1.Clinical Observation of Montelukast Combined with Budesonide in the Treatment of Bronchial Asthma in Children

OBJECTIVE:To investigate clinical efficacy and safety of montelukast combined with budesonide in the treatment of bronchial asthma(BA),and it effects on clinical symptom,inflammatory factor and immune function of BA children. METHODS:To-tally 106 BA children selected from pediatrics department of our hospital during Jan. 2014-Dec. 2015 were divided into observation group and control group according to random number table,with 53 cases in each group. Based on routine treatment,control group was given Budesonide suspension for inhalation 400μg,bid. Observation group was additionally given Montelukast sodium tablets oral-ly,4 mg for under 5 years old,qd,5 mg for 5 years old or older,qd,on the basis of control group. Treatment courses of 2 groups last-ed for 8 weeks. Clinical efficacies of 2 groups were compared as well as clinical symptom scores,inflammatory factor and immune fac-tor levels before and after treatment. The occurrence of ADR was also compared between 2 groups. RESULTS:The total response rate of observation group(94.34%)was significantly higher than that of control group(77.36%);the symptom relief time and lung signs disappearance time were significantly shorter than control group,with statistical significance(P<0.05). Before treatment,there was no statistical significance in clinical symptom scores,inflammatory factor and immune factor levels between 2 groups(P>0.05). After treatment,daytime and nighttime symptom scores,the levels of IL-6,IL-8,TNF-αand IgE in 2 groups were significantly decreased, while the levels of IgA and IgG were increased significantly;the observation group was significantly better than control group,with sta-tistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(5.66% vs. 7.55%) (P<0.05). CONCLUSIONS:Montelukast combined with budesonide help to reduce the level of serum inflammatory cytokines in BA children,improve immune function and clinical symptoms with good safety.
2.Comparative analysis of the core competence between the upgraded and the full-time bachelor nurses

Objective To provide the effective references for reasonable arrangement of nursing post by comparison on the core competence level and characteristics between the upgraded and the fulltime bachelor nurses.Methods Questionnaire survey based on the basic demographic data and the Competency Inventory for Chinese Registered Nurse for 391 clinical nurses from 13 three level of firstclass hospitals.Results The core competence score of the upgraded and the full-time bachelor nurses showed a moderate degree of satisfaction; the full-time bachelor nurses' total scores were higher than those of the upgraded nurses,and there were statistically significant differences among 6 dimensions included critical thinking and scientific research ability; there was no significant difference in clinical nursing dimension.Conclusions The full-time bachelor nurses' core competence is higher than that of the upgraded nurses in general,but no significant difference exists in clinical nursing dimension.
3.The Audiological Characteristics of Neonate with Severe Hyperbilirubinemia Need to be Exchanged Transfusion

Objective To study the change of audiological characteristics of neonate with severe hyperbilirubi‐nemia need to be exchanged transfusion ,and to explore the correlation between the peak concentrations of serum to‐tal bilirubin and hearing loss .Methods A total of 130 cases of neonate diagnosed with severe hyperbilirubinemia need to be exchanged transfusion were included in this study .Hearing tests of auditory brainstem response (ABR) , distortion product otoacoustic emission (DPOAE) and acoustic immittance were administered to these neonates and they were followed up three months old .They were divided into the normal group ,the mild - moderate group and the severe - extreme group according to the hearing tests results of three months .The peak concentrations of ser‐um total bilirubin and hearing condition among the three groups were analyzed .Results There were 85 neonates with normal hearing (65 .38% ,85/130) ,45 with hearing loss (33 .85% ,45/130 .For 88 ears ,there were 2 cases of single ear and 43 cases of both ears) ,including 11 cases of mild - moderate group (22 ears ,24 .44% ,11/45) and 34 cases with severe - extreme group (66 ears ,75 .56% ,34/45) .There were 16 neonates (32 ears) met the crite‐rion of auditory neuropathy (12 .31% ,16/130) .The difference of the peak concentrations of serum total bilirubin a‐mong the three groups was statistically significant (F=16 .525 ,P=16 .525) .And positive correlation was observed between ABR threshold and peak serum total bilirubin concentration (r=0 .584 ,P<0 .001) .Conclusion The ma‐jor feature of the neonate with severe hyperbilirubinemia need to be exchanged transfusion was extremely severe sen‐sorineural hearing loss in both ears ;and the higher probability of serious hearing loss with the higher peak concen‐trations of serum total bilirubin .
4.Changes of the cardiac morphology and size after transcatheter closure of atrial septal defect

Objective To evaluate the changes of the right atrium,right ventriculum,left atrium and left ventriculum after transcatheter closure of atrial septal defect(ASD) during a short to mid-term follow-up.Methods The right ventricular end-diastolic anterior-posterior diameter(RVEDD),right atrial long diameter(RADl),right atrial transverse diameter(RADt),left ventricular end-diastolic ante-posterior diameter(LVEDD),left ventricular end-diastolic volume(LVEDV) and left atrial anterior-posterior diameter(LAD) in 36 patients with secundum ASD were measured before ASD closure,after 3 days,3 months and 6 months of ASD closure with transthoracic echocardiography(TTE).Results RVEDD,RADl and RADt were significantly decreased,while LVEDD,LVEDV and LAD significantly increased 3 days after ASD closure.During 3 months follow-up,RVEDD,RADl and RADt continuously became smaller;LVEDD,LVEDV and LAD continuously became larger.At 6 months,RVEDD was significantly smaller and LVEDD,LVEDV were significantly larger than those at 3 months.No remarkable difference of RADl,RADt and LAD was found between 6 months and 3months follow-up.Conclusion Transcatheter closure of ASD not only decreases the preload of right heart and causes right atrium and right ventriculum become smaller,but also improves the geometry of left heart and causes the narrowed left atrium and left ventriculum gradually return to almost normal status.
5.Clinical findings of a group of children with auditory neuropathy spectrum disorder.

OBJECTIVE:
In order to extend the knowledge of auditory neuropathy spectrum disorder (ANSD), the clinical data of a group of infants and young children with this disorder was reported.
METHOD:
This retrospective study involved 84 cases (151 ears). The subjects aged from 2 months to 6 years old. Descriptive analysis was carried out on case history, audiological and imaging results and the outcome of cochlear implantation.
RESULT:
There were a variety of case history, including 11 of hyperbilirubinemia, 2 of family history of deafness, 3 of hypoxia, 1 of prematurity, 1 of nuclei basales pathology, 2 of congenital atelencephalia,and 13 of cochlear nerve deficiency (CND). The audiological results varied too. Click ABR ranged from no response at the maximum output level to Wave V thresholds as low as 70 dB nHL. Behavioral thresholds were mild in 1, moderate in 2 and profound in 20. The inner ear MRI were normal in 16 cases, CND in 12 and CND accompanied by periventricular leukomalacia in 1. 5 cases were implanted in the affected ear. The results in 3 were comparable with that of cochlear loss, 1 were less effective than that of cochlear loss, 1 became none user of the device.
CONCLUSION
ANSD is a heterogeneous group with a wide variety on case history, audiological and imaging results and the outcome of cochlear implant. Therefore, comprehensive clinical examinations are necessary so that individualized intervention program can be made.

6.The normal values of multiple-frequency tympanometry in normal newborns.

OBJECTIVE:
To discuss the multiple-frequency probe tones tympanograms and the normal ranges of admittance, susceptance and conductance in normal newborns.
METHOD:
Tympanometries with 226 Hz, 678 Hz and 1000 Hz probe tones were obtained from newborn infants with normal TEOAE and DPOAE(55 infants, 110 ears), analysed multiple-frequency prone tones tympanograms and values of admittance, susceptance and conductance by using GSI-33 middle ear analyzer.
RESULT:
226 Hz tympanograms for admittance, susceptance and conductance main were W-shaped, the percentages were 90.0%, 99.1% and 85.5%. 678 Hz tympanograms for admittance, susceptance and conductance main were single-peaked, the percentages of single-peaked type were 62.7%, 77.3% and 62.7%, and the percentages of W-shaped type were 34.6%, 20.9% and 31.8%, had a little three-peaked type. 1000 Hz tympanograms for admittance, susceptance and conductance main were single-peaked, and percentages were 96.4%, 99.1% and 97.3%, and had a little W-shape type but no three-peaked type. There were significant differences between admittance and susceptance, susceptance and conductance in 226 Hz and 678 Hz probe tones tympanogram. There were significant differences between admittance and susceptance, susceptance and conductance, admittance and conductance in 1000 Hz probe tones tympanogram.
CONCLUSION
Multiple-frequency probe tones tympanograms and the normal ranges of admittance, susceptance and conductance of normal newborn infants is obtained, and 1000 Hz probe tone tympanometry is a sensitive test for function of middle ear in newborn infants.

7.The effect of ERAS on VAS score and serum immunoglobulin in children undergoing low-temperature plasma radiofrequency tonsillectomy

Objective:To explore the effects of accelerated rehabilitation surgery (ERAS) on visual analogue scale (VAS) and serum immunoglobulin in children undergoing low-temperature plasma radiofrequency tonsillectomy.Methods:A prospective study was conducted on 200 children who underwent bilateral tonsillectomy with low-temperature plasma radiofrequency from January 2022 to April 2023 at the Sixth Affiliated Hospital of South China University of Technology. They were randomly divided into an observation group ( n=100) and a control group ( n=100) using a random number table method. The control group received traditional perioperative treatment, while the observation group received perioperative treatment under ERAS mode. We compared the postoperative conditions, pain VAS at different time points, and incidence of complications between two groups, as well as serum immunoglobulins and inflammatory markers before and 2 days after surgery. Results:The observation group had shorter postoperative pseudo film detachment time time, white membrane formation time, dietary recovery time, and hospital stay than the control group (all P<0.05); The VAS of the observation group was lower than that of the control group at different time points after surgery (all P<0.05); Two days after surgery, the levels of immunoglobulin A (IgA), immunoglobulin G (IgG), and immunoglobulin M (IgM) in both groups decreased, with the observation group showing a smaller decrease than the control group (all P<0.05); Two days after surgery, the levels of interleukin-8 (IL-8), interleukin-6 (IL-6), and C-reactive protein (CRP) significantly increased in both groups, with the observation group showing a lower increase than the control group (all P<0.05); The total incidence of complications in the observation group was 5.00%(5/100), significantly lower than the control group′s 13.00%(13/100, P<0.05). Conclusions:After undergoing ERAS intervention during the perioperative period, children undergoing low-temperature plasma tonsillectomy can accelerate their recovery process, shorten hospital stay, reduce pain VAS and inflammatory indicators, and reduce the impact on their serum immunoglobulin levels.
