1.Review of medical surge capacity for catastrophic events

Following an introduction to the concept of medical surge capacity against catastrophic events,the paper made a systematic examination of the studies on the definition and classification of the capacity,surge capacity assessment,and surge capacity planning from the aspects of management science.On this basis,the authors call for medical surge capacity optimization and resource allocation study,in light of the guideline of multi-disciplinary study,development and evolution principles of such events,as well as the operation management thinking and mathematical modeling approach.These efforts will enable medical institutions in their emergency means against such events.
2.Central role of GRP78 in growth of gastric carcinoma cells

AIM:To explore the effect of glucose-regulated protein 78 (GRP78) on the gastric carcinogenesis. METHODS:GPR78 expression patterns were examined in 34 specimens from gastric carcinoma patients using the immu-nohistochemistry (IHC) assay, and in 10 specimens using Western blotting analysis .In addition, the expression of GPR78 and cyclin D1 was detected in human gastric cancer cell lines SGC 7901 and SGC7901-H78 (overexpressing GRP78) by Western blotting.RESULTS:By IHC assay, GRP78 was found to be highly expressed in the cytoplasm of gastric carcino-mas as compared with the adjacent non-malignant tissues and corresponding normal tissues .GRP78 expression was positive-ly correlated with gender and histological differentiation (P<0.05), but not with age, tumor stage and lymph node metas-tasis (P>0.05).Furthermore, we found that with the increased expression of GRP 78 in SGC7901-H78 cells, the expres-sion of cyclin D1 was also elevated .CONCLUSION:GRP78 might be a key player to be involved in the growth of gastric cancer.
3.Study on relationship between changes of DNA content in human spleen nuclei and the postmortem interval by image analysis

Objective To study changes of DNA content in human spleen nuclei and seek an experimental method for estimation of the postmortem interval(PMI)using computerized image-analysis technique(CIAT).Methods Smear sections from spleen sampled were collected in 36 cadavers with known the accurate PMI respectively every hour within the first 36 hours after death,which were then fixed with cold Carony fixation.The smeared sections were stained by Feulgen-van's staining method.3 indices for spleen nucleic DNA including integral optical density(IOD),average optical density(AOD)and average gray(AG)were measured using the CIAT.Results IOD and AOD in the spleen nuclei declined regularly,whereas AG increased with the extension of PMIs in 36 hours.Conclusion There are definite relationships between the PMI and gray parameters(IOD、AOD and AG)representing the DAN content of nucleic DNA in the spleen in 36 hours after death,which may be used for estimation of PMI.
4.Construction of mixed ability in eight-year program clinical teaching of obstetrics and gynecology

Huazhong University of Science and Technology is one of the five schools that trial implementing eight-year program medical education of first batch.Eight-year program medical students in Tongji college came into obstetrics and gynecology clinical contact in 2010.According to the characteristics of students and discipline,obstetrics and gynecology department used multiple teaching methods and means as PBL teaching,integrated case management,typical clinical case conference,clinical diagnosis and treatment,academic lecture,research report,experimental design,scientific research and experi-ment,etc.Available practice mode was reformed.Those students accomplished clinical practice,and achieved expected effect.
5.Enhanced bioavailability of total paeony glycoside by self-microemulsifying drug delivery system.

Total paeony glycoside (TPG) is extracted and purified from a traditional Chinese herbal medicine. It has many biological and pharmacological activities. However, there are few dosage forms of TPG in the market because of its low bioavailability. Self-microemulsifying drug delivery system (SMEDDS) is a vital tool in solving low bioavailability of poor absorption drugs. So the objective of this study is to develop a new TPG-SMEDDS for the oral delivery of poorly soluble TPG. Through the construction of pseudo-ternary phase diagrams, the optimum prescription was obtained, which consisted of 18.70% TPG, 16.27% ethyl oleate as oil, 43.34% Cremophor RH40 as surfactant and 21.73% Transcutol P as cosurfactant. The characterizations of TPG-SMEDDS including morphological characterization, droplet size, zeta-potential, emulsification time, and dissolution study of TPG-SMEDDS were evaluated. The results showed that TPG-SMEDDS is stable and its release rate is high in four different media (0.1 mol x L(-1) HCl, pH 6.8 PBS, pH 7.4 PBS, and water). The relative bioavailability of SMEDDS was dramatically enhanced in an average of 1.52-fold that of TPG-suspension. It is concluded that the bioavailability of TPG is enhanced greatly by SMEDDS.
6.Effect of Acupuncture Combined with Low Frequency Electrical Stimulation on Cerebral Palsy Salivation

Objective To explore the effect of oral sensorimotor therapy (OSMT), acupuncture, and low frequency electrical stimulation on salivation in patients with cerebral palsy. Methods From September, 2014 to October, 2015, 80 cerebral palsy patients with salivation were recruited. They were randomly divided into four treatment groups:group A was treated with OSMT, group B was treated with acupunc-ture, group C was treated with frequency electrical stimulation, and group D was treated with acupuncture and low frequency electrical stim-ulation, for eight weeks. Meanwhile, 20 cerebral palsy patients with salivation from outpatients were chosen as control group. Results There was no improvement in the root mean square (RMS) value of surface electromyography (sEMG) after treatment in both the control group and group A (t<1.668, P>0.05), and the RMS value significantly improved in the groups B, C and D (t>8.983, P<0.001). There was no sig-nificant difference in the salivation score and RMS value between the control group and group A (W=389.5, t=2.041, P>0.05), and they was better in the groups B, C and D than in the control group and group A (W>226.0, t>8.534, P<0.01), as well as in group D than in groups B and C (W>306.0, t>3.663, P<0.05). Conclusion Acupuncture and low frequency electrical stimulation could effectively improve salivation in children with cerebral palsy, and their combination was superior to either ones, while OSMT did not show obvious benefit.
7.The relationship between the resection extent and number of lymph node dissection and the effect of metastatic lymph node number on the prognosis of proximal gastric cancer

Objective To study the relation between the surgical extent and number of lymph node dissection and the effect of metastatic lymph node number on the prognosis of proximal gastric cancer. Method Thirty-one patients with proximal gastric cancer underwent D_2 or D_3 operation, and lymph node were harvested from specimen. The number of lymph node dissection in proximal gastrectomy was compared with that in total gastrectomy. Results A total of 1971 lymph nodes were obtained from the 31 specimens, the mean was 63 per case. There were 57 lymph nodes per case for patients with proximal gastrectomy and 71 per case with total gastrectomy,64 per case with left half pancreaticosplenectomy and 63 per case with splenectomy. According to the new 5th N stage system, the 5 year survival of N_1,N_2,N_3 were 36%, 11%, and 0 respectively. Conclusion Along with the wide invasion of the carcinoma wide resection with extended lymph node dissection is mandatory, reserved left half of the pancreas has no influence on the number of harvested lymphnode, the new quantitative N staging is superior to the old in predicting the prognosis.
8.Effect of Electromyographic Biofeedback on Pointed Foot in Children with Spastic Cerebral Palsy

Objective To explore the clinical curative effect of electromyographic biofeedback on pointed foot in children with spastic cerebral palsy. Methods From June, 2014 to December, 2015, 80 children with spastic cerebral palsy and pointed foot were divided into con-ventional group (n=40) and electromyographic biofeedback group (n=40). The conventional group received exercise and massage, while the electromyographic biofeedback group received electromyographic biofeedback in addition. The passive range of motion (PROM) of ankle, the surface electromyographic results and the selective control results were compared before and eight weeks after treatment. Results After treatment, the PROM significantly decreased in both groups (t>9.142, P<0.001);the integrated electromyography and root mean square, and the selective control increased in both groups (t>3.456, Z>3.178, P<0.01), which were better in the electromyographic biofeedback group than in the conventional group (t>3.737, Z=-2.748, P<0.01). Conclusion Electromyographic biofeedback can further improve the foot dor-sal flexure and pointed foot gait of spastic cerebral palsy children.
9.Prognostic significance of the number of lymph nodes removed surgically in patients with colorectal cancer

ResultsWT5”BZ Analysis revealed that among node-negative patients, the 5-yerar's survival rate was 23% higher in patients with more than 50 nodes removed than those with less than 50. For node-positive patients, the difference of 5-year′s survival rate between the related two subgroups reached 36%.WT5”HZConclusion The number of lymph node resected has great impact on the long term survival of patients with colorectal cancer.
10.Surveillance on MIC of Antibiotic Resistance of Meticillin-resistant Staphylococcus aureus

OBJECTIVE To review and analyze the change in the MICs of vancomycin,teicoplanin and linezolid in meticillin-resistant Staphylococcus aureus(MRSA) strains isolated in our hospital from 2003 to 2007. METHODS The MICs of vancomycin,teicoplanin and linezolid were tested by Etest method on a sample of randomly selected MRSA strains. RESULTS The incidences of MRSA increased from 52.2% in 2003 to 74.5% in 2007.MIC of vancomycin increased from 1.85 ?g/ml in 2003 to 2.15 ?g/ml in 2007,and teicoplanin MIC geometric mean increased even more markedly from 1.28 ?g/ml in 2003 to 2.07 ?g/ml in 2007.The linezolid MIC remained almost unchanged. CONCLUSIONS The incidences of MRSA were increasing from 2003 to 2007.There is a upward trend in MIC of glycopeptide over the years,in which the increase for teicoplanin is higher than others two.
