1.Nursing of children dermatomyositis complicated with interstitial pneumonia

Objectives To summarize the experience of nursing care to children with dermatomyositis complicated with interstitial pneumonia.Methods The nursing histories of 11 cases of children dermatomyositis complicated with interstitial pneumonia were retrospectively looked into for summarizing the nursing measures.Results The diseases were both effectively controlled in all the children,who were given maintenance therapy by oral taking of hormone after discharge.Conclusion Such nursing measures as close observation of disease,enhanced nursing and first aid of respiratory failure,nursing of infection and secondary infection,care after mesenchymal stem cells transplantation,strengthened disinfection and isolation,are of significance for controlling the disease.
2.The relationship between homocysteine, folate, vitamin B12, lipids and carotid artery atherosclerosis in older patients

Objectives To investigate the relationship between homocysteine (Hcy), folic acid, vitamin B12(VitB12),lipids and the carotid artery atherosclerosis(CAAs) and the significance of their levels in predicting cardiac and cerebral vascular events of older patients. Methods Both sides of the common carotid artery and internal carotid artery were measured by B-mode ultrasound in 74 examinees in A, B, C, and D groups and a control group. With fasting serum, cholesterol, triglyceriade, apolipoprotein AI(Apo AI), apolipoprotein B (Apo B), lipoprotein (a) [Lp(a)], creatinine, glucose, folic acid, Vit B12 and Hcy were detected. Results When carotid artery atherosclerosis became severer, serum Hcy was higher. The levels of the control and A, B, C, D groups were (10.2±3.6) μmol/L, (11.5±4.4) μmol/L, (17.9±4.5) μmol/L, (24.7±10.3) μmol/L, and (41.4±22.3) μmol/L respectively. SNK test showed a significant difference between D and other groups (P>0.05). Folic acid and Vit B12 were negatively correlated with the severity of carotid artery atherosclerosis, but only A and D groups showed statistical significance. Spearman correlation coefficient showed that serum Hcy had significant negative correlation with folic acid and Apo AI(P=0.035 and P=0.000).Multiple linear regression analysis between CAAs and various risk factors showed Apo AI had negative correlation, but Apo B, Lp(a) and Hcy had positive correlation with CAAs, especially Hcy(P=0.009).Conclusion Hyperhomocysteinaemia is an important risk factor of artery atherosclerosis and the degree of CAAs is highly correlated with Hcy concentration. Hcy, Apo AⅠ,Apo B, Lp(a) are sensitive and reliable indexes for evaluating and predicting CAAs.
3.The Therapeutic Effect of Allergic Rhinitis by Different Acupoints Application Therapy for Wintertime Diseases in Summer Treatment

[Objective] To observe the effect of different acupoint application therapies :a summer treatment for the winter disease on allergic rhinitis ,and compare the relationship between the different acupoints and therapeutic effect. [Methods ] The 81 cases of allergic rhinitis who met inclusion criteria were divided into 12 acupoint group and 6 acupoint group. The time of application was in first ten-day period of the hot season. The duration of application was 2h. The days later, namely in the second ten-day period of the hot season, and twenty days later, in the third ten-day period of the hot season, the application was repeated respectively. Before treatment and half year after the treatment, the score of quality of life, symptoms of allergic rhinitis were observed to compare the relationship between the different acupoints and therapeutic effect. [Results] The effective rate was higher in 12 acupoints group than 6 acupoints group. After treatment, the score of quality of life, symptoms of allergic rhinitis were significantly improved( P<0.05 or P<0.01). And the comparison of the therapeutic effect of two groups showed significant difference in statistics( P<0.01). [Conclusion] The acupoint application therapy is effective for allergic rhinitis, and the comprehensive combination of acupoint is influential on the effect.
4.The applied analysis of brain CT and cerebrospinal fluid examination in the diagnosis and treatment of viral encephalitis in children

Objective:To sum up the characteristics of cerebrospinal fluid and brain CT findings of viral encephalitis in children, and provide guidance for clinical diagnosis and treatment of viral encephalitis.Methods: 117 cases of viral encephalitis were selected in our hospital and the results were analyzed by statistical analysis of cerebrospinal fluid and CT in children with viral encephalitis in order to compare the results of cerebrospinal fluid examination and CT examination.Results:Cerebrospinal fluid examination showed that the increased intracranial pressure were 52.14%(61/117), protein>0.5 g/L are 36.75%(43/117), elevated white blood cell levels were 57.26%(67/117), the neutrophils increased 33.33% (39/117), lymphocytes increased 31.62%(37/117), and the sugar concentration <3.3 mol/L are 14.53% (17/117). Head CT in the diagnosis of abnormalities in children were 98 cases, in which 21 were mild abnormal, 57 were moderately abnormal and 20 cases were severe abnormalities. Abnormal cerebrospinal fluid diagnosis rate of 94.02% was significantly higher than the rate of CT abnormalities 83.76% and the difference was statistically significant (x2=6.231,P<0.05).Conclusion: The abnormal rates of CT examination in the diagnosis of viral encephalitis were higher in patients with viral encephalitis. The combination of the two examinations could be more comprehensive than that of the patients with the disease and focus. It could be benefit for clinical treatment.
5.Differentiation of benign and malignant lymph nodes with ADC value on diffusion MR: a Metaanalysis

Objective To summarize the diagnostic efficacy of ADC value for differentiation of benign and malignant lymph nodes on diffusion MRI with Meta-analysis. Methods Published papers on differentiation of benign and malignant lymph nodes with ADC value were searched and reviewed.Quality evaluation was performed for the eligible papers before data extraction.Test for heterogeneity was performed first,then appropriate model was selected to calculate the weighted mean difference,sensitivity,specificity,positive likelihood ratio,negative likelihood ratio,diagnostic odds ratio,pretest and posttest probability.The potential of ADC value for differentiation of benign and malignant lymph nodes was assessed qualitatively and quantitatively.Results Fifteen papers including 735 cases and 1963 lymph nodes were selected.According to Meta-regression analysis,subgroup analysis and robust analysis,two studies with benign lymph nodes in patients with benign lesion and one study using chemical shift saturation technique were excluded because of their impact on the robustness of the pooled results. The weighted mean difference (WMD) between malignant and benign lymph nodes was -0.355 × 10-3mm2/s[95% confidence interval (CI):-0.423 ×10-3- -0.288 × 10-3 mm2/s].Although the cutoff of ADC value for differentiation in each study was different,the diagnostic efficacy was stable,the pooled sensitivity,specificity,positive likelihood ratio,negative likelihood ratio,diagnostic odds ratio and area under summarized receiver operator's curve were 0.87 (95% CI:0.79-0.92),0.87 (95% CI:0.82-0.90),6.5 (95% CI:4.7-9.2),0.15 (95% CI:0.09-0.25 ),43 ( 95% CI:21-87 ),0.93 ( 95 % CI:0.90-0.95 ).The posttest malignancy probability of benign lymph node indicated by ADC was 6%,while that of malignant lymph node was 72%.Conclusion The ADC value can be used to differentiate benign and malignant lymph nodes with good sensitivity and specificity noninvasively.
6.Application of light-cure resin-modified glass ionomer cement in orthodontic practice

BACKGROUND: Resin-modified glass ionomer (RMGI) cements have higher bond strength, especially can release fluoride. But there are fewer reports of the clinical application for the prevention of decalcification.OBJECTIVE: To test the benefit from using RMGI cement instead of a conventional composite resin in bracket bonding for patients with malocclusion, and observe bracket-failure rates and decalcification on enamel surfaces at pretreatment and at debonding.DESIGN: Observational and comparative trial.SETTING: The Second Hospital Affiliated to Hebei Medical University.PARTICIPANTS: Forty successive patients (358 teeth) with malocclusion admitted to the Department of Orthodontics in the Second Hospital Affiliated to Hebei Medical University, were selected for the study from July to August in 2002. All the patients (21 females and 19 males, mean age 16 years) had normal and complete anterior teeth, good oral hygiene. There were no obvious differences in bilateral teeth. Informed consents were obtained from all the subjects. The experiment was also approved by the ethical committee of the hospital. Experimental materials were RMGI adhesive (Fuji, GC, Japan, Lot 0005111) and composite resin cement (enamel adhesive of Beijing and Tianjin, Tianjin product, Lot 020402). Brackets produced from Hangzhou 3B and 37% phosphoric acid were used.METHODS: ①Bonding brackets: Subjects selected according to random procedure were divided into two groups, each with 20. GroupⅠ: The left buccal surfaces bonded with light-cure RMGI were etching for 30 seconds with 37% phosphoric acid, rinsed with water; the right buccal surfaces bonded with composite resin cement were etching for 60 seconds with 37% phosphoric acid, rinsed with water and dried; Group Ⅱ: After etching for 30 seconds with 37% phosphoric acid, the right buccal surfaces were rinsed with water and bonded brackets with light-cure RMGI. The left buccal surfaces were bonded brackets with composite resin cement after etching for 60 seconds with 37% phosphoric acid, rinsing with water and drying; Attachment of 0.036-cm NiTi wires with ligature to the brackets was conducted 10 minutes after light-curing. The information about differences in bilateral bonding materials was not told to patients. To ensure an equal bonding materials containing fluoride and minimize the error, all the patients were instructed to use toothpaste containing fluoride, a fluoride mouthwash was not prescribed. The treatment period was 9-26 months (mean 18 months).②Patients were rechecked at intervals of 4 weeks postoperatively. Each bonded tooth was checked for loose or missing brackets, and failures were recorded. A color transparency of anterior teeth area was taken using a standardized photographic technique. The enamel surface conditions were classified at a magnification of ×10. The condition of enamel surface recorded was made according to the scoring system by Geiger before treatment and at debonding.MAIN OUTCOME MEASURES: ①The number and site of bonding failures.②Enamel surface conditions at before treatment and debonding.RESULTS: Forty patients were all involved in the result analysis. Eliminating 4 teeth occurring bond failure and 4 teeth of opposite side at anterior teeth, a total of 232 teeth were evaluated.①The number and sit of bonding failures: There was no significant difference between the failure rates of RMGI adhesive and composite resin cement (P > 0.05). Significantly more premolar brackets failed than incisor brackets.②Decalcification of enamel surface: At debonding after treatment, the incidence rates of white spots in the surfaces bonded with the RMGI were significantly lower than that in the composite resin (25.9%, 38.8%, P < 0.05).CONCLUSION:The use of RMGI for brackets bonding results in a significant reduction in the incidence of white spot at debonding. Reducing etching time may obtain a similar survival rate with the routine etching time.
7.Effect of EDTA combined with ciprofloxacin on Pseudomonas aeruginose biofilm infection in guineapig

Objective To investigate the effect of EDTA combined with ciprofloxacin on Pseudomonas aeruginose biofilm in vivo.Methods Pseudomonas aeruginosa was inhaled into the lung of guinea pigs and colonized,formed biofilm.After 7 days,the model was treated with ciprofloxacin,EDTA alone,or a combination of both for 7 days.The number of colony in the lungs is measured by agar plate.The pathological change of the lung is observed by hematoxylin and eosin (HE) staining and scanning electron microscope.Results EDTA combined with ciprofloxacin make the number of bacteria in the lungs reduced from l05 CFU/g to 10 CFU/g(t =24.67,P<0.05),the lung lesion was less-sever histophathologically.Conclusion The combination of EDTA with ciprofloxacin has significant activity to remove mucoid PA biofilm in vivo.
8.Study on morphology and function of mitral valve leaflets and mitral annulus in patients with mild mitral regurgitation by real-time three-dimensional echocardiography

Objective To measure the parameters of mitral valve leaflets and mitral annulus in patientswithmildmitralregurgitation( MR )byreal timethree-dimensionaltransesophagealechocardiography (RT-3D-TEE),and explored the mechanism of MR.MethodsFifty-seven MR subjects were selected and twenty-eight subjects without mitral regurgitation were served as control group,all subjects were examined by RT-3D-TEE and acquired image,mitral valve quantification (MVQ) software was used for post-processing.Mitral annulus parameters (H/DAIPm,E2D,θAv-Mv,mitral annulus θnpa) and mitral valve leaflets parameters(A3DE,L2DAIPm,VA1-3tentVp1-3tentVtentHtentθnpa ) at the end of systolic were measured.The results of two groups were compared,and the most affected parameters to mild mitralregurgitation were selected.Results Compared with control group,VA3tent was decreased,mitral annulus θnpa and L2DAIPm increased,and the mitral valve leaflets θnpa was independently correlation factor of mild mitral regurgitation.ConclusionsThe mitral annulus geometry to flat in subjects with mild MR,the mitral valve local area is increased in subjects with mild MR,the mitral valve leaflets θnpa is independently correlation factor of mild mitral regurgitation.
9.Effects of bypassing the emergency department on outcomes of patients with acute myocardial infarction underwent primary percutaneous coronary intervention

Objective A retrospective analysis were conducted to identify the effect of bypassing the emergency department on 30-days outcomes of patients with acute myocardial infarction undergone primary percutaneous coronary intervention ( PPCI) . Methods From June 2014 to April 2015, 187 patients underwent PPCI in Kunming General Hospital were included. 13 patients were excluded owing to their incomplete follow-up data. The total 174 patients were divided into two groups: the control group (n =59) who did not bypass the emergency department, and the bypass group ( n = 115) who bypassed the emergency department and directly received PPCI. The data of all patients were collected and analyzed. Results There were no significant differences in baseline characteristics and PPCI related data (including percentage of thrombus aspiration catheter used, length or diameter of stents applied between two groups (all P ﹥ 0. 05) . The bypass group had shorter door-to-ballon ( D2B) than the control group [ (67. 7 ± 21. 5) min vs. (89. 4 ± 23. 6) min, P ﹤ 0. 001] . There were no significant differences in 30-days all-cause mortality, re-myocardial infacrtion and target ressel revascularization (TVR) between the two groups (P ﹥ 0. 05) . Total MACEs rate in the bypass group was lower than in the control group (10. 2% vs. 1. 7% , P = 0. 012) . Logistic regression analysis showed that age, diabetes, pain-to-door (PTD) time and CK peak value were the main influencing factors for 30-day MACEs rate of patients receiving PPCI ( P ﹤0. 05) . Conclusions Bypassing the emergency department can shorten D2B time and reduce 30-days MACEs post-PPCI, but reducing the total ischemic time will be more beneficial to patients with acute myocardial infarction.
10.Chromosomal and Subcellular Localization and Expression of Cell Cycle-related Regulator DCT1

Objective To investigate the chromosomal and subcellular localization of DOC-1R terminal 1(DCT1),and detect its expression in human tissues.Methods Chromosome localization of DCT1 was detected by radiation hybrid.pEGFP-DCT1 was constructed,and HeLa cells were transfected with the plasmid.The subcellular localization of DCT1 protein was observed by fluorescence microscope.Real-time PCR was performed for the determination of DCT1 expression level in 16 kinds of human tissues.Results DCT1 was demonstrated to localize in 5q31,and its encoding protein was detected on the nuclear membrane.Additionally,DCT1 was proved to express universally in all the 16 kinds of human tissues and it was expressed at the highest level in spleen.Conclusion DCT1 might be a regulator in cell cycle,and ubiquitously express in human tissues.
