BACKGROUND: Nerve growth factor (NGF) plays a regulatory effect on survival, growth, differentiation, and egeneration of neurons. OBJECTIVE: To verify the therapy effects and the expression of Fos protein with different dose of NGF on the spinal cord of rats with neuropathic pain.DESIGN, TIME AND SETTING: The randomized controlled animal experiment was performed at the Shanghai Ninth People's Hospital.MATERIALS: A total of 72 male, health, adult, Wistar rats, weighing 180-200 g, were randomly divided into 4 groups, namely,model group and NGF with 4, 8, 16 Ijg groups, with 18 animals in each group.METHODS: All rats were prepared sciatic nerve chronic constriction injury models. Rats were intraperitoneal injected NGF with 4,8, 16 μg/(kg·d) in the experimental groups.MAIN OUTCOME MEASURES: ① Behavioral observation and mechanical pain threshold testing was performed prior to and at days 1,2, 3, 5, 7, 10, 14 after operation. ②Six of rats in each group were sacrificed at days 2, 7, and 14 after operation, and the expression of Fos protein in the spinal cord were assayed with the immunohistochemicat methods and image analysis.RESULTS: In the model group, rats raised and licked feet spontaneously after operatiOn, and the mechanical pain threshold was decreased from the 3rd day, and reached the lowest at day 14. However, in the NGF groups, rat exhibited no pontaneous pain,without subnormal period of mechanical pain threshold. The mechanical pain threshold in rats of the model group as significant different to the other groups at days 10 and 14 after operation (P < 0.05). The mechanical pain threshold in rats of all groups increased, but which of 16 μg NGF group was lower than the other groups on the first day after operation (P < 0.01 ) and lower than that of the 4 μg NGF group at 2 days after operation (P < 0.05). The expression of Fos protein in the spinal cord of the model group was increased than that of the other groups after operation (P < 0.01 ). The expression of Fos protein in the spinal cord of the 16 μg NGF group was less than that of model and 4 μg NGF groups at day 2 after operation (P < 0.01 ).CONCLUSION: NGF has therapeutical effect on neuropathic pain of rats, especially with large dosage. The mechanisms may be inhibiting the expression of Fos protein in the spinal cord of rats.

Objective To determine the association of anomalies of female reproductive system with that of urinary system. Methods Two hundred and eighty-nine cases, including 59 with congenital vaginal inclined septum syndrome, 135 with septate uterus, 4 with unicornuate uterus, 13 with rudimentary horn of uterus, 58 with congenital absence of the uterus and vagina, 14 with imperforated hymen, and 6 with atresia of vagina, treated during the past 4 years, were analysed retrospectively. Results Anomalies of urinary system were more frequently found in patients with anisomerous anomalies of reproductive system(85. 53% , 65/76 cases). In 59 cases of congenital vaginal inclined septum syndrome, 43 had ipsilateral renal agenesis, 2 had one single pelvic kidney, 1 had bilateral horseshoe kidneys, 1 had left-side horseshoe kidney, 1 had one pair of renal pelvis and renal calices with ureters incorporated at middle and inferior segments on both sides. Ipsilateral renal agenesis was also seen in patients with rudimentary horn of uterus(14/14 cases) and unicornuate uterus(4/4 cases). However, there was no urinary system malformation seen in septate uterus patients (0/135 cases), imperforated hymen (0/16) and atresia of vagina (0/4 cases). Only 1 case of congenital absence of the uterus and vagina associated with ipsilateral renal agenesis. Conclusion Anomalies of reproductive ducts associated with renal malformations, especially anisomerous anomalies of reproductive system are not unusual. Early recognition and diagnosis is very important to prevention and treatment of such diseases.

Objective To find out about the attitudes of hospital runners, academic leaders and key members of the technical force in public hospitals about structural reform in their institutions and to explore modes and methods for such reform. Methods Survey questionnaires were designed, anonymous surveys were conducted on the spot, and statistical analyses were made of the data collected. Results Surveys on 1904 people in 55 second- and third-tier hospitals indicated that 84.9% were in favor of structural reform in their institutions; 89.7% played an active part in the reform; 86.8% held that structural reform should first be carried out in large and medium-sized hospitals; 96.6% were in favor of such reform modes as government-controlled stock systems, government-owned and civilian-run hospitals, and non-governmental hospitals, with 66% of them supporting the mode of government-controlled stock systems. Conclusion The majority of hospital runners, academic leaders and key members of the technical force in public hospitals actively support structural reform in their institutions and their views with regard to the modes and methods for such reform are in accordance with the guidelines put forward by the government. Sound ideological foundations have been laid for duly promoting structural reform in public hospitals.

Objective:To explore relation of TCM syndrome types in patient with sexual hormones of hyperplasia of mammary glands,so as to establish objective indexes of TCM syndrome types of hyperplasia of mammary glands.Methods:112 cases of hyperplasia of mammary glands were divided into 3 types:stagnation of liver-Qi,intermingled phlegm and blood stasis,disharmany of thoroughfare vessel and conception vessel.Level of estradiol(E_2),progestin(PT),testosterone(TT),prolactin(PRL),follicle- stimulating hormone(FSH)and luteotropic hormone(LH)during ovulatory period in the patients were determined.The relation of hormone level with TCM syndrome types was analyzed.Results:Level of LH and E_2 decreased,PRL and PT increased and level of TT and FSH did not significantly change during ovulatory period in the patient of hyperplasia of mammary glands.LH and E_2 level in the patients with stagnation of liver-Qi or intermingled phlegm and blood stasis was significantly lower than normal values,and the level of PRL and PT was significantly higher than that of normal values.The level of LH and E_2 was also lower than the normal value in the patient with disharmany of thoroughfare vessel and conception vessel,but the level of FSH was significantly higher than normal value.Conclusion:The patient of hyperplasia of mammary glands during ovulatory period manifests endocrine dysfunction,and the patients with different TCM syndrome are different in secretion of sexual hormones; Changes of sexual hormones can be used as objective indexes of TCM syndrome types and basis of clinical medicinal application.

Objective To investigate the long-term protective effects of the waistband onto lumbar spins.Based on the practical situation,to search for some effective and convenient methods,such as wearing waistband and mutual massage,to prevent from lumbago for the army drivers worked in plateau.Methods 372 army drivers,young males,healthy and all worked in plateau,ware involved as the treatment group in the present study.Another 545 army drivers with the similar physical qualities,also worked in plateau,were selected as the control group.For the treatment group,a kind of special waistband was employed for the drivers,and mutual massage on hack of each other was performed among them;while in control group the army drivers were given no measures for lumbar protection and lumbago pre- vention.All the drivers in the both groups ware in normal driving conditions.The therapeutic effects of the waistband and the lumbago symptoms were investigated by epidemiologic survey 16 months later,Results The epidemiologic survey results showed that the incidence of lumbago in the drivers of control group(72.8%)was significantly higher than that in the treatment group(30.9%,P

Objective To know the impact of nursing intervention mode of parent-child interactive on the adaption of postpartum family roles.Methods One hundred families (maternal,husbands,baby at birth) were divided into the intervention group and the control group.The intervention group was cared by the responsibility nurses who were trained to master the parent-child interactive nursing intervention.The control group was cared by conventional nursing methods.The role adaptation of the fathers and the mothers of the two groups,and self-confidence of the parents to take care of newborns were observed.Results In the intervention group,the role adaptation of the fathers and the mothers was better than the control group; self-confidence of taking care of the newborn was better than the control group.The sleep time of the newborns was prolonged and the crying times were less than the control group.Conclusions The parent-child interactive nursing intervention model can help the family to adapt to the father and the mother role as soon as possible; teach the parents to comprehensively understand newborns,give stimulation of touch,sight,hearing,smell,so as to achieve positive response of newborns and increase the confidence of rearing newborns of the parents.

Objective To investigate the pile protein (paxillin) and carbonic anhydrases Ⅸ (CA Ⅸ)expression and its significance in duodenal papilla carcinoma patients.Methods Eighty-seven cases of duodenal papilla carcinoma were enrolled as our subject.Immunohistochemical staining was used to detect the expressions of paxillin and CAⅨ in site of tumor,adjacent tissue and duodenal papillitis tissue.Results Sixtyfive cases (74.7％ (65/87)) were showed a higher expression of paxillin in cancer site than that of adjacent tissue (41％ (16/39),P ＜ 0.05) and duodenal papillitis tissue (41.3％ (19/46),P ＜ 0.05),and the difference was statisically significant (x2 =19.869,P ＜0.05).While a higher expression of CA Ⅸ in 62 cases (71.3％) in cancer site was seen,higher than that of adjacent tissue (33.3％ (13/39),P ＜ 0.05) and duodenal papillitis tissue (30.4％ (14/46),P ＜ 0.05),and the difference was statisically significant (x2 =26.936,P ＜ 0.05).As the increase of the cancer tissue malignant degree and the surrounding tissue invasion or distant metastasis degree,paxillin and CA Ⅸ expression increased.Conclusion The paxillin and CAⅨ might served as the early indicators for diagnosis and judgment of prognosis of duodenal papilla cancer.

Objective To describe practice experience of the male nurses in obstetrics and gynecology department.Methods Phenomenological methodology was adopted in this study.In-depth interview was conducted on 12 male nurses who practiced in obstetrics and gynecology department from March 2012 to May 2013.Data were analyzed by Colaizzi's analysis method.Results The main experience included:lack of patient understanding and support,a sense of loss,decline in the enthusiasm to learn,inconvenience of live and study,dissatisfied with specialist operations.Conclusions Male nurses in obstetrics and gynecology has many adverse experiences,clinical teachers should take measures to help them successfully complete the practice and prevent occurrence of adverse experience.

Objective To investigate the changes in the expression of neuromedin U receptor 2 (NMUR2) in spinal dorsal horn in a rat model of bone cancer pain (BCP).Methods Thirty-two female Sprague-Dawley rats,weighing 150-180 g,were randomly divided into 2 groups (n =16 each):sham operation group (group S) and BCP group.BCP was induced by inoculating Walker 256 mammary gland carcinoma cells (1 × 105) into the medullary cavity of left tibia.Heat-killed Walker 256 cells (1 × 105) were injected into the medullary cavity of left tibia in S group.Eight rats were chosen from each group and the paw withdrawal threshold (PWT) to yon Frey filaments was measured at 1 day before operation (baseline) and 1,3,6,9,12 and 15 days after operation.Bone destruction was shown by X-ray at 15 days after operation.At 1 day before operation and 15 days after operation,4 rats in each were chosen and sacrificed,and L4,5 segments of the spinal cord were removed for measurement of the expression of NMUR2 mRNA (by real-time PCR) and protein (using Western blot analysis) in the spinal dorsal horn.Results Compared with S group,the PWT was significantly decreased at day 6-15 after operation and the expression of NMUR2 mRNA and protein in the spinal dorsal horn was up-regulated at 15 days after operation in BCP group (P ＜ 0.05 or 0.01).Compared with the baseline value,the PWT was significantly decreased at day 6-15 after operation and the expression of NMUR2 mRNA and protein in the spinal dorsal horn was up-regulated at 15 days after operation in BCP group (P ＜ 0.05 or 0.01).X-ray showed defect of bone trabecula and cortical bone destruction in BCP group.Conclusion The expression of spinal NMUR2 is up-regulated in rats with BCP and this change may be involved in the development and maintenance of BCP.
