functional HDL or dysfunctional HDL with the endothelial function.

The discovery of microRNAs (miRNAs) has introduced a new paradigm into gene regulatory systems. Since inception, computational methods have been an invaluable tool complementing experimental approaches, and many discoveries have been obtained through combination of experimental and computational approaches. The knowledge that has been accumulated about the principles of miRNAs and target recognition were reviewed. The currently available computational methodologies and software for prediction of miRNA and their target genes also have been discussed.

Objective To compare the efficacy of amisulpride and other SGAs in treating the negative symptoms of Schizophrenia.Methods The randomized controlled trials (RCTs) about Schizophrenia treated with amisulpride and other SGAs from Jan 1995 to Mar 2013 were searched in The Pubmed,EMbase,Cochrane Library,WanFang Data,CNKI and VIP.Two reviewers independently screened the literatures,extracted the data,and evaluated the methodological quality.Than meta-analyses were conducted by using RevMan 5.1 and Stata 12.0 software.Results The totall3 RCTs were included.Among the 1814 patients involved.The results of meta-analyses showed that the score of PANSS-N was no significant differences between two groups (MD =-0.33,95％ CI:(-0.87,0.21),Z =1.20,P =0.23) ; and the score of SANS was no significant differences between two groups (MD =-0.21,95％ CI:(-1.51,1.50),Z =0.31,P =0.76).The side effects were more in other SGAs group than those in amisulpride group.Conclusion Amisulpride is as effective as other SGAs for the treatment of schizophrenia with predominantly negative symptoms,and it has more advantage than other SGAs in safety.

OBJECTIVE To study and find reasonable methods of nursing care of incision through analyzing the risk factors in liver transplant patients.METHODS A total of 416 liver transplantation operations in our hospital from Sep 2004 to May 2006 were evaluated.The risk factors that resulted in the incision infection were considered.RESULTS The incision infection rate was decreased from 13.9% to 4.3% after improvement.CONCLUSIONS Nursing care is of great significance in preventing the incision infection in liver transplant patients.

Objective To discuss the perioperative nursing measures and their effectiveness for patients with malignant airway stenosis who are receiving airway stent implantation once more due to airway re-stenosis.Methods The clinical data of 7 patients,who received airway stent implantation once more due to airway re-stenosis during the period from May 2014 to July 2016,were retrospectively analyzed.The effectiveness of nursing intervention for stent implantation was assessed.Results The symptoms of dyspnea were relieved immediately after the stent re-placement procedure in all the 7 patients,and no severe complications,such as massive hemorrhage,respiratory failure or asphyxia occurred.Conclusion The key points to ensure a successful second airway stent implantation include following measures and preparations:cooperation mode of medical care integration,preoperative individualized psychological intervention,guiding the patients to cooperate with the treatment,ready for in-operation emergency rescue,rapid and effective assistant cooperation for stent placement,and postoperative close observation for the occurrence of respiratory tract infection,bleeding and other complications.

Objective:To study the bone-conductivity and absorbability of nano-hydroxyapatite/collagen(nHAC) composite implanted under the calvarial periosteum in rabbits.Methods:24 nHAC samples and 24 HA samples were prepaired in the shape of round disk with the diameter of 8 mm and thickness of 3 mm.nHAC samples were implanted under calvarial periosteum on the left side and HA samples on the right of 24 rabbits.The bone-conductivity and absorbability of the samples were examined by new bone height measuring and residual implant materia measuring 2,4,8 and 12 weeks after operation. Results:2,4,8 and 12 weeks after implantation new bone height(mm) in nHAC group was 0.54?0.09,0.72?0.12,1.83?0.14 and 2.63?0.07,that in HA group 0.13?0.11,0.31?0.12,1.23?0.05 and 1.75?0.14,respectively(P

Objective To investigate the association of polymorphisms of arylhydrocarbon receptor (AhR) - 1661G/A with glutathione S-transferase pi ( GSTP1 ) - 313A/G and the susceptibility to endometriosis in southern Han Chinese.Methods Total of 432 endometriosis patients undergoing laparoscopic or laparotomy surgery matched with 493 patients with fallopian tube ligation,tubal recanalization,laparoscopic hydrotubation,benign ovarian tumor and teratoma surgeries without endometriosis as control group were enrolled in this study.The single nucleotide polymorphism (SNP) of AhR -1661G/A and GSTP1 -313A/G were detected by using a fluorescent quantitative PCR-based high resolution melting (HRM).Results The numbers of combined genotypes AhR - 1661G/A and GSTP1 -313A/G were 120 patients with AG + AA,64 patients with AG + AG,8 patients with AG + GG,109 patients with GG +AA,84 patients with GG + AG,4 patients with GG + GG,31 patients with AA + AA,10 patients with AA + AG,1 patient with AA + GG at endometriosis group and 131 patients with AG + AA,68 patients with AG + AG,6 patients with AG + GG,157 patients with GG + AA,66 patients with GG + AG,4 patients with GG + GG,35 patients with AA + AA,20 patients with AA + AG,3 patients with AA + GG at endometriosis group.There was no statistically different frequencies of genotypes between endometriosis group and control group (x2 = 12.558,P = 0.128 ).Compared with genotype GG + AA,the risk of endometriosis with genotype GG + AG was increased 1.833 time (95％CI:1.233-2.274).Conclusion The combined genotype GG + AG [ from AhR - 1661G/A (GG) and GSTP1 - 313A/G (AG) ] might be related with susceptibility to endometriosis.

Objective To assess whether metformin prevents body weight gain in patients with schizophrenia who are treated with second-generation antipsychotics(SGA).Methods The randomized controlled trials (RCT) about metformin for prevention of weight gain with SGA for schizophrenia from January 1998 to July 2012 were selected in the Cochrane Library,Medline,Wanfang Data,CNKI and VIP.Two reviewers independently screened the literatures,extracted the data,and evaluated the methodological quality.Than meta-analyses were conducted by using RevMan 5.1 software and Stata 12.0 software.Results The total 7 RCTs were selected and 394 patients were involved.The results of meta-analyses showed that the efficacy of the metformin group was superior to that of the control group in lessen body mass (MD =-3.07,95％ CI:-4.17,-1.98,Z =5.50,P ＜ 0.01),BWI (MD =-1.21,95％ CI:-1.50,-0.92,Z=8.18,P＜0.01) with significant differences.Conclusion Metformin addition therapy is effective in attenuating SGA-induced weight gain.

Objective　To study the clinica features, diagnosis and surgical treatment of simple and complex macromastia. Methods　The clinical and pathological data of 21 patients with macromastia in our hospital from 1990 to 2000 were analysed retrospectively. Simple macromastia in 16 patients with 29 breasts, complex macromastia in 5 patients with 8 breasts. Results　The average age of patients was 23 years old. All the 21 patients were treated surgically. Among the 5 patients with complex macromastia, subcutaneous mastectomy and reconstruction were performed on 2 patients with breast fibroadenomatosis; of the 2 patients with giant fibroadenoma, one underwent tumor excision, the others were subjected to tumor excision with reduction mammaplasty; the last one with breast fibrous disease, selective partial breast excision and reduction mammaplasty was performed. All the patients had satisfactory mammary appearance and hadn't recurrrence of the breast disease in follow up period. Conclusions　(1) Macromastia often happens in the adolescent and young women. (2) It is necessary to do biopsy during operation for distinguishing the simple from the complex macromastia. (3) The treatment for simple macromatia is reduction mammaplasty; for complex macromatia is dissection of the disease area completely and redution mammaplasty simultaneously. For macromastia with fibroadenomatosis, subcutaneous mastectomy with reconstruction can prevent tumor recurrence and get better mammary appearance.
