Objective To evaluate and compare the safety and advantage of different kinds of electrode system (monopolar and plasmakinetic techniques) in transcervical resection of myoma (TCRM). Methods A total of 60 patients undergoing TCRM were enrolled. Serum sodium (Na~+), chlorine (Cl~-), potassium (K+) and glucose (Glu) were measured before and after the operation. Meanwhile, the operation time, absorption volume and bleeding volume were also recorded after the operation. Results ① There were significant decreases in Na~+, Cl~-, K~+ and Glu concentrations in the monopolar group (P<0.05), but Glu increased significantly (P<0.05). Na~+, Cl~-, K~+ and Glu concentrations in plasmakinetic group, however, did not change significantly before and after the operation (P>0.05). ② After the operation, there were significant differences between the two groups in Na~+, Cl~- and Glu changes (P<0.05). The absorption volume and volume of bleeding during the operation were increased in monopolar group compared with those in plasmakinetic group (P<0.05). ③ In monopolar group, Na~+, Cl~- and Glu concentrations, the operation time, absorption volume and volume of bleeding were significantly higher in patients with type Ⅱ myoma than in those with type Ⅰ and 0. In plasmakinetic group, the operation time, absorption volume and volume of bleeding were significantly increased in type Ⅱ myoma patients. No significant difference was observed of Na~+, Cl~- and Glu concentrations between patients with the three types of myoma (P>0.05). Conclusion Plasmakinetic technique is superior to monopolar one in that it does not cause obvious changes in blood electrolytes and glucose and there is less bleeding during the operation. Myoma type has a greater effect on the absorption volume, operation time and volume of bleeding during operation, which are increased in type Ⅱ myoma than in type 0 and 1.

BACKGROUND:Keloid is a very chalenging problem in plastic surgery. Its pathogenesis is very complex, resulting from the combined action of many factors, such as various cytokines, signal transduction pathways, extracelular matrix, etc. At present, a critical role for mechanical force and mechanotransduction in the pathogenesis of keloids has been broadly concerned and becomes the focus of studying the pathogenesis of keloids.
OBJECTIVE:To summarize the progress of the mechanosignaling pathway involved in the pathogenesis of keloids in order to further understand the pathogenesis of keloids and provide new ideas for the prevention of keloids.
METHODS: The PubMed database and Elsevier database were retrieved for articles published from January 2000 to July 2014 by computer with key words of “keloid, molecular mechanism, mechanical stress, cutaneous scar, mechanobiology, mechanosignaling pathway” in English. A total of 23 articles were included which related to the molecular signal transduction mechanism and mechanosignaling pathway about keloids.
RESULTS AND CONCLUSION:The mechanosignaling transduction pathways, such as transforming growth factor-β/Smad, MAPK, integrin, Wnt/β-catenin, RhoA/ROCK and tumor necrosis factor-α/nuclear factor-κB signaling pathway, play an important role in the formation and development of keloids. A number of clinical trials have also shown the effectiveness of a part of mechanosignaling transduction pathway inhibitors in wound healing and reducing scar hyperplasia. The research about mechanosignaling transduction pathways involved in keloids has made some progress, but most stil remain in animal experimental stage. Secondly, various mechanosignaling transduction pathways about correlation and intersectionality stil need further studies to achieve a breakthrough in the prevention of keloids.

Objective To discuss the preventive measures of anterior palatal fistula by modified the operation of cleft palate. Methods For 23 patients of complete cleft palate with alveolar ridge cleft, bilateral mucoperiosteum flap was moved forward as possible so as to close fistula front of hard palate, and incised junction of the hard and soft palate to prolong soft palate and the wound was repaired by buccal mucosal flap. The incidence of anterior palatal fistula and velopharyngeal closure after operation was observed. Results Twenty-three patients were rechecked 1 month after operation ,there was no anterior palatal fistula occurring,10 cases were examined by epipharyngoscope 1 year after operation, the velopharyngeal closure was 90%-100%. Six cases were followed up for 6 months, the velopharyngeal closure was 80%-85%. Conclusions For second-stage operation methods of anterior palatal fistula, there are too many discussions of selection criterias, advantages and disadvantages. If first-stage operation is taken measures to prevent anterior palatal fistula or decrease the diameter of fistula as possible. It reduces percentage of second-stage operation or decreases the difficulty. It should get more attention in the clinical works.

AIM: This study was aimed to assess the effect of mineral trioxide aggregate (MTA) combined with fibronectin on reparative dentinogenesis and odontoblast differentiation, investigate the possible mechanism underlying traumatized pulp repair, which would facilitate the choosing of ideal pulp capping agents. METHODS: 192 teeth from 16 adult cats were used. Class V cavties were prepared on the facial surface. The pulps of cats were exposed mechanically and then capped directly with MTA, MTA combined with fibronectin, calcium hydroxide, fibronectin and starch as control. The pulpal tissue reactions were assessed by light microscopy at healing interval of 1 and 4 weeks.RESULTS: 1 week after pulp capping, no reparative dentin formation was observed in each group. 4 weeks after pulp capping, the dental pulps capped with MTA/fibronectin showed no or slight cellular inflammatory responses and odontoblast differentiation with tublar dentin bridge on the exposed sites. In MTA group, the dental pulps showed slight to moderate cellular inflammatory responses with osteodentin formation. In calcium hydroxide group, the pulps showed moderate to severe inflammatory responses with calcified matrix. In fibronectin and starch group, no dentin bridge formation was observed, the pulps showed severe cellular inflammatory responses.CONCLUSION: (1) MTA combined with fibronectin induced to form complete and tublar dentin bridge. In MTA group, most of the formed dentin bridge was irregular and osteodentin. (2) MTA combined with fibronectin is a bioactive pulp-capping agent and has a great worth in vital pulp therapy.

Objective To identify gene mutations and explore the molecular mechanism of a pedigree with inherited coagulation F Ⅶ deficiency. Methods The levels of F Ⅶ: Ag in the proband and other family members were measured by ELISA assay. The values of PT, F Ⅶ: C and other coagulant parameters were determined by one-stage clotting for laboratory phenotype diagnosis. All the exons,exon-intron boundaries and 5',3' untranslated sequences of F7 gene were amplified by direct sequencing. The detected mutations were further confirmed by sequencing the other stand. The CLC Protein Workbench software was used to analyze the species conservation of the mutated site and the protein secondary structure. 100 healthy individuals were selected to exclude gene polymorphism. Results PT, FⅦ∶C and FⅦ: Ag in the proband and his sister were abnormal, which were 36. 3 s, 5.0%, 40. 7% and 33.4 s,5. 0%, 37.4%, respectively. Both PT and FⅦ∶C in the proband's father, mother, daughter and niece were slightly abnormal, which were 14.9 s, 14. 6 s, 15.5 s, 14. 6 s and 70%, 85%, 59%, 79%, respectively. The heterozygous mutations c. 784T > C and c. 964T > G in exon 8 of F7 gene were found in the proband, resulting in the substitutions of Ser269Pro and Cys329Gly respectively. Compound heterozygous mutations c. 784T > C and c. 964T > G were found in the proband's sister. The proband's mother was heterozygous for c. 784T > C. His father, daughter and niece were heterozygous for c. 964T > G. The protein biological characteristics analysis revealed that the Cys329Gly caused the change of spatial configuration, and Ser269Pro led to the change of amino acid polarity and hydrophobicity. Conclusion Compound heterozygous mutations of Cys329Gly and Ser269 Pro in F7 gene may be the underlying molecule mechanism of FⅦ deficiency in this pedigree.

Objective To investigate the variety and clinical value of the neutrophil volume and cytoplasm-nucleus complex in patients with bacterial infection, cardiovascular or cerebrovascular accident and major surgery operation. Methods 125 patients with bacterial infection, 64 patients with acute cardiovascular or cerebrovascular accident, 66 patients after major surgery operation and 69 normal subjects were selected in the study. Total WBC counts (WBC), percentage of neutrophils (NE), and the VCS parameters of neutrephils including the mean channels of cell volume ( NEV), conductivity ( NEC), light scatter(NES) and the SD of these parameters( NEVSD, NECSD, NESSD)were measured by automatic blood cell analyzing instrument. The sensitivity and specificity of the WBC, NE and the VCS parameters of neutrophils were analyzed with receive operating characteristic (ROC) curve. Results The levels of NEV, NES and NEVSD in acute bacterial infection group were 154.3 ± 15.2, 135.7 ± 9.9, 26.8 ± 4.2 respectively. The levels of NEV, NES and NEVSD in post major surgery operation group were 147.2±8.9, 141.5 ± 7.7, 23.0 ± 2. 8 respectively. The levels of NEV, NES and NEVSD in acute cardiovascular or cerebruvascular accident group were 144.9 ± 5. 2, 146.0 ±5.0, 19. 6±1.6 respectively. The levels of NEV, NES and NEVSD in healthy control group were 139.7±4.6, 145.0±3.8, 18.2±1.3 respectively. The differences of these parameters among these groups had statistical significance ( F = 17. 650, 38. 122, 54. 604,P<0. 05). And the changes of NEV, NES and NEVSD in bacterial infection group were most obvious among those three groups. The levels of NEV, NES and NEVSD were 146.5±9.5, 144.3 ± 9.4, 21.3 ± 3.3 respectively in stress diseases groups which included acute cardiovascular or cerebrovascular accident group and post major surgery operation group. The differences of these parameters between stress diseases group and acute bacterial infection group had statistical significance ( t = - 2.840, 7.533, - 8.999,P<0.01). The areas under the ROC curve of NEVSD, NEC, NES and NECSD were 0.893, 0. 845, 0. 833 and 0. 849 respectively. The sensitivity of 83. 3% and specificity of 82. 0% could be achieved by selecting the cut-off equal to or greater than 24. 0. Conclusions The variety of neutrophil volume, nuclear size and cytoplasmic granularity changed obviously in patients with acute bacterial infection and common stress diseases, and the variety in acute bacterial infection is more obvious than that in common stress diseases. The sensitivity and specificity of VCS parameters of neutrophils are higher than those of WBC or NE for predicting infection, and the NEVSD is the most predictable indicator of acute bacterial infection.
