Objective To explore the feasibility of a novel technique of anvil placement in combined use of laparoscopic and thoracoscopic Ivor Lewis esophagectomy. Methods From June 2011 to October 2012, we performed a novel technique of minimally invasive Ivor Lewis esophagogastrectomy in 20 patients with midthoracic esophageal cancer.The anvil was placed to connection with the esophageal stump under thoracoscopic vision, and an apical pleural stomach esophagus anastomosis was performed by using normal stapling. Results The mean operation time lasted for 200 minutes (150-300 minutes) with a mean blood loss of 180 ml (150-250 ml) without blood transfusion.The mean number of lymph nodes resected was 18 (12-44).No additional incision was required. There were no complications at post-operation and no anastomic leakage.The patient’s hospital stay lasted a mean of 10 days (8-12 days) .Follow-up reviews were conducted for 6 months and no recurrence was found in all the 20 patients. Conclusion Laparoscopic and thoracoscopic Ivor Lewis esophagectomy with novel technique of anvil placement for the treatment of esophageal carcinoma ensures the feasibility and safety.

Mycoplasma pneumoniae(MP) is the smallest organism that dose not need a host cell for replication.It is one of the most common pathogens of community acquired pneumonia in children.In recent years,the incidence of pneumonia mycoplasma pneumonia increased year by year,and severe pneumonia mycoplasma pneumonia,refractory pneumonia mycoplasma pneumonia cases also increased.MP causes a variety of clinical presentations,in addition to respiratory infections,there are also many extrapulmonary manifestations.Immune response plays a very important part in pathogenesis of MP infection.This article focused on the immune aspects in pathogenesis and extends to some diagnosis and therapeutic implications.

Objective To observe the immunohistochemical expression of lymphocyte subpopulationin in brain and spinal cord tissue,and explore the mechanism of Zuogui wan and Yougui wan on rats with experimental autoimmune encephalomyelitis(EAE).Methods Lewis rats were immunized with the myelin basic protein(MBP).120 rats were grouping randomly into normal group,EAE group,prednisone group,Zuoguiwan group and Youguiwan group after post immunization(PI).Rats in normal group and EAE group were administered normal soline,each 3 mL/d.Rats in prednisone group were administered suspension of prednisone after developed clinical signs,each 5 mg/kg.Rats in Zuoguiwan group were administered suspension of Zuoguiwan,each 2 g/kg,and rats in Youguiwan group were administered suspension of Youguiwan,each 3 g/kg.On 15th and 27th day after PI,rats were killed and the immunohistochemical staining was performed on the sections of brain and spinal cord.Results On 15th and 27th day after PI,the expression of CD4+ in brain and spinal cord tissue of EAE group were significantly higher than that in normal group(P

Objective To evaluate the applied value of CT guiding the liver cancer ablation with cluster electrode radiofrequency hyperthermia. Methods 105 cases with local lung cancer were ablated with cluster electrode radiofrequency hyperthermia by whole body CT-ELSCINT 2400ELECT. Results Success rate of 105 cases CT guiding was 98.3% and the total remission rate was 82.8%. Conclusion It is with CT that the location of percutaneous puncture can be optimally selected, and the point view and depth of the electrode can been detected anytime; also, the internal local focus can be monitored. So, the CT guiding is the optimal method to guide cluster electrode radiofrequency hyperthermia ablation for lung cancer.

Objective To study the difference between images of DR with or without equilibrated reconstruction. Methods 200 samples undergoing DR abdominal imaging from April 2003 are chosen orderly. Results All details of different tissue can be displayed in an image by adopting Equilibrated Reconstruction, while standard DR images should adjust the width and position of the window to get the same effect. Conclusion Equilibrated Reconstruction technology could improve image quality of different tissues without loss of details of other parts, and make DR more consummate and operation more convenient. It raise the ratio of displaying lesions.

Objective To study the clinical and laboratory characteristics and treatments in children with chronic myelogenous leukemia (CML). Methods Respective analyze of the clinical and laboratory data of twelve pediatric patients with CML from December 1994 to October 2009 was performed. Results The disease predominately affected children from 5 to 9 years old (50 % of the patients), with a higher prevalence in boys than girls (gender ratio: 5:1). The duration before the diagnosis was different from three days to one year. The main presenting symptoms were fever, bleeding,asthenia, and complaints related to splenomegaly.Some cases were diagnosed incidentally. Markedly raised leukocyte and platelet counts were common.Conclusion CML in children is rare, and average level of WBC count seems to be much higher than adult.

Objective:To explore the effect of glycynhizinatis on serum IgE level of rat model and patients with asthma.Methods:Trie rats were divided into four groups randomly: normal control, asthmatic model, model treated with GL, model treated with DS. The rats were sensitized and challenged with ovalbumin to establish an asthmatic model. The established models were measured by breathing curves, pathological changes in pulmonary tissue slices and the level of serum IgE. Furthermore, serum IgE level of patients with asthma was also detected. Results: High amplitude of breathing curve and fast rhythm were showed in group of asthmatic model, there were more inflammatary cells infiltration in the tiny bronchial wall, hyperplasia of cup-shaped cells and thicker smooth muscle in the pulmonary tissue slices. Compared with normal control, the level of serum IgE in model group increased obviously. The rats treated with glycyrrhizinatis showed low amplitude of breathing curve, decreasing inflammatary cells infiltration in pulmonary tissue slices of tiny bronchial wall, no hyperplasia of cup-shaped cells, and decreased serum IgE level. There were no significant difference between the groups of GL and DS. At the same time, the serum IgE level in patients with asthma was observed. Objective To evaluate the reliability and validity of Chinese version of Arthritis Self-Efficacy Scale.Methods Arthritis Self-Efficacy Scale was translated and adapted according to Chinese culture,and content validity was evaluated by panel of experts.Then preliminary test was proceeded and revised the scale.90 effective questionnaires were recollected,and 40 patients were tested once again after two weeks randomly.Correlation analysis and factor analysis was conducted.Results The Cronbach's α of Arthritis Self-Efficacy Scale was 0.920 and the Cronbach's α of each subscale was ranged from 0.807 to 0.831.The test-retest reliability was 0.800 to 0.921.The correlations among subscales were ranged from 0.705 to 0.799,the correlations between each subscale and total scale were ranged from 0.875 to 0.921.Component analysis revealed three factors accounting for 56.47％ of the sample variance.Conclusions Chinese version of Arthritis Self-Efficacy Scale is of better reliability and validity,it could be used in the evaluation of self-efficacy for arthritis patients.

Objective To investigate the effects of aloperine (ALO) on a model of dextran sulphate sodium (DSS)-induced chronic colitis in C57BL/6 mice.Methods Repeated colitis was induced by administration of four cycles of 4％ DSS.The severity of colitis was assessed on the basis of clinical signs,ratio of colon weight and colon length,and histological grading scores.Moreover,secretory immunoglobulin A (S-IgA) and plasma haptoglobin (HP) were analyzed by enzyme-linked immunosorbent assay,and the changes of mRNA expression of ICAM-1and MIF gene in colorectal tissue were detected by quantitative reverse transcriptase real-time polymerase chain reaction using SYBR Green Ⅰ.Results ALO administration significantly attenuated the colon damage,caused substantial reductions of the rise in HP,and maintained the level of cecum S-IgA.ALO inhibited the ICAM-1mRNA expression and had no effect on MIF mRNA expression.Conclusion The effect of ALO on DSS-induced chronic colitis in mice is investigated for the first time,which suggests that ALO could be an attractive therapeutic candidate in the treatment of inflammatory bowel disease.
