1.Fluorescence in situ hybridization and immunohistochemistry for detecting HER-2 gene amplification and protein expression in breast cancer tissue

Objective To analyze and compare the fluorescence in situ hybridization(FISH) and immunohistochemical(IHC) for detecting HER-2 gene amplification and protein expression in breast cancer tissues .Methods 110 cases of breast cancer from Janu-ary 2008 to May 2012 receiving the modified radical mastectomy were selected .The resected breast cancer tissue was detected by FISH and IHC and the detected results were performed the comparative analysis .Results Among 110 cases of breast cancer tissue , 25 cases(22 .73% ) were the HER-2 protein expression(+ + + ) ,44 cases(40 .00% ) were(+ + ) ,26 cases(23 .64% ) were(+ ) and 15 cases(13 .64% ) were(-) .Among 110 cases ,the gene amplification was in 28 cases(25 .45% ) and no gene amplification was in 82 cases(74 .55% ) .The positive(+ + + ) of the IHC detection was coincident with that of FISH ,and the negative(+ /-) of the IHC detection was also coincident with that of FISH ,there was statistical difference between the suspicious positive of the IHC de-tection and the results of FISH (P<0 .05) .But the total coincidence of the IHC detection results and FISH test results was 89 .29%(25/28) ,and the two detection methods had the positive correlation (χ2 =84 .89 ,P<0 .01) .Conclusion The positive and negative expression of the IHC detection has better consistency with that of the FISH detection .However ,the coincidence of the IHC suspi-cious positive expression and the FISH results is poor ,indicating that the suspicious positive sample of the IHC detection needs to be detected by the FISH detection .
2.Study of the efficacy and toxicity of gemcitabine combined with nedaplatin in anthracycline and/or taxane -pretreated patients with metastatic triple -negative breast cancer

Objective To evaluate the short -term efficacy and adverse reactions of gemcitabin combined with nedaplatin to patients with metastatic triple -negative breast cancer after taxane and/or anthracycline treatment. Methods Thirty -three patients with anthracycline and/or taxane -resistant metastatic triple -negative breast cancer received gemcitabin and nedaplatin regimen:gemcitabin 1 000 mg/m2,d1,8 days,intravenous;nedaplatin 80mg/m2,d1,intravenous.Treatments were repeated every 21 days and response rate was evaluated every two cycles. Results Of 33 patients, complete remission ( CR) in 1 case ( 3.03%), partial remission ( PR) in 13 cases (39.39%),stable disease(SD) in 11 cases(33.33%),progressive disease(PD) in 8 cases(24.24%),objective response rate was 42.42%.The main adverse reaction was bone marrow suppression and gastrointestinal reaction. Conclusion Combination therapy of gemcitabin and nedaplatin is an effective and well tolerated rescue regimen in anthracycline and/or taxane -resistant metastatic triple -negative breast cancer patients.
3.Effect of type 2 diabetes on middle cerebral artery occlusion-induced focal cerebral ischemia

Objective To study the function of vascular endothelial growth factor (VEGF) in type 2 diabetes model rats and its effect on focal cerebral ischemia induced by middle cerebral artery occlusion in these rats. Methods Focal cerebral ischemia was induced by middle cerebral artery occlusion for 6 hours in type 2 diabetes rats and normal control rats.Blood vessels morphology was examined by ink perfusion,infarct size was measured by TTC and expression of VEGF and CD34 were evaluated by immunohistochemistry staining. Results Ink perfusion revealed increased number of small vessels in type 2 diabetes rats. Infarct size was significantly smaller in type 2 diabetes rats ( ( 80. 07 ± 11.21 ) mm3 ) than that in normal controls ((98. 91 ± 14. 86) mm3,t = 2.48,P = 0. 0326). There were more hemorrhage lesions in the ischemic hemisphere in type 2 diabetes rats when comparing with the controls. VEGF and CD34 showed significantly higher expression in type 2 diabetes rats than in normal controls. Conclusions High expression of VEGF and CD34 are found in type 2 diabetes rats after middle cerebral artery occlusion. There is cerebrolvascular remodeling in diabetes rats. While this diabetes-induced remodeling appears to prevent infarct expansion,the changes also increase the risk of hemorrhagic transformation. The latter may result in poor prognosis.
4.Effects of Panax Notoginseng Saponins on Expression of ZO-1 of Brain Microvascular Endothelial Cells Caused by Aβ42

Objective To study the effects of Panax Notoginseng Saponins (PNS) on the damaged expression of ZO-1 of brain microvascular endothelial cells caused by Aβ42.MethodsBrain microvascular endothelial cells were divided into normal control group, model group, PNS low-, medium- and high concentration groups. They were incubated for 24h in 5% CO2 incubator at 37℃ . Then cell vitality was detected by MTT colorimetric method and ZO-1 protein expression was tested by Western blot.Results Stimulation of Aβ42 reduced the activity of microvascular endothelial cells (P<0.01) and suppressed the expression of ZO-1 protein (P<0.01). Compared with the model group, the activity of microvascular endothelial cells of PNS groups, especially the high and medium dose groups (P<0.05), and increased the ZO-1 protein expression. Conclusion PNS can partly recover the barrier function of blood brain barrier through inhibiting the decrease of the activity of microvascular endothelial cells caused by Aβ42.
5.Relationship between expression of Her-2 gene and the transfer number of axillary lymph node and its influence on prognosis

Objective To investigate the relationship between expression of Her-2 gene and the transfer number of axillary lymph node and its influence on prognosis.Methods A total of 351 cases with primary breast cancer from January 2008 to January 2011 were selected.The expression of Her-2 gene was detected by immunohistochemical method,and analyzed the relationship between it and the transfer number of axillary lymph node and its influence on prognosis.Results The expression of Her-2-,+,++ had no correlation with the transfer number of axillary lymph node (x2 =3.757,1.650,2.379,P > 0.05),while the expression of Her-2 +++ had correlation with the transfer number of axillary lymph node (x2 =8.681,P < 0.05).The 2 years survival rates in the expression of Her-2-,+,++,+++ were 77.01% (201/261),85.00% (34/40),29.17%(7/24),1 1.54% (3/26),and which in the expression of Her-2--+ was significantly higher than that in the expression of Her-2 ++-+++ (x2 =61.605,P < 0.01).The transfer number of axillary lymph node was 0 and 1-3,the 2 years survival rate in the expression of Her-2--+ was significantly higher than that in the expression of Her-2 ++-+++,and there was significant difference (x2 =61.605,14.747,P < 0.05).The transfer number of axillary lymph node was 4-9 and ≥ 10,there was no significant difference in the 2 years survival rate between the expression of Her-2--+ and Her-2 ++-+++ (x2 =3.691,3.482,P > 0.05).Conclusions The expression of Her-2-,+,++ has no correlation with axillary lymph node metastasis,and the 2 years survival rate in the expression of Her-2--+ is higher than that in the expression of Her-2 ++-+++,while Her-2-has no difference with Her-2 + in prognosis.While the transfer number of axillary lymph node ≤ 3,the expression of Her-2 gene may be an important prognostic factor.
6.Expression and significance of SNAT1 protein in brain glioma tissue

Objective To explore the expression of human sodium coupled neutral amino acid transporter 1 (SNAT1) in human glioma tissues and its relationship with clinical pathological parameters and prognosis. Methods Immu?nohistochemical and western blotting were used to detect SNAT1 expression in glioma tissue and tumor peripheral tissue from 89 cases of glioma patients including 55 cases of low grade gliomas (WHO I-II), and 34 cases of high grade gliomas (WHO grade III-IV).χ2 test and was used to analyze the relationship between expression and clinical pathological param?eters of SNAT1. Kaplan-Meier method was used to analyze the effect of different expression of SNAT1 on the prognosis of patients and to establish the Cox regression model. Results The expression of SNAT1 was significantly higher in gliomas than in tumor peripheral tissue (t=-9.803, P=0.001). The expression of SNAT1 was significantly higher in high pathologi?cal grade tissues than in low grade of glioma tissues (t=-6.682, P=0.003). SNAT1 expression was associated with tumor di?ameter and pathological grade (χ2=4.963, 8.527, P<0.05);Cox regression model showed that the tumor pathological grade and different SNAT1 protein expression were independent risk factors for the prognosis of patients with glioma. Conclu?sions The expression of SNAT1 protein is closely associated with the pathological grade of gliomas and the prognosis of the patients, which may be a new target to judge the biological characteristics and to evaluate the prognosis of gliomas.
7.The application of "internet +" disease management based on the AISAS model on young and middle-aged patients after PCI therapy

Objective:To investigate the effect of the application of "internet +" disease management based on the AISAS model on the young and middle-aged patients after PCI therapy.Method:A total of 90 young and middle-aged patients hospitalized in cardiological department of the First Affiliated Hospital of University of Science and Technology of China (Anhui Provincial Hospital) were enrolled between June 2018 to October 2019, and they were randomly divided into observation group and control group with 45 cases in each group. Patients in the control group received telephone follow-up while "internet +" disease management based on the AISAS model was applied in the observation group. The drug therapy compliance, acquisition of the disease knowledge, quality of life, anxiety and depression levels, return to workand incidence of cardiovascular events.Results:One year after the intervention, the drug therapy compliance score, disease-related knowledge score, quality of life score, SAS and SDS scores of the observation group were 7.55±1.21, 7.29±1.27, 701.17±74.86, 32.55± 4.31, 34.74±4.16, the scores of patients in the control group were 6.48±1.56, 6.12±1.94, 670.58±65.29, 41.72±4.33, 40.79±4.17. The difference was statistically significant ( t value was 2.066-11.203, P<0.05). The comparison between the return of the patients and the incidence of cardiovascular events in the two groups was statistically significant ( χ2 value was 5.031, 11.275, P<0.05). Conclusion:This management model can increase the knowledge of disease PCI postoperative patients, improve their quality of life, make patients return to society earlier, and promote the improvement of the quality of continuous nursing service.
8.Analysis of influenza vaccination status of primary and secondary school students in Shijingshan District of Beijing from 2019 to 2023

Objective:
To provide scientific support for the prevention and control of influenza epidemic and the vaccination of influenza vaccine for primary and secondary school students by analyzing the influenza vaccination situation in Shijingshan District, Beijing from 2019 to 2023.
Methods:
To make a data analysis collected from Beijing Immunization Planning Information Management System by descriptive epidemiological methods and Chi square test method.
Results:
There were totally 71 230 primary and secondary school students taking influenza vaccine in Shijingshan District from 2019 to 2023, within which the vaccination rate ranged from 62.24% to 77.64%, with an average annual vaccination rate of 69.36%. There was a statistically significant difference ( χ 2=926.39, P <0.05) between the vaccination rates in each year. The vaccination ratio didn t vary by sex( 69.16 %, 69.59%)( χ 2=0.40, P > 0.05 ). The average vaccination rate of students in primary,middle and high school were respectively 80.86%, 47.09%, 43.88% with a statistically significant difference( χ 2=610.36,2 113.30,6 720.88,12 886.79, P <0.05). The average vaccination rate of students in west area, middle area and east area, showing an increasing trend, were respectively 81.88%, 70.01%, 56.63% with a statistically significant difference( χ 2=3 679.19, P <0.05). In sum, there were significant differences in vaccination rate for both different years and areas( χ 2=570.25, 987.18, 99.60, 1 111.99, P <0.05).
Conclusion
The annual vaccination rate of influenza vaccine in primary and secondary schools in Shijingshan District from 2019 to 2023 has met the requirements for effective vaccination rates, but it should be recognized that not all schools and their students form the immune barrier. It is necessary to further strengthen publicity and education to guide students and parents to form a correct understanding, so as to reduce the occurrence of influenza epidemic.
9.Concomitant whole brain radiotherapy and FUDR+VM-26+DDP chemotherapy in brain metastasis of non-small cell lung cancer: a report of short term efficacy.

BACKGROUNDTo evaluate the efficacy and toxicity of concomitant chemoradiotherapy in patients with brain metastases from non-small cell lung cancer (NSCLC).
METHODSThirty patients suffering from NSCLC with brain metastasis were prospectively included in this study. Twenty-four patients had neurological symptoms and an ECOG performance index between 0 and 3. Treatment consisted of concomitant whole brain radiotherapy (WBRT) with a dose of 30 Gy in 15 fractions, followed by a local boosted dose of 20 Gy in 10 fractions for those that the number of the remained lesions were less than 3, or by WBRT with a total dose of 50 Gy for those that the number of the remained lesions were more than 3. Concomitant chemotherapy of FVP regimen with floxuridine 600 mg/(m²*d), teniposide 60 mg/(m²*d), cisplatin 20 mg/(m²*d) on d1 to d5,repeating every 3 or 4 weeks. The response was evaluated by brain CT or MRI after WBRT and 2 cycles of chemotherapy being completed.
RESULTSAll the patients completed WBRT and concomitant chemotherapy including 68 cycles (2 to 4 cycles for each patient). The follow-up rate was 93.3% with a median survival duration of 11.3 months. Total response rate was 46.7%, with CR for 2 and PR for 12. Specific evaluation of brain response demonstrated CR for 8 patients, and PR for 10 patients (the objective brain response rate, 60.0% ). The objective primary disease response rate was 18% for 22 cases of previously untreated primary NSCLC. Other specific evaluation of metastases included 1 PR patient in 6 patients with lung metastases, 3 CR patients and 4 PR patients in 17 patients with lymph node metastases, 1 PR patient with liver metastases, and 1 PR patient with eye metastasis. Twenty four patients with neurological symptoms benefited improvements to different extent. The main adverse effects were myelotoxicity, nausea/vomiting, constipation and alopecia. Grade III and IV toxicities were observed as following: leucopenia (19.1%), anemia (10.3%), thrombocytopenia (7.4%), nausea/vomiting (4.4%), diarrhea (2.9%), alopecia (5.9%), glutamio oxaloacetic transaminase (GOT) and glutamio pyruvic transaminase (GPT) elevation (1.5%). Dehydration therapy was needed at 2 weeks after WBRT in all patients.
CONCLUSIONSConcomitant WBRT plus FUDR+VM-26+DDP chemotherapy is tolerable in NSCLC patients with brain metastases and the short term response is comparable to the results of others.
10.Retrograde recanalization technique for TASC Ⅱ C/D femoropopliteal arteriosclerosis obliterans

Objective To explore the value of retrograde recanalization technique in treatment of TASCⅡ C/D femoropopliteal arteriosclerosis obliterans.Methods Totally 36 patients with TASCⅡ C/D femoropopliteal arteriosclerosis obliterans were retrospectively analyzed.Preoperative and postoperative ankle brachial index (ABI) and intermittent claudication were compared,as well as postoperative vascular patency rate and complications were analyzed.Results The retrograde recanalization technique were successfully performed in all 36 patients.The walking distance and ABI of 7 days,and 3,6,12 months postoperation were increased significantly compared with preoperation (all P<0.05).The postoperative vascular patency rate of 3,6 and 12 months was 97.22%,91.67% and 72.22%,respectively.There was no death nor amputation during the follow-up period,and the incidence of perioperative complications was 11.11 % (4/36).Conclusion Retrograde recanalization technique can be used to treat TASC Ⅱ C/D femoropopliteal arteriosclerosis obliterans when anterograde access failed.
