1.Research progress of antiemetics in tumor therapy

Chemotherapy plays a significant role in cancer therapy,but chemotherapy-induced nausea and vomiting(CINV)common happened and affects patients satisfaction with treatment.With the development ot physiology on emesis,the new antiemetics such as 5-HT3 receptor antagonist(5-HT3RA),Aprepitant and Olanzapine greatly improved the control of CINV,the guidelines for antiemeties were deftned.Research progress of antiemetics was summarized.
2.Risk Factors of HBV Intrauterine Transmission

【Objective】 To study the risk factors of Hepatitis B Virus intrauterine transmission. 【Method】 Cordal blood from 87 HBV carrier's newborns was tested for HBV markers and PCR-HBV-DNA. Maternal venous blood po stpartum was tested for HBV markers. 【Results】 HBV-DNA positive rate of corda l blood is 1.2%(1/86). Odds Ratio(OR) of positive maternal HBsAb, HBeAg, HBeAb and HBcAb for intrauterine infection are 0.697(0.586~0.786), 50.792(13.082 ~197.204), 0.103(0.032~0.338) and 2.261(0.587~8.712), respectively. Co rdal HBsAg positive rate of maternal HBeAg positive group was significantly high er than that of only HBsAg positive group (31.0% vs 3.92%), so was cordal HBeA g positive rate (72.4% vs 3.92%). Maternal HBeAg was significantly correla ted with cordal HBsAg, especially HBeAg. Newborn sex wasn′t correlated with intraut erine infection. 【Conclusion】 Positive maternal HBeAg was one risk factor of i ntrauterine HBV infection. Newborn sex wasn′t correlated with intrauterine infe ction.
3.Gastrointestinal Hormones and Oxygen Free Radicals in IBS Rabbits of Spleen Deficiency

[Objective] To observe the effect of Leweiyin of traditional Chinese compounds prescription on gastrointestinal hormones and oxygen free radicals in IBS rabbits of spleen deficiency,and to explore the possible mechanism for the treatment of IBS with spleen deficiency.[Methods] 36 two-month-old rabbits with white feather and black eyes(WHBE rabbit),half male and half female,were randomly divided into 6 groups:normal group,model group,Xiangsha Liujun pill group,Lizhu Changle group,low-dose Leweiyin-treated group,high-dose Leweiyin-treated group.Except the normal group,others were made into IBS model of spleen deficiency which was induced by damp heat stress and gavage with Fanxieye as well as method of either hunger or gorge.The drug groups were administered preventively from the medium-term of the model reproduction to the end of modeling.The rabbits of each group were observed.NO,NOS,MDA,SOD in serum and SP in colonic membrane were detected.[Results] NO,NOS,MDA in serum and SP in colonic membrane significantly more increased than the normal group(P
4.One-stage surgical management and allograft in the treatment of multisegments spinal tuberculosis

[Objective]To assess the efficacy of one-stage anterior radical debridement,decompression,and fusion with anterior and/or posterior spinal instrumentation in the treatment of multisegments spinal tuberculosis.[Method]From 2001 and 2005,a total of 15 patients included thoracic,thoracolumbar and lumbar spinal tuberculosis underwent anterior debridement,allograft bone fusion,and stabilization with anterior and posterior instrumentation.The mini-invasive technique including thoracoscope and X-tube were applied in the later 8 cases.[Result]An average of 2.1 years' follow-up was achieved in 16 patients.The overall fusion rate was 100% in the dynamic lateral views.There was no graft problems.The kyphosis angles that were corrected postoperatively averaged 19.9?.All patients' neurological function were improved except that there was no change in 1 patient with Frankel grade B preoperatively.[Conclusion]Reconstructing the stability of spine is very important for the treatment of spinal tuberculosis.The application of internal instrument and allograft,choosing appropriate surgery can greatly improve the clinical results.Mini-invasive technique reduced blood lost and made the operation easier.
5.Extreme lateral transforaminal lumbar interbody fusion (E-TLIF) designed via digital technology

ObjectiveTo design a new operation approach:extreme lateral transforaminal lumbar interbody fusion (E-TLIF) via digital technology and to discuss its feasibility in treatment of degenerative lumbar diseases.MethodsCT scan image data of lumbar vertebra were obtained from volunteer (a healthy male of 26 years old,67 kg in weight and 172 cm in height).Mimics was used to read and reconstruct the data into 3D images.We observe the anatomical bone structures of intervertebral foramen and nerve roots through CT scan and 3D reconstruction in Mimics to analysis the feature of this area.Then E-TLIF operation,pedicle screw insertion and cage placement were simulated by computer via digital technology in Mimics.The feasibility of this new approach was proved through operating on lumbar of formalin doused body with E-TLIF approach.ResultsAn incision of 9 cm lateral form the central line and 45° from the sagittal plane is operated to fully expose the foraminal area.Remove superior facet while save inferior facet is feasible with this approach in E-TLIF.There is enough room for fully revealing and excision of intervertebral disc,and cage placement through enlarged foraminal area.Digital technology is a more economic,efficient way to design a new operation approach.The results are in accord with the formalin doused body test.ConclusionMimics is an effectively,reliable and economic method to design an operation approach.And the result suggested that E-TLIF is a safe,less minimal invasive and more efficient operation approach in treatment of degenerative lumbar diseases.
6.Relationship between expression of cydooxygenase-2, Bcl-2 and chemosensitivities in lymph node metastases of gastric carcinoma

Objective To investigate the relationship between expression of cyclooxygenase-2 (COX-2), Bcl-2 and chemosensitivities in lymph node metastases (LNMs) of gastric carcinoma. Methods The chemosenisitivities to 9 drugs were measured by MTT assay, and the expression of COX-2, Bcl-2 was determined immunohistochemically in 40 paired primary tumor (PT) and lymph node metastases(LNMs)of gastric carcinoma. Results The positive rate of COX-2 and Bcl-2 in PT were 52.5%, 45.0% respectively, and in LNMs, the positive rate were 72.5% and 60.0%. The expression of COX-2 was higher in LNMs than in PT(χ~2=4, P<0.05). There was no statistically difference in the expression of Bcl-2 between PT and LNMs(χ~2=3, P>0.05). There was positive correlation of COX-2, Bcl-2 between PT and LNMs(r=0.3403, 0.4560, beth P<0.05). There was positive correlation between COX-2 and Bcl-2 in PT and LNMs (r=0.6014, 0.5330, both P<0.01). In PT the inhibition rate for 5-FU, VCR and eADM in COX-2 high expression group were lower than those in low expression group (t=2.29, 2.18, 2.41, all P< 0.05). The inhibition rate to 5-FU, PTX and eADM was significantly lower for the Bcl-2 high expression group in PT (t=2.46, 2.23, 2.22, all P<0.05). In LNMs, there were lower inhibition rates for VCR and MTX in COX-2 strong expression group (t=2.17, 2.35, both P<0.05); the inhibition rates to 5-FU, VP-16, PTX and MTX were significantly lower for the Bcl-2 strong expression group in LNMs (t=2.32, 2.29, 2.50, 2.25, all P<0.05). Conclusions COX-2 and Bcl-2 are involved in MDR of gastric carcinoma. The LNMs of gastric carcinoma are heterogeneous with respect to expression of COX-2, Bcl-2 and response to chemosensitivities. Effective adjuvant chemotherapy in gastric carcinoma depends on targeting the metastatic component of the tumor.
7.Treatment of olecranon fracture with biodegradable tension band in 9 cases

BACKGROUND: Internal fixations with plate and screw or K-wira tension band are widely used in treating olecranon fracture; however, the internal fixations need to remove by a second surgery. OBJECTIVE: To retrospective analyze the therapeutic effect of treating olecranon fracture using biodegradable tension band. METHODS: Nine olecranon fracture patients received at the Shanghai East Hospital of Tongji University from July 2006 to April 2009 were selected. All patients were treated by biodegradable tension band fixation which comprises absorbable screw and absorbable suture. Elbow joint functional exercises were performed at 3 weeks after plaster extema! fixation. The patients were followed up for 12 months, and the therapeutic effect was measured by X-ray films and Mayo score of elbow joint function. RESULTS AND CONCLUSION: Imaging examination showed that the fracture healing well in all cases, and the fracture line was blurred. No ulnar nerve compression, fracture, or fracture displacement could be seen. Biodegradable tension band fixation can provide adequate intensity for functional exercises, which has the virtue of effective fixation without a second surgery to remove fixation. It is believed to be a good method for the treatment of olecranon fractures.
8.Effects of pulsed ultrasound and pulsed electromagnetic field on the extracellular matrix secretion of rat bone marrow mesenchymal stem cell pellets in chondrogenesis.

OBJECTIVETo study the effects of pulsed ultrasound (PUS) and pulsed electromagnetic fields (PEMF) on the secretion of extracellular matrix from a culture complex during in vitro chondrogenesis.
METHODSAll the rat bone marrow mesen- chymal stem cell pellets were cultured in achondrogenic medium. Different intensities of PUS (100, 150, and 200 mW · cm⁻²) and PEMF (1, 2, and 5 mT) were applied to the cell pellets for 2 weeks. Group N was cultured without PUS and PEMF stimu- lation as control. The culture medium was collected after 2 weeks of culture. Enzyme-linked immunosorbent assay (ELISA) was used to detect the type of collagen and glycosaminoglycan (GAG) in the culture medium.
RESULTSPUS increased the secreting-type collagen and GAG from cell pellets compared with group N (P < 0.05), whereas there was no difference in different intensities (P > 0.05). PEMF had no significant effect on the secretion of the type of collagen (P > 0.05). A PEMF of 1 mT had no significant effect on the secretion of GAG (P > 0.05). A PEMF 2 and 5 mT decreased the secretion of GAG (P < 0.05).
CONCLUSIONTo prevent the secretary of extracellular matrix may play a role in chondrogenic effect of PEMF.

9.Chitosan liquid improves wound healing in rats

Objective To study the promotive effect of chitosan liquid on wound healing.Methods Wound model was established on the back of 60 rats,which were classified into 3 groups to be treated with topical chitosan liquid (group A),basic fibroblast growth factor (group B),and sodium chloride physiological solution (group C),respectively.The time required for the healing of wound was recorded.Regenerated tissues were resected from the rats on day 3,7 and 14 after the establishment of wound model,and observed with light microscopy.Results The time required for the healing of wound was 17.3 ± 1.35 days in group A,18.2 ± 1.15 days in group B,and 24.0 ± 1.37 days in group C.For the time required for the healing of wound,no significant difference was observed between the group A and B (P > 0.05),but group C significantly differed from group A and B (both P < 0.05).Chitosan liquid accelerated the generation of capillary sprouts and vascular endothelial cells at the early stage of wound healing,promoted the production of fibroblasts and collagen fibers at the middle stage,and improved the organization of collagen fibers at the late stage.Conclusion Chitosan liquid exerts a promising promotive effect on wound healing.
10.Effect of Gegen Qinlian Decoction on Rotavirus-inducing Diarrhea in Neonatal Mice

Objective To investigate the anti-rotavirus effect of Gegen Qinlian decoction on rotavirus (RV)-inducing diarrhea neonatal mice model, and provide evidences for pharmacological and pharmacodynamic studies. Methods Neonatal diarrhea mice model of Kunming family infected with RV-Wa was developed. Symptom, diarrhea ratio and severity level, histological changes of neonatal mice’s small intestine and lung was assessed after Gegen Qinlian decoction (0.78 g/kg) treatment. Results Diarrhea ratio, diarrhea severity level, histological changes of neonatal mice’s small intestine and lung were obviously decreased after Gegen Qinlian decoction (0.78 g/kg) treatment. Conclusion Gegen Qinlian decoction has anti-rotavirus effect on RV induced diarrhea.
