4.Changes of vascular cell adhesion molecule-1 in serum and marrow stroma cell supernatant of patients with relapsed acute leukemia

BACKGROUND: Relapse of acute leukemia is possibly correlated with abnormal expression of cellular adhesion molecule-1 (VCAM-1).OBJECTIVE: To observe the level of VCAM-1 in serum and bone marrow stromal cell (MSC) supernatant of patients with relapsed acute leukemia. METHODS: Samples of serum and MSC supernatant were collected from 17 patients with remission-phase and relapse-phase acute leukemia hospitalized in the Department of Hematology, the Fifth Affiliated Hospital of Sun Yat-sen University between June 2006 and March 2008. The levels of VCAM-1 were measured with ELISA in remission and relapse phases.RESULTS AND CONCLUSION: Compared with the remission phase, the VCAM-1 level of serum was significantly increased in the relapse phase (P < 0.05). At 1 and 3 weeks after in vitro culture, the VCAM-1 level in the MSC supernatant was not changed (P > 0.05), but it was significantly increased on the fourth week (P < 0.05). The results demonstrated that VCAM-1 expression of serum and MSC supernatant in the relapse phase was greater than that in the remission phase, suggesting that abnormal expression of VCAM-1 was possibly correlated with relapse of acute leukemia.
5.Effects of verapamil on the sustained depolarization of rat diaphragm myocyte pretreated with neostigmine or 3,4-diaminopyridine

AIM To study the effects of verapamil on end-plate potentials(EPPs) in isolated non-uniform stretched muscle preparation(INSMP) of rat diaphragms pretreated with neostigmine or 3,4-diaminopyridine(3,4-DAP). METHODS Using conventional intracellular microelectrode recording technique. RESULTS A sustained depolarization could be induced at the end-plate area pre-incubated with 0.2-5.0 μmol·L-1 neostigmine or 1.0-4.0 mmol·L-1 3,4-DAP. In normal Tyrode solution, verapamil at a concentration of 1, 5, 10 or 20 μmol·L-1 had no significant effect on evoked endplate potentials and miniature endplate potentials. However, the sustained depolarization due to neostigmine or 3,4-DAP could be antagonized by verapamil at 5-20 μmol·L-1 and the duration of the depolarization was shortened in a concentration-dependent manner. CONCLUSIONL-type calcium ion channels can be activated by accumulated acetylcholine in the synaptic cleft and may be involved in producing sustained depolarization, while they play no role in transmitter release under normal physiological conditions.
6.Early diagnosis for developmental dysplasia of the hip:what can we do in China?

Universal uhrasound screening in newborn has been accepted for reducing the incidence of late detected developmental dysplasia of the hip (DDH).Unfortunately, the neonatal ultrasound screening strategy has not been applied in the mainland of China, which leads to a high incidence of late-detected DDH.Before implementing the neonatal ultrasound screening strategy, the clinical screening by limited abduction is valid to early diagnose the dislocated hip joint.Additionally,it is necessary to screen all the newborn with high risk factors of DDH, such as positive family history, breech presentation and postnatal hip extended swaddling.Early screening of DDH is the common responsibility of child care doctors, including community health physician, pediatrician or pediatric orthopedist.Currently, it is imperative to build regional neonatal ultrasound screening system for DDH in the regions with tertiary pediatric orthopedic center.
7.Immune relevant issues on Mycoplasma pneumoniae infection

Mycoplasma pneumoniae(MP) is the smallest organism that dose not need a host cell for replication.It is one of the most common pathogens of community acquired pneumonia in children.In recent years,the incidence of pneumonia mycoplasma pneumonia increased year by year,and severe pneumonia mycoplasma pneumonia,refractory pneumonia mycoplasma pneumonia cases also increased.MP causes a variety of clinical presentations,in addition to respiratory infections,there are also many extrapulmonary manifestations.Immune response plays a very important part in pathogenesis of MP infection.This article focused on the immune aspects in pathogenesis and extends to some diagnosis and therapeutic implications.
8.Discussion on synergetic action of Zhenqi Fuzheng granule in prevention and treatment of infection in critically ill patients

Objective To observe the synergetic action of Zhenqi Fuzheng granule in prevention and treatment of critically ill patients with severe infection. Methods A retrospective study was conducted. 154 cases with critical illness were enrolled in the First People's Hospital of Baiyin in Gansu Province from January 2009 to December 2013. The control group consisted of 76 cases and only basic treatments were given;in the treatment group, there were 78 cases,besides the basic treatments,they also taken nasal feeding of Zhenqi Fuzheng granule 5 g twice a day,7 days constituting for 1 therapeutic course,a total of 1-3 courses. The incidences of pulmonary infection, urinary tract infection,deep venous catheter-related infections,rate of using antibiotics,fatality rate,infection relapse rate,disease course,pathogen flora distribution were observed in the two groups. Results Compared with the control group,the rates of lung infection,urethral infection,deep vein catheter-related infection,the rate of using antibiotics, fatality rate,infection relapse rate,the discovery rates of drug resistant Acinetobacter baumannii,Pseudomonas aeruginosa,Stenotrophomonas maltophilia,Staphylococcus aureus,and Blastomyces albicans were obviously reduced in treatment group〔lung infection:15.38%(12 cases)vs. 47.37%(36 cases),urethral infection:7.69%(6 cases) vs. 23.68%(18 cases),deep vein catheter infection:5.13%(4 cases)vs. 18.42%(14 cases),antibacterial drug using rate:48.72%(38 cases)vs. 77.63%(59 cases),fatality rate:10.26%(8 cases)vs. 18.42%(14 cases),infection relapse rate:28.21%(22 cases)vs. 48.68%(37 cases),the discovery rate of Acinetobacter baumannii:3.85%(3 cases) vs. 11.84%(9 cases), of Pseudomonas aeruginosa:2.56%(2 cases) vs. 9.21%(7 cases), of Stenotrophomonas maltophilia:5.13%(4 cases)vs. 13.16%(10 cases),of Staphylococcus aureus:5.13%(4 cases) vs. 6.58%(5 cases),of Blastomyces albicans:6.41%(5 cases)vs. 10.52%(8 cases),all P<0.05〕;the therapeutic course was obviously shortened(day:5.79±1.89 vs. 13.65±2.73,P<0.05). Conclusion In critically ill patients with severe infection,treatment of Zhenqi Fuzheng granule can reduce the incidences of severe lung infection, urinary tract infection,deep venous catheter-related infection,the 5-year relapse rate,fatality rate,and shorten the therapeutic course,lower the usage of antibiotics and detection rate of drug-resistant bacteria,therefore,clinically it is effective in such patients.
9.Progress of interventional therapy of congenital valvular aortic stenosis

Congenital vahular aortic stenosis (AS),which refers to a congenital malformation of the aortic valve,is the most common causes of left ventricular outflow tract obstruction in chiidren.In 1983,percutaneous balloon aortic valvuloplasty (PBAV) was firstly described as the treatment for congenital AS in children.Over the past 30 years advancement in technique and equipment had radically improved the safety and outcome of balloon valvuloplasty of aortic valve.Nowadays the technique is widely regarded as the therapy of first choice for children with congenital AS.However,it is essential to make decisions about treatment and define which approach has the best chance of success for any given patient before the initial intervention,especially for the neonate with critical AS.PBAV is usually effective for acutely relieving left ventricular outflow obstruction and has excellent short term results in most childen with congenital AS.The mid-and long-trem outcome after a successful balloon dilation of congenital AS in childhood is also good,but late restenosis and valve regurgitation eventually necessitate reintervention in the majority of children,which included aortic valve replacement.PBAV was found to have a high incidence of complications and remains one of the most challenging procedures in the inttcventional therapy of congenital heart disease.Operators should not only acquire the necessary manual skills,but also have the ability to make sound clinical judgment and deal with critical events.Owing to its palliative nature of balloon dilation for congenital AS,the follow-up care of patients after procedure is quite important.
10.Changes in platelet-activating factor receptor binding characteristics in cerebral thrombotic core and penumbra of tree shrews

AIM: To observe the changes in platelet-activating factor(PAF) receptor binding characteristics and explore the action of PAF on formation of thrombotic core and penumbra following local cerebral ischemia.METHODS: Neuron's membrane protein was abstracted, and the local cerebral ischemia model were induced by photochemistry in tree shrews.
