Objective To study the expressions and clinical significance of matrix metalloproteinases-2(MMP-2) and vascular endothelial growth factor-C(VEGF-C) in patients with papillary thyroid carcinoma(PTC).Methods SP immunohistochemical technique was used to detect the expressions of MMP-2 and VEGF-C in 78 cases of PTC and 18 cases of thyroid benign tumors.Results The positive expression rates of MMP-2 and VEGF-C in PTC(80.77%,75.64%) were significantly higher than those of the thyroid benign tumor(11.11%,22.22%),P

3.Optimal timing for perioperative internal aortic balloon pumb support in high-risk coronary artery bypass grafting

Objective To study the clinical experience and determine the optimal timing for periop-erative internal aortic balloon pumb (IABP) support in high-risk patients who had coronary artery bypass grafting (CABG) had been demonstrated. Methods Fifteen cases of emergency CABG with IABP were performed from September 2003 to April 2008. Eight cases were supported by IABP before operation,1 case was supported after succeeding in cardiopulmonary resuscitation,2 cases were supported by IABP for acute myocardial infarction,5 cases were supported by IABP for acute coronary syndrome. Seven cases were supported by IABP in the time of intra-or postoperation,2 cases were supported by IABP in order to help re-moving cardiopulmonary bypass in the operation. Result Three cases supported by IABP in the time of in-tra-or postoperation were dead, the other 12 cases were healed. Conclusions IABP can improve the heart function effectively and therefore can increased the success rate of operation.The beneficial effect of preoperative IABP in high-risk patients who have CABG is confirmed.

5.Function-preserving parotidectomy for benign tumors involving the deep parotid lobe

Objective:To evaluate the advantages of function-preserving parotidectomy for benign tumors involving deep parotid lobe. Methods:17 patients with benign tumor involving deep parotid lobe were treated by limited partial parotidectomy.All of the tumors were not more than 3 cm of diameter,12 of them were in the deep lobe and 5 involving both superficial and deep lobes.In the operation fascia glandular flap was made,the tumor with the surrounding gland tissue was thoroughly dissected and the glandular parenchyma was preserved.Results:All the operations were successful and the wounds healed well with symmetrical facial contours.No patient experi-enced salivary fistula.Temporary facial palsy was observed in 2 cases(11.76%).No tumor recurrence was observed.Facial nerve function of all patients completely recovered after a median follow-up of 11 months.Conclusion:Function-preserving parotidectomy for benign tumor involving the parotid deep lobe may improve functional outcomes without compromising local tumor control.

The excimer laser diopter correction has proven to be efficient and safe. This paper presents the principle of excimer laser refractive surgery. Based on analyzing the mathematics model of the human eye cornea, the authors have proposed a new model which can be used to proceed the myopia, hyperopia, astigmatism diopter correction. Also studied were the excimer laser's ablation mechanism and the flying-spot scanning technology. The research results have been directly applied to Ophthalmic excimer laser system. The correction of diopter is well improved.

8.The effect of paclitaxel liposome on the apoposis of cancer cells in the lymph node with the metastasis of rabbit tongue cancer

Objective:To study the effect of paclitaxel liposome on the apoptosis of cancer cells in neck metastatic lymph node of rabbit tongue cancer.Methods:24 New Zealand rabbits with VX-2 tumor at tongue edge were divided into 6 groups randomly after neck lymph node metastasis.Paclitaxel liposome,free paclitaxel and 5% glucose were injected into the lingual arterial and ear marginal vein,respectively.A week after chemotherapy metastasis lymph nodes were collected.The apoptosis of the cancer cells in the lymph nodes was examined by transmission electron microscope,flow cytometry,and SP immunohistochemical method for P53 protein expression of the cancer tissue.Results:The apoptosis rate of cancer cells in the intra-arterial chemotherapy group was higher than that in the intra-venous chemotherapy group,in the paclitaxel liposome injection group was higher than that in the free paclitaxel injection group(P < 0.05).The positive rates of P53 protein expression in paclitaxel liposome group by intra-arterial injection was the lowest (P < 0.05).Conclusion:Paclitaxel liposome is more effective than free paclitaxel,regional arterial infusion is more effective than intra-venous infusion in the apoptosis induction of lymph node metastasis cancer cells.
9.Early predictors of refractory epilepsy: a case-control study

ObjectiveTo identify the early predictors of refractory epilepsy (RE). MethodsAll 173 epileptic patients with correct diagnosis and reasonable treatment were enrolled. The 106 patients were classified as drug non-responsive epilepsy (DNR-EP). The remaining 63 patients were classified as drugresponsive epilepsy (DR-EP). With multiple logistic regression, the clinical characteristics between the two groups were compared to identify the early predictors of RE. ResultsMultiple logistic regression analysis demonstrated that more than 10 seizures before treatment (OR =4. 46, 95% CI 1.60-12. 40, P =0. 004),mental retardation at early time ( OR =19. 87, 95% CI 3. 60-109. 78, P =0. 001 ) and abnormal electroencephalogram(EEG) with epileptiform wave after treatment ( OR =7.57, 95% CI 2. 54-22. 56,P <0. 01 ) were independent predictors of RE.Response to initial therapy was a protective factor of RE (OR=0.05, 95% CI 0.018-0. 139, P<0.01). ConclusionPatients who have many seizures before treatment, mental retardation at the early time, epileptiform abnormality in EEG after treatment and who are resistant to initial therapy are likely to develop into refractory epilepsy.
10.The value of T cell subset monitoring for traumatic sepsis patients defined by sepsis-3

Objective To estimate the value of T cell subset monitoring on early diagnosis and prognosis in traumatic sepsis patients defined by sepsis-3.Methods A total of 102 severe trauma patients were prospectively collected in an Emergency Intensive Care Unit (EICU) of a tertiary hospital from 1st July 2017 to 30th September 2017.Finally 28 patients were confirmed as sepsis according to the sepsis-3 guideline.The levels of blood T cell subsets (CD3,CD4,and CD8) were measured at day 1,3 and 7 after ICU admission and after diagnosis.Receiver operating characteristic curve and multivariate regression analysis were used.Results The levels of CD3 and CD4,and CD4/CD8 ratio in severe trauma patients first decreased and then increased within one week after admission,while CD8 level first increased and then decreased.The levels of CD3 and CD4 and CD4/CD8 ratio showed a decreased tendency and CD8 level showed an increased tendency in the death and traumatic sepsis groups.The CD4/CD8 ratio at day 1 after admission was relatively effective in diagnosing traumatic sepsis.The area under the ROC curve was 0.901 with a sensitivity and specificity of 88.89% and 83.33%,respectively.The CD4/CD8 ratio at day 1 after diagnosis of traumatic sepsis was a risk factor for predicting patient mortality (OR=1.43;95%CI:1.27-1.89).Conclusions Cellular immunity in severe trauma patients presents a process of inhibition before recovery.The CD4/CD8 ratio can better diagnose traumatic sepsis and evaluate the prognosis..
