Objective To investigate the suppression effects of tumor suppressor gene p53 alone and apoptosis incombination with TNF-? in a hepatocellular carcinoma cell line Hep3B. Methods Hep3B cells were transfected with a wild-type p53 cDNA(wt-p53)and plain vector(pNeo) respectively. Then the cell were cultured for 12 hours, one of the transfected p53 groups was added TNF-?(20 ?g/ml). The expression of p53 was detected by immunological fluorescence assay. Determination of apoptosis was used by DNA fragmentation, TUNEL assay. Results Both a small dose TNF-? and wt-p53 can induce apoptosis more efficiently comparing with non-transfected cultures(P

Professor ZHANG Liang-ying in Yunnan University of Traditional Chinese Medicine conducted TCM adjuvant therapy for in vitro fertilization - embryo transfer (IVF-ET) of infertility patients, which has obtained satisfactory results. The use of kidney-based, disease differentiation combined with the treatment, emphasizing preoperative conditioning, psychological adjustment, and appropriate choice, can help improve the IVF-ET pregnancy rate and complications of many problems.
4.Exploration on the Origin of Qilu Modern Nursing and Some Thoughts

In the early 20th century,the term Nursing was introduced into China.In 1884,with the arrival of the first batch of missionary nurse,modern nursing initiated by Nightingale was introduced into China and greatly changed the way of care.The missionary nurses were full of enthusiasm for the spreading Gospel of Christ,the love for nursing care and the empathy for Chinese people;and encountered the lack of staff and material,the mis-understanding and resistance from the local people and other multiple tests.With the untiring effort of them and the trained local nurses,China had led the international care in nursing education and practice standard.Based on this,nursing in Shandong gradually rose and had an unprecedented development as a new medical subject.By clearing up and consulting Chinese and English historical materials,this paper reviewed the 60 years of the mission-ary nurses in China,saw the early development of nursing in our country from the budding and development of Shandong nursing and provided more insights for understanding and solving the complex problems contemporary Chinese nursing faces.
5.A recombinant adenovirus vector expressing human IFN-λ1 inhibits the growth of orthotopic gastric cancer and its possible mechanism

Objective To investigate the inhibitory effects of a recombinant adenovirus vector ex-pressing human IFN-λ1 ( r-Ad-hIFN-λ1) on the growth of orthotopic gastric cancer and to analyze its possi-ble mechanism in a nude mice model of orthotopic human gastric carcinoma .Methods Orthotopic trans-plantation was performed to establish the nude mice model of orthotopic gastric cancer .Thirty mice were ran-domly divided into three groups including PBS control group , Ad-Lac Z empty vector group and r-Ad-hIFN-λ1 experiment group .The mice in each group were given the corresponding interventions once a week for three times.The sizes of tumor were detected by using B-mode ultrasound at different time points to draw growth curves .The expression of IFN-λ1 at mRNA level in skeletal muscle tissues was analyzed by RT-PCR.Western blot assay and immunohistochemical staining were used to detect IFN-λ1 protein in gastric tumor samples .The apoptosis of cells in paraffin-embedded tumor tissues was detected by TUNEL .The per-centages of natural killer ( NK) cells in spleen tissues were analyzed by flow cytometry .Results Compared with control group and empty vector group , the mice in r-Ad-hIFN-λ1 experiment group showed the smallest tumor size [(331.25±6.00) mm3, (322.92±5.92) mm3 vs (248.39±7.60) mm3; P<0.05].hIFN-λ1 was transfected into skeletal muscle successfully and expressed in gastric cancer tissue .Highly expressed hIFN-λ1 was observed in cytoplasm of tumor cells from experiment group by immunohistochemical staining . The apoptotic index of tumor tissues for experiment group was 0.700±0.059 which was significant different from that of control group (0.271±0.026, P<0.05) and empty vector group (0.333±0.028, P<0.05). The percentage of NK cells in spleens from mice in experiment group [(26.49±1.89)%] was significantly higher than that of control group [(13.94 ±1.31)%, P<0.05)] and empty vector group [(19.12 ±1.69)%, P<0.05)].Conclusion Transfection of r-Ad-hIFN-λ1 and expression of hIFN-λin skeletal muscle could significantly inhibit the growth of gastric cancer by inducing tumor cells ′apoptosis and enhan-cing NK cells.
6.Performance evaluation of laboratories based on AHP and DEA

Objective:By calculating the efficiency of the scientific research laboratory, which reflects the level of the scientific research input and output capacity, provide reference for the evaluation and decision-making of its scientific research sustainable development capacity.Methods:20 scientific research laboratories in a hospital were selected as the research subjects, annual input data were used as the input index. Weighted quantitative scores of the performance of each laboratory in research capacity and contribution, research team construction, discipline development and personnel training, operation management, papers and monographs, patents and transfer, awards, graduate-student training, standards and norms, and academic conferences. All these factors mentioned above were used as output indicators. Then Analytic Hierarchy Process (AHP) and Data Envelopment Analysis (DEA) are used to evaluate the scientific efficiency of each laboratory.Results:The performance of the laboratory is weak in the aspects of patent transfer, awards, standardization. The technical efficiency of laboratory 20 is the lowest, and the scale efficiency of laboratory 12 is the lowest.Conclusions:Scientific Research Laboratories should enhance the effectiveness through input adjustment and output enhancement, meanwhile each laboratory should pay attention to the transformation of scientific achievements and also the optimization of construction system.
7.Chronic granulomatous inflammation of the tibial nerve: a case report

Granulomatous disorder is a group of chronic proliferative inflammatory diseases.Both infectious and noninfectious factors can lead to granuloma formation.Its diagnosis and treatment should be dependent on clinical manifestations and pathological results of patients.The existence of systematic diseases and immune system abnormalities is the key factor in determining final therapy in this condition.It is a rare clinical case of granulomatosis with simple invasion of one single peripheral nerve,which is challenging in daily clinical work.Here we present a case of granulomatous inflammation involving the right tibial nerve only,and analyze the difficulties and key points in the diagnosis and treatment of these patients with literature review.
8.The application value of FilmArray detection in children with acute lower respiratory tract infection

Objective:To explore the application value of FilmArray detection in children with acute lower respiratory tract infection and conduct economics analysis.Methods:From December 1, 2016 to November 30, 2017, 1 380 patients were enrolled in our study.Some children(FilmArray group) were tested for respiratory pathogens with FilmArray, while others (control group) were tested by 7-box antigen test of respiratory virus, gold colloid test of influenza and mycoplasma antibody.Those with underlying diseases were excluded.A total of 160 cases in the FilmArray group and 160 cases in the control group were obtained with tendency score matching method.The physical examination of pathogens, clinical indicators, usage of anti-infective drugs and hospitalization related costs were compared.Results:The positive rate of FilmArray test was significantly higher than that in control group (86.88% vs. 45.91%). The most common pathogens detected by FilmArray were adenovirus(39 cases), rhinovirus(34 cases), and parainfluenza virus(30 cases). In the FilmArray group, nine cases were positive for botulinum pertussis, accounting for 5.6% of the total.The hospitalization time of FilmArray group was shorter than that in control group [(8.89±6.23 days vs.(11.51±14.43)days]. In FilmArray group, the antibiotics were used for a shorter time, and 18 children did not use antibiotics during hospitalization.Compared with the control group, the hospitalization cost had no significant difference in the FilmArray group, but the antibiotic cost was less, as well as hospitalization time was shorter.The average hospitalization cost saved by using the FilmArray test was nearly 2 000 yuan per person. Conclusion:The application of FilmArray detection in children with acute lower respiratory tract infection can quickly and accurately identify a various infections of virus, bacteria and atypical pathogen, which guides using anti-infective drugs more reasonably.The application of FilmArray detection shortens the average hospitalization days of children, increases the utilization efficiency of medical resources, and reduces the medical cost and indirect economic loss of children’s families, which has certain economics significance.
9.Challenges, countermeasures and key points of ethical review for decentralized clinical trials

Decentralized clinical trials (DCT) are bringing new changes to clinical trials, the core concept of “patient-centered” must be adhered, and the safety and rights of research participants must be adequately safeguarded. Although DCTs have similarities with traditional clinical trials, they are not suitable for all clinical trials, and are more suitable for oral administration, stable condition of study participants, and longer duration of treatment in clinical trials. DCT, elements must be selected based on the characteristics of clinical trials, and strike a balance between improving the sense of gain, safety, and rights and dignity of research participants, as well as the quality of data collected. Currently, DCT in China face many challenges. To actively promote DCTs in China, regulatory regulations and guiding principles should be improved. Based on patient-centered design principles, the first discussion requires real-time online communication. Researchers should provide more clinical trial services, improve risk control and accessibility of medical assistance, use third-party convenient payments, ensure fair recruitment of research participants, consider compliance, validate digital health technology, remote monitoring and electronic informed consent compliance, ensure the security of data collection, transmission, and analysis, as well as strengthen multi-party training. Ethical review is one of the important means to protect the safety and rights of research participants. Ethics committees should focus on appropriate DCTs elements that are patient-centered, compliance and effectiveness of electronic informed consent, processing and reporting of security information, privacy protection of data collection, compliance of remote monitoring, family health follow-up, management of biological samples and experimental medical products, and training programs. The author believed that the complementarity and combination with traditional clinical trials will make DCTs elements implemented more easily and smoothly. By leveraging the power of scientific and technological advances, DCTs will boost the speed of China’s drug and medical device research and development, enhance international competitiveness, and benefit more patients.
10.The clinical studies of 51 patients with thrombotic thrombocytopenic purpura.

OBJECTIVETo comprehensively analyze the clinical characteristics, treatment strategies and outcome of patients with thrombotic thrombocytopenic purpura (TTP).
METHODSA retrospective survey of 51 TTP patients confirmed in our database. Relevant statistical analyzes were performed by GraphPad Prism 5 software.
RESULTS51 cases of patients with acquired TTP were identified as idiopathic TTP. In our study, only 18 cases (35.29%) had typical pentalogy of TTP, where thrombocytopenia (100.00%), microangiopathic hemolytic anemia (92.16%) and neurologic abnormalities (88.24%) were more common than fever (72.55%) and renal abnormalities (70.59%). Plasma ADAMTS13 activity was detected in 37 patients with TTP with ADAMTS13 deficiency confirmed in 31 patients (83.78%). Plasma exchange with response of 72.3% was still the preferred strategy in TTP with individuation. Among 36 survival TTP patients, 8 patients (22.22%) relapsed. 15 patients (29.41%) died in our study. The mean ages of responders and deaths were of (37.5±14.5) and (50.1±18.9) respectively; whereas total bilirubin level of responders and deaths were of (43.3±23.5)μmol/L and (63.7±39.7) μmol/L respectively, the differences were statistically significant. Conversely, body temperature, WBC, HGB, PLT, serum creatinine and LDH showed no significant differences (P>0.05).
CONCLUSIONThe diagnosis of TTP was based on comprehensive analysis of clinical manifestations. Plasma ADAMTS13 activity test had a higher clinical practical value. The therapeutic alliance with corticosteroids, immunosuppressive agents and Rituximab significantly improved its outcome. The age and high total bilirubin level at onset were associated with less sensitive to plasmapheresis and poor prognosis.
