1.Evaluation of the intensity modulated radiotherapy in early stage cervical cancer with vaginal stump recurrence after surgery

Objective To evaluate the efficacy of the intensity modulated radiotherapy in early stage cervical cancer with vaginal stump recurrence after surgery.Methods A retrospective concurrent comparative study included 60 patients with vaginal recurrence after surgery in early stage cervical cancer.30 cases of the experimental group were treated with intensity-modulated radiotherapy (IMRT),and 30 cases of the control group were treated with conventional radiotherapy.The efficacy and complications between two groups were compared after 3 treatment cycles.Results Of 30 patients in experimental group,22 patients achieved complete remission,7 had partial and 1 progressed.But in the control group,14 had complete remission,5 reached partial remission,7 were stable and 4 progressed.The effective rates were 96.67 % (29/30) and 63.33 % (19/30),respectively,in the experimental and control group,which showed statistical significance (P =0.009).In the experimental group,there were 1 case with grade 1 radiation cystitis and 1 case with grade 1 radiation proctitis.In the control group,there were 3 cases of vaginal-rectal fistula,1 case of grade 3 radiation proctitis,2 cases of grade 2 radiation cystitis and 2 cases of grade 1 radiation proctitis.Conclusions IMRT for treating vaginal recurrence of early cervical cancer achieves satisfactory short-term effect.In comparison with conventional radiotherapy,IMRT has higher efficacy but less side effects.
2.Surgical excision and botulinum toxin A injection for vocal process granuloma.

OBJECTIVE:
To study the efficacy of treatment with microsurgery in combination with local injection of type A botulinum toxin for vocal process granuloma.
METHOD:
28 patients with vocal process granuloma received endotracheal intubation under general anesthesia. The lesion was removed with micro-scissor and CO2 laster under a self-retaining laryngoscope and microscope. The incision and mucous membrane surrounding the wound was closed with 8-0 absorbable suture. 4-point injection of botulinum toxin type A 8-15 u was then performed along the thyroarytenoid muscle and arytenoid muscle of the same side. Postoperative medication was administered based on disease causes.
RESULT:
All patients experienced vocal cord dyskinesia of the injected side 2-3 days after surgery. At 1 month after the surgery, wound healing was good in all the 28 patients, and the vocal cord movement was limited at the injected side. At 3 months, movement of the bilateral vocal cords was normal, and the vocal cord process mucosa was smooth. Patients were followed up for more than a year, and only one patient had recurrence in 2 months after surgery. The cure rate was 96. 4%.
CONCLUSION
Combination of laryngeal microsurgery and type A botulinum toxin local injection can shorten the treatment course of vocal process granuloma.

3.Correlation of angiopoietin-2 and angiopoietin-2 receptor expressions in serum and placenta with preeclampsia

Objective To investigate the correlation of the expressions of angiopoietin-2 (Ang-2) and angiopoietin-2 receptor(Tie-2)in serum and placenta with preeclampsia. Methods From May 2009 to April 2010, 62 women with preeclampsia who delivered in Affiliated Hospital of Qingdao University Medical College were recruited in the study, including 30 women with moderate preeclampsia (MPE group) and 32 women with severe preeclampsia (SPE group). Another 30 healthy pregnant women were taken as control group. ELISA was used to measure the serum Ang-2 in these women. Semiquantitative reverse transcription (RT)-PCR was used to investigate the expressions of Ang-2 mRNA and Tie-2 mRNA in placenta. Western blot was used to determine the expression of Ang-2 protein in placenta. Results (1) The serum concentrations of Ang-2 in MPE group and SPE group were (5.4 ± 1.8) μg/L and (5. 1 ± 1.7) μg/L,respectively. Both were significantly lower than that in control group (16. 2 ± 4. 5) μg/L (P<0. 01).There was no significant difference between MPE group and SPE group (P > 0. 05). (2) The expressions of Ang-2 mRNA in placenta of MPE group (2. 1 ± 0. 7) and SPE group (2. 0 ± 0. 6) were both significantly lower than that of control group (5.8 ± 0. 8; P<0. 01). But there was no significant difference in Ang-2 mRNA expression between MPE group and SPE group (P>0. 05). (3) No significant difference was found in the expressions of Tie-2 mRNA in placenta among MPE group (1. 33 ±0. 04), SPE group (1.35 ±0. 05) and control group (1.34 ± 0. 04; P > 0. 05). (4) The expressions of Ang-2 protein in placenta of MPE group (2.0 ± 0. 8) and SPE group (2. 0 ± 0. 8) were both significantly lower than that of control group (5.7 ±0. 9; P <0. 0l), while no significant difference was found between MPE group and SPE group (P >0. 05) . (5) In MPE group and SPE group, the serum concentrations of Ang-2 were positively correlated with the levels of Ang-2 mRNA and Ang-2 protein in placenta(r =0. 651, 0. 627; P <0. 01). Conclusions Decreased expressions of Ang-2 mRNA and Ang-2 protein in placenta reduced serum concentration of Ang-2. Low expression of Ang-2 may be involved in the pathophysiological process of preeclampsia by affecting the formation of placenta in early pregnancy.

Objective PSGL-1 is specifically expressed in leucocytes.The aim of this study was to explore the changes of myeloid-derived suppressor cells (MDSCs) in the spleen and bone marrow in PSGL-1-deficient mice.Methods PSGL-1 -/-mice were used in the experiment.After identification of the offsprings, flow cytometry was used to test the expression of CD11b and Gr-1 in C57 and PSGL-1 -/-mice.Results Compared with the C57 mice, the expression of MDSCs was up-regulated in the PSGL-1-deficient mice ( P <0.001).Conclusion The expression of MDSCs is upregulated in PSGl-1-deficient mice.
6.The effect of CPAP treatment in short time on severe OSAHS patients with revised Uvulopalatoph aryngoplasty

OBJECTIVE To investigate the effects of continuous positive airway pressure (CPAP) on patients with severe obstructive sleep apnea hypopnea syndrome (OSAHS) who underwent the revised uvulopalatopharyngoplasty (UPPP). METHODS 18 cases of severe OSAHS patients (therapy group) were selected for CPAP treatment in the 2nd week of postoperation. The average time of CPAP treatment was 34.3 days. The other 20 cases (control group) onlyunderwent operation. All patients were monitored with PSG before operation and in the 6 months postoperatively. RESULTS Compared with the result of pre-operative PSG, apnea hypopnea index (AHI) and Lowest SaO2 (LSaO_2) was improved (P<0.001) in two groups. Compared with control group, LSaO_2 of the therapy group in the sixth month postoperatively was improved (P<0.001), but the decreasing of AHI was not significant (P>0.05). Contrast to the control group, the length of stage 1 and 2 decreased and that of REM increased. (P<0.05). CONCLUSION For severe OSAHS patients who underwent uvulopalatopharyngo plasty, CPAP treatment for one month postoperatively could improve the hypoxia of patients, and prolong REM stage, shorten the stage 1 and 2 sleep. CPAP treatment in short time would be helpful for the severe OSAHS patients with revised UPPP.
7.Influence of hyperbaric oxygen therapy for middle aged and elderly patients with acute cerebral infarction and its effect on M-CSF, ox-LDL and sICAM-1

Objective To study the clinical efficacy of hyperbaric oxygen therapy in the treatment of middle-aged and elderly patients with acute cerebral infarction and their effects on M-CSF,ox-LDL and sICAM-1.Methods A total of 100 elderly patients with acute cerebral infarction who were treated in our hospital were divided into control group and study group according to the random number table method,with 50 cases in each group.The patients in the control group were treated with conventional therapy,and the patients in the study group were treated with hyperbaric oxygen therapy.The levels of serum markers,nerve defect function and cerebral cerebral blood flow (CBF),cerebral blood volume (CBV) were observed and compared between the two groups before and after treatment.Results After treatment,the levels of M-CSF,ox-LDL and sICAM-1 in the two groups were lower than treatment before,and study group was lower than the control group (P < 0.05);The total clinical outcomes was higher in the study group than that in the control group (P < 0.05).After treatment,the CBF and CBV levels of the two groups were higher than treatment before,and the study group was higher than the control group(P <0.05).Conclusion Hyperbaric oxygen therapy in the treatment of the elderly with acute cerebral infarction can effectively reduce the expression of M-CSF,ox-LDL,sICAM-1,reduce cerebral vascular injury,inhibit the formation of thrombosis,improve the function of nerve deficit,relieve the insufficient of oxygen supply,so it is worthy of clinical promotion.

9.Value of albumin in diagnosis of neonatal intrahepatic cholestasis caused by citrin deficiency

Objective To investigate the clinical value of albumin (Alb) in the diagnosis of neonatal intrahepatic cholestasis caused by citrin deficiency (NICCD).Methods A retrospective analysis was performed for the clinical data of 90 children with NICCD who visited Children's Hospital of Fudan University from January 2007 to December 2014,and according to the content of Alb,these children were divided into Alb < 30 g/L (LA) group with 20 children and Alb ≥ 30 g/L (NA) group with 70 children.The clinical manifestations,results of laboratory examination,results of blood tandem mass spectrometry and urine gas chromatographymass spectrometry,and gene detection results were compared between the two groups.The t-test and the chi-square test were used for statistical analysis..Results There were significant differences between the LA group and the NA group in splenomegaly degree (3.28±1.95 cm vs 1.92t1.06 cm,P =0.030),aspartate aminotransferase/alanine aminotransferase ratio [3.15 (0.38-5.93) vs 2.14 (0.26-6.67),P =0.010],activated partial thromboplastin time (53.27±11.68 s vs 45.06±9.79 s,P =0.003),and international normalized ratio (1.92±1.35 vs 1.29±0.33,P=0.001).The SLC25A13 mutation 1851_854de14 was associated with Alb (22 =4.76,P =0.025).Conclusion As for the children with Alb < 30g/L who are highly suspected of having NICCD,SLC25A13 gene detection and blood/urine mass spectrometry should be performed as early as possible,in order to initiate intervention treatment as soon as possible,prevent and treat complications,and improve prognosis.
10.The application of micro-suture technology in laryngeal microsurgery.

OBJECTIVE:
To explore the effect of micro-suture technology in laryngeal surgery.
METHOD:
Sixty-one patients with benign laryngeal disease underwent microsurgery resection under general anesthesia, and the wound surface of the mucosa was sutured intermittently.
RESULT:
The postoperative healing time was shortened, the cicatrix on the vocal cord mucosa was reduced, the voice quality was improved significantly and the recurrence rate was reduced.
CONCLUSION
The micro-suture technology is effective in improving the voice quality and surgery effect significantly.
