1.The detection and significance of hyper sensitivity C-reactive protein, iipoprotein (a) and total cholesterol/high density lipoprotein cholesterol in coronary heart disease

ObjectiveTo analyze the significance of serum hyper sensitivity C-reactive protein (hs-CRP),lipoprotein (a) [ Lp(a) ] and total cholesterol (TC)/high density lipoprotein cholesterol ( HDL-C )in coronary heart disease.Methods From September 2010 to September 2011,the levels of serum hs-CRP,Lp (a) and TC/HDL-C were detected in 176 patients of coronary heart disease (coronary heart disease group ) and 60 healthy controls (control group).Coronary heart disease group included acute myocardial infarction with 56 cases,stable angina with 58 cases and unstable angina with 62 cases.ResultsThe levels of serum hs-C RP,Lp ( a ) and TC/HDL-C in coronary h cart disease group were (34.51 ± 9.65 ) mg/L,(295.16 ±104.57) mg/L and 4.23 ±0.91,while those in control group were (1.26 ±0.69) mg/L,(145.26 ±42.19)mg/L and 2.54 ± 0.57.There were significant differences between two groups (P < 0.05 ).The levels of serum hs-CRP,Lp(a) and TC/HDL-C in acute myocardial infarction patients were significantly higher than those in unstable angina patients and stable angina patients (P < 0.05).The levels of serum hs-CRP,Lp (a) and TC/HDL-C in unstable angina patients were significantly higher than those in stable angina patients(P < 0.05).Conclusions Serum hs-CRP,Lp (a) and TC/HDL-C are significantly increased in coronary heart disease,and can reflex the severity of coronary heart disease.
2.Analysis of Peripheral Blood Lymphocyte Immunophenotypes in Syphilitic Patients with Flow Cytometry

Objective To characterize the human cell mediated immune response to Treponema pallidum(TP), and to determine the effect of TP infection on the various lymphocyte subpopulations and natural killer cells in the peripheral blood of the patients. Methods Twenty four untreated patients with early syphilis were analyzed,including 3 groups of 8 patients with disseminated syphilides,palmoplantar syphilides and latent syphilis each. Monoclonal antibodies of CD labeled with either fluorescein isothiocyanate or phycoerythrin were used on a flow cytometry to detect the changing of peripheral blood lymphocyte immunophenotypes. Results The results showed that the percentages of panT,T H,NK cells and the ratio of T H/T S were significantly lower (P0.05). Conclusion It suggests that there is prominent suppression of cellular immunity in the untreated patients suffering from early syphilis. Meanwhile, the difference of 3 groups in skin lesions was due to the different dermatobarrier function against the systemic invasion of TP, rather than attributed to the alteration of the lymphocyte immunophenotypes in the peripheral blood.
3.Brain protection of adenosine preconditioning in infants undergoing radical correction for fallot tetrad

Objective To investigate the brain protection of adenosine preconditioning in infants with tetralngy of Fallot (TOF) undergoing open heart surgery. Methods 120 patients with TOF undergoing open heart surgery were randomly divided into two groups: adenosine group and control group. Total dosage of 1.5 mg/kg adenosine was intravenously dripped in the adenosine group before the cardiopulmonary bypass. Serum S-100β and neurone specific enolase (NSE) were measured before the cardipulminary bypass, 30 min and 12 hours after the aorta off clamping. Results At 30 min and 12 hour after opening the aorta, the S-100β[(0. 346±0. 162)μg/L vs. (0.717±0.260) μg/L and (0.541±0.272) μg/L vs. (1.140±0.567) μg/L,P <0.001 ]and NSE[(10.43±5.54) μg/L vs. (15.75±7.96)μg/Land (15.54±7. 98)μg/L vs. (23.67±8.00)μg/L,P<0.001]were both lower in adenosine group than that in control group. Conclusion Adenosine preconditioning can reduce the S-100β and NES in infants with TOF undergoing open heart surgery and has brain protection.
4.Therapeutic Observation of Umbilicus Application withDa Huang (Rheum Officinale) during 5-7 a.m. for Senile Constipation after Hip Fracture

Objective To observe the clinical efficacy of umbilicus application withDa Huang (Rheum Officinale) during 5-7 a.m. in treating senile constipation after hip fracture.Method Fifty-nine elderly patients with constipation after hip fracture were randomized into a treatment group of 30 cases and a control group of 29 cases. The treatment group was intervened by umbilicus application withDa Huang (Rheum Officinale) during 5-7 a.m, while the control group was by umbilicus application withDa Huang (Rheum Officinale) at any time but avoiding 5-7 a.m. period. After a treatment course, the therapeutic efficacy was evaluated based on bowel movement frequency and shape of stool, and the two groups were compared with each other.Result The total effective rate and markedly-effective rate were respectively 93.3% and 70.0% in the treatment group versus 82.8% and 37.9% in the control group. The between-group difference in the total effective rate was statistically insignificant (P>0.05); there was a significant difference in comparing the markedly-effective rate between the two groups (P<0.01).Conclusion Umbilicus application withDa Huang (Rheum Officinale) during 5-7 a.m. is an effective method in treating senile constipation after hip fracture.
5.A study on mammary hamartomas (8 cases)

Objective To study the clinical, histological and immunohistochemical features of breast hamartomas. Methods The specimens were fixed in 10% formalin and embedded in paraffin. The sections were stained with hematoxylin and eosin for histological diagnosis. SP technique was used in the immunohistochemical study.Results We have 8 cases of breast hamartomas with well defined borders microscopically, histologically composed of ducts, lobules, fibrous stroma, adipose tissue. None of the epithelial elements showed any evidence of atypia. Immunohistochemical analysis showed that there was a variable number of epithelial cells showing a nuclear staining for estrogen receptor(ER) in all cases.Conclution Mammary hamartomas are benign tumor like lesions, histologically dissimillar from other breast tumors. It is affected by hormonal factors. The diagnosis is achieved by combining clinical, radiological and pathologic results.
6.Performance validation of CLINITEK Atlas urine dry chemistry analyzer and its supplementary strips in clinical laboratory

Objective To evaluate the clinical performance of CLINITEK Atlas urine dry chemistry analyzer and its supplementary strips,which could be used for other hospitals as reference.Methods Five hundred and one samples of random fresh urine were collected and analyzed by CLINITEK Atlas urine dry chemistry analyzer.10 parameters were reported for each sample,including SG,pH,BLD,LEU,PRO,GLU,KET,UBG,BIL and NIT.According to the medicals standard of the People's Republic of China,General Technical Requirements for Urine Analyzer(YY/T 0475-2004),General Technical Requirements for Chemical Reagent Strips for Urinalysis(YY/T 0478-2004)and physical,Chemical and Microscopic Examination of Urine(WS/T 229-2002),the precision,accuracy,carryover,stability,sensitivity and consistency of each parameter were evaluated.The agreement was assessed between the results for BLD and LEU obtained from CLINITEK Atlas analyzer and phase contrast microscope,and calculated the sensitivity and specificity of CLINITEK Atlas analyzer for BLD and LEU using phase contrast microscope as the gold standard.SG and pH test was performed among 200 specimens by CLINITEK Atlas analyzer,and then compared with the results obtained from MASTER-SUR-NM specific gravity refractometer and pH precision test strips respectively.In addition to SG and pH,the other eight parameters were compared with the results obtained from CLINITEK 500 urine analyzer,and Kappa value and consistency were calculated.Results The accuracy,precision,sensitivity,carryover and stability of 10 parameters could meet all the requirement of standards.SG and pH had good correlation with urine specific gravity refractometer (r =0.9838,P <0.001)and pH meter (r =0.8884,P <0.001),respectively.Compared with phase contrast microscope,BLD and LEU had coincidence rates of 90.4% and 90.8%,respectively; Sensitivities were 90.7% (301/332) and 83.3% (200/240) ; Specificities were 89.9% (152/169) and 97.1% (255/261).Compared with CLINITEK 500,all the parameters,except for SG and pH,had good coincidence rates of > 87.6%.Conclusion The performance of CLINITEK Atlas urine dry chemistry analyzer can meet the clinical requirements of all standards.
7.Effect of bone marrow stromal cells transfeted by human sFlt-1 gene on growth of K562 cells

Objective To get the protein expression of sFlt-1 by transfection and to investigate the effects of sFlt-1 gene transfection on the growth of K562 cells.Methods The recombinant plasmid pcDNA3-sFlt-1D4 was constructed.The recombinant plasmid pcDNA3-sFlt-1D4 was transfected into the bone marrow stromal cells by Lipofectamine 2000,which was identified by RT-PCR,ELISA and MTT.Results The transfection efficiency identified by flow cytometry was 9.27%.The protein expression of sFlt-1D4 was found in the culture supernatant 24 h,48 h and 4 weeks after transfetion by ELISA and the expression concentrations were(0.104?0.078),(0.158?0.022) and(0.171?0.069) ?g?L-1,respectively.The content of VEGF secreted by K562 culturing with transfectant cells culture supernatant was reduced compared with control.The inhibitoy rates on the proliferation of K562 cells via MTT assay were 9.41%?4.71%,23.63%?7.50%,and 33.13%?6.93%,respectively.Conclusion The bone marrow stromal cells transfected with recombinant plasmid pcDNA3-sFlt-1D4 could secrete sFlt-1D4 and inhibit the proliferation of K562 cells.
8.Detection of IL-2 and IL-6 in human breast tumor Bcap-37 cells treated by hematoporphyrin nonomethyl ether mediated photodynamic therapy

Objective To observe IL-2 and IL-6 changes in the breast tumor Bcap-37 cells reated by hematoporphyrin nonomethyl ether mediated photodynamic therapy (HMME-PDT).Methods Cells in logarithmic growth phase were collected among breast cancer cells cultured in conventional methods.According to blank control group or the experimental group (laser irradiation group,photosensitive agent group and HMME-PDT group),PDT in addition to HMME and HMME-PDT were conducted.The changes of IL-2,IL-6 were detected by radioimmunoassay.Results After HMME-PDT,IL-2 was increased as time passed.After 12,24 and 48 h,compared with IL-2 level in the control group,in laser irradiation group or photosensitive agent group,the levels of IL-2 in HMME-PDT group was significantly differences (P < 0.05).But IL-6 levels decreased.The most obvious changes of IL-6 levels happened at 12h and 24h.There was significant differences between IL-6 in HMME-PDT group with the control group,laser irradiation group or photosensitive agent group (P < 0.05).Conclusion HMME-PDT maybe have destruction effect by altering IL-2,IL-6 activity on breast tumor cells,which provides objective indicators for clinical patients to regulate immune function and auxiliary diagnosis.
9.Detection of peripheral CD4+CD25+ regulatory T (Treg) cells in patients with atopic dermatitis

Objective To explore the action mechanism and clinical significance of CD4+CD25+ regulatory T (Treg) cells in the development of atopic dermatitis (AD). Methods Peripheral blood mononuclear cells (PBMC) were obtained from 46 patients with AD and 20 normal human controls. Flow cytometry was performed to detect the number of CD4+CD25+ Treg cells, real-time fluorescence PCR assay to measure the Foxp3 mRNA level in PBMC, ELISA to determine the serum levels of IL-2, IL-4, IL-10 and IFN-γ. Results A statistical decrease was observed in the percentages of peripheral CD4+CD25+ Treg cells among CD3+ T cells and CD4+ T cells in AD patients compared with normal controls (t' = 3.775, 4.533, both P< 0.01 ), and in the percentage of peripheral CD4+CD25+ Treg cells among CD3+ T cells in patients with acute AD compared with those with chronic AD (t = 2.217, P < 0.05), but no significant difference was noted between patients with acute AD and those with subacute AD or between those with subacute AD and those with chronic AD in the percentage of peripheral CD4+CD25+ Treg cells among CD3+ T cells (t = 1.558, 0.49, both P > 0.05). The mRNA level of Foxp3 in PBMC from AD patients was statistically decreased compred with that from normal controls (z =-2.368, P < 0.05 ). The count of CD4+CD25+ Treg cells was positively correlated with serum levels of IL-2 and IL-10 (r = 0.512, 0.494, both P < 0.05), but had no significant correlation with serum levels of IL-4 and IFN-γ (r = -0.110, -0.237, both P > 0.05). Conclusions In AD patients, there is a decrease in the count of CD4+CD25+ Treg cells and in the level of Foxp3 mRNA, which may suppress the proliferation of and secretion of Foxp3 mRNA by Th2 cells, lead to Th2 predominance, participate in the development of AD.
10.The effect of three kinds of reverse trendelenburg position angle in block plane of prone position after subarachnoid anesthesia

Objective To compare the effect of three kinds of reverse trendelenburg position angle in block plane of prone position after subarachnoid anesthesia, in order to acquire the conformable reverse trendelenburg position angle of prolapse and hemorrhoids (PPH). Methods Sixty patients having underwent the selective PPH were divided into 10° group, 20° group and 30° group by random digits table method with 20 cases each. Subarachnoid puncture was performed in the L3 - 4 processus spinosus gap, then 0.5%heavy density ropivacaine 2 ml was injected. After anesthesia, the operation bed was adjusted to reverse trendelenburg position 10° (10° group), 20° (20° group) and 30° (30° group). At the same time the patients changed to prone position, and the operation bed was changed to horizontal position before operation. The patients changed to jackknife position 10 min after drug injection of subarachnoid anesthesia. The sensory blockade after drug infusion, circulation function change, anesthetic effect and perioperative adverse reaction were observed. Results There were no statistical differences in operation start time and fixed time of anesthesia plane among 3 groups (P>0.05). The upper bound of sensory nerve block before horizontal position, upper bound of sensory nerve block before jackknife position, highest plane of sensory nerve block in 10° group were significantly higher than those in 20° group and 30° group: T(10.47 ± 0.65) vs. T(12.36 ± 0.72) and T(12.50 ± 0.54), T(10.12 ± 0.56) vs. T(11.82 ± 0.66) and T(11.99 ± 0.72), T(9.53 ± 0.71) vs. T(11.32 ± 0.78) and T(11.54 ± 0.83), and there were statistical differences (P<0.05); but there were no statistical difference between 20° group and 30° group (P>0.05). The systolic blood pressure and diastolic blood pressure levels 15 and 20 min after drug infusion in 10° group were significantly lower than those in 20° group and 30° group, systolic blood pressure:(120.40 ± 7.38) mmHg (1 mmHg=0.133 kPa) vs. (131.82 ± 7.88) and (130.47 ± 7.57) mmHg, (119.55 ± 7.65) mmHg vs. (131.25 ± 9.67) and (130.23 ± 8.69) mmHg, diastolic blood pressure: (63.74 ± 6.54) mmHg vs. (71.36 ± 8.49) and (74.32 ± 7.54) mmHg, (61.59 ± 6.23) mmHg vs. (72.98 ± 7.39) and (73.75 ± 6.34) mmHg, and there were statistical differences (P<0.05);but there were no statistical difference between 20° group and 30° group (P>0.05). No patients′ anesthesia effect was poor in 3 groups, but 4 cases in 30° group were good. Hypotension and tachycardia occurred in 2 cases in 10° group, respectively. Conclusions Immediately prone position after subarachnoid anesthesia can be adjusted by reverse trendelenburg position angle to get the appropriate level of anesthesia, and conformable reverse trendelenburg position angle of PPH is 20°.
