1.Eight-step method in clinical research of traditional Chinese medicine.

With the progress and development of science and technology, biomedical technology has been widely used in traditional Chinese medicine (TCM) research. However, TCM and Western medicine developed and came into being at different times and from different cultural backgrounds. Their targets are to study and explore the objective laws of human life activities from different angles and by different means. To study and evaluate TCM simply by the diagnosis and treatment standards of Western medicine cannot truly reflect the characteristics of TCM. Lots of clinical phenomena in the diagnosis and treatment in TCM cannot be revealed scientifically. It might be a significant pattern to combine the study of TCM with Western medicine, based on double-screening model of combining disease with syndrome types, following up evidence-based medical research steps, and making use of systematic review and data mining to analyze the inherent laws of TCM symptoms.

From the thinking to results , we made knowledge of the situation in order to provide references for the further building of clinical research base of TCM .
3.Relevance Research between Traditional Chinese Medicine Syndromes and Chemotherapy and Targeted Therapy on Primary Lung Cancer

This article was aimed to study the relevance between traditional Chinese medicine ( TCM ) syndromes and the treatment of chemotherapy and targeted therapy , in order to provide theoretical support for TCM syndromes of lung cancer in the clinical application . Uniform TCM syndromes questionnaire was used in the TCM syndrome differentiation among lung cancer patients from the Oncology Department of Longhua Hospital, Shanghai University of Traditional Chinese Medicine and TCM Department of Shanghai Pulmonary Hospital. The analysis was made on correlation between distribution rules of TCM syndromes and chemotherapy and targeted therapy . The results showed that among primary lung cancer patients , the most TCM syndromes were syndrome of lung-yin deficiency , lung-q i deficiency , blood stasis in lung collaterals , spleen-q i deficiency , lung-yin deficiency with fire excess . There were certain correlation in TCM syndromes and different TCM therapeutic methods , in which targeted therapy had great significant effect on syndrome of lung-yin deficiency ( P < 0 . 05 ) , after targeted therapy syndrome of lung-yin deficiency increased obviously . It was concluded that there were some regulations in TCM syndromes of lung cancer which take syndrome of lung-yin deficiency, lung-qi deficiency, blood stasis in lung collaterals, spleen-qi deficiency, lung-yin deficiency with fire excess. There were certain correlation in TCM syndromes and different TCM therapeutic methods , after targeted therapy syndrome of lung-yin deficiency increased obviously .
4.A prospective comparison cohort study between baseline serum uric acid level and coronary artery disease in first-degree relatives and non-first-degree relatives of type 2 diabetes males

Objective To investigate the relationship between baseline serum uric acid and the severity of coronary artery disease ( CAD ) in the first-degree relatives or non-first-degree relatives of men with type 2 diabetes. Methods Three hundred and eighty-one men with negative coronary angiography for the first time were divided into diabetes and non-diabetes groups and followed-up for 5 years. The primary outcome was acute coronary syndrome suspected during subsequent 5 years, and the coronary angiography was conducted simultaneously. The severity of CAD was assessed by the coronary stenosis index ( CSI) and the number of coronary lesion vessels. Results In normal blood glucose group, serum uric acid was higher in the first-degree relatives of diabetics compared with non-first-degree relatives(P<0. 01), along with higher morbidity of CAD, CSI, and coronary lesion vessels (all P<0.01). Correlation analysis showed that CSI(r=0. 250, P=0. 041) and coronary lesion vessels(r=0. 252, P=0. 040) in non-diabetics group were associated with baseline levels of serum uric acid. Conclusion The elevation of serum uric acid was closely related to subsequent CAD, especially in first-degree relatives of male with type 2 diabetes, which could be used as an early indicator for CAD prediction.
5.Traditional Chinese Medicine Decoction Combined Bisphosphonates in Treatment of Bone Metastasis of Malignant Tumors:A Systematic Reviews and Meta-analysis

This study was aimed to systematically appraise the effectiveness and safety of traditional Chinese medicine (TCM) decoction in combination with bisphosphonates in treatment of bone metastasis of malignant tumors. Related literatures were searched in the PubMed, Embase, CBM, CNKI and Cochrane Databases. The endpoints included cancer pain, activity of daily living, sclerotin reparation, KPS and adverse events. The Stata 11.0 software was used in the statistical analysis. The results showed that a total of 19 articles involving 1080 patients were included. It showed that TCM in combination with bisphosphonates can significantly relieve pain (RR=0.8079,95%CI =0.754-0.85,P <0.001), improve the activity of daily living (RR =0.787,95%CI =0.69-0.897,P <0.001), promote sclerotin reparation (RR =0.71,95%CI =0.52-0.96,P =0.026), and improve KPS (RR=0.7268,95%CI =0.61-0.85,0.58-0.81,P <0.001). It was concluded that TCM decoction in combination with bisphosphonates can significantly improve its therapeutic effect as well as improve the patients’ quality of life.
6.Advances in research on genetic susceptibility to silicosis

Occupational silicosis (hereinafter referred to as "silicosis") exhibited individual differences in disease susceptibility, with genetic factors playing a crucial role in its onset and progression. Cytokines, such as interleukin (IL)-1RA +2018T>C locus, tumor necrosis factor-α -308G>A and -238G>A locus, transforming growth factor (TGF)-β1 +915G>C locus, were related to the development of silicosis. However, relationship between IL-17F +7488A>G and TGF-β1 -509T>C locus with silicosis had shown inconsistent results across different studies. Regulatory proteins such as matrix metalloproteinase (MMP)-2 -735C>T locus, MMP-9 rs3918242 locus, heat shock protein (HSP) 70-1+190G>C locus, carboxypeptidase M rs12812500 locus, family sequence similarity gene 13A (FAM13A) rs2609255 locus, and desmoplakin rs2076304 locus were also related to the development of silicosis. The A allele of the non-coding RNA miRNA-4508 rs6576457 increased the risk of developing silicosis-related pulmonary fibrosis in dust-exposed workers. The polymorphism in the long non-coding RNA ADGRG3 rs1814521 was related to silicosis susceptibility. Additionally, six circular RNAs of small nucleolar RNA host gene 14 rs17115143 sequence might be potential biomarkers of silicosis. Human leukocyte antigen-DR (HLA-DR) genes demonstrated a dual role in both risk and protection against silicosis, while angiotensin I-converting enzyme (ACE) gene polymorphisms likely affected silicosis development by modulating serum ACE activity. However, the mechanisms by which certain genetic variations affected susceptibility to silicosis remain unclear. Prospective studies with large-scale samples combining genetics, epidemiology, bioinformatics, and biofunctional studies are needed to promote the development of biomarkers for silicosis susceptibility and disease course, and clinical therapies.
7.Chemotherapy in conjunction with traditional Chinese medicine for survival of elderly patients with advanced non-small-cell lung cancer: protocol for a randomized double-blind controlled trial.

Traditional Chinese medicine (TCM) is considered an important complementary therapy with beneficial effects for cancer patients. Elderly patients with non-small-cell lung cancer (NSCLC) are a complex patient group with increasing co-morbidity and shrinking physiological reserve, and may derive substantial benefit from the supportive aspects of TCM. Researchers from Shanghai Longhua Hospital found that qi and yin deficiency is a common syndrome in patients with stage III or IV lung cancer. This project was designed to study the combination of single-agent chemotherapy with TCM methods of benefiting qi and yin in elderly patients with advanced NSCLC.
8.The correlation between serum human β-defensin 3 and interleukin-12 levels and postoperative infection in hip replacement patients with femoral neck fractures

Objective:To explore the relationship between periprosthetic infection and preoperative serum levels of human β-defensin 3 (HBd-3) and interleukin-12 (IL-12) in patients with femoral neck fracture after hip replacement surgery.Methods:A prospective study was conducted to 268 patients with femoral neck fracture who underwent hip replacement surgery at Chengde Central Hospital from April 2020 to April 2022. They were divided into an infected group and an uninfected group based on whether they had periprosthetic infection within 2 weeks after surgery. The baseline data and preoperative laboratory indicators of the two groups were compared, and the relationship between preoperative serum HBd-3 and IL-12 levels and postoperative periprosthetic infection in patients was mainly analyzed.Results:Seventeen out of 268 patients with femoral neck fractures experienced periprosthetic infections after hip replacement surgery, with an infection rate of 6.34%(17/268). The body mass index, operation time, postoperative drainage time and IL-12 in the infected group were higher than those in the uninfected group: (23.75 ± 2.18) kg/m 2 vs. (21.86 ± 1.95) kg/m 2, (124.82 ± 15.93) min vs. (116.92 ± 14.75) min, (57.41±7.81) min vs. (49.65 ± 5.73) min, (64.82 ± 11.59) ng/L vs. (54.23 ± 9.55) ng/L, the preoperative albumin and HBd-3 were lower than those in the uninfected group: (32.56±4.63) g/L vs. (37.25 ± 5.81) g/L, (38.03 ± 6.74) μg/L vs. (46.84 ± 8.93) μg/L, and the differences were statistically significant ( P<0.05). The results of Logistic regression analysis showed that high body mass index, long surgical and postoperative drainage time, and high preoperative IL-12 levels were risk factors for periprosthetic infection after hip replacement surgery in patients with femoral neck fractures ( OR>1, P<0.05), while preoperative serum albumin and HBd-3 levels were protective factors ( OR<1, P<0.05). Receiver operating characteristic curve results showed that preoperative serum HBd-3 and IL-12 alone and in combination predict AUC>0.70 for periprosthetic infection in patients with femoral neck fracture after hip replacement surgery, which had certain predictive value. The restricted cubic spline method combining spline function and Logistic regression was used for analysis, and the results showed a linear dose-response relationship between the levels of HBd-3 and IL-12 and the strength of the association with periprosthetic infection after hip replacement surgery in patients with femoral neck fracture ( P<0.05). Conclusions:Preoperative serum HBd-3 and IL-12 levels are associated with periprosthetic infection in patients with femoral neck fracture after hip replacement surgery, and can serve as predictive indicators for infection occurrence.
9.A severe acute intermittent porphyria patient: Successful treatment and management

Acute intermittent porphyria(AIP) is a rare inherited metabolic disease that can cause severe and fatal acute attacks. This article shares the treatment and management of a severe AIP patient. It is proposed that (1) avoiding incentives is essential; (2) emotional problems easily overlooked should be paid attention; (3) long-term follow-up and patient education can improve the prognosis. The patient underwent renal biopsy during the remission period. We found a red-brown-yellow-white refractive index crystal under a polarized light microscope that had not been reported in the previous literature, which was speculated to be a porphyrin crystal.
10.Status Quo Analysis of Outcome Indexes and Evaluation Tool in Treatment of Skin Rash Caused by EGFR-TKI with Traditional Chinese Medicine

Objective To analyze the outcome indexes selected in the randomized controlled trials(RCTs)on traditional Chinese medicine(TCM)for skin rash caused by EGFR-TKI.Methods Seven databases were researched and the literature was screened according to the inclusion and exclusion criteria,then summarize and categorize outcome indexes to calculate frequency of outcomes and analyze.Results 46 out of 2241 papers were included.The outcome indexes of 40 RCTs were mainly divided into seven categories,that is physical symptoms/signs(37.97%),quality of life(17.65%),safety indicators(13.37%),traditional Chinese medicine symptoms/syndromes(16.58%),long-term prognosis(1.60%),blood biochemical index(9.09%)and others(3.74%).Problems as follow:First,there is a high risk of clinical trial bias in the treatment of EGFR-TKI-associated rash with TCM.Besides,the measurement time points was largely different with the severity of rash not considered.What's more,oversimplify the reports of compound outcome indicators and safety evaluation,lack of treatment time window and full process observation.Last,grading criteria for rash were inconsistent and the function of quality of life assessment tool was relatively simple.Conclusion At present,the use of outcome indicators has not been standardized in the RCTs for skin rash related EGFR-TKI by TCM,and relevant construction work should be carried out in the future to build a core index set with the characteristics of TCM treatment.
