Objective To evaluate the value of fecal high mobility group box-1 (HMGB 1 ) in early diagnosis and prediction of the severity of neonatal necrotizing enterocolitis (NEC). Methods From July 2013 to June 2015 , the neonates who had distention, vomit, or gross blood in stool and were suspected of NEC were recruited as NEC group while hospitalized children without abdominal distension, vomiting, bloody diarrhea, or other gastrointestinal symptoms were recruited as the control group. Stool samples were collected on day 1 , 3 , 5 and 7 after admission. The level of HMGB 1 was measured by enzyme linked immunosorbent assay (ELISA). Results In the end, there were 46 cases in NEC group and 15 cases in control group. In NEC group, 29 cases were conifrmed of stageⅠof NEC by abdominal radiograph within 24 h after hospitalized, all of them were deteriorated to stageⅡphase in 4 days, and 10 cases were deteriorated to stageⅢ. Seventeen cases were conifrmed of stageⅡby abdominal radiograph within 24 h after hospitalized, 7 cases were deteriorated to stageⅢ. In 17 stageⅢcases, 11 cases received surgical treatment and 6 cases gave up. Eight cases survived and 3 died after surgery. The levels of HMGB 1 in NEC group on day 1 , 3 , 5 and 7 after hospitalized were higher than those in control group (P0 . 05 ). With the exacerbation of NEC from stage I toⅢ, the levels of HMGB 1 increased gradually (P0 . 05 ). Conclusions With the exacerbation of NEC, the level of HMGB 1 is gradually increased, which indicates that it has certain clinical value in early diagnosis and prediction of severity of NEC.

Objective Based oncomputer-aided design (CAD) technology, we compared the optimized method of locating inserting point (2 cm lateral to and 1 cm below angulus oris) with traditional ones and assessed its clinical value by simulating foramen ovale (FO) cannulation. Methods Skull CT data of 100 primary trigeminal neuralgia patients were used in this study and three-dimensional reconstruction with FO cannulation simulation were conducted. With self-control test method, each patient was divided into three groups: group of optimized insertion point (method O) : point 2 cm lateral to and 1 cm below angulus oris as the insertion point; group of traditional entry point A (method A) : 2.5 cm lateral the angulus oris as the insertion point; group of traditional entry point B (method B) : intersecting the external extension of angulus oris and the vertical line of the lateral eye angle as the entry point. Puncture simulation was performed along connecting line of insertion point and the central point of the FO. We recorded results of simulation and measured the available area and short diameter of FO in the direction of puncture. Results The cases that the puncture route was obstructed of methods O, A and B were 15, 28 and 31, respectively. The occlusion rate of method O was significantly lower than those of methods A and B (P < 0.01 or P < 0.05). The available area of methods O, A and of B were (9.45 ± 6.57) mm2, (6.91 ± 5.96) mm2, (6.59 ± 5.86) mm2, respectively. The available area of method O was significantly larger than those of methods A and B (P < 0.01 or P < 0.05). The short diameters of methods O, A and B were (2.25 ± 1.04) mm, (1.81 ± 0.97) mm, (1.71 ± 0.92) mm, respectivtly. Short diameters of method O was significantly longer than those of methods A and B (P < 0.01 or P < 0.05). Conclusion compared with the traditional positioning method, the optimized method can provide better condition for puncture through Hartel approach.

Objective:To investigate the epidemiological characteristics of classic human astrovirus (HAstV) among children under five years old with acute diarrhea, and to understand the role of HAstV in children acute diarrhea. Methods:A total of 1 010 fecal specimens were collected in 1 010 outpatients under five years old with acute diarrhea admitted to Children′s Hospital of Fudan University, Shanghai from January 2012 to December 2016. Reverse transcription polymerase chain reaction (PCR) or PCR was used for screening classic HAstV, group A rotavirus, norovirus and adenovirus. Genotypes of classic HAstV were determined by nucleotide sequencing and phylogenetic tree analysis. Results:The overall positive rate of classic HAstV was 2.7%(27/1 010). The detection rates of classic HAstV from 2012 to 2016 were 6.9%(10/144), 3.5%(5/144), 2.1%(3/144), 1.5%(4/265) and 1.6%(5/313), respectively. Almost 96.3%(26/27) of children infected with HAstV were 0 to 36 months of age. The prevalence of classic HAstV infections displayed a typical autumn/winter seasonality except in 2016. All the positive classic HAstV strains were genotyped as HAstV-1 with two lineages of HAstV-1a and HAstV-1b. Among them, the lineage of HAstV-1a was the predominant subtype (63.0%, 17/27). There were 77.8%(21/27) of the children with acute diarrhea only infected with classic HAstV, whereas for the remaining cases a variety of other enteric viruses were detected (three cases co-infected with HAstV and group A rotavirus, two cases co-infected with HAstV and adenovirus, and one case co-infected with HAstV, group A rotavirus and adenovirus). Conclusions:Children infected with HAstV are mainly less than 36 months of age. Although the genotype of classic HAstV detected in this study is single, but the lineages are in a state of dynamic change. Long-time and continuous monitor for the epidemiology of classic HAstV is needed to avoid outbreak of diarrhea in children.

Objective To predict and assess biomechanical responses and injury mechanisms of the thorax and abdomen for small-sized females in vehicle collisions. Methods The accurate geometric model of the thorax and abdomen was constructed based on CT images of Chinese 5th percentile female volunteers. A thoracic-abdominal finite element model of Chinese 5th percentile female with detailed anatomical structure was developed by using the corresponding software. The model was validated by reconstructing three groups of cadaver experiments (namely, test of blunt anteroposterior impact on the thorax, test of bar anteroposterior impact on the abdomen, test of blunt lateral impact on the chest and abdomen). Results The force-deformation curves and injury biomechanical responses of the organs from the simulations were consistent with the cadaver experiment results, which validated effectiveness of the model. Conclusions The model can be used for studying injury mechanisms of the thorax and abdomen for small-sized female, as well as developing small-sized occupant restraint systems and analyzing the forensic cases, which lays foundation for developing the whole body finite element model of Chinese 5th percentile female.

OBJECTIVETo compare the effect difference for stroke between meridian-collateral diagnosis and therapy byand conventional acupuncture with syndrome differentiation.
METHODSTotally 148 patients were assigned into an observation group(72 cases) and a control group(76 cases) by random number table, with 10 cases dropping out in the observation group. In the observation group, meridians were examined and differentiated and then the treating meridians and acupoints were defined. Corresponding acupuncture was used according to them. In the control group, acupuncture was applied at acupoints by internal differentiation and experience. Treatment was given once a day and five times a week, with total 20 times. The motion function of limbs and coloboma degree of nerve function were assessed by Fugl-Meyer score and National Institutes of Health Stroke Score(NIHSS) before and after treatment as well as at three-month follow-up.
RESULTSAfter treatment, the Fugl-Meyer scores increased and the NIHSS scores decreased in the two groups compared with those before treatment(all<0.05). At follow-up three months after treatment,Fugl-Meyer score upgraded in the observation group (<0.05) and NIHSS score declined in the two groups (both<0.01) than those before treatment, and NIHSS scores were statistically different between the two groups(<0.05).
CONCLUSIONSMeridian-collateral diagnosis and therapy byhas better long-term efficacy when it is compared with conventional acupuncture with syndrome differentiation for motion function of limbs and nerve function of stroke.
