1.Applicated effect of systematic nursing intervention in patients with severe pneumonia

Objective To discuss the applicated value of systematic nursing intervention in patients with severe pneumonia.Methods 100 patients with severe pneumonia were randomly divided into the study group and the control group,50 cases in each group.The control group was given routine nursing measures.The study group received systematic nursing intervention on the basis of routine nursing measures.The hospitalization time,satisfaction degree, treatment compliance and mentality were observed.Results The SAS scores of the study group before and after nurs-ing were (56.67 ±5.99)points and (38.92 ±5.31 )points,there was statistically significant differences between before and after treatment (t =9.235,P <0.05).The SDS scores of the study group before and after nursing were (62.58 ±6.14)points and (41.15 ±5.26 )points,there was statistically significant difference (t =8.623, P <0.05 ).The hospitalization time ,satisfaction degree and treatment compliance of the study group were (32.9 ± 6.6)days,(8.21 ±1.29)days,6.00%,86.00%,74.00%,which were significantly lower than (45.1 ±10.2)days, (13.56 ±1.77)days,16.00%,52.00%,48.00% of the control group (t =5.254,3.829,χ2 =5.236,8.292, 8.1561,all P <0.05 ).Conclusion Hsystematic nursing intervention can significantly improve the postoperative anxiety and depression,and improve the quality of life of patients with severe pneumonia.
2.Use the technique of three layers suspension to lift the mid-low face

Objective To explore an ideal surgical method for mid-low face lifting.Methods In this procedure,the first surgical step was subcutaneous undermining that was performed through the incision along temporal hairline,anteriorear,and mastoideae.It extended distally beyond the furthest skin fold and along the mandibular border or the midline of the neck if nacessary.Then three layers suspension were performed,which included three procedures:first,SMAS was purse string sutured in certain direction;secondly,the malar fat pad was suspended to tow directions backward and upward and anchored in the periosteum of zygoma and SMAS too;lastly,through traction suture of reducing tension step-by-step,lots of the tension was relieved from the incision;at the same time,new retaining ligaments between flap and SMAS were reestablished.And redundant skin was removed in incision at the end of the procedure.Results A total of 22 cases received this operation.16 cases were followed up from 3 months to 24 months after operation.The results of operation were divided into three degrees:excellent,acceptable and poor;and those were evaluated by patients themselves.14 cases obtained excellent result (87.5 %),2 cases regarded that their results were acceptable (12.5 %).The total satisfactory rate was 100 %.None serious complication occurred in those eases except some slight complications,such as local edema,skin numbness and small hematoma which relieved in 3 to 6 months after operation.Scar was inconspicuous.Conclusions This technique can correct the posies of lower tow-third face better,especially for mitigating the nasolabial fold and enhancing the elastic of face skin.Simultaneously,it is an effective,safe and simple technique.
3.Controlled randomized trial on therapeutic effects of Biqi Capsule and VitB combined with FENG'S manipulation(FSM) on lubar intervertebral disc protrusion mainly with paresthesia of skin

Objective:To evaluate the therapeutic effects of capsule of Biqi Capsule and VitB on lubar intervertebral disc protrusion(LIDP).Methods:78 Patients with LIDP were divided randomly into 2 groups:treatment and control group.39 Patients in treatment group were treated with Biqi Capsule and FENG's bone-setting manipulation,39 patients in control group were treated with vitamine B and FENG's bone-setting manipulation.All patients were followed up for 1 month after treatment for 2 weeks and therapeutic effects were evaluated.Results:In treatment group,24 patients were cured,6 patients had remarkable effects,5 effective,4 ineffective.In control group,the data were 16,8,8,7 correspondly.The therapeutic effects in treatment group were obviously better than those of control group(P
4.Effect of a novel oncolytic herpes simplex virus type Ⅱ on lung adenocarcinoma

Objective To establish the subcutaneous transplantation tumor models with Lewis lung adenocarcinoma in C57BL/6 mice, and to observe the influence of oHSV2, DDP and drug combination on tumor volume, median survival time and weight of tumor-burdened mice.Methods Subcutaneous transplantation tumor models were established with Lewis lung adenocarcinoma in tumor-burdened mice.Tumor-burdened mice were randomly divided into the control group, oHSV2 group, DDP group, oHSV2/DDP sequential group, DDP/oHSV2 sequential group and oHSV2+DDP combination group with 12 rats in each group using the random number table method.The tumor size and weight of mice were measured every 3 days.Results On the 21st day, the tumor size of tumor-burdened mice in every group was as follows: control group (1.82±0.06)cm3, oHSV2 group (0.63±0.05)cm3, DDP group (0.58±0.03)cm3, oHSV2/DDP sequential group (0.49±0.05)cm3, DDP/oHSV2 sequential group (0.42±0.04)cm3, and the difference was statistically significant (F=1 359.01, P=0.000).The data in oHSV2+DDP group were put away because of premature death in mice.The differences were statistically significant between control group and oHSV2 group (P=0.000), control group and DDP group (P=0.000), control group and oHSV2/DDP sequential group (P=0.000), control group and DDP/oHSV2 sequential group (P=0.000), oHSV2 group and DDP group (P=0.017), DDP group and DDP/oHSV2 sequential group (P=0.000), oHSV2/DDP sequential group and DDP/oHSV2 sequential group (P=0.001).The weight of tumor-burdened mice in every group was listed as follows: control group (21.64±0.40)g, oHSV2 group (21.34±0.37)g, DDP group (15.96±0.43)g, oHSV2/DDP sequential group (19.04±0.31)g, DDP/oHSV2 sequential group (16.34±0.30)g, and the difference was statistically significant (F=588.67, P=0.000).The difference was not statistically significant between control group and oHSV2 group (P=0.076).However, the differences were statistically significant between control group and DDP group (P=0.000), control group and oHSV2/DDP sequential group (P=0.000), control group and DDP/oHSV2 sequential group (P=0.000), oHSV2 group and DDP group (P=0.000), oHSV2 group and oHSV2/DDP sequential group (P=0.000), DDP group and DDP/oHSV2 group (P=0.013), oHSV2/DDP sequential group and DDP/oHSV2 sequential group (P=0.000).The median survival time of tumor-burdened mice in every group was displayed as follows: control group 23 d , oHSV2 group 32 d, DDP group 30 d, oHSV2/DDP sequential group 37 d, DDP/oHSV2 sequential group 39 d, oHSV2+DDP combination group 16 d, and the difference was statistically significant (χ2=120.81, P=0.000).The differences were statistically significant between control group and oHSV2 group (χ2=10.88, P=0.001), control group and DDP group (χ2=10.69, P=0.001), oHSV2 group and DDP/oHSV2 sequential group (χ2=10.09, P=0.001), DDP group and DDP/oHSV2 sequential group (χ2=9.67, P=0.002).However, the differences were not statistically significant between oHSV2 group and DDP group (χ2=0.00, P=0.996), oHSV2/DDP sequential group and DDP/oHSV2 sequential group (χ2=2.70, P=0.100).Conclusion On the premise of that the weight of mice is no affected, oHSV2 can inhibit the tumor size and prolong the median survival time of tumor-burdened mice effectively, and the effect of DDP/oHSV2 sequential group is the most significant.This article provides an experimental basis for exploring therapeutic methods of lung adenocarcinoma.
5.Shikonin induce HeLa cell apoptosis via a caspase-dependent mechanism

AIM To study the mechanisms of shikonin-induced HeLa cell apoptosis. METHODS Cytotoxicity assay by MTT, DNA fragmentation assay, LDH activity-based cytotoxicity assay and Western blot analysis were carried out. RESULTS Shikonin inhibited HeLa cell growth in a dose- and time-dependent manner. Apoptotic cell death was observed within 24 h after 40 ?mol?L -1 shikonin treatment. Shikonin induced HeLa cell apoptosis involved in the activation of caspase-3 and caspase-8, which is similar to the Fas or TNF-? signaling transduction pathways. CONCLUSION Shikonin induced caspase-dependent apoptosis in HeLa cells.
6.Study on correlation between ankylosing spondylitis and HLA -B27 subgenotype

Objective To explore the correlation between different HLA‐B27 subgenotype and ankylosing spondylitis (AS ) . Methods The whole venous blood was collected from the outpatients and inpatients of the orthopedics ,acupuncture and rheuma‐tism departments and HLA‐B27 was qualitatively detected by using the gene analysis method .Among them ,380 cases of AS were HLA‐B27 positive ,and 50 cases of HLA‐B27 positive were selected as the healthy control group .Then the HLA‐B27 subgenotypes were detected by using the sequence specific primers PCR (PCR‐SSP) technology .Results Among 380 cases of AS ,217 cases (57 .1% ) of HLA‐AS B2704 subgenotype ,143 cases (37 .6% ) of HLA‐B2705 subgenotype ,11 cases (2 .9% ) of HLA‐B2707 sub‐genotype and 9 cases (2 .4% ) of HLA‐B2711 subgenotype were detected out ;among 50 cases of HLA‐B27 positive in the healthy control group ,23 cases (46 .0% ) of HLA‐B2706 subgenotype ,21 cases (42 .0% ) of HLA‐B2709 subgenotype ,4 cases (8 .0% ) of HLA B2704 subgenotype and 2 cases (4 .0% ) of HLA‐B2705 subgenotype were detected out ,the differences between the two groups were statistically significant (P=0 .002) .Conclusion The subgenotypes of HLA‐B27 among the AS patients in Baoji area are dominated by the genotype B2704 and B2705 ,which is strongly correlated with the occurrence of AS among Han population in Baoji area ;B2706 and B2709 are the protective subgenotypes in this area .
7.Mechanical principle of the respiration-driven variation of cardiac function——Echocardiographic study of the mechanical models

Objective To reveal the exact mechanical principle of the respiration-driven variation of cardiac function by observing the effect of respiratory intrathoracic pressure change (RIPC) on the mechanical models using echocardiography.Methods Model 1 was designed to observe the influence of RIPC on systemic and pulmonary venous return systems (SVR and PVR) respectively.Model 2,as an equivalent mechanical model of septal swing,was used to study the influence of RIPC on the motion of the interventricular septum (IVS).Results Model 1 demonstrated that the simulated RIPC had totally different influences on the simulated SVR and PVR.It increased the volume of the simulated right ventricle when the internal pressure was kept constant (8.16 cm H2O),while it had the opposite effect on PVR.Model 2demonstrated that there was a corresponding relationship between RIPC and the position of the simulated IVS which might be called pressure-position relationship.Conclusions The different anatomical arrangement of the two venous return systems leads to a different effect of RIPC on right and left ventricles,and thus generates a pressure gradient across IVS that tends to shift IVS left-and right-wards with respiration.The swing of the IVS changes the short axis diameters of the ventricles,thus their fillings and then their functions reciprocally.
8.Radiotherapy standard and progress in locally advanced rectal cancer.

Recently, treatment strategy optimization for neoadjuvant therapy of rectal cancer includes two aspects: (1) Increasing treatment intensity may improve pathological complete response rate, including increasing radiation dose or concurrent chemotherapy intensity, or shifting adjuvant chemotherapy; (2) Short-course radiotherapy or neoadjuvant chemotherapy which can promise treatment efficacy will decrease toxicity and lead to better tolerance. Long-course chemoradiotherapy is the recent treatment standard for locally advanced rectal cancer. NCCN guidelines do not recommend combined chemotherapy in the radiotherapy period. However, it is important for individualized treatment of rectal cancer if appropriate patients who may benefit from the combined concurrent chemotherapy can be selected. Short-course radiotherapy is defined as 5 Gy × 5. It is recommended for T3 or N+ rectal cancer in NCCN guidelines, but not for T4 patients. In ESMO guidelines, stratified patients of intermediate risk by MRI can be treated with either short-course or long-course radiotherapy, but short-course radiotherapy is not recommended for T4 or positive mesorectum fascia (MRF+) patients with high risk. Neoadjuvant chemotherapy incorporated in the neoadjuvant part has been a therapeutic choice in NCCN guidelines. However, It is still unclear whether chemotherapy upfront as a component of neoadjuvant treatment or even completion of chemotherapy before surgery can improve treatment outcome or not. There are phase II( studies focused on this issue and final results are pending.

10.Study on fetal pulmonary venous Doppler spectrum in cardiorespiratory function

ObjectiveTo explore the characteristics of the fetal pulmonary venous Doppler spectrum in medium-term and terminal pregnancy and to evaluate its application in the cardiac function assessment and lung maturation.Methods The study group consisted of 86 normal fetuses aged from 23rd to 41st week's gestation.The Doppler echocardiography was used to record the pulmonary venous spectra.The peak diastolic velocity (D),deceleration time of D wave,deceleration of D wave (Ddec),the peak systolic velocity (S),acceleration time of S wave,and acceleration of S wave of the right superior pulmonary vein (Sac.c) were measured.Sacc/Ddec was calculated.The Tei index of the left ventricle was calculated.ResultsThe D and deceleration time of D wave and Ddec increased with gestational age (P < 0.05).S,the acceleration time of S wave and Sacc showed no significant correlation with gestational age.Sacc/Ddec ratio was more likely smaller than 1 with the gestational age less than 28 weeks and larger than 1 with the gestational age longer than 28 weeks ( P <0.05).ConclusionsD wave flow velocity,deceleration time of D wave and the Ddec of the right superior pulmonary vein had a good correlation with the global left ventricular function.The Sac/Ddec ratio might be new index for assessing cardiac function and lung maturation.
