Malaysian Journal of Medical Sciences 2020;27(1):124-133
Introduction: Acceptance of a screening programme for thalassemia is important in
managing the disease and its associated complications. The objective of this study was to determine
the knowledge of thalassemia and factors associated with thalassemia screening refusal among
parents of secondary school children.
Methods: This cross-sectional study was carried out from May 2017 to October 2017 among
parents of fourth form students in three schools in Besut, Terengganu, Malaysia. Parents who are
able to read and understand Malay and consented to the study were required to answer a validated
questionnaire on their knowledge regarding thalassemia. They were also asked the reason for their
acceptance or refusal of the thalassemia screening of their children.
Results: In total, 273 participants took part in the study. The mean thalassemia knowledge
score was 11.8 out of a maximum score of 21. Low knowledge scores (adjusted odds ratio [adj
OR] 0.87; 95% confidence interval [CI]: 0.79, 0.95; P = 0.002) and female sex (adj OR 2.60;
95% CI: 1.04, 6.53; P = 0.040) had significant associations with parental thalassemia screening
refusal. The main reason for screening refusal was that parents perceived that their children were
not at risk for the disease since they did not have a family member with thalassemia.
Conclusion: The mean thalassemia knowledge score among parents remains
unsatisfactory. A high knowledge score is important since it is associated with parental acceptance
of thalassemia screening for their children.

Objective:To optimize the formula of film-forming materials for compound rhizoma corydalis plastics. Methods:Poly-vinyl alcohol 124 (PVA124) and bletilla striata gum were selected as the film-forming materials. The amounts of PVA 124, bletilla striata gum, glycerol and absolute ethanol were used as the influencing factors in an orthogonal test with the composite score of film-forming time and appearance as the indices to screen out the best composition of film-forming materials. Results:The best composition of film-forming materials was as follows:8 g PVA 124, 1 g bletilla striata gum, 2 ml glycerol and 15 ml ethanol. Conclusion:The op-timized film-forming materials can be used for the preparation of compound rhizoma corydalis plastics.

Nevirapine ( NVP) is the first-generation non-nucleoside reverse transcriptase inhibitor, which is a front-line drug for the treatment of AIDS and the prevention of mother-to-child transmission of HIV-1. NVP is with poor solubility and low bioavailability. With the development of new dosage forms, such as solid dispersion, self-emulsifying, microparticles, micelles, sustained-release tab-lets and floating microspheres, the bioavailability of NVP can be increased and the adverse reactions can be decreased. Research pro-gress in the new dosage forms of NVP at home and abroad was reviewed through searching literatures in order to provide basis for the development of new dosage forms of NVP.

Objective To evaluate the effect of Bortezomib in the treatment of secondary plasma cell leukemia in serum M protein and clinical efficacy.Methods20 cases of patients with secondary plasma cell leukemia were randomly divided into the control group and treatment group, 9 cases in the control group,11 cases in the treatment group. The control group was given Prednisolone acetate tablets 60mg/(m2·d),oral, d1~d4 + Melphalan tablets, 8mg/(m2·d),oral, d1~d4+Thalidomide 100~200mg/d, oral, bedtime;on the basis of the control group, the experimental group was treated with Bortezomib for Injection, 1.3mg/(m2·d), intravenous injection, d1、d4、d8、d11.Before and after treatment, compared between the two groups of patients with the ratio of bone marrow plasma cells, serum monoclonicity Immunoglobulin A (IgA), monoclonicity immunoglobulin G (IgG) levels, efficiency of treatment and safety.ResultsAfter treatment, compared with the control group, the plasma cell ratio, the serum levels of IgA and IgG were lower in the experimental group (P<0.05);the treatment efficiency of the experimental group (54.55%) was significantly higher than that of the control group (11.11%), the results were statistically significant.ConclusionThe Bortezomib can significantly reduce the ratio of bone marrow plasma cells and the serum levels of monoclonicity IgA、IgG in patients with secondary plasma cell leukemia, improve clinical efficacy, and the safety was high.

OBJECTIVE To investigate the distribution of pathogenic bacteria in blood culture and their drug-sensitivity in our hospital in the past year.METHODS The blood samples were cultivated in the BacT/Alert 3D system and the isolated colonies were identified by the VITEK32 system or traditional routine procedures. The K-B method was carried out for the drug-sensitivity festing.RESULTS From the 716 blood samples received,we identified 109 strains of pathogens in which Gram-positive cocci accounted for 50.46%,Gram-negative bacilli for 44.04% and fungi for 5.5%.CONCLUSIONS There are many kinds of pathogens identified from blood culture and they have high resistance to many antibiotices. It is very important for doctors to get the results of blood culture in time in order to give the patients correct antibiotic therapy and to improve the cure rate and to control cross-infection in hospials.

Pulmonary drug delivery has become a research focus in recent years, while the action between particles and pulmonary macrophages after particles transportation to lung tissue was paid little attention. Therapeutic efficacy can be enhanced by regulating the particles uptake action of pulmonary macrophages according to different diseases. Referring to a lot of articles, the relationship between common diseases and macrophages was reviewed and the influencing factors in the particles uptake of alveolar macrophages were sum-marized. The review laid foundation for the development of preparations and clinical application of pulmonary drug delivery systems.

Vaginal delivery system has a unique therapeutic advantage. In recent years, the rapid development of dosage forms shows a good prospect of development. The related literatures at home and abroad in recent years were analyzed and summarized. The research progress in the drug absorption model of various vaginal administrations was reviewed. The review provided reference for the research of new drug preparations and drug absorption mechanisms of vaginal administration.

OBJECTIVE:
To analyze the clinical characteristics and genetic variant of a child featuring X-linked mental retardation.
METHODS:
Whole exome sequencing and Sanger sequencing were used for the detection of variant and pedigree validation, respectively. Clinical manifestation of patients with DDX3X gene variants were also reviewed.
RESULTS:
The child was found to harbor a heterozygous NM_001193416.3: c.1332_1333delCT (p.Leu445Serfs*19) variant of the DDX3X gene. The same variant was not found in either of her parents.
CONCLUSION
The child was diagnosed with X-linked mental retardation due to variant of the DDX3X gene. Above finding has enriched the spectrum of DDX3X gene variants and provided a basis for clinical diagnosis and prenatal diagnosis for this pedigrees.
