1.Fully automatic AI segmentation of oral surgery-related tissues based on cone beam computed tomography images

Accurate segmentation of oral surgery-related tissues from cone beam computed tomography(CBCT)images can significantly accelerate treatment planning and improve surgical accuracy.In this paper,we propose a fully automated tissue segmentation system for dental implant surgery.Specifically,we propose an image preprocessing method based on data distribution histograms,which can adaptively process CBCT images with different parameters.Based on this,we use the bone segmentation network to obtain the segmentation results of alveolar bone,teeth,and maxillary sinus.We use the tooth and mandibular regions as the ROI regions of tooth segmentation and mandibular nerve tube segmentation to achieve the corresponding tasks.The tooth segmentation results can obtain the order information of the dentition.The corresponding experimental results show that our method can achieve higher segmentation accuracy and efficiency compared to existing methods.Its average Dice scores on the tooth,alveolar bone,maxillary sinus,and mandibular canal segmentation tasks were 96.5%,95.4%,93.6%,and 94.8%,respectively.These results demonstrate that it can accelerate the development of digital dentistry.

Objective To compare the simple cases of hand-foot-mouth disease(HFMD) with HFMD patients complicated with encephalitis and HFMD cases complicated with pulmonary edema (PE). To explore predictor factors of disease progression and unfavorable prognosis. Methods Forty-one EV71-infected children admitted to the Fuyang First People's Hospital in Anhui Province from March to May in 2008 were investigated in the research, who were classified as encephalitis-complicated cases ( encephalitis group, n = 15 ), PE-complicated cases ( PE group, n = 15 ) and simple cases (simple group, n= 11 ). Their clinical manifestation, laboratory findings, and immunophenotypes of peripheral blood lymphocyte were analyzed to find predictors associated with disease progression and unfavorable outcomes. Results The mortality rate in PE group was 66.7%, which was significantly higher than that in encephalitis group. Ninty-three point three percent cases in PE group and encephalitis group were younger than 3 years old, with statistic difference compared to simple group. Patients in PE group had higher total blood white cell (WBC) counts and higher absolute neutrophil counts and tended to have higher breathing rate, heart i'ate and glucose level than encephalitis group. The percentages of T cells and natural killer (NK) cells were significantly lower among patients complicated with encephalitis than simple HFMD patients.Conclusions PE is one predictor for poor prognosis. Factors correlated with unfavorable outcome include high WBC, high absolute neutrophil counts; elevated breathing rate, heart rate and glucose level. The immunophenotypes of peripheral blood lymphocytes can also predict the disease progression.
4.Application of perpendicular plate of ethmoid bone in short nose plasty in Chinese with insufficient septal cartilage

Objective To evaluate the applied value of septal cartilage combined with perpendicular plate of ethmoid bone graft in short nose plasty in Chinese people with insufficient septal cartilage.Methods From February 2015 to March 2017,thirty-five women with short noses underwent septal extension grafting in our institute.We used perpendicular plate of ethmoid bone to enhance the L-strut structure.An L-strut,comprising 0.6 cm segments of the caudal and dorsal cartilaginous septum,was left altered to harvest more cartilages for septal extension.The harvested septal cartilage was grafted on one side of the caudal septum.Then the alar cartilage was fixed at the end of the septal cartilage graft.Nasal lengths and nasolabial angles were measured pre-and postoperatively.Results The average nasal lengthening was (3.37±1.87) mm (P<0.001).There was no infection,cerebral spinal fluid leakage,hemorrhage and other complications postoperatively.The average follow-up was 10 months (6 months to 2 years).After operation,33 patients were satisfied with the nasal contour,1 patient complained of high tip projection whereas one patient complained of low tip projection,and the dissatisfactions were corrected by revision surgeries.Conclusions Combining the septal cartilage with perpendicular plate of ethmoid bone graft presents an adequate nose lengthening and a decreased nostril show,even in patients with very little septal cartilage.With minimum complications,this novel technique provides promising clinical outcomes and high patient satisfaction.
5.Arthroscopic treatment of elbow stiffness using an additional auxiliary soft-point portal

Objective To investigate the feasibility of adding an auxiliary soft-point portal for the arthroscopic treatment of stiff elbow.Methods From January 2016 to August 2017,20 patients with stiff elbow were treated at Department of Orthopaedic Surgery,The Second Affiliated Hospital to Nanchang University.They were 13 males and 7 females,with a mean age of 30.8 years (from 18 to 46 years).Their elbow stiffness time averaged 8.4 months (from 6 to 14 months).Their stiff elbow was released by elbow arthroscopy after their surgical contraindications were controlled.In addition to conventional portals,an auxiliary soft-point portal was used.Analgesia was conducted postoperatively and staged rehabilitation encouraged immediately after operation.The therapeutic effects were evaluated using Hospital for Special Surgery (HSS) elbow score and the Mayo scoring system at final follow-ups.Results The 20 patients were followed up for an average time of 8.9 months (from 4 to 15 months).Their preoperative maximum elbow flexion (62.3°±21.4°),maximum elbow extension (30.4° ± 13.6°) and total range of elbow motion (32.5° ± 22.4°) were significantly improved to 112.6° ± 23.4°,15.3° ± 10.4° and 98.4° ± 15.3°,respectively,at final follow-ups (P <0.05).According to their HSS elbow scores,13 cases were excellent and 7 good,yielding an excellent to good rate of 100%;their preoperative Mayo scores (64.1 ± 12.8) were significantly improved to 85.6 ± 7.4 points at final follow-ups (P < 0.05).Conclusion Addition of an auxiliary soft-point portal in the arthroscopic treatment of elbow stiffness can simplify operative maneuver and shorten operation time,leading to fine curative efficacy.
6.Morphological and functional evaluation of digital technology combined with endoscope-assisted surgery of crooked nose

Objective:To explore the clinical effect of digital endoscopic-assisted one-stage rhinoplasty and septoplasty by using subjective and objective methods.Methods:Thirty-two patients with skeletal crooked nose and nasal septum deviated who underwent endoscopic-assisted rhinoplasty and septoplasty were included in this study from January 2015 to January 2020. This study used objective measurements such as 3D digital imaging technology and CT scans, as well as subjective measurements such as Visual Analogue Scale (VAS), Rhinoplasty Outcomes Evaluation (ROE) scale, Nasal Obstruction Symptom Evaluation (NOSE) and Functional Rhinoplasty Outcome Inventory (FROI-17) to evaluate the crooked nose morphology and nasal respiratory function before and after surgery.Results:Compared with preoperatively, postoperative 3D facial imaging results showed that the deviation distance and deviation angle of the crooked nose were significantly improved (both P<0.05), and the long-term effect of the operation was stable. Subjectively, the patient's appearance VAS score and ROE score were significantly higher than those before surgery, while the NOSE score and FROI-17 score of nasal congestion symptoms were lower than those before surgery. Conclusions:Endoscope-assisted one-stage rhinoplasty and septoplasty can achieve the purpose of repairing the nose appearance and improving the nasal respiratory function at the same time. Through a combination of subjective and objective evaluations, our study found that this procedure had the advantages of minimally invasive, stable effect and shorter recover time; meanwhile, this procedure has high patients' satisfaction and is worthy of clinical promotion.
7.Correlation of serum calprotectin level with the range of coronary lesion in patients with acute coronary syndrome.

OBJECTIVE:
To examine the serum levels of S100 calcium-binding protein A8/A9 complex (S100A8/ A9) in patients with acute coronary syndrome (ACS) and to explore the relation between the serum levels of S100A8/A9 and the degree of coronary lesion.
METHODS:
A total of 126 patients with coronary heart disease were enrolled from Xiangya Hospital of Central South University between September 2010 and January 2011, which included 51 patients with unstable angina pectoris (UAP group, n=51), 50 patients with acute myocardial infarction (AMI group, n=50), and 25 patients with stable angina pectoris (SAP group, n=25). Twenty-five healthy volunteers were served as a normal control group (NC group, n=25). According to the coronary artery lesion area, ACS patients were also divided into a single-branch group, a double-branch group and a triple-branch group. Serum levels of S100A8/A9 were measured by enzyme-linked immunosorbent assay on the day when the patients admitted to the hospital and on the day after one-week treatment (UAP group + AMI group). The serum levels were compared among the various branch groups. The short-term prognosis in patients with ACS was investigated by phone follow-up after 3 months.
RESULTS:
1) The S100A8/A9 level in the SAP group was significantly higher than that in the normal control group (P<0.05). The serum levels of S100A8/A9 in the UAP group and the AMI group were significantly higher than that in the SAP group (all P<0.05); There was no significant difference in the S100A8/A9 level between the UAP group and the AMI group (P>0.05); 2) After one-week standard treatment, the serum levels of S100A8/A9 in patients with ACS were significantly reduced compared with that at the admission (P<0.01), but it was still elevated compared with that in the normal control group (P<0.01); 3) The serum level of S100A8/A9 in the triple-branch group was significantly higher than that in the single-branch group and the double-branch group (both P<0.05); 4) The short-term prognosis in patients with ACS was not correlated with the serum level of S100A8/A9 (r=0.012, P> 0.05).
CONCLUSION
The serum level of S100A8/A9 is significantly elevated in patients with ACS, which might be positively correlated with the number of the coronary lesion branches.

8.Application of early respiratory training program based on 4E model in children with bronchiolitis obliterans

Objective:To explore application effect of early respiratory training schemes based on 4E mode (Engage, Educate, Execute, Evaluate) in children with bronchiolitis obliterans (BO) and provide evidence for the clinical implementation of early respiratory rehabilitation in children with BO.Methods:This was a quasi-experimental study. The children with BO who were admitted to 2 wards of the Department of Respiratory Medicine of Hunan Children's Hospital from January 1 to December 31, 2021 were selected as the research objects. They were randomly divided into the control group and the experimental group, with 23 cases in each group. The control group received routine treatment, nursing and rehabilitation guidance. The experimental group established a multidisciplinary team based on the control group, and used the early respiratory training program based on the 4E model to implement intervention. The clinical symptom severity scale was used to evaluate the improvement of the clinical symptoms of the children within 24 hours of being diagnosed as BO, the day of discharge, and 1 and 3 months after discharge for re-examination, and the hospitalization time of the children and the incidence of adverse events related to respiratory training were counted by using medical records and questionnaires.Results:The clinical symptom severity scores of the experimental group within 24 hours of admission diagnosis and the day of discharge were (20.00 ± 2.51) and (11.30 ± 2.46)points respectively, while those of the control group were (20.57 ± 2.21) and (11.70 ± 2.42) points respectively, with no statistically significant difference ( t=0.81, 0.54, both P>0.05). The clinical symptom severity scores of the experimental group were(10.52 ± 2.31) and (8.55 ± 1.06) points, lower than (12.32 ± 1.39) and (12.45 ± 2.19) points of the control group when they returned to the hospital for re-examination 1 and 3 months after discharge, with a statistically significant difference ( t=3.14, 7.25, both P<0.05). The experimental group was hospitalized for (11.78 ± 1.17) days, which was showter than (13.74 ± 1.63) days in the control group, with a statistically significant difference ( t=4.68, P<0.05). No respiratory training-related adverse events occurred in both groups of children during hospitalization. During home respiratory training after discharge, 1 and 2 respiratory training-related adverse events occurred in the experimental group 1 and 3 months after discharge, respectively, compared with 6 and 9 in the control group. The difference was statistically significant ( χ2=4.64, 5.94, both P<0.05). Conclusions:Early respiratory training solutions based on the 4E mode can improve the clinical symptoms of BO children, shorten the hospitalization time, reduce the number of adverse events related to respiratory training, and promote the recovery of children.
9.Application of fast track surgery in the perioperative nursing of patients with abdominal operation

Objective To analyze the application effect of fast track surgery in the perioperative nursing of patients with abdominal operation .Methods Ninety-one patients with abdominal operation were chosen and divided into the control group (n=45) and the observation group (n=46) according the stratified randomization grouping method.The control group received the routine nursing , and the observation group received the fast track surgery.The preoperative discomfort , postoperative pain, time of gastrointestinal function recovery and the incidence rate of complications after operation were compared between two groups .Results Compared with the control group , the cases of patients with the sensation of thirst , hunger , anxiety in the observation group were significantly less than those in the control group , and the differences were statistically significant (χ2 =3.965, 11.984,17.036, respectively;P<0.05).The time of bowel movement after operation was (50.79 ±2.52) h in the observation group, and was (57.25 ±2.57) h in the control group, and the difference was statistically significant (t=1.795,P<0.05).The degree of postoperative pain in the observation group was lower than that of the control group, and the difference was statistically significant (Z=-4.768,P<0.01).The incidence rate of complications after operation in the observation group was 8.7%, and was lower than 28.9% in the control group, and the difference was statistically significant (χ2 =6.106,P<0.05).Conclusions Compared with conventional nursing , the nursing intervention of fast track surgery has obvious advantage to promote early recovery of patients with abdominal operation .
10.Effects of HSYA on expression of bFGF protein and MMP-9 in BGC-823 transplantation tumor of nude mice.

OBJECTIVETo investigate the effects of hydroxy safflor yellow A (HSYA) on the expression of bFGF protein and MMP-9 mRNA or protein of transplantation tumor of gastric adenocarcinoma cell line BGC-823 in nude mice.
METHODThe BGC-823 cells were subcutaneously injected into the right anterior armpit of BALB/C nu/nu nude mice, and the animal model of transplantation tumor was established. The experimental groups were treated with HSYA at concentration of 0.056 and 0.028 g x L(-1) and cyclophosphamide at 2 g x L(-1), or with physiologic saline. The tumor inhibitory effect was observed, and the mRNA expression of MMP-9 of transplantation tumor was detected by real time-fluorescent quantitation PCR and the protein expression of MMP-9 and bFGF were detected by enzyme linked immunosorbent assay.
RESULTThe IR in the group with HSYA at the concentration of 0.028 g x L(-1) is higher than in the group with normal sodium. After treatment with HSYA, the mRNA expression of MMP-9 has significant difference at the concentration of 0.028 g x L(-1) as compared with physiologic saline-treated group (P < 0.05), but the protein expression of MMP-9 and bFGF is obviously less than that in the physiologic saline-treated group (P < 0.05).
CONCLUSIONThe possible mechanism of HSYA in given concentration to antagonize tumor angiogenesis may be related with inhibiting the protein expression of MMP-9 and bFGF or the mRNA expression of MMP-9 in tumor tissue to reduce the degradation of blood vessel basilar membrane, and to restrain the migration of blood vessel and decrease the tumor vascularization.
