1.Clinical Observation of Enalapril Folic Acid Tablet Combined with Shenkang Injection in the Treatment of Hypertensive Nephropathy

OBJECTIVE:To observe the clinical efficacy and safety of Enalapril folic acid tablet combined with Shenkang injec-tion in the treatment of hypertensive nephropathy. METHODS:90 patients with hypertensive nephropathy were randomly divided in-to control group and observation group. All patients were given anti-infection,regulating electrolyte balance,supplying amino acids and other conventional treatment. Based on it,control group was orally given Maleic acid enalaprilat folic acid tablet 1 tablet in the morning,once a day;observation group was additionally given 100 ml Shenkang injection adding into 300 ml 10% Glucose injec-tion by intravenous infusion,20-30 drops/min,once a day. The treatment course for both groups was 4 weeks. The clinical effica-cy,and clearance rate of creatinine(Ccr),serum creatinine (Scr),morning urine test urine protein (Up)/urine creatinine (Ucr), serum malondialdehyde(MDA),superoxide dismutase(SOD),total antioxidant capacity(T-AOC)before and after treatment and incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in observation group was significantly higher than control group,the difference was statistically significant(P<0.05). After treatment,the Ccr,SOD and T-AOC in 2 groups were significantly higher than before,and observation group was higher than control group,Scr and MDA were significant-ly lower than before,and observation group was lower than control group,the differences were statistically significant(P<0.05). Morning urine test Up/Ucr,systolic blood pressure and diastolic blood pressure were significantly lower than before,and the differ-ences were statistically significant(P<0.05),however,there was no significant difference between 2 groups(P>0.05). And there was no significant difference in the incidence of adverse reactions(P>0.05). CONCLUSIONS:Based on the conventional treat-ment,Enalapril folic acid tablet combined with Shenkang injection has better efficacy than only Enalapril folic acid tablet in the treatment of hypertensive nephropathy,with similar safety.
2.Angiographic diagnosis and interventional therapy of severe gastrointestinal bleeding

Objective To explore the value of angiographic diagnosis and interventional therapy of severe gastrointestinal bleeding.Methods 33 patients in a critically ill state(1000ml of bleeding greater than alimentary canal),who could not or were not willing to go fibre gastroscope/intestines ytterbium checks,were diagnosed by selectivie or surpass-selectivie stomach-bowel angiography,and embolismed or poured into by the surpass-selection artery art after the bleeding artery definiting at the same time.Results The selective stomach-bowel artery radiography group identified 27 patients site of bleeding,the masculine rate was 82%.The surpass-selectivie stomach-bowel artery radiography group identified 31 patients site of bleeding,the masculine rate was 94%.2 patients had no abnormality,26 patients were cured by the embolism,7 patients were treated successfully by pouring into the pharmaceuticals.Among them 8 patients were bleeding again,4 patients were cured through the surgical operation without complication.Conclusions DSA surpass-selectivie stomach-bowel artery radiography can define the site of severe gastrointestinal bleeding and raise the hemorrhage kitchen range recall rate,the embolism can successfully control the bleeding.Throughout the pouring into the pharmaceuticals,those who can't be treated by embolism can save time for the surgical operation,make part of patient cure further gaining time for the surgical operation.
3.Upregulation of phagocytic clearance of apoptotic cells by autoimmune regulator.

To investigate the effect of autoimmune regulator (AIRE) on phagocytic clearance of apoptotic cells, a recombinant expression vector containing full-length human AIRE cDNA was transfected into 16HBE cells. After incubation with transfected 16HBE cells, engulfment of apoptotic HL-60 cells induced by camptothecin was detected by myeloperoxidase (MPO) staining. The change in the expression of Rac 1 in transfected 16HBE cells was determined by RT-PCR and Western blotting. The results showed that the phagocytosis percentage of the experimental group, the mock transfection group and the negative control group (non-apoptotic cells) was (25.50+/-3.67)%, (6.25+/-1.58)% and (1.0+/-0.67)%, respectively. Moreover, the expressions of Rac 1 mRNA and protein were up-regulated in AIRE-transfected 16HBE cells, suggesting that AIRE may function as a regulator in the phagocytic clearance of apoptotic cells by promoting the expression of Rac 1.
4.Application of E-flow in ultrasound examination of fetal pulmonary vein

Objective To explore the value of E-flow technique in ultrasound examination of fetal pulmonary vein.Methods The pulmonary veins of 332 fetus were examined using lungs as acoustic window by two-dimensional echocardiography(2DE), color Doppler flow imaging (CDFI) and E-flow.Visualization ratios of the three methods were compared.Results The fetal pulmonary veins were displayed as following:7 cases were showed by 2DE,49 cases by CDFI and 73 cases by E-flow among 118 cases during 12 - 22 gestational weeks.The visualization ratio of E-flow was higher than that of the other two methods (P <0.001).Fourty-one cases were showed by 2DE, 171 cases by CDFI and 177 cases by E-flow among 214 cases during 23 - 40 gestational weeks.The visualization ratios of E-flow and CDFI were higher than that of 2DE (P < 0.001).The visualization ratio of E-flow and CDFI has no distinct differrence (P > 0.0167).Conclusions E-flow was sensitive to pulmonary vein blood flow and better than 2DE and CDFI in early stage of the second trimester.
5.Clinical characteristics of hyperplastic pyloric canal polyp in infant and toddler:an analysis of 5 cases

Objective To review the diagnosis and treatment of pyloric canal polyp in infants and toddlers suffering progressive gastric outlet obstruction.MethodsClinical data of 5 male infants and toddlers,at age of 10 to 26 months(mean 15.6 months)with pyloric canal polyp admitted in our department from January 1993 to February 2009 were analyzed.ResultsOf 5 cases,the starting was acute and severe.the conservative treatment was out of effect and no any assistant examination was helpful during diagnosis,and every case was diagnosed clearly through exploratory laparotomy,cured with polypectomy and classic pyloroplasty.ConclusionCurrently,the exploratory laparotomy is an important way to diagnose pyloric canal polyp in infants and toddlers,while other assistant examination seems useless in this aspect.Polypectomy and classic pyloroplasty should be carried out during exploration,and it is dangerous for their life without correct therapy in time.
6.Research in quality of life in patients after lung transplantation and its related factors

Objective To acknowledge the quality of life (QOL) in patients after lung transplantation and to explore related factors. Methods A cross- sectional study design and a convenience sampling were performed in this research. Totally 30 patients after lung transplantation were investigated. The questionnaires which used to explore the quality of life were Short Form 36 Health Survey Questionnaires (SF-36) and Revised Airways Questionnaires 20 (AQ20-R). Related state were surveyed by questionnaires consisted of demographic questionnaires, Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS) and Medical Coping Mode Questionnaires (MCMQ). Statistical analysis was performed by SPSS 20.0 software. Correlation analysis and multivariate analysis were performed using Pearson linear correlation analysis and multi-linear regression. Results The scores among the 8 dimensions of SF-36 were(34.48±16.73)-( 71.63±22.83), lower than those of norms(Z=-9.684--2.817, P<0.05 or 0.01). Somatic pain scored the highest (71.63±22.83), while physiological function scored the lowest (34.48±16.73). The mean score of AQ20-R was 7.93±5.21. The major two problems that manifested QOL were: uncomfortable feeling of lung caused by strong scent, smog or perfume, exhausted feeling after having a cold. The mean scores of SAS and SDS were 44.33±9.33,48.05±9.80, higher than those of norms, which were 33.80 ± 5.90, 41.88 ± 10.57 (t=6.1833, 3.4458, P < 0.01). The scores among the 3 dimensions of MCMQ were 8.93 ± 2.08, 16.10 ± 2.28, 3.63 ± 1.33 and the differences were significant in 3 dimensions compared with those norms, which were 19.48±3.81, 14.44±2.97, 8.81±3.17(t=-27.7281, 3.9885,-21.3878, P<0.01). Conclusions In SF-36, role-physical of patients after lung transplantation was the worst dimension, while bodily pain was the best dimension of QOL. The QOL of patients after lung transplantation were almost worse than normal people. The related factors of QOL in patients after lung transplantation maybe:anxiety, depression and medical coping style.
7.Role of the E gene in the infectivity of SINV and SINL in infected cells

Objective To elucidate the molecular basis on the differences of infectivity in infected cells between Sindbis virus(SINV:YN87448 virus)and Sindbis like virus(SINLV:XJ-160 virus).Methods Compare the E(glycoprotein)gene sequence and secondary structure of YN87448 virus and XJ-160 virus by bioinformatics analysis.Analyze the contribution of E gene to the biological differences between SINV and SINLV by constructing recombinant virus.Results By bioinformatics analysis,YN87448 virus and XJ-160 virus have the same genomic structure,which has 11 717 nt and 11 626 nt respectively.There are 82 amino acid differences between E gene of these two viruses,and showed scattered distribution.The main peak is basically the same for the hydrophobic of the E gene protein,but in some region existing small differences.The recombinant virus which exchanged the E gene of XJ-160 virus with YN87448 virus totally showed the biological character of YN87448 virus,either in the showing time of CPE,plaque forming time and plaque diameter,or in expression of functional proteins.Conclusion E gene plays a major role in the differences of infectivity in infected cells between SINV and SINLV,this result provide the molecular biological evidences for elucidating the biological differences between SINV and SINLV.
8.Blockage of PD-L1/PD-1 pathway for enhancing the chemotherapeutic efficacy of cisplatin

Objective To explore the anti-tumor effect and the influence of antitumor immunity of PD-L1/PD-1 blocked by PD-1 antibody combined with cisplatin. Methods Tumor models were established by injecting TC-1 cells into C57BL/6 mice, and the mice were divided into four groups (n = 4). The tumor growth curves and survival curves were drawn to observe the anti-tumor effect. The tumors were then removed; and the PD-L1 and CD8+ T cells were analyzed by immunohistochemical method. Results The anti-tumor effect was greater in the cisplatin group , PD-1 antibody group , and PD-1 antibody plus cisplatin group than in the control group (P < 0.05). Expression of PD-L1 in the tumor tissues was markedly increased in the cisplatin group and it was obviously decreased in the combination group (P < 0.05). CD8+ T cells decreased in the cisplatin group; and expression of CD8+ T cells was significantly increased the combination group (P < 0.05). Conclusion The anti-tumor effect and anti-tumor immunity of cisplatin are enhanced by blocking PD-L1/PD-1 pathway with PD-1 antibody.
9.Effect of interventional treatment on absolute alcohol in different types of liver cancer

Objective To investigate the effect of routine ultrasound examination and interventional treatment of absolute alcohol in different types of liver cancer.Methods Thirty cases of liver cancer patients who were treated in the 3rd People' s Hospital of Yangquan from May 2010 to May 2014 were selected as ours subjects.They were performed ultrasound exam.The ultrasound characteristic was recorded.Patients were divided into intrahepatic cholangiocarcinoma (ICC) group (11 cases),hepatocellular cholangiocarcinom (HCC) group (14cases) and mixed cholangiocarcinoma group(5 cases) based on pathology results.Nine HCC postoperative recurrence patients and 7 ICC postoperative recurrence patients were treated with absolute alcohol and the clinical effect was observed.Results There were significant differences in the three groups in terms of gender,boundary definition and tumor location (x2 =8.01,6.16,7.32 ; P < 0.05).While the distribution of ultrasound echo intensity was not statistically significant(x =3.44,P > 0.05).The effective rate of interventional treatment of absolute alcohol in HCC patients was 88.89%,and 42.86% in ICC group,and the difference was statistically significant (x2 =3.88,P < 0.05).Conclusion The ultrasound characteristics of different pathological liver cancer are different.Ethanol interventional therapy on HCC patients is significantly better than the effect of ICC patients,suggesting that ICC postoperative recurrence patients can be treated combined with other methods to help improve the effect.
10.The increased express of TIM-1, TIM-3 and other association genes in peripheral blood mononuclear cells from patients with Graves disease

Objective To detect the levels of the mRNA expression of TIM-3,TIM-1,T-bet,GATA-3,IFN-γ,IL-4 and Galectin-9 in the peripheral blood monocytes (PBMCs) of the patients with Graves disease(GD),and to explore their potential role in the pathogenesis of GD.Methods We used fluorescence quantitative real-time reverse transcription-polymerase chain reaction to measure the mRNA expression of TIM-3,TIM-1 and other associated genes in PBMCs of 70 patients with GD and 22 healthy controls.In addition,we analyzed the relationship of TIM-3,TIM-1 and other associated genes.Results The expression of TIM-3 and TIM-1 mRNA in the PBMCs from GD patients were abnormally higher,the GD patients with Graves' ophthalmopathy group had significantly higher level of TIM-3 mRNA expression than that of GD patients without Graves' ophthalmopathy group,but no statistically significant difference was found in the expression of TIM-1 mRNA.Untreated GD patients had significantly higher level of TIM-3 mRNA expression than that of GD patients in recurrence group,however the expression of TIM-1 mRAN was opposite.But no statistically significant difference was found in TIM-3 mRNA expression of recovery GD patient and healthy control group.Though the expression of TIM-1 mRNA was significantly decreased,it was still higher than that of the normal control group.Conclusion TIM-3 and TIM-1 may participate in the occurrence,development and turnover of GD.TIM-3 or TIM-1 may prove to be an important target for developing new drugs and treatments to GD.
