1.Analysis of psychological pressure of nurses in wards of retired cadres and application effect of incentive mechanism

Objective To understand psychological pressure of nurses in wards of retired cadres,eatablish incentive mechanism accordingly and further improve nursing quality. Methods Incentive mechanism was applied to nurses in wards of retired cadres to settle their psychological pressure. Results After application of incentive mechanism,14 nurses and 1 nursing worker showed placid psychological state,keep forging ahead actively.Nursing quality of the wards increased. Conclusion Application of incentive mechanism can effectively alleviate psychological pressure of nurses.
2.Centralizing Management of Recycled Medical Instruments by Center for Sterilization,Supply and Distribution of a Hospital

OBJECTIVE To study the effect and development of centralizing management of the overall recycled medical instruments toward the hospital nursing management by the Center for Sterilization,Supply and Distribution.METHODS The centralizing method and decentralizing method toward the recycled handling of the medical instruments were adopted and compared their effect toward nursing management quality,vocational safety,hospital infection management,and of the two methods cost and benefit management,respectively.RESULTS The centralizing management group had distinct advantage than the decentralizing one on the qualified rate of cleaning,packaging,the qualified rate of the sampled incubation of the aseptic products and work efficiency.CONCLUSIONS Through the centralizing management of the Center for Sterilization,Supply and Distribution toward the overall recycled hospital instruments,it can improve the hospital′s nursing staff resource utilization and nursing quality,help to perfect the nursing staff′s vocational safety;decline the hospital infection risk;increase the output of the hospital nursing work and reduce its operational cost;and can make for improving and protecting the environment.
3.Expression of NLRP3 inflammasome in the BSA-overloaded rats kidney

Objective To observe NLRP3 inflammasome expression and inflammatory cells infiltration in the BSA-overloaded rats kidney,and to investigate the potential mechanism of renal injury induced by proteinuria.Methods After unilateral right nephrectomy,eighteen healthy male Wistar rats were randomly divided into two groups:protein overload nephropathy model group (n=10),treated with intraperitoneal injections of bovine serum albumin (BSA); control group (n=8),treated with intraperitoneal injections of 0.9% saline for 9 weeks.Body weigh were measured every week and 24 h urine were collected in 0,2,5,7,9 week.The plasma levels of blood total protein (TP),albumin (Alb),serum creatinine (Scr) and blood urea nitrogen (BUN) were determined by automatic analyzers.Renal pathological changes were evaluated by PAS and Masson stains.Immunohistochemical staining was used to detect the expression of NLRP3,caspase-1,IL-1β,and IL-18,as well as the types of inflammatory cells.The NLRP3,caspase-1,IL-1β,and IL-18 protein and mRNA levels were also analyzed by Western blot and real-time PCR in two groups.Results It was found that there was a significant increase of proteinuria and BUN in model group compare to that in control group (all P < 0.05).However,there were no significant changes in body weight,TP,Alb and Scr between the two groups.Morphological study demonstrated that renal tubular epithelial cell injury,proteinaceous casts in tubular lumen,accompanying with the dominant macrophages and lymphocytes infiltration in interstitium in model group.The immunohistochemistry showed that there were more T (CD3+),B cells (CD20+) and macrophages (CD68+) in renal interstitium in model group than that in control group (P < 0.05).Tubulointerstitial injury score was higher than that of the control group (P<0.05).Immunohistochemistry,Western blot and real-time PCR all showed that the expression of NLRP3,caspase-1,IL-18 and IL-1 β were significantly increased compared to those in control group (P < 0.05).Furthermore,there were significant correlations between proteinuria and IL-lβ/IL-18 expression (P < 0.05).Conclusion NLRP3 inflammasome activation is involved in tubulointerstitial inflammation caused by proteinuria.
4.THE EFFECT OF ?-ELEMECE ON REVERSAL MULTIDRUG RESISTANCE AND THE EXPRESSION OF P-GLYCOPROTEIN IN K562/ADM

Objective To explore the effect of ?-elemece on the reverse of drug resistance and the expression of P-glycoprotein(P-gp) in MDR K562/ADM cells. Methods MTT method was used to detect the sensitivity of cell and its reverse of drug resistance.Immunohistochemistry and immunoelectron microscopy were for observing the expression of P-gp.It's quantitative analysis was determined by flow cytometry.All experimental data were dealt with by SPSS(10.0 production pacility) soft ware. Results 1.Non-cytotoxic dose of ?-elemece(4.0?mg/L) could obviously decrease the IC-(50) value of K562/ADM cells to ADM.The reversing fold was 2.18;2.The level of P-gp expression was higher in K562/ADM cells than that in its parent K562 cells.Our studies also showed that the distribution of P-gp was on membrane of K562/ADM cells;3.Non-cytotoxic dose of ?-elemece(4.0?mg/L) could remarkably decrease the P-gp expression in MDR K562/ADM cells.Conclusion\ ?-elemece could evidently reverse drug resistance of K562/ADM to ADM.Decreasing the P-gp expression in drug resistance cell is one of the main mechanism of reversion.

The relationship between C936T polymorphism at 3'-untranslated region of vascular endothelial growth factor (VEGF) gene and diabetic nephropathy (DN) was analysed in 194 type 2 diabetic patients. The frequencies of genotype CC and allele C were significantly higher in DN group than those in non-DN group and control group. Allele C and genotype CC of VEGF may be a genetic marker susceptible to DN.
6.Comparison on two methods of HPLC-MS/MS and MEIA in monitoring the tacrolimus concentrations for organ transplantation patients

AIM: To compare two methods of HPLC-MS/MS and MEIA to monitor the tacrolimus concentrations for organ transplantation patient.METHODS: After developing and validating the HPLC-MS/MS method to determine the concentrations of tacrolimus,the tacrolimus samples were quantitated by two methods of HPLC-MS/MS and MEIA,respectively.Evaluate two methods by statistical analysis of quantitative results.RESULTS: The mean concentration were(4.86?0.46) ng/mL by HPLC-MS/MS and(5.52?0.43) ng/mL by MEIA.The coefficient correlation of two methods was 0.8771,and the two methods have good correlations in statistics.The mean concentration ratio was(90.3?5.3)%.CONCLUSION: The HPLC-MS/MS method is a more precise method for to determining the effective concentration of tacrolimus,which is suitable for daily TDM.
7.Influence of albumin on expression of NLRP3 inflammasome in renal tubular epithelial cells

Objective To investigate the effect of albumin on expression of NLRP3 inflammasome and its downstream cytokines IL-1β and IL-18 in tubular epithelial cells.Methods Thirty mesangioproliferative glomerulonephritis (MsPGN) patients with different levels of proteinuria were selected, and their renal biopsy samples were stained by PAS and Masson to observe tubular epithelial cells injury and inflammatory cells infiltration.NLRP3, caspase-1, IL-1β and IL-18, as well as different inflammatory cells, were detected by immunohistostaining.In vitro, Western blotting and real-time PCR were employed to detect NLRP3, caspase-1, IL-1β and IL-18 protein and mRNA in HK-2 cells stimulated by bovine serum albumin (BSA) (20 g/L).Results In MsPGN patients with high levels of proteinuria, there were obvious renal tubular epithelial cell injury and inflammatory cells infiltration (all P < 0.05), and the expressions of NLRP3, caspase-1, IL-1β and IL-18 were up-regulated compared to patients with low levels of proteinuria (all P < 0.05).Furthermore, IL-1β and IL-18expressions were positively correlated with the degree of proteinuria (r=0.836, P < 0.05;r=0.901, P <0.05).NLRP3, caspase-1, IL-1β and IL-18 protein and mRNA were significantly increased in HK-2cells stimulated by BSA compared to the control group (all P < 0.05).Conclusions Albumin is able to induce NLRP3 inflammasome activation in tubular epithelial cells, which may be the mechanism of tubulointerstitial injury and inflammation caused by proteinuria.
8.Free carnitine levels in peripheral blood of healthy pregnant women in third trimester and their relationship with maternal and fetal cardiac function and structure

Objective:To investigate the levels of periphreal blood free carnitine and amino acids in healthy pregnant women in the third trimester and their association with maternal, fetal, and neonatal cardiac function and structure.Methods:This prospective descriptive study included healthy singleton pregnancies who underwent routine obstetric examination and delivered in two district maternal and child health hospitals (one in the urban and one in the suburb an area) in Beijing from June 2017 to February 2018. All recruiters had serology Down's syndrome screening test at (18±1) gestational weeks. Besides measurement of amino acids and free carnitine levels in whole blood and urine samples by liquid chromatography-tandem mass spectrometry, all cases underwent maternal and fetal echocardiography at (35±1) weeks of gestation. And neonatal echocardiography was performed after delivery to assess the heart function and structure. Antenatal factors were also collected, including maternal education background, age at first marriage and conception, gravidity, and folic acid supplement in early pregnancy. Statistical analysis was performed using t-test, ANOVA, Chi-square test, Pearson correlation coefficient, and Kappa test. Results:A total of 493 mother-neonate dyads were enrolled in this study. Blood free carnitine levels in the healthy pregnant women in the third trimester ranged from 5.09 to 59.17 μmol/L (reference value: 10.00-50.00 μmol/L) with an average value of (13.03±3.87) μmol/L. None was found with structural abnormalities by cardiac ultrasound, showing an average left ventricular end diastolic diameter (LVEDD) and end systolic diameter (LVESD) of (45.70±3.08) mm and (29.17±3.12) mm, respectively, and left ventricular ejection fraction (LVEF) of all cases were over 55%. No cardiac malformation was detected by the third-trimester fetal echocardiography. The average birth weight of the 493 newborns was (3 340±313) g. Those whose birth weight <2 500 g and >4 000 g were accounted for 1.0% (5 cases) and 3.0% (15 cases) with the average maternal blood free carnitine level of (13.25±2.17) μmol/L (10.46-19.21 μmol/L) and (12.64±2.50) μmol/L (8.78-17.73 μmol/L) ( t=0.42, P>0.05). The average LVEDD and LVESD of the 493 newborns were (17.21±1.27) mm and (11.03±1.30) mm, respectively. For the 64 newborns (13.0%) whose LVEF<60%, the maternal blood free carnitine level was (12.93±2.78) μmol/L (7.34-22.13 μmol/L), showing no statistical difference ( t=-0.29, P>0.05) with those 59 neonates (12.0%) whose LVEF over 75% and maternal carnitine level of (13.09±3.24) μmol/L (8.66-27.49 μmol/L). All cases were divided into four groups based on the quartiles of maternal blood free carnitine level and no significant difference in maternal or neonatal LVEDD or LVEF was observed among these groups (all P>0.05). Conclusions:Blood free carnitine concentration in healthy pregnant women in the third trimester is at the lower limit of normal range, and no significant effect on maternal cardiac function and fetal cardiac structure is seen. However, the effect of low maternal carnitine level in the third trimester on children's myocardial function and whether carnitine should be supplemented in the third trimester are worthy of further investigation with larger sample size.
9.Esthetic reconstruction of the anterior teeth area following a combined periodontic-orthodontic treatment in adult periodontal patients.

OBJECTIVETo evaluate the role of the combined periodontic-orthodontic treatment in the esthetic reconstruc- tion of the anterior teeth area following periodontitis.
METHODSThirteen adult patients with anterior teeth displacements were treated. The probing pocket depth (PD; 102 teeth, 612 sites), bleeding on probing (102 teeth, 204 sites), papilla index (PI; 128 papillae), and papillary height (PH; 128 papillae) of each patient were assessed at baseline, 3 months after the initial therapy, and the end of the orthodontic treatment. Non-parametric and paired-sample t tests were carried out for the statistical analysis of the data.
RESULTSThree months after initial therapy, the sites with PD ≤ 3 mm accounted for 79.58% (487/612) of the observed teeth, and 88.73% (181/204) of the buccal and lingual sites of the teeth showed negative bleeding on probing. These findings were better than those at baseline [26.31% (161/612) and 22.06% (45/204), respectively] (P < 0.05), but no sig- nificant difference was observed compared with pro-orthodontic treatment (P > 0.05). Prior to orthodontic treatment, the levels of the PI of 8 and 21 papillae were III and II, respectively, among the 128 observed papillae. After the orthodontic treatment, 51 papillae were at level III and 68 papillae were at level II. The PH of the 102 papillae was 2.84 mm ± 0.62 mm after ortho- dontic treatment. This result indicated significant difference compared with that of pre-orthodontic treatment (1.69 mm ± 0.57 mm) (P < 0.05).
CONCLUSIONAfter initial therapy, moderate orthodontic teeth movements may reconstruct the interproximal soft tissue, with esthetic improvement of the papillary level and resolution of the periodontal defects.

10.Control observation between mild moxibustion and TDP for obsolete collateral ligament injury of interphalangeal joints.

OBJECTIVETo compare the effects between mild moxibustion and specific electromagnetic spectrum therapy apparatus (TDP) for obsolete collateral ligament injury of interphalangeal joints.
METHODSSixty patients were randomly divided into a mild moxibustion group and a TDP group, 30 cases in each one. In the mild moxibustion group, pure moxa sticks were used at the affected digital joints locally for 20-30 min a time. In the TDP group, TDP was applied at the affected digital joints locally for 20-30 min a time. The treatment was given once a day for two courses, and 10-day treatment was made into a course. Visual analogue scale (VAS) for pain, swelling degree of the affected digital joints before and after treatment were observed and the clinical efficacy and safety were evaluated in the two groups.
RESULTSThe excellent rate was 56.7% (17/30) and the excellent and, good rate was 83.4% (25/30) in the mild moxibustion group,which were better than 36.7% (11/30) and 76.7% (23/30) in the TDP group respectively (both P < 0.01). After treatment the score of VAS and digital joints swelling degree were improved than those before treatment in the two groups (P < 0.01, P < 0.05), and the improvements of the mild moxibustion group were better than those of the TDP group (P < 0.01, P < 0.05).
CONCLUSIONMild moxibustion can apparently relieve the painful and swelling degree of obsolete collateral ligament injury of interphalangeal joints, which is superior to TDP.
