1.A comparative study on two liver suspension methods in laparoscopic radical gastrectomy

Objective:To compare liver adhesive suspension method and V-shaped liver suspension of anterior abdominal wall in laparoscopic radical gastrectomy.Methods:This study retrospectively evaluated 115 patients who underwent laparoscopic radical gastrectomy from Jan 2018 to Dec 2019 at Shanxi Provincial People′s Hospital, 59 cases were under liver adhesive suspension (study group) and 56 cases under V-shaped liver suspension of anterior abdominal wall (control group). Intraoperative and postoperative conditions were observed and compared between the two groups.Results:All the 115 cases successfully completed surgery. The liver suspension time in study group was (2.4±0.5)min, which was significantly shorter than that of the control group((12.5±2.6) min)( t=-28.100, P<0.01). The total operation time in was (284±69) min and (307±78) min, respectively, ( t=-1.656, P=0.10). There were no statistically significant differences between the two groups in preoperative ALT, AST, total bilirubin, intraoperative incidence of hepatic lacerations and subcapsular hematoma, postoperative total bilirubin level, AST on the day 7 postoperatively, tumor location, TNM staging, surgical method, postoperative duration of hospital stay, and VAS score ( P>0.05). Conclusions:Both methods of liver suspension are safe and feasible in laparoscopic surgery for gastric cancer.Compared with V-shaped liver suspension of anterior abdominal wall, liver adhesive suspension has the advantages of less invasive, safe, and time-saving.
2.Down-regulation of microRNA-187*expression in colorectal cancer and its roles in promoting cell ;apoptosis

Background and purpose: MicroRNAs (miRNAs) play an important role in tumor biological behavior. miRNAs are down-regulated or up-regulated in various cancer types, triggering abnormal cell differentiation, proliferation and apoptosis. This study was designed to investigate the expression and clinical signiifcance of miR-187*in colorectal cancer (CRC), and further to investigate its roles in promoting cell apoptosis. Methods:The expressions of miR-187* in 40 CRC cases were examined by real-time quantitative reverse transcription-PCR (qRT-PCR). The relationship between miR-187*expression and clinical features of CRC was analyzed. HCT116 cells were transfected with a miR-187*mimic and the apoptosis of the transfected cells were examined by lfow cytometry (FCM). Results:The expression of miR-187*was down-regulated in CRC tissues 0.165 (0.106, 0.428) compared with those in normal tissues 0.334 (0.211, 0.712) (P<0.05), especially in mucinous carcinoma and older age CRC (P<0.05). Transfection of HCT116 cells with a miR-187*mimic up-regulated the expression of miR-187*and increased cell early apoptosis (P<0.05). Conclusion: The expression level of miR-187* was lower in CRC. miR-187* expression correlates with histological type and age. Transfection of HCT116 cells with a miR-187*mimic accelerates apoptosis of tumor cells, suggesting that miR-187*is a potent tumor suppressor.
3.Expression and Clinical Significance of miR-224 and miR-378e in Colorectal Cancer Tissues

Objective To investigate the expression and clinical significance of microRNA-224 and microRNA-378e in colorectal cancer tissues and normal mucosa adjacent to tumor lesions. Methods The gene chip technology was used to detect the different expression of miRNA in colorectal carcinoma tissues and adjacent normal tissues, which was then confirmed by real-time PCR. The relationship between the pathology and clinical data was analyzed. Results The expres-sion level of miR-224 was significantly up-regulated in tumor tissue, while miR-378e was down-regulated in tumor tissue, which was confirmed by real-time PCR. The expression of miR-224 was strongly associated with histological types, while miR-378e was strongly associated with the infiltration depth of colorectal cancer. Conclusion miR-224 is a potent tumor promoter, while miR-378e is a potent tumor suppressor. Both miR-224 and miR-378e can be used as potential colorectal cancer molecular markers.
4.Transumbilical multi-stab laparoscopic pyeloplasty for infants younger than 3 months with severe hydronephrosis: our initial experience

Objective To present our initial experience of using transumbilical multi-stab laparoscopic pyeloplasty(TMLP) as a treatment for the infants younger than 3 months with severe hydronephrosis caused by ureteropelvic junction obstruction (UPJO).Methods Seventy-two infants younger than 3 months with severe hydronephrosis caused by UPJO underwent TMLP from June 2010 to March 2013 in our center.The average age received operation was 34 days (7-87 days).The operative indications included:① prenatal diagnosis of Society of Fetal Urology Grade 4 hydronephrosis; ②ipsilateral differential renal function being less than 40%.Patients were followed up with physical examinations,ultrasound and radionuclide scans.Results The operations were successfully performed in all the 72 patients.Neither conversion nor additional trocars placement was required and there was no intraoperative complication.The average operative time was 75 min (range,53-118 min).The patients were followed up for 6-36 months (mean,12 months).The renal pelvic anteroposterior diameters were reduced and the renal functions were improved (P< 0.05).The anastomoses were proved to be patent and the renal parenchymal thickeness increased.In addition,the scars were barely noticeable.Conclusions TMLP for the infants younger than 3 months with severe hydronephrosis caused by UPJO is feasible,safe and less invasive.The cosmetic results are excellent.But the surgery must be operated by rich experienced surgeon.
5.Transumbilical laparoendoscopic single-site pyeloplasty in infants and children: initial experience

Objective To present the surgical technique of transumbilical laparoendoscopic singlesite pyeloplasty (LESS-P) for pediatric patients with ureteropelvic junction obstruction (UPJO).Methods Twenty-four pediatric patients with UPJO had transumbilical LESS-P performed by the same surgeon from June to December 2010.Sixteen patients were male and eight female aged from 2 to 62 months with an average age of 14 months.Eighteen patients had obstruction on the left side and six on the right side.Dismembered LESS-P was carried out with the Anderson-Hynes anastomosis where 5-0 sutures were uswed over a double J ureteric stent.Results All operations were successful.None was converted to open surgery and no additional sheath tube or incision besides the umbilicus was needed.No intraoperative complications occurred.Ectopic blood vessels were found in two patients during surgery.The mean operative time was 145 min,and the average blood loss about 10 ml.Abdominal drainage tubes remained for 2 -9 d after surgery.The mean postoperative hospital time was 7 d.Two patients had postoperative urinary fistula,which naturally disappeared at 4 and 7 d postoperation,respectively.Follow-up with ultrasound and diuretic renal scintigraphy found 23 patients had significantly decreased renal pelvis diameter,the remaining case showed no obvious change,but diuretic renography showed significantly improved excretion.ConclusionsPediatric transumbilical LESS-P could be safe and effective.LESS-P could achieve comparable clinic outcomes to standard laparoscopy.
6.Reoperative laparoscopic surgery in children with failed pyeloplasty

Objective To investigate the etilolgy of failed pyeloplasty in children and to study the feasibility of redo laparoscopic surgery for recurrent nreterupelvic junction obstruction.Methods The clinical data of 39 patients with recurrent ureteropelvic junction obstruction after the primary pyeloplasty underwent redo laparoscopic surgery were analyzed retrospectively between September 2009 and June 2016 in our institution.There were 31 males and 8 females with a mean age of 66 months,ranged from 4 to 204 months.28 patients had left obstructions and 11 had fight obstructions,who were diagnosed by ultrasonography,MRU,and MAG3 renal scan et a1.Under general anesthesia,we identified that two patients (5.1%) had renal calices strictures,four patients (10.3%) had unsolved disease ureter and anastomotic strictures,two patients (5.1%) had adhesion band and anastomotic strictures,one patient had adhesion band,high ureteropelvic anastomosis and ureter torsion,and 30 (76.9%) patients had anastomotic strictures.Thirty-three patients underwent cdismembered pyeloplasty,five patients underwent onlay appendiculoureteroplasty and one patient underwent ureterocalicostomy.Results All surgeries were successfully completed without conversion.No intraoperative complication was encountered.One patient had persistent,frequent and intolerable flank pain with severe hydronephrosis after surgery,he subsequently underwent nephrectomy.One patient had persistent severe hydronephrosis which was repaired with on-lay appendiculoureteroplasty.One patient had anastomotic adhesion and balloon dilatation was performed with no further obstruction in follow up imaging.The rest of the patients got complete clinical or radiologic resolution.The successive rate was 36 of 39 (92.3%) at a mean follow up of 25 months (ranging 3-60 months).Conclusions Unsolved ureteric pathologies,anastomotic stricture,renal calices stricture,adhesion band,torsion of ureter and high ureteropelvic anastomosis all contribute to recurrent obstruction after the primary pyeloplasty.Laparoscopic redo pyeloplasty is safe and feasible for skilled surgeons.
7.Application of GVF snake model in segmentation of whole body bone SPECT image.

Limited by the imaging principle of whole body bone SPECT image, the gray value of bladder area is quite high, which affects the image's brightness, contrast and readability. In the meantime, the similarity between bladder area and focus makes it difficult for some images to be segmented automatically. In this paper, an improved Snake model, GVF Snake, is adopted to automatically segment bladder area, preparing for further processing of whole body bone SPECT images.

8.Multimodal medical image registration using cubic spline interpolation method.

Based on the characteristic of the PET-CT multimodal image series, a novel image registration and fusion method is proposed, in which the cubic spline interpolation method is applied to realize the interpolation of PET-CT image series, then registration is carried out by using mutual information algorithm and finally the improved principal component analysis method is used for the fusion of PET-CT multimodal images to enhance the visual effect of PET image, thus satisfied registration and fusion results are obtained. The cubic spline interpolation method is used for reconstruction to restore the missed information between image slices, which can compensate for the shortage of previous registration methods, improve the accuracy of the registration, and make the fused multimodal images more similar to the real image. Finally, the cubic spline interpolation method has been successfully applied in developing 3D-CRT (3D Conformal Radiation Therapy) system.

9.Detection of lung mini-nodules using multi-feature tracking.

How to accurately identify mini-nodules in a large amount of high resolution computed tomography (HRCT) images is always a significant and difficult issue in lung nodule computer-aided detection (CAD). This paper describes a new mini-nodules detection method which is based on a multi-feature tracking algorithm. Our detection method began after running the Da-Jing algorithm and morphological operation to extract the lung region of every HRCT image in a sequence. Once the lung had been extracted, a hybrid algorithm, combining gray threshold and improved template matching, was used to obtain the regions of interest (ROD). Next, several characteristics of each ROI were calculated to identify the final results by using multi-feature tracking throughout the whole HRCT image sequence. The results showed that the proposed method would be of high accuracy with a low occurrence of false positives.

10.Fast multi-resolution volume rendering method of large medical data sets.

A Fast Multi-resolution Volume Rendering Method (FMVRM) based on wavelet and Shear-Warp is herein proposed. In this method, the medical volume data is compressed using wavelet transformation first. Then based on the set resolution, the medical volume data is decompressed guided by Opacity transfer function (OTF). Finally, the 3D medical image is reconstructed on the basis of Shear-Warp using Block-based run length encoded (BRLE) data structure, in which, the aliasing artifacts resulting from under-sampling in Shear-Warp is avoided by the pre-integrated volume rendering technology. Experiments demonstrate the good performance of the proposed method.
