Objective:To introduce the working mode and training process of specialized clinical pharmacists in the United States. Methods:By visiting and learning the experience of specialized clinical pharmacist training in the United States, the process and re-quirements of specialized clinical pharmacy were illustrated. Through the content analysis on pharmaceutical care of specialized clinical pharmacists in the United States, the work patterns and responsibilities were understood. Results and Conclusion:The duties of spe-cialized clinical pharmacists in USA are more refining. A three-step pattern is clearly presented in the training system of clincial phar-macists in USA, and specialized clinical pharmacists can play important roles in pharmaceutical care.

AIM:To detect the effect and underlying mechanism of parthenolide ( PN) on neointimal hyperpla-sia.METHODS:After 1 week of high-fat feeding, 30 male New Zealand white rabbits (2.0~2.3 kg) were randomly di-vided into 6 groups: sham +NS, rabbits received 0.9% normal saline after sham operation; sham +DMSO, rabbits re-ceived DMSO after sham operation;balloon injury(BI)+NS, rabbits received NS after balloon injury;BI+DMSO, rabbits received DMSO after balloon injury;BI+PN low, rabbits received PN at 1 mg/kg after balloon injury;BI+PN high, rab-bits received PN at 2 mg/kg after balloon injury .The drugs were intraperitoneal injected once a day after the operation until sacrifice.After fed with high-fat diet for 4 weeks, the intima-media thickness, the expression of caspase-1, IL-1β, the lev-els of IL-8, TC, TG, LDL and HDL in the serum were measured .RESULTS:Compared with sham +DMSO group, the thickness of intima, the amount of caspase-1, IL-1βand IL-8 in BI +DMSO group were significantly increased ( P <0.05).The levels of caspase-1, IL-1βand IL-8 were significantly decreased in BI +PN high group compared with BI +DMSO group (P<0.05).CONCLUSION:Neointimal hyperplasia is suppressed by PN after balloon injury , the potential mechanism may be associated with its anti-inflammatory role .

Objective To study the influence of different bed rest time on orthostatic hypotension,and explore a feasible and appropriate bed rest time for elderly patients after hemodialysis.Methods 58 elderly patients with regular hemodialysis were selected from January 2011 to May 2012.By self-control,they were divided into three groups according to different bed rest time:T0 group who got up immediately after the hemodialysis,T10 group who got up after 10-minute bed rest,T20 group who got up after 20-minute bed rest.The incidence of orthostatic hypotension was observed and compared between three groups.Results Compared to T0 group,the incidence of orthostatic hypotension in T10 and T20 groups was significantly reduced.However,no difference was seen between T10 and T20 groups.The acceptability rate in T10 group was 67.24％,higher than 17.24％ of the T20 group.Conclusions Elderly hemodialysis patients should continue to lie down for at least 10 minutes after hemodialysis,and then slowly get up,this measure can reduce the occurrence of orthostatic hypotension.

Objective To explore the relationship between type 2 diabetes mellitus(T2DM)and thyroid diseases by analyzing the abnormal status of thyroid hormone and antibodies in T2DM patients and the healthy control group.Methods The serum thyroid stimulating hormone (TSH),free triiodothyronine (FT3),free thyroxine (FT4),anti-thyroglobulin antibody (TGAb)and anti -thyroid peroxidase antibody (TPOAb)were measured in 396 patients with T2DM and 411 healthy persons.Results In the healthy group,the incidence rate of abnormal thyroid hormone was 7.5%,in which the male was 5.5% and female was 9.4%,the incidence rate of subclinical hypothyroidism in females was 4.7%,which was higher than 1.5% in males,there was statistically significant difference between them(P <0.05);the positive rate of TPOAb in the healthy control group was 10.2% and the positive rate of TGAb was 6.6%;in T2DM patients,the incidence rate of thyroid hormone abnormalities was 16.2%,which in males and females were 12.3% and 20.5% respectively,among them the incidence rate of subclinical hypothyroidism in females was 9.2%,which was significantly higher than 4.3% in males,the difference had statistical significance (P <0.01),T2DM patients with subclinical hypo-thyroidism accounted for 40.6% of thyroid dysfunction incidence rate;the positive rate of TPOAb was 15.2% and the positive rate of TGAb was 7.1 %.Conclusion The incidence rate of thyroid diseases in T2DM patients is significantly increased compared with the healthy control group,subclinical hypothyroidism in females is predominant;screening thyroid autoantibodies and monitoring FT3,FT4 and TSH at regular intervals in T2DM have the important clinical significance for evaluating the disease condition,jud-ging prognosis and guiding treatment.

Objective To study the value of multiphasic contrast-enhanced CT in diagnosing focal noudlar hyperplasia(FNH).Methods 12 patients with FNH underwent plain CT scan and multiphasic contrast-enhanced CT scan.Arterial phases was at 25~30 s,venous phases was at 55~60 s,and delayed scans were taken after 180 s.Results The lesions showed hypodensity and slight hypodensity on plain CT in 10 cases,among them,irregular center lower density was found in 5 cases and isodensity was found in 2 cases.In the arterial phases,the lesions appeared obvious in 9 cases,among which 4 cases were enhanced homogeneously and 5 were heterogeneously.No obvious enhancement in the center scar.The supply arteries were seen in 3 cases,speckled enhancement in peripheral area of lesions was seen in 2 cases,no obvious enhancement in 1 case.In the venous phases,the lesions showed slightly hyperdensity in 6 cases,isodensity in 4 cases,and mixture of hyperdensity and hypodensity in 2 cases.12 lesions were hypodense as compared with liver in the delayed phases.Conclusion Multiphasic contrast-enhanced CT scan is specific and accurate in diagnosing FNH and can be used to guide the clinical treatment.

Objective To explore the impact of comprehensive musing intervention on the accuracy of non-stress test (NST). Methods With sealed envelope method, 400 cases of primiparous pregnant women without high risk factors were randomly divided into the research group and the control group with 200 cases in each group. The research group was given comprehensive care intervention, the control group received routine nursing. The incidence of non reaction of non-stress test, Apgar score no higher than 7 points, amniotic fluid pollution and cesarean section rate were compared between the two groups. Results The incidence of non reaction of NST was 15.08% in the research group, which was lower than 29.40% of the control group. The incidence of Apgar score no higher than 7 points and amniotic fluid pollution were 8.5% and 19.0% respectively in the research group, which showed no significant difference compared with those of the control group. The cesarean section rate in pregnant was 38.5% in the research group, which was significantly lower than 65.5% of the control group. Conclusions Comprehensive care intervention is effectively to improve the accuracy of NST , so that it can reduce cesarean section rate.

OBJECTIVE To analyze the monitoring performance and testing efficacy and reliability of standardized disposable B-D test pack and standard linen test pack,and summarize the corrective actions based on failure analyses for future reference.METHODS The two different test packs were employed to evaluate the vacuum performance of dynamic air removal sterilizer.RESULTS We enrolled 400 standardized disposable B-D test packs and standard linen test packs respectively.The disposable pack failed 4 cases with the success ratio 99%,while the linen pack failed 30 cases with the success ratio 92.5%.CONCLUSIONS The poor conformity of hand-made linen test pack,variation of steam pressure,sterilizer failure,and unprofessionalism of sterilizer operator contribute as main failure causes in B-D test.Standardized disposable B-D test pack can decrease subjective factors significantly,and the test results are more reliable and standardized.

OBJECTIVE To study the period of validity of iodine skin disinfectant povidone-iodine(Iodophor) in different seasons,different hours and different using frequency.METHODS In spring and summer respectively,the iodine was used for 10 days continually after the bottle opened.Then,the sample drawing from bottle was detected for iodine content,pathogenic bacteria and contamination of bacteria quantification.RESULTS The available iodine content was coming down gradually with the increasing in usage time and frequency.And the changing in the content differed in spring and summer.Microorganism was not found in povidone-iodine under using time,different seasons and using frequency.CONCLUSIONS The skin disinfectant can be used for 10 days at least under standard range after begining using.

Objective To systematically assess the safety of recombinant human interferon α1b(rhIFNα1b) as therapy for viral diseases in children,so as to provide on evidence-based medicine for the clinical treatment.Methods Randomized controlled trails (RCTs) of rhIFNα1b for viral diseases in children were investigated through PubMed literature retrieval service system,Science Citation Index,China National Knowledge Infrastructure,WanFang Database;RCTs were selected according to the inclusion and exclusion criteria.Related data were extracted and the Meta-analysis was performed.Results Nineteen RCTs were involved,including 2 731 patients.In the overall,59/1 437 cases (4.1％) in the rhIFNα1b treatment group and 79/1 294 cases (6.1％) in the control group had adverse reactions.The Meta-analysis revealed that in the overall and in atomization inhalation subgroup,the incidence of adverse reactions was significantly lower in the rhIFNα1b treatment group than that in the control group [Z =2.18 (P =0.03),RR =0.71(95％ CI:0.52-0.97);Z =2.44(P =0.01),RR =0.53 (95％ CI:0.32-0.88)].But,there was no significant difference in the incidence of adverse reactions between the rhIFNα1b treatment group and the control group in intramuscular injection subgroup,and the test for overall effect was Z =0.78 (P =0.43),RR =0.86 (95 ％ CI:0.58-1.26).The incidence of adverse reactions of the control group was significantly higher than that of the rhIFNα1b treatment group in gastrointestinal adverse reaction [Z =2.20 (P =0.03),RR =0.60 (95 ％ CI:0.39-0.95)],and the incidence of adverse reactions of the rhIFNα1b treatment group was significantly higher than that of the control group in nervous system symptoms [Z=2.09(P=0.04),RR =4.28(95％ CI:1.10-16.72)].Conclusion Compared with other antiviral drugs,the treatment of pediatric common viral diseases with rhIFNα1 b has good safety,low incidence of adverse reactions,and the incidence of adverse reactions through atomization inhalation can be lower than that of intramuscular injection.
