1.Advances in the treatment of acute pancreatitis

With the enhancing treatment concept and the improvement of diagnosis and treatment technology in recent years,multidisciplinary cooperation and minimally invasive treatment has been the dominant treatment methods for acute pancreatitis,and they reduces the mortality effectively.At the same time,a series of new treatment methods such as step-up approach,which has obvious advantages over traditional surgery,improves the prognosis of severe acute pancreatitis significantly,and it has been recognized by scholars both at home and abroad.
2.Effects of recombinant human interleukin-11 on the vaginal epithelium mitosis of estrone periodical mice and the expression of PCNA

Objective To investigate the therapeutic effects of recombinant human interleukin-11 (rhIL-11) on the vaginal epithelium mitosis of estrone periodical mice and the exprssion of PCNA.Methods The vaginal epithelium mitosis of estrone periodical mice was used as the epidermis hyperplasia model, rhIL-11 action in regulating the epidermis hyperplasia was observed and the expression of PCNA was detected by immunohistochemistry.Results The rhIL-11 significantly inhibited the mitosis of mouse vaginal epithelium, decreased the expression of PCNA ( P <0.01).Conclusion The rhIL-11 has good efficacy in treating the epidermis hyperplasia of psoriasis by inhibiting the mitosis of epithelium and decreasing the expression of PCNA.
3.Research progress of systemic inflammatory response syndrome in acute pancreatitis-associated lung injury

As a result of the early amplification of the inflammatory response in the acute pancreatitis, systemic inflammatory response syndrome (SIRS) is a main cause of acute pancreatitis-associated lung injury (APALI) , while early combined acute lung injury or acute respiratory distress syndrome causes a high mortality of acute pancreatitis.A series of inflammatory mediators and cytokines play important roles in the process of SIRS and APALI, therefore, inflammatory reaction restoring a balance becomes a key point of the treatment of pancreatitis lung injury.
4.The Pre-attentive Processes of Chinese Tones Transmitted by Cochlear Implant Users and Normal Hearing Subjects

Objective Event related potentials (ERP), especially mismatch negativity (MMN), as index, was utilized to study the processing of Chinese tones recognized by cochlear implant (CI) users and normal hearing subjects. Methods The auditory materials were Chinese words with four tones consisting of three different oddball blocks. The ERP and MMN of CI users and normal hearing subjects were recorded and analyzed in relation to the discrimination of those tones as tested. Results The ERP and MMN of CI users and normal hearing subjects were presented. The MMN of CI users includes two negative waves: the first negative wave (MMN1) occurred approxi-mately 100 ms, and the second wave (MMN2) at 300 ms. The MMN of normal hearing subjects includes two nega-tive waves too: the first negative wave (MMN1) occurred approximately 150 ms, and the second wave (MMN2) al-so at 300 ms. The MMN1 peak latency of CI users was obviously shorter than that of normal hearing subjects and the MMN2 peak latency of CI users shorter than that of normal hearing subjects too, but the difference of MMN2 peak latency between CI users and normal hearing subjects was not as significant as that of MMN1. Conclusion In the preattentive stage, CI users could process Chinese tones as normal hearing subjects, but compared with the pro-cessing of normal hearing subjects, there are some differences in the processing of CI users.
5.Practice and experience on performing a better medical ultrasound teaching for undergraduate medical students

Currently, medical ultrasound teaching focuses on the delivering the image infor-mation itself, while its connection with basic medicine and clinical medicine is neglected. Such a teaching method is not good for students to establish a systemic view. To conduct the ultrasound medi-cine education effectively in undergraduate students, we have tried some reforms, including: perform-ing diseases or problem based teaching, integrating basic medicine, ultrasound medicine and clinical medicine throughout the whole teaching process, providing as many practice opportunities as possible for the graduates and integrating the ultrasound imaging with other medical imaging modalities. The practice shows that with the reform, the students have a better understanding of ultrasonic knowledge itself, a better grasp of the whole medical knowledge structure, and also have a preliminary impression on how to diagnose the disease through reasonable application of different treatments. In addition, students have more passion for study.
6.The clinical study on conservative treatment for the patients with tube pregnancy under laparoscopy

Ojective To investigate therapeutic results of conservative operation with laparoscopy for all kinds of patients with tube pregnancy. Methods 62 patients with tube pregnancy were performed the conservative surgery, fenestration on the position of tube pregnancy or take out embryos from the tube crevice combined with injection of MTX under laparoscopy. Results all of 61 patients' tube were retained successfully. no complications occurred, urine HCG recovered to normal levels two weeks after operation. Only one patient continue to pregnancy and completely recovered after intramuscular injection MTX. 18 patients were carried out HSG 1~4 months after operations. 15 patients' tube among them were unobstructed. Opening rate was 88.3%. Within postopeative four years, 12 patients were repregnant. Pregnancy rate was 66.7%. Conclusions Tube pregnancy conservative treatments under laparoscopy not only is safe and minimal invasion, but also had less complications, therefore it is a more suitable method for retaining the fertile females' tubes.
7.Effect of nordihydroguaiaretic acid on chronic atrophic gastritis in rats

Objective To study the preventive effect of lipoxygenase inhibitor nordihydroguaiaretic acid (NDGA) on chronic atrophic gastritis (CAG). Methods To construct CAG rat model, 72 male rats were randomly divided into 6 groups, including normal group, CAG model group, NDGA group with different dose and positive groups (folacin). After feeding for 24 weeks, all rats were executed, their stomach mucous membrane was picked out and stained with H.E. The expression of 5-LOX and P16 protein in mucous membrane epithelia cells were detected by immunohistochemistry. Results The CAG incidence rate in model group was significant higher than normal group (77.8% vs 0% , P <0. 05), which indicated that C AG model rat was successfully established. The CAG incidence rate of model group was significantly higher than NDGA groups (77.8% vs 25% ,27.3% ,25%, P<0.05), while no significant difference was found between positive group and NDGA groups (30% vs 25% ,27.3% ,25% , P>0.05) .The expression of 5-lox in model group was higher than NDGA groups (44% vs 25% ,27% ,25%, P<0.05). The expression of P16 protein in model group was lower than NDGA groups (66.7% vs 83.3% ,81.8% ,83.3%,P<0.05) , and there were no significant differences between NDGA groups and positive group (83. 3%,81.8% ,83.3% vs 80%, P>0.05). Conclusions NDGA could prevent the occurrence of N-ethyl-N-nitro-N nitrosoguanidine-induced chronic atrophic gastritis in rat. NDGA could down-regulate the expression of 5-LOX and up-regulate the expression of P16 in stomach mucous membrane of N-ethyl-N-nitro -N-nitrosoguanidine-induced chronic atrophic gastritis in rat.
8.Effect of dexmedetomidine hydrochloride on hemodynamics,immune function and inflammatory factors in patients gynecological laparoscopic operation

Objective To investigate the effect of dexmedetomidine hydrochloride on hemodynamics,immune function and inflammatory factors in patients with gynecological laparoscopic operation.Methods According to the random comparison table,86 elderly patients with gynecological laparoscopic operation were divided into observation group(43cases) and control group(43cases).The observation group received conventional general anesthesia,and intravenously pumped with dexmedetomidine hydrochloride.The the control group received conventional general anesthesia,and intravenously pumped with normal saline.The changes of hemodynamics,immune function and inflammatory factors were observed and compared in different periods in the two groups.Results Compared with before induction,the level of HR increased significantly at intubation(t=-3.257,-5.019,all P<0.05),the level of HR decreased significantly at 15 min and 30 min after pneumoperitoneum in the two groups(15 min after pneumoperitoneum:t=2.111,-5.019;15 min after pneumoperitoneum:t=3.078,3.926,all P<0.05),but the changes of the observation group variation were significant smaller(at intubation:t=-2.078;15 min after pneumoperitoneum:t=2.105;30 min after pneumoperitoneum:t=1.993,all P<0.05),the level of MAP had no significant difference at intubation in the observation group(t=1.893,P>0.05),the level of MAP increased significantly at intubation in control group(t=-2.122,P<0.05),and there was significant difference at intubation in the two groups(t=-3.477,P<0.05).Compared with before induction,the levels of HR and MAP recovered in the two groups,there were no statistically significant difference at 24 h after operation(HR:t=0.474,0.233;MAP:t=0.853,-0.273,all P>0.05);Compared with before induction,the levels of CRP and IL-6 increased significantly at after operation and 24 h after operation in the two groups(CRP:t=-9.974,-16.872;IL-6:t=-7.284,-11.449,all P<0.05),and the levels of CRP and IL-6 increased more significantly in the control group(CRP:t=-7.509;IL-6:t=-4.744,all P<0.05);Compared with before induction,the levels of CD+3,CD+4,CD+4/CD+8 and NK had no statistically significant differences at after operation and 24 h after operation in the observation group(t=0.154,1.204,1.546,-0.289,all P>0.05),the levels of CD+3,CD+4,CD+4/CD+8 and NK decreased significantly after operation in the control group(t=11.514,10.317,9.180,6.815,all P<0.05),there were statistically significant differences after operation in the two groups(t=10.232,10.298,7.728,4.900,all P<0.05),and the level of NK decreased significantly at 24h after operation in the control group(t=4.362,P<0.05).Conclusion Dexmedetomidine hydrochloride can keep hemodynamics stability,reduce immune function damage and inflammation.
9.Effects of respiration on hemodynamics in patients with pericardial effusion by color Dopplere chocardiography

ObjectiveTo observe and quantify the effec ts of respiration on hemodynamics in patients with pericardial effusion (PE) and to verify our new proposed hypothesis about the mechanism of respiratory effects on cardiac function from clinical point of view. MethodsPulsed-wave Doppler spectra of the four cardiac valves were recorded with Acuson Sequoia 512 and 128 XP/10 in 10 patients with PE and 18 healthy control subjects with no apparent cardiac and pulmonary abnormalities. Electrocardiogram and respiratory tracing by a nasal thermistor were recorded simultaneously with the echocardiograms. The blood flow velocities of each valve during inspiration and expiration were averaged separately and those were again averaged for 5 consecutive respiratory cycles, and the respiratory variation indices (RVI) were calculated both in the patients and the control groups. ResultsThe peak E velocity across the mitral valve and that across the aortic valve significantly increased during expiration and decreased during inspiration, while that across the tricuspid valve and that across the pulmonary valve increased remarkably during inspiration and decreased during expiration in all of the subjects. 10.Studies on the effect of ulinastatin on hippocampal neuron and MMP-9 expression after ischemia-reperfusion injury rats

Objective To investigate the effect of ulinastatin on hippocampal neuron and matrix metalloproteinase-9 ( MMP-9 ) expression after ischemia-reperfusion injury rats, to reveal the mechanism of protection on hippocampus of ulinastatin.Methods The focal ischemia reperfusion model was made by thread embolism of middle cerebra1 artery, and ulinastatin was used.The animals were killed at 24 h after reperfusion was started.The sections of the brains were processed with Nissl staining and by immunohistochemistry with antibody of MMP-9.Results Corrected optical density (COD) of MMP-9 expression was significantly decreased after administration of ulinastatin.Also ulinastatin can obviously protect the structure of hippocampal neuron.Conclusion Ulinastatin could protect pyramidal cells in CA1 region from damage of ischemia-reperfusion injury and increase the number of normal neurons.Inhibition on MMP-9 expression was involved in the mechanism.
