1.Observation of the effect of bisoprolol combined with simvastatin in the treatment of elderly patients with unstable angina

Objective To observe the clinical effect of bisoprolol combined with simvastatin in the treatment of elderly patients with unstable angina pectoris.Methods 60 elderly patients with unstable angina pectoris were randomly divided into control group and study group according to the digital table method.The patients in the control group were treated with simvastatin,and the patients in the study group were treated with bisoprolol and simvastatin.The clinical therapeutic effect and adverse reactions of the two groups were compared.Results The total effective rate and the rate of ECG (86.67% and 80%)in the study group were significantly higher than those of the control group (76.67% and 46.67%)(χ2 =4.02,7.1 8,all P <0.05 ).The total incidence rate of adverse reactions (26.67%)in the study group had no significant difference compared with the control group (χ2 =0.08,P >0.05). Conclusion Bisoprolol combined with simvastatin had better clinical effect and small adverse reactions in the treat-ment of elderly patients with unstable angina.It was worthy of clinical use.

3.Effect of Milrinone injection on hypertensive heart disease complicated with heart failure

Objective To observe and analyze the clinical effect of Milrinone injection in the treatment of hypertensive heart disease complicated with heart failure .Methods From February 2016 to January 2017,80 patients with hypertensive heart disease complicated with heart failure in Cilin Hospital were selected and randomly divided into observation group and control group according to the digital table ,with 40 cases in each group.The control group was given conventional therapy ,including antihypertensive therapy and the treatment of heart failure and so on .The observation group was treated with Milrinone injection on the basis of the control group .The clinical manifestations of the two groups were compared and analyzed .Results The cardiac function indicators ,B-type natriuretic peptide (BNP) and 6MWD in the observation group were better than those in the control group after treatment [observation group: HR (59.31 ±2.03) times/min,LVEF (55.98 ±5.31)%,LVEDd (54.37 ±0.83) mm,SBP (119.53 ± 7.04)mmHg,DBP(67.31 ±2.94) mmHg,BNP (198.32 ±21.54) ng/L,6MWD (332.61 ±52.70) m; control group: HR (67.63 ±2.95)min,LVEF (40.51 ±4.96)%,LVEDd (60.55 ±1.47)mm,SBP (128.47 ±8.35)mmHg,DBP (76.37 ±3.75)mmHg,BNP (227.43 ±25.96)ng/L,6MWD (254.78 ±26.16)m],the differences were statistically significant (t=14.6943,13.4651,23.1532,5.1769,12.0250,20.5215,8.3663,all P<0.05).The incidence rate of adverse reactions of the observation group (7.5%) was lower than that of the control group (10.0%),the differ-ence was not statistically significant (χ2=0.156,P<0.05).Conclusion Milrinone injection in the treatment of hypertensive heart disease has good effect , can obviously decrease BNP concentration , increase the patients'life quality,and with no obvious adverse conditions .
4.Evaluation of radiation dose in multislice spiral CT scan for children embedded teeth

Objective To evaluate the radiation dose in both low radiation dose and to conventional radiation dose mode in multislice spiral CT scan for children with embedded teeth,and explore the lowest conditions of exposure in multislice spiral CT scan for children embedded teeth.Methods Eighty cases of children with embedded teeth examined by 16 slices spiral CT were divided into 5 ,25,125 and 250 mAs scanning groups.The scanned pictures of four groups were evaluated by two chief physicians and three associate chief physicians in the radiological department with double blind method.The CTDI_w and DLP in different radiation dose mode were recorded and analyzed with statistics.Results All the four groups scanned pictures in different radiation dose mode could accurately show the structure and the adjacency relation of embedded teeth.Even though the resolution of scanned pictures in S mAs group were inferior to the other three groups on bone trabecula,gingiva and their surrounding soft tissue,it still could meet the requirements of examination.The average DLP in 5 mAs group was only 2% of value in 250 mAs group.Conclusions MSCT in 5 mAs group can not only meet the requirements of examination,but also reduce the radiation dose to patients to the greatest extent.
5.Combination of the contrast-enhanced ultrasound and color Doppler's grade to diagnose breast masses

Objective Comparing the enhancement of contrast-enhanced ultrasound(CEUS) with the intensity of the blood signals of breast masses, and producing the parameter of peak intensity (PI), to determine whether they can reflect the differentiation of the benign breast masses from the malignant ones.Methods Fifty patients with the breast masses (25 benign,25 malignant) were implemented the contrastenhanced ultrasound inspection.The blood signals of the masses could be got before performing the CEUS,then the CEUS was performed.The enhancement of the masses was divided into 4 grades according to the enhancement of breast which was around the mass (no enhancement, low enhancement, equal enhancement,and high enhancement as well).The PIs of all masses and high enhanced massed were calculated by software in machine,then them were compared according to "the groups which had been classified by their maximal diameters.Results Forty-one of 50 cases showed an obvious enhancement using CEUS compared with the routine CDFI.Malignant masses were more obviously than that of benign ones ( P<0.05).In the 50 cases,the no enhanceed ones( n = 2) and equal enhanced ones( n = 5) were benign,and 1 case of the low enhanced masses( n = 9) was malignant.The high enhanced masses ( n = 34) were malignant or benigh.About the high enhanced masses, there were statistics meanings using the parameter of PI for the masses whose maximal diameters<2 cm( P<0.05),and no statistics meanings when their maximal diameters≥2 cm(P>0.05).Conclusions The CEUS of breast can improve the appreance of the tumor' s blood vessel obviously, especially for malignant masses.The PI of the breast benign masses are different from the malignant ones.Combination of them can help to discriminate benign masses from malignant ones.The parameter of PI is useless for differentially diagnosing the breast masses if their maximal diameters≥2 cm and the blood flow grade Ⅲ before CEUS.

Objective To investigate the analgesia effects of ultrasound-guided suprascapular nerve blocks for shoulder arthroscopy without impacting respiratory function.Methods Forty seven patients scheduled for shoulder arthroscopy,17 males and 30 females,aged 26-78 years,weighing 50-75 kg,ASA physical status Ⅰ or Ⅱ,were randomized into suprascapular nerve blocks group (group S,n=24) and interscalene plexus blocks group (group Ⅰ,n =23).0.375% ropivacaine 20 ml and 1 % lidocaine plus 1 ml of dexamethasone 5 mg.The vital capacity before and 6 h after surgery,extubation time,total PACU duration,VAS scores at awake time and 6 h,12 h,24 h after surgery,total opiods consumption,and other complications were recorded.Results The patients in group S exbuted significantly earlier than in group Ⅰ [(13.0±3.9) min vs (21.2±4.0) min,P<0.05].The mean vital capacity 6 h after surgery significantly decreased in group Ⅰ [(2 909±502) ml vs (3 533±726) ml,P<0.05].There were no significant differences of VAS scores or opiods consumption between the two groups.Conclusion Selective suprascapular nerve blocks can provide effective analgesia for shoulder arthroscopy without impacting the vital capacity.

Objective To assess the stiffness and thickness of the plantar fascia using shear wave elastography (SWE) in healthy volunteers of different ages and in patients with plantar fasciitis. Methods The bilateral feet of 30 healthy volunteers and 23 patients with plantar fasciitis were examined with SWE. The plantar fascia thickness and elasticity modulus value were measured at the insertion of the calcaneus and at 1 cm from the insertion. Results The elderly volunteers had a significantly greater plantar fascia thickness measured using conventional ultrasound (P=0.005) and a significantly lower elasticity modulus value than the young volunteers (P=0.000). The patients with fasciitis had a significantly greater plantar fascia thickness (P=0.001) and a lower elasticity modulus value than the elderly volunteers (P=0.000). The elasticity modulus value was significantly lower at the calcaneus insertion than at 1 cm from the insertion in patients with fasciitis (P=0.000) but showed no significantly difference between the two points in the elderly or young volunteers (P=0.172, P=0.126). Conclusion SWE allows quantitative assessment of the stiffness of the plantar fascia, which decreases with aging and in patients with plantar fasciitis.

10.Effects of long-term exposure to new types of light emitting diode sources on neurobehavior of rats

Objective To investigate the effects of long-term exposure to three new types of light emitting diode (LED) sources on the behavior, learning, and memory of rats. Methods A total of 160 specific pathogen-free SD rats were divided into eight groups as followed, trichromatic fluorescent lamps color temperature control group, violet-chip full-spectrum white LED group, blue-chip white LED group, and blue-chip full-spectrum white LED group based on the light sources types, with color temperature of 4 000 K and 6 500 K groups in each group using the 4×2 factorial design. There were 20 rats in each group, with half of the rats were males and half females. Rats were exposed to artificial lighting, and the illumination was set at 750 lx. The rats in each group were exposed to different lighting environments for 12 hours per day for 24 weeks. The open-field and step-down tests were conducted in rats after 24 weeks exposure, followed by sacrifice of rats and measurement of organ coefficients. Differences in body weight, organ coefficients, and neurobehavioral indexes of rats in different groups were compared. Results The spleen coefficient of female rats decreased in blue-chip white LED of 6 500 K color temperature group, and the liver coefficient of male rats decreased in the violet-chip full-spectrum white LED of 4 000 K color temperature, blue-chip full-spectrum white LED of 4 000 K color temperature, and blue-chip full-spectrum white LED of 6 500 K color temperature groups, compared with the same-sex rats in trichromatic fluorescent lamps with same-color temperature control group (all P<0.05). The result of different types of light sources compared in the open-field test showed that the index of total distance and movement speed of female rats in the blue-chip full-spectrum white LED group were lower than those in the other three groups, and the time cost to the central area was longer than that in the blue-chip white LED group and the violet-chip full-spectrum white LED group (all P<0.05). The total distance and movement speed of male rats in the blue-chip full-spectrum white LED group were longer or higher than those in the violet-chip full-spectrum white LED group (all P<0.05). Based on the comparison of color temperature, the time and total distance of male rats in 6 500 K color temperature group were lower than that in the 4 000 K color temperature group (both P<0.05). In the step-down test, both male and female rats in the blue-chip full-spectrum white LED group made more errors compared with other three groups with the same gender (all P<0.05). Conclusion Based on the experimental conditions of this study, the blue-chip full-spectrum white light LED affects behavior, learning and memory of the rats, and trichromatic fluorescent lamp has the lowest effect on neurobehavior. The color temperature also affects behavior of the rats, and high color temperature has higher risk.
