1.Clinical Observation of Montelukast Combined with Budesonide in the Treatment of Bronchial Asthma in Children

OBJECTIVE:To investigate clinical efficacy and safety of montelukast combined with budesonide in the treatment of bronchial asthma(BA),and it effects on clinical symptom,inflammatory factor and immune function of BA children. METHODS:To-tally 106 BA children selected from pediatrics department of our hospital during Jan. 2014-Dec. 2015 were divided into observation group and control group according to random number table,with 53 cases in each group. Based on routine treatment,control group was given Budesonide suspension for inhalation 400μg,bid. Observation group was additionally given Montelukast sodium tablets oral-ly,4 mg for under 5 years old,qd,5 mg for 5 years old or older,qd,on the basis of control group. Treatment courses of 2 groups last-ed for 8 weeks. Clinical efficacies of 2 groups were compared as well as clinical symptom scores,inflammatory factor and immune fac-tor levels before and after treatment. The occurrence of ADR was also compared between 2 groups. RESULTS:The total response rate of observation group(94.34%)was significantly higher than that of control group(77.36%);the symptom relief time and lung signs disappearance time were significantly shorter than control group,with statistical significance(P<0.05). Before treatment,there was no statistical significance in clinical symptom scores,inflammatory factor and immune factor levels between 2 groups(P>0.05). After treatment,daytime and nighttime symptom scores,the levels of IL-6,IL-8,TNF-αand IgE in 2 groups were significantly decreased, while the levels of IgA and IgG were increased significantly;the observation group was significantly better than control group,with sta-tistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(5.66% vs. 7.55%) (P<0.05). CONCLUSIONS:Montelukast combined with budesonide help to reduce the level of serum inflammatory cytokines in BA children,improve immune function and clinical symptoms with good safety.

4.Features of coronary angiography in type 2 diabetic patients combining various risk factors

BACKGROUND:With the increasing morbidity of type 2 diabetes, people should pay more attention to the early intervention of cardiovascular complications in order to reduce the incidence of its complications. OBJECTIVE:To observe the risk factors of type 2 diabetes combining coronary heart disease(CHD) and the images of coronary angiography. DESIGN:A cross-sectional observational study. SETTING:Internal medicine department of a municipal hospital. PARTICIPANTS:The study was completed in the Department of Endocrine Harbin First Hospital from May 1998 to May 2003.Inclusive criteria:Type 2 diabetic patients aged above 30 years old and combined with CHD with either sex, who met the diagnostic standard of diabetes set by ADA in 1997 and Naming and Diagnostic standard of ischemic heart disease set by WHO in 1979. Exclusive criteria:Patients suffering from severe heart,liver,kidney disease and serious infection,rheumatoid heart disease,pulmonary heart disease and so on.There were 98 inpatients, who met the above mentioned inclusive criteria, were set as the diabetic group with 56 males and 42 females,aged from 35 to 70 years old;and 85 CHD patients without combining diabetes admitted to hospital at the same time were chosen as the control group with 53 males and 32 females, aged from 40 to 75 years old. INTERVENTIONS:The blood glucose was observed with the glucose oxidase method,fasting insulin and C peptide with radioimmunoassay (RIA),glycosylated hemoglobin with affinity chromatography, triglyceride(TG),total cholesterol(TC),high density lipoprotein-cholesterol(HDL-C) and low density lipoprotein-cholesterol (LDL-C) with automatic biochemical analysis,and selective right coronary angiography was applied by JUDKINS method. Of coronary arteries. RESULTS:The comparison of the coronary angiographic images between the two groups showed that there was no significant difference in the lesion of left main trunk and circumflex artery between the diabetic group and control group(P >0.05);While there were more cases with lesion of anterior descending artery and right coronary artery in the diabetic group than in the control group(P< 0.05). The pathological changes in the diabetic group mainly manifested with lesion of three branches and single branch in which represented diffusive changes.Lesion of three branches was the main pathological change of coronary artery in type 2 diabetic patients with diffusive changes. CONCLUSION:With the extension of course of type 2 diabetes in patients,the incidence of CHD will gradually increase with extensive pathological changes and much severe conditions.
5.The comparistion of application of three adhesive membrane in cesarean section

Objective To investigate the effect of adhesive membrane that has anti-side leakage and can be drained during the operation of cegarean section.Methods 300 cageg of lying-in women with cessrean section were randomly divided into three groups,the test 1 group,test 2 group and the control group.The test 1 group used adhesive membrane specially adopted in cerebral surgery,the test 2 group used the"U"shape adhesive membrane that had anti-side leakage and could be drained during the operation,the control group used the routine adhesive membrane,and then the contamination by amniotic fluid and blood of the thine groups was observed.Resulls Compared with test 1 group and the control group,the pollution by amniofic fluid and blood was significanfly decreased in the test 2 group,P<0.01.Conclusiom This modified designed"U"shape adhesive membrane can drain amniotic fluid and blood during cesarean section in time and simply.
6.Effect of Thunder-fire Moxibustion on Blood Lipid and Hemorheology in Patients with Coronary Heart Disease Undergone Interventional Treatment

Objective To observe the effect of moxibustion on the blood lipid and hemorheology in senile patients with coronary heart disease (CHD) due to heart blood stagnation who underwent interventional treatment.Method A hundred senile CHD patients were randomized into a treatment group and a control group, 50 cases in each group. The control group was intervened by conventional medications, while the treatment group was additionally by thunder-fire moxibustion. The blood lipid and hemorheology were observed before the treatment and after 3 treatment courses.Result The blood lipid parameters (including CHO, TG, HDL-C and LDL-C) were changed significantly in the treatment group after the intervention (P<0.05). The CHO, TG, and LDL-C levels of the treatment group were significantly different from that of the control group (P<0.05). The hemorheology parameters (whole blood viscosities at a high-, middle-, and low-shear rate, and fibrinogen) were significantly changed in the treatment group after the intervention (P<0.05). The whole blood viscosities at a high-, middle-, and low-shear of the treatment group were markedly different from that of the control group (P<0.05).Conclusion Thunder-fire moxibustion can down-regulate the blood lipid and blood viscosity of CHD patients undergone interventional treatment, and thus improve myocardial ischemia.
7.Twist regulation of EMT and its clinical significance in monitoring circulating tumor cells and evaluating effects of anticancer drugs

Tumor cell plasticity, including epithelial to mesenchymal transition (EMT) and its reverse program, mesenchymal to epithe-lial transition (MET), regulates circulating tumor cells and carcinoma metastasis. Twist is overexpressed in rhabdomyosarcoma, breast cancer, gastric cancer, and other tumors. Twist, as a transcriptional factor, cross-talks with multiple signaling pathways, forming a com-plex network to participate in the regulation of EMT/MET in circulating tumor cells, which in turn promotes metastasis of tumor cells. Therefore, monitoring the level of Twist and epithelial–mesenchymal phenotypic molecules is important as it may be beneficial for in-creasing the detection ratio of circulating tumor cells as tumor biomarkers and for evaluating the effects of anticancer drugs.
8.“Diffusion index”used to evaluate the prognosis of ARDS

Objective To propose the concept of “Diffusion Index”to replace Oxygenation Index as independent indicators to evaluate prognosis on ARDS patients under mechanical ventilation treatments,and comparison carried out between them preliminarily.Methods Calculation of “1 000 × (PaO2 /FiO2 /PEEP)”was taken as “Diffusion Index”.A total of 130 ARDS patients under mechanical ventilation support (MVS)were admitted to Peking Union Medical College Hospital ICU from July 2013 to July 2014.The data of these patients were retrospectively analyzed.Of them,15 patients were excluded because these patients refused invasive ventilation support.Respirator parameter setting and haemogas figures were recorded accordingly. Both Diffusion Index and Oxygenation Index were used separately to predict detachment of MVS from patients in 28 days,and the correlation between these two indexes and ARDS prognosis were determined.Results According to the outcomes of patients in 28 days,patients were divided into 3 groups:detached group (n =44),failed to detach group (n =14)and death group (n =57).There was obvious difference in trend diagrams observed among three groups between diffusion index and oxygenation index.COX regression analysis of survival curve demonstrated that if Diffusion Index kept greater than 405.8,probability of detachment of MVS from patients was higher and the correlation was significant (P =0.009 ).Conclusions Compared with Oxygenation Index,“Diffusion Index” is a comprehensive indicator for ARDS prognosis with better reliability and validity.
9.Incision Infection after Abdominal Operation:Investigation and Strategy

OBJECTIVE To analyze the high incision infection rate in abdominal operation in order to take effective preventive measures.METHODS Totally 2302 cases of abdominal operation in general surgery were investigated retrospectively from Jan 2005 to May 2007.The diagnosis standard was based on the Diagnosis Standard of Hospital Infection(Draft) published by Ministry of Health of the People′s Republic of China in Jan 2001.RESULTS From them 113 cases suffered incision infection.The infection rate was 4.85%,accounted for 81.54% of all surgical operation incision infections.The pathogenic bacteria in the samples taken during the operation were the same as those in the secretion of postoperative incision in terms of species and source.CONCLUSIONS The key points of prevention of incision infection are sterile operation,flushing of abdominal cavity and enhanced detection of environment hygiene in operating room.The infection rate could be lowered through observation and nursing care of postoperative incision and proper use of antibiotics.
10.Hospital Infection Management Community Health Service Centers Affiliated to Hospital:Problems and Countermeasures

OBJECTIVE To strengthen the hospital infection management in community health service centers,in order to reduce the onset of hospital infection.METHODS According to Hospital infection administration regnltions and Shenzhen medical service hospital infection control quality evaluation management demands we had investigated and evaluated the hospital infection management in 9 community health service centers.RESULTS The monitoring,surveillance and management of hospital infection in community health service centers as a quality evaluation index of hospital medical service still had many problems,it should further enhance the quality in the field of administration management,sterilization and disinfection,surveillance and sevage treatment.CONCLUSIONS The hospital infection management in community health service centers is a part of whole hospital medical service,but their quality at present is weak.To strengthen the hospital infection management is important to guarantee the medical service quality and medical care safety.
