1.Development of titanium-posted keeper-removable magnetic attachment and its effect on magnetic resonance imaging

Objective: To develop titanium posted keeper removable magnetic attachment and to decrease its influence on magnetic resonance imaging(MRI). Methods: Pre fabricated soft magnetic alloy pin cap keeper(PSAPK), casting non precious alloycoping keeper(CNPAK), casting precious alloy coping keeper(CPACK), titanium post keeper(TPK) and titanium post(TP) were prepared and applied on the maxilla of a volunteer, MRI was taken and measured. Results: ①The attachments producing artifacts in MRI from the biggest to the smallest were listed in following rank: PSAPK, CNPAK, CPACK, TPK and TP. ②After removing the keeper from titanium post keeper, the post had no artifact on MRI. ③The artifact was the smallest taken with SE sequence. Conclusion: Titanium post keeper is an effective way to solve the artifact problem and will bring benefits to the patients who need MRI examination of head.
2.Effect of goal-directed fluid therapy on the intestinal blocking of the elderly patients with colon cancer surgery

Objective To evaluate the effects of two fluid therapy methods guided by SVV and CVP re-spectively on intestinal barrier of colon cancer surgery with elderly patients. Methods 50 elderly patients with ASA Ⅱ~Ⅲ level were randomly divided into Group S (fluid therapy guided by SVV) and Group C (convention-al fluid therapy). Each group has 25 cases. Patients in group S were treated under goal-directed fluid infusion strategy with target of 10%≤SVV≤12%. Group C received conventional infusion characterized by monitoring central venous pressure (CVP) and mean arterial pressure (MAP). Fluid volume, surgery time, urine volume, blood loss, exhausting time, post-operative hospitalization days in Group S and Group C were recorded. MAP, HR, CVP, SVV, HCO3- of patients were recorded at the same time before anesthesia (T0), beginning of surgery (T1), one hour after surgery (T2) and ending of surgery (T3) respectively. Venous blood samples were collected at the time points of T0, T1, T2 and T3 to detect DAO and D-lactic acid levels. Results MAP and CVP at time points of T2 and T3 in Group S were obviously lower than that in Group C; total intraoperative fluid volume in Group S was obviously less than that in Group C (P < 0. 05); postoperative exhausting time and hospitalization days in Group S decreased significantly (P < 0.05) and the content of DAO and D-lactic acid in Group S were lower than that in Group C (P < 0.05). Conclusion For elderly patients with colon cancer surgery, fluid thera-py guided by SVV is better than conventional fluid therapy guided by CVP in protecting intestinal barrier.
3.Clinical study of laparoscopic treatment of inguinal hernia in children

Objective To investigate the laparoscopic treatment of ki-67 inguinal hernia in children compared with traditional surgical superiority.Methods Sixty cases of pediatric hernia clinical data were retrospectively analyzed,divided into two groups,one group was treated with traditional surgical (n =30),and another using laparoscopic surgery(n =30).Compare the difference of surgical results,postoperative recovery,and postoperative complications.Results Laparoscopic surgery over traditional surgery in operative time,blood loss,postoperative pain,postoperative ambulation time,gastrointestinal recovery time,hospital stay time comparison,P < 0.05,there is a significant difference; postoperative pain,scar,postoperative complications,recurrence rate,etc.Compare,P < 0.05,there are significant differences.Conclusions Laparoscopic surgery over traditional surgery in shorter operative time,less bleeding,postoperative pain,postoperative ambulation earlier,gastrointestinal recovery time is short,shorter hospital stay,wound scar subtle,fewer complications,recurrence rate low,so the treatnent of pediatric inguinal hernia laparoscopic surgery has the advantages of minimally invasive.
4.Effect of different doses of ambroxol on efficacy and lung function of elderly patients with chronic obstructive pulmonary disease

Objective To investigate the effect of different doses of ambroxol on efficacy and lung function of elderly patients with chronic obstructive pulmonary disease.Methods 200 patients with COPD were selected.They were divided into two groups randomly.The high dose group (102 cases) were given anmbroxol hydrochloride in high doses.The low dose group (98 cases) were given anmbroxol hydrochloride in low doses.The effect of different doses of ambroxol on efficacy and lung function of elderly patients with chronic obstructive pulmonary disease was evaluated by the efficacy,TNF-α,CRP,pulmonary function indexes and adverse reaction during treatment.Results The effective rate was 89.2% in the high dose group and 75.5% in the low dose group.The effective rate of high dose group was higher than that of the low dose group (P < 0.05).Before treatment,there were no statistical significance on CRP and TNF-α between two groups.After treatment,the CRP and TNF-α were decreased in two groups and lower in the high dose group (P < 0.05).Before treatment,there were no statistical significance on FVC,FEV1 and FEV1/FVC% between two groups.After treatment,FVC,FEV1 and FEV1/FVC were increased and higher in the high dose group (P < 0.05).During treatment,there were no statistical significance on adverse reaction between two groups.Conclusion High dose of ambroxol was effective and safe for COPD.It could reduce inflammation and improve pulmonary ventilation with clinical use.

Secreted frizzled-related protein (SFRP) can inhibit the expression of Wnt signaling pathway through Frizzled protein.The silencing of SFRP gene promoter methylation is associated with the occurrence and metastasis of many cancers such as colorectal cancer,gastric cancer,liver carcinoma,lung cancer and ovarian cancer.Several studies have found that SFRP gene has latent clinical value,which is expected to become the novel target for the gene diagnosis and treatment of cancer.
6.Free airbags, free fixed completely peritoneal laparoscopic inguinal hernia repair clinical research

Objective To investigate the feasibility and applicability of free airbags,free fixed totally extraperitoneal laparoscopic inguinal hernia repair (TEP) treatment of inguinal hernia.Methods In Baotou Medical College,Department of General Surgery,the First Affiliated Hospital,the 40 cases of patients confirmed with inguinal hernia surgery were choosed from Jan.2010 to Dec.2012 as the observed object,depending on the surgical methods.Divided the cases into the TEP group and the open surgery group to observe statistics including postoperative pain,extent of gastrointestinal function recovery time,the time of bedside movement,the number of days of hospitalization,the incidence of postoperative complications,hospital fees indicators.Results The operation time of TEP group was about 20 minutes longer than the open group and the difference was statistically significant (P < 0.05).Compared with the open group,TEP group' s postoperative pain was milder,postoperative feeding time,the time of bedside movement and postoperative exhaust time were earlier,hospital stays were shoter,incidence of postoperative complications was lower,the differences were statistically significant (P < 0.05).The difference was not statistically significant in hospital costs(P > 0.05).Conclusion TEP inguinal hernia greater advantages compared with traditional open surgery,may be appropriate to carry out in my area.
7.Expression and purification of nucleocapsid protein of MERS coronavirus in E.coli

Objective To construct a prokaryotic expression vector pET-22b+with Middle East respiratory syndrome ( MERS) coronavirus nuclocapsid protein( NP) gene and to express and purify N protein.Methods N gene amplified by PCR was inserted into the prokaryotic expression vector pET-22b+.Recombinant plasmid was confirmed using DNA elec-trophoresis and sequencing.NP was expressed in E.coli BL21(DE3) by IPTG induced and purified by cation exchange chromatography using the AKTA purification system.Results The NP gene sequence was proved to be correct by sequen-cing and the protein was expressed in both soluble and insoluble forms in E.coli BL21 ( DE3 ) after IPTG induction.The purity and concentration of recombinant protein was improved obviously by cation exchange chromatography and enrich-ment.Conclusion Recombiant NP of high purity and concentration is purified and will facilitate NP functional research.
8.Typing methods and DL MRSA Library database in study of MRSA causing nosocomial infection

Objective To investigate the drug resistance,source and molecular epidemiology of methicillin-resistant Staphyloccus aureus(MRSA)causing nosocomial infection. Methods Fifty-seven pathogenic MRSA strains were isolated from Beijing Tongren Hospital during 2007 and 2008.K-B method,MIC assay,multiple PCR,automatic repetitive element sequence-based PCR(REP-PCR)typing platform and DL MRSA Library were used to identify the resistant phenotypes,Panton-Valentine leukocidin gene (pvl)and REP-PCR types of the MRSA.Results All strains were classified as 6 antibiotic resistant phenotypes(a-f)based on the resistance to rifampin,clindamycin,levofloxacin and cotrimoxazole.The MRSAs with Staphylococcal cassette chromosome mec(SCCmec)Ⅲ and SCCmec Ⅱ accounted for 91.23% (52/57)and 5.26%(3/57)of all strains,respectively.Only one strain was pvl positive.All strains were typed as REP-A-F(6 types)and three single clones by automatic REP-PCR typing platform,in which REP-C was predominant(30/57,52.63%).Three out of 6 REP-D strains were from laryngology wards.The REP-C-SCCmec Ⅲ were genetically most close to the Brazilian clone-SCCmec Ⅲ in DL MRSA Library.Conclusion s REP-C-SCCmec Ⅲ-a type are the major epidemic hospital-associated MRSA and the REP-D-SCCmec Ⅲ-d is usually isolated from patients received laryngeal surgery. Automatic REP-PCR typingplatform combined with DL MRSA Library database is an effective approach to study the nosocomial infection.
9.Morphology and location of mitochondria during mitosis impacted by ROS

Objective To detect the impact of reactive oxygen species ( ROS) on mitochondrial morphology and distri-bution during mitosis.Methods A viral vector in which the fluorescence gene was specifically under the control of mito-chondrial promoter was constructed and confirmed through DNA sequencing and Western blotting.After transfecting HeLa s3 cell with packaged virus, the HeLa s3-COX4tp-EGFP cell line stably expressing the mitochondrial fluorescence signal was obtained.With immunofluorescent staining, the impact of ROS on the morphology and distribution of mitochondria dur-ing mitosis was inspected.Result The cell line constantly expressing mitochondrial fluorescence signals was successfully constructed.Meanwhile,it was found that H2 O2 treatment could significantly change the morphology and distribution of mi-tochondria during mitosis by confocal microscopy.Conclusion Our study demonstrates that ROS can affect the morphology and distribution of mitochondria during mitosis.This research help study the relationship between the mitochondrial function and the regulation of mitosis in the future.
