1.Minimally Invasive Coronary Bypass by Using Bilateral Internal Mammary Arteries Combine with Percutaneous Coronary Intervention for Multivessel Coronary Artery Disease:Case Report

[Summary] A 60-year-old man was admitted with main complaint of chest pain on exertion.Coronary angiogram showed severe triple vessel disease.He underwent the hybrid technique of treatment through two left minithoracotomy incisions.Both internal thoracic arteries were harvested under direct vision.The left internal mammary artery (LIMA)and right internal mammary artery (RIMA) compositing Y grafts was constructed. The LIMA was anastomosed to the left anterior descending branch and the RIMA was anastomosed to the ramus intermedius.The operation time was 306 min.The amount of bleeding during operation was 50 ml.Five days later he underwent percutaneous coronary intervention at right coronary artery lesion successfully.He was discharged from hospital 8 days after operation,and didn’t complain of chest pain during fellow-up for one month.This technique demonstrates to be an effective and minimally invasive alternative to treat patients with multivessel coronary artery disease.
2.ARIMA Product Season Model for Predicting Number of Inpatient and Hospitalized Expense of Malignant Tumor

Objective To explore the application of auto-regressive integrated moving average (ARIMA) product season model in predicting number of inpatient and hospitalized expense of malignant tumor,and to provide scientific basis for hospital business management.Methods We collected inpatient data of malignant tumor from January 2007 to December 2015 in one hospital for model fitting,and used monthly data 2016 to verify the effect of model prediction.We predicted the number of inpatient and hospitalized expense of malignant tumor in 2017.Results ARIMA (0,1,1) (1,1,0) 12 was the best model for number of inpatient and hospitalized expense of malignant tumor,with prediction fitting errors of 1.1% and 1.47 %,respectively.The number of inpatient and hospitalized expense of malignant tumor in 2017 were predicted to be 7631 and 0.336 billion.Conclusion ARIMA product season model can better applied in the predicting of hospital business management.

6.L-carnitine and cardiac metabolism and remodeling indices in alcoholic cardiomyopathy

Objective To observe the preventive and therapeutic effects of L-carnitine on the metabolic disorder and cardiac remodeling in alcoholic cardiomyopathy. Methods Experimental animals were divided into three groups: alcohol-fed group(A), an alcohol/L-carnitine fed group(B) and a control group(C). Free fatty acid(FFA) and earnitine were detected in the blood serum at different time. mRNA and protein expressions of peroxisome proliferator-activated receptors (PPARα and PPARγ), retinoic acid receptor retinoid X receptor alpha (RXRα), earnitine palmitoyl transferase isoform and medium-chain acyl-CoA dehydrogenase (MCAD) were observed with RT-PCR and Western blotting methods. Results (1) When group A and B were compared with group C, FFA was increased and carnitine was decreased;mRNA and protein expressions of PPARα, RXRα, CPT-Ⅰ and MCAD were decreased with the development of alcoholic cardiomyopathy (ACM), being more significantly in group A than group B (P < 0.05). (2) mRNA and protein expressions of PPARγ had no statistical significance between these three groups at the end of 2 and 4 months(P>0.05), but after 6 months, they were increased in group B and decreased in group A (A vs. C,P<0.01;B vs. C,P<0.05). Conclusion Metabolic disorder and cardiac remodeling occur in the development process of ACM; they are partly prevented by L-carnitine through downregulating mRNA and protein expressions of PPARct, RXRα, CPT-Ⅰ, MCAD and PPARγ.
7.Effect observation of float needle combined with reperfusion activity for pain induced by cyclomastopathy.

OBJECTIVETo observe the clinical effect of float needle therapy combined with reperfusion activity for pain induced by cyclomastopathy.
METHODSThirty-five female patients with cyclomastopathy were randomly divided into a comprehensive group (18 cases) and a float needle group (17 cases). In the comprehensive group, float needle manipulation was used at the centre of the biceps brachii belly or the ligature between the affected nipple and breast nodule, about 4 cm beside the exterior margin of the breast, and the reperfusion activity of the affected upper limb and breast was combined. In the float needle group, simple float needle therapy was adopted. In the two groups, treatment was started 7 ± 3 days before menstruation, once every other day. After 3-time treatment, the changes of short form McGill Pain Questionnaire (SF-MPQ) scores before and after treatment, the time of pain relieved during the first treatment, recurrence in 3 months after treatment and the adverse reaction were observed. Also, the clinical effects of the two groups were compared.
RESULTSImmediately at the end of the first treatment,after 3-time treatment and while followed up for one month, each item score and the total score of SF-MPQ were decreased apparently than the scores before treatment (all P < 0.05), and all the scores mentioned above of the comprehensive group were declined more obviously than those of the float needle group (all P < 0.05). The time of pain relieved during the first treatment of the comprehensive group was much shorter than that of the float needle group [(94.72 ± 33.67) s vs (162.06 ± 29.16) s, P < 0.01]. The recurrence rate of the comprehensive group in 3 months after treatment was 5.9% (1/17), which was better than 20.0% (3/15) of the float needle group (P < 0.01).
CONCLUSIONFloat needle therapy combined with reperfusion activity and simple float needle therapy can both safely and effectively improve cyclomastopathy, and the combination therapy is better than simple float needle therapy in the aspects of pain relieving effect at once and the long term effect.

8.Diagnostic Value of Multi-slice Spiral CT in Pulmonary Embolism

Objective To analyze spiral CT manifestations and the diagnostic value of pulmonary embolism (PE).Methods Spiral CT pulmonary angiography (SCTPA) and chest plain CT scan were performed in 25 cases with highly-suspected PE. CT findings were retrospectively analyzed.Results The direct signs of PE appeared as complete or partial filling defect within pulmonary arteriae on SCTPA.191 branches of pulmonary artery were involved in all cases,of them, 44 branches were centrally located(23.0%), 115 branches were eccentrically located (60.2%),7 branches were mural filling defect (3.7%), 25 branches were complete occlusion (13.1%).The indirect signs of PE included irregular consolidation (n=15), patchy ground glass opacities (n=6),local streak shadows(n=4),"mosaic"sign (n=5), pleural effusion(n=16) ,pericardial effusion (n=3)and simple emboli no other signs(n=3).Conclusion SCTPA is a fast ,effective, security and non-invasive diagnostic method for PE .
9.Prevalence of macrosomia and its risk factors in China: a national survey in 2011

Objective To investigate the prevalence of macrosomia in China and the risk factors.Methods By stratified cluster random sampling,the medical records of 101 723 singleton term infants born in 37 hospitals nation wide (in 14 provinces) from January 1,2011 to December 31,2011 were reviewed.Adjusted odd ratio (aOR) and 95% confidence interval (CI) were used to describe the risk factors.Population attribute risk (PAR) was used to quantitatively describe the associations between risk factors and the prevalence of macrosomia.Results The general prevalence of macrosomia was 7.3% (7403/101 723).The prevalence varied among provinces (4.1% to 13.4 %).Logistic regression analysis showed that macrosomia were positively associated with living in northern China (aOR =1.4,95 % CI:1.2 ~ 1.6,PAR =19.9 %),maternal age ≥ 35 (aOR =1.1,95% CI:0.9~1.3,PAR=8.7%),pre-pregnant BMI≥28 (aOR=3.6,95% CI:2.9~4.5,PAR=15.1%),gravidity>1 (aOR=1.2,95% CI:1.1~1.4,PAR=10.8%),parity>1 (aOR=1.8,95% CI:1.5~ 2.1,PAR=10.1%),maternal height≥160 cm (aOR =1.6,95% CI:1.4~ 1.9,PAR=29.5%),gestational weight gain (GWG) ≥16 kg (aOR=2.2,95% CI:1.9~2.4,PAR=25.2%),gestational diabetes mellitus (aOR=1.2,95% CI:1.0~ 1.3,PAR =3.2%),gestational weeks≥40(aOR=2.4,95% CI:2.1~ 2.7,PAR=34.4%)and baby boy (aOR=l.6,95% CI:1.4~1.8,PAR=25.4%).Maternal BMI,gestational week and GWG were most strongly associated with macrosomia.Conclusions The prevalence of macrosomia varied dramatically between different areas in China.Pre-pregnancy BMI,gestational week and GWG,as the main modifiable risk factors for macrosomia,need more attention from health care providers.
10.The clinical study of laparoscopic radical cystectomy for bladder cancer aftar partial cystectomy

Objective:To discuss the clinical efficacy of laparoscopic radical cystectomy in the treatment of bladder cancer after partial cystectomy.Methods:The clinical data of 30 patients who underwent laparoscopic radical cystectomy after PC in Sichuan Provincial People's Hospital from March 2016 to August 2020 were retrospectively analyzed. Including 24 males and 6 females with an average age was 62.5 (45.5-82.5)years.6 out of 30 cases underwent pelvic lymph node dissection during PC. All patients had definite pathological diagnosis for the high-grade urothelial carcinoma after PC, and the tumor staging was pT 2-3bN 0M 0.5 patients received postoperative adjuvant chemotherapy with gemcitabine and cisplatin, 6 received postoperative adjuvant radiotherapy, 13 received postoperative adjuvant radiotherapy and chemotherapy, and all patients were received maintenance intravesical instillation. Median time for local tumor recurrence after PC was 9(5-29) months, all patients had pathological diagnosis for the high-grade papillary urothelial carcinoma, cT 2-4N 0M 0 stage.The average tumor diameter was 3.5(2.5-4.5)cm, an average number of tumors was 2(1-3). Laparoscopic salvage cystectomy was performed after recurrence.General anesthesia, supine position, 5 ports were inserted through the abdominal approach. Standard pelvic lymph node dissection (PLND) was used to clean the pelvic lymph nodes. Those who had underwent PLND no longer clean the obturator and peripheral iliac vessels, but including the common iliac vessel and the bifurcation of the abdominal aorta and lymphatic tissues around the inferior vena cava, as well as the presacral lymph nodes. Results:All 30surgeries were successfully performed. The average operative time was 270(240-310)min, average estimated intraoperative blood loss was 180(50-300)ml, and there was no blood transfusion during the perioperative period.The average number of lymph nodes dissected was 18 (10-27). There were 4 cases with positive lymph nodes, of which 3 cases were positive for 2 obturator lymph nodes, and 1 case was positive for 3 obturator and external iliac lymph nodes. No serious intraoperative complications occurred.No lymphatic leakage occurred. The average drainage duration was 4(3-7) d, and postoperative hospital stays was 9(7-20)d. The postoperative pathology was invasive high-grade papillary urothelial carcinoma, and pathological TNM stage was pT 2-4aN 0-2M 0.13 patients received postoperative adjuvant chemotherapy. The average postoperative follow-up time was 23(3-31) months. There were 2 cases of pelvic recurrence and 1 case of retroperitoneal lymph node metastasis. These 3 cases received adjuvant chemotherapy and radiotherapy. Conclusions:Radical cystectomy should be the primary treatment for recurrence of bladder cancer after partial cystectomy.
