1.Relationship of nutrition with cancer and its epigenetic DNA methylation mechanism

DNA methylation is the best studied of the epigenetic changes that influence the gene expression,and changes in genomic methylation patterns have been observed in many cancer cell lines.Nutrients as methyl donors play essential roles in DNA methylation and may modify cancer risk.This review,with an emphasis on the relationship between nutrition,DNA methylation and cancer,firstly defines methylation with a description of its correlated mechanism,then approaches the molecular mechanism of cancer initiation and development at the methylation level,and lastly summarizes the mechanism underlying the effect of methylation-related nutrients on the process of cancer initiation and development at the molecular level.It also offers some suggestions on cancer prevention at the group nutrition level.
2.Influencing factors of lipid control success rate among patients with coronary heart disease

Objective To investigate the factors influencing the lipid control success rate among patients with coronary heart disease (CHD).Methods Questionnaire and clinical data reviews were conducted to investigate the general information,disease characteristics and patients controlling blood lipid and by blood biochemical tests were done to detect the lipid levels.The relationships between lipid control success rate and general information,disease characteristics and patients' behaviors were analyzed by chi-square test and logistic regression analysis.Results The blood lipid success rate was only 67.09% in patients with CHD combined dyslipidemia.The education level,cardiovascular disease risk grading,complications,body mass index,medication compliance and dietary evaluation were the main factors influencing the patients' success in blood lipid control.Conclusion The lipid control success rate can be raised by taking measurements to help patients with CHD and dyslipidemia control weight and increase medication compliance and dietary behavior.
3.Molecular mechanism of oxLDL inducing lipid accumulation and inflammation in macrophages to promot atherosclerosisvia TLR4 signaling pathway

Objective · To investigate the possible role of TLR4 signaling pathway in the mediation of atherosclerosis. Methods · TLR4 were knocked down via transfection with TLR4-specific siRNA, and the lipid accumulation was further detected in control and TLR4-knockdown groups by oil red O staining. The expression of CD36 andLectin-like oxLDL receptor 1 (LOX-1) in macrophages were detected by Western blotting to investigate the role of TLR4 in the expression of oxLDL-related receptors. Cytokines such as interleukin-6 (IL-6), IL-8, monocyte chemoattractant protein 1 (MCP-1), and matrix metalloproteinase-9 (MMP-9) were tested by ELISA to confirm the possible role of TLR4 in the secretion of inflammatory factors. Results · Macrophages (namely CD68+ cells) were found to accumulate within atherosclerosisplaques with TLR4 highly expressed on the surface of macrophages; the stimulation with oxLDL promoted the lipid accumulation (P<0.01), the secretion of inflammatory factors (P<0.01), and the expression of CD36 and LOX-1. The oxLDL-associated expression of CD36 was decreased but the expression of LOX-1 was not affected. The knockdown of TLR4 inhibits oxLDL-induced lipid accumulation (P<0.01)and inflammatory cytokines (IL-6, IL-8, MCP-1 and MMP-9) secretion (P<0.01). Conclusion · TLR4 signaling pathway possibly promotes the lipid accumulation and the secretion of inflammatory factors via up-regulating the expression of CD36 to affect the formation and development of atherosclerosis.
4.DETERMINATION OF VITAMIN E IN PEANUT OIL AND SOME FOODS BY FLUOROMETER AND TLC SCANNER

Vitamine E in peanut oil was determined by means of f Juorometric method (Ex = 285nm, Em=324nm) and TLC scanning method (? = 292nm). Both methods include saponification of the oil, isolation o'f the unsaponifiable matters and the respective final measurements. The result of the former method indicates the total vitamine E in terms of ?-vitamine E and the latter method may indicate their ?,?,? and 6 analogues separately. These two methods are simple, time-saving and reliable. The recovery of VE and the standard deviation of replication of fluorometric method and TLC scanning method are 99.3%, ? 0.587mg/100g sample and 103.04%, ?0.959mg/100g sample respectively. Both methods are also applicable to other foods such as fortified cakes, confectionaries, infant foods etc., but the oil in these samples must be extracted before saponification.
5.The Expression and Clinical Significance of CD~4+CD25~+ Regulatory T cell in Non-Hodgkin's Lymphoma

Objective To evaluate the expression of CD4+CD25+CD127low regulatory T cells(Tregs) in the peripheral blood of the patients with NHL and discuss the relationship between expression of Tregs and prognostics factors of NHL.Methods 58 NHL patients was enrolled for analyzing.Among of them,23 were new patients without any treatment,35 patients received 3 to 8 cycles of chemotherapy,and 16 were normal controls.The expression of CD4+CD25+CD127low Tregs was detected by Flow Cytometry and the proportions of Tregs were analyzed among the groups of the patients before or after chemotherapy and the normal controls.Bivariate Correlation was used to analyze the correlation among the expression of Tregs in the peripheral blood and prognostic factors,such as age,sex,PS state,LDH,?2-MG,IPI,clinical stage and pathology category of the new patients.Results The proportion of Tregs in the peripheral blood of B cell lymphoma(B-NHL) group(3.89?2.49)% was significantly higher than that of normal control(2.52%?0.88%)(P=0.005).In B-NHL,Tregs was signifantly higher in the aggressive NHL(DLBCL,MCL) than that of nonaggressive HHL(FL,MZL)(P=0.002).In T cell lymphoma,Tregs was increased markedly in the highly aggressive T-NHL(T-LBL,NK/TCL) as compared with aggressive T-NHL(ALCL,PTCL)(P=0.006).Tregs expression was significantly higher in newly diagnosed NHL patints than that of normal control(P=0.004) and post-chemotherapy group(P=0.034).The expression of Tregs in peripheral blood was significantly correlated with ?2-MG,LDH and IPI of the patients in newly diagnosed patients group(P=0.003,0.003 and 0.038),whereas was not significantly correlated with the age,sex,PS state,LDH,clinical stage and pathology category of the patients.Conclusion The expression of CD4+CD25+CD127low Tregs in peripheral blood of B-NHL group was significantly higher than that of the patiens post-chemotherapy and normal control,which means that it may be an useful prognostic factor for NHL patients.
6.The value of D-dimer in assessing severity and predicting long-term prognosis in patients with community acquired pneumonia

Objective To evaluate the value of D-dimer in assessing severity and predicting longterm prognosis in patients with community acquired pneumonia (CAP).Methods From June 2009 to December 2010,a total of 189 patients with CAP were enrolled.After admission,D-dimer,procalcitonin (PCT) and C-reactive protein (CRP) were measured,and Pneumonia Severity Index (PSI) was calculated.They were assigned into two groups according to their D-dimer levels:high D-dimer levels group (D-dimer levels≥500 μg/L) and normal D-dimer levels group (D-dimer levels < 500 μg/L).The followup time was one year.A Kaplan-Meier survive curve was constructed to assess the 1-year mortality,and multivariate logistic regression analysis were used to assess the value of D-dimer for predicting long-term prognosis.Results D-dimer levels increased with increasing PSI class [class Ⅰ-Ⅲ:378.37 μg/L (216.74,649.50) μg/L; class Ⅳ:673.41 μg/L (544.77,866.85) μg/L; class Ⅴ:831.58 μg/L (591.78,1066.39) μg/L,x2 =56.58,P < 0.01].The Kaplan-Meier survival curve showed that 1-year mortality rate of high D-dimer levels group was higher than normal D-dimer levels group (log-rank test,x2 =52.51,P < 0.01).The multivariate logistic regression analysis showed an independent relationship between higher D-dimer levels and long-term mortality (OR =2.05,95% CI:1.48-2.61,P < 0.01).Conclusion D-dimer is an independent predictor of severity and long-term prognosis in patients with CAP.
7.Evaluation of the diagnostic accuracy of modified Berlin questionnaire on predicting obstructive sleep apnea-hypopnea syndrome in adults.

OBJECTIVE:
To evaluate the diagnostic value of modified Berlin questionnaire on predicting obstructive sleep apnea-hypopnea syndrome (OSAHS)in Chinese adults.
METHOD:
Differ from the original version, BMI cut-off point was adjusted to 25.0 in modified Berlin questionnaire according to the Asia -Pacific obesity definition. A total number of 244 samples who experienced polysomnography (PSG) were included. After well informed, each patient finished questionnaire by an interview. The results of the original and modified questionnaires and polysomnography reports were compared with polysomnography reports to evaluate the diagnostic accuracy of the modified Berlin questionnaire.
RESULT:
In male population, the sensitivity and specificity of original Berlin questionnaire were 74.03% and 65.71%, respectively. The percentage of diagnostic consistency was 72.49%, and Kappa coefficient was 0.304 (P < 0.01). In aspect of modified version, the sensitivity and specificity were 92.21% and 48.57%. The percentage of diagnostic consistency was 84.13%, and Kappa coefficient was 0.437 (P < 0.01). In female population, the sensitivity and specificity of original version were 50.00% and 61.90%, respectively. The percentage of diagnostic consistency was 54.55%, and Kappa coefficient was 0.110 (P > 0.01). When comes to the modified Berlin questionnaire, the sensitivity and specificity were 76.47% and 47.62% in female subjects, and the percentage of consistency agreement was 65.45%, with a Kappa coefficient 0.248 (P > 0.01).
CONCLUSION
Compared to the original version, the modified Berlin questionnaire has a better diagnostic consistency with a considerable sensitivity and specificity in male population. However, in female subjects group, the consistency of the modified Berlin questionnaire was still not significant remains unsatisfactory, although P value has been improved. Modified Berlin questionnaire could be. used as a primary screening tool for male OSAHS patients. However, but the screening questionnaire for female should still be need to be further explored.

8.Parainfluenza virus pneumonia after lung transplantation :a case report and literature review

Objective To explore the clinical characteristics ,diagnosis ,treatment and prognosis of parainfluenza virus (PIV) pneumonia after lung transplantation .Methods One case of PIV pneumonia after lung transplantation was retrospectively analyzed . The relevant domestic and foreign cases and literature review were summarized .Results The recipient underwent sequential bilateral lung transplantation for chronic obstructive pulmonary disease ,bullae and respiratory failure .Donor lung was sourced from donation after cardiac death .Routine anti-rejection therapy was prescribed postoperatively .At 14 months ,cough and shortness of breath lead to hospitalization for over 1 month .At 15 months ,sputum/fungal smear and culture showed that nucleic acid of PIV was positive . The definite diagnosis was PIV pneumonia after lung transplantation .After ribavirin antiviral therapy ,tracheal intubation and invasive ventilation ,followed by imipenem plus doxycycline plus anti-infective therapy ,ganciclovir antiviral therapy ,repeated bronchoscopic sputum aspiration and lavage treatment ,the patient's condition deteriorated and died from breathing failure and septic shock at 16 months .Conclusions Preventing PIV infection after lung transplantation is of vital importance .PCR is essential for a rapid detection of virus infection .However ,there is no curative treatment of PIV infection .Specific parainfluenza immunoglobulin and DAS 181 aerosol inhalation may be applied for future treatment of PIV infection in lung transplant recipients .
9.39 cases of malignant tumors originated from external and middle ear.

OBJECTIVE:
To describe our experience in the clinical manifestation and treatment of malignant tumors of the external and middle ear.
METHOD:
The study reviewed 39 patients between 1994-2011 in our hospital, including 15 pinna tumors, 18 external canal tumors and 6 middle ear tumors. 23 males and 16 females were enrolled in this study. The mean age of patients at the time of surgery was 59. Radiotherapy or radiotherapy and chemotherapy were the only possible treatment in 6 cases. Thirty-three patients were treated surgically, and 9 patients also received radiotherapy after surgery.
RESULT:
All of the patients had been followed up over 3 years, except for 1 case of external canal and 1 case of middle ear tumor. The 3-year survival of pinna, external canal and middle ear tumors were 86.7%, 82.4% and 60.0% respectively. At the last follow up, the pinna tumors showed that the survival rate was 100% in T1, T2 and Tx stage, and 0% in T4 stage; the external canal tumors showed that the survival rate was 90% in T1 stage, and 66.7% in T2, T3 stage; the middle ear tumors showed that the survival rate was 100% in T1 and T2 stage, 0% in T3 stage.
CONCLUSION
The T staging system is for an important prognostic factor, and it is important for an early diagnosis and radical surgery to achieve a better therapeutical result.

Diabetic kidney disease (DKD) is one of the main complications of diabetes, and also the leading cause of the end-stage renal disease (ESRD).The main clinical manifestations are albuminuria and decreased glomerular filtration rate.DKD seriously affects the quality of life of sufferers and places a huge financial burden on them. Traditional Chinese medicine (TCM) has accumulated rich experience in treating DKD.This paper analyzed and summarized the recent treatment of DKD with traditional Chinese medicine from three aspects: active ingredients of TCM, TCM pairs and TCM prescriptions, so as to provide new ideas for the majority of researchers in experimental research.
