1.Relationship between serum SREBP-1, SAP and endocrine metabolism in patients with polycystic ovary syndrome

Objective:To investigate the relationship between serum sterol regulatory element binding protein-1 (SREBP-1) , serum amyloid P (SAP) and endocrine metabolism in patients with polycystic ovary syndrome.Methods:75 patients with polycystic ovary syndrome (study group) and 70 healthy women (control group) admitted to the First People’s Hospital of Yuhang District from Mar. 2018 to Feb. 2020 were enrolled. Various indexes were detected in both groups, including body mass index (BMI) , SREBP-1, SAP, triglyceride (TG) , total cholesterol (TC) , high-density lipoprotein (HDL-C) , and low-density lipoprotein (LDL-C) , fasting blood glucose (FBG) , fasting insulin (FINS) , and insulin resistance index (HOMA-IR) . The study group was further divided into two subgroups according to BMI, overweight group and normal group. SREBP-1 and SAP were compared between the two groups. Pearson correlation analysis was used to analyze the correlation between SREBP-1, SAP, blood lipid index, blood glucose index and BMI in patients with polycystic ovary syndrome.Results:The body weight and BMI index of study group were higher than those of control group [ (72.23±4.84) kg vs (58.23±3.25) kg, (25.02±2.75) kg/m 2 vs (22.11±1.34) kg/m 2, P<0.05]. The levels of serum SREBP-1 and SAP in the study group were significantly higher than those in the control group [ (334.78±32.06) pg/ml vs (206.34±25.71) pg/ml, (206.34±25.71) mg/U vs (39.16 ±0.58) mg/U, P<0.05]. The expression levels of TG, TC, LDL-C, FBG, FINS, and HOMA-IR in the study group were higher than those in the control group [ (2.32±0.71) vs (1.53±0.52) , (4.85±0.54) vs (3.41±0.66) , (3.06±0.75) vs (2.11±0.89) , (6.45±0.62) vs (5.59±0.76) , (16.14±1.03) vs (13.02±1.34) , (1.67±0.38) vs (1.18± 0.26) , P<0.05]; HDL-C expression level in the study group was lower than that in the control group [ (1.43±0.56) vs (1.71±0.42) , P<0.05]. The levels of SREBP-1 and SAP in overweight group were higher than those in normal group [ (339.19±27.63) pg/ml vs (281.67±20.18) pg/ml, (53.26±0.59) mg/U vs (42.48±0.67) mg/U, P<0.05]. Serum SREBP-1 and SAP in the study group were positively correlated with TG, TC, LDL-C, HOMA-IR, and BMI, and were negatively correlated with HDL-C ( P<0.05) . Conclusion:SREBP-1 and SAP levels are elevated in patients with polycystic ovary syndrome, which are significantly correlated with TG, TC, LDL-C, HOMA-IR, BMI, and HDL-C, and can cause endocrine disorder by affecting glucose and lipid metabolism.
2.Correlation analysis and clinical significance of lymph node metastasis in right recurrent laryngeal nerve of papillary thyroid carcinoma

Objective To explore the correlation factors and clinical significance of lymph node metastasis in right recurrent laryngeal nerve of thyroid papillary carcinoma (PTC).Methods Ninty-eight consecutive patients with PTC who were underwent total thyroidectomy with routine central lymph node dissection in the Second People's Hospital of Hefei from January 2015 to August 2017 were analyzed.The right paratracheal lymph nodes in the central compartment lymph nodes were divided into the level Ⅵ-A (anterior) and level Ⅵ-B (posterior,that was lymph node posterior to recurrent laryngeal nerve) compartments by recurrent laryngeal nerve.The lymph node metastasis of Ⅵ-B area during central compartment lymph node dissection was analyzed.We drew the receiver-operating characteristic curve (ROC) for right neck Ⅵ-A number of lymph node metastasis,and calculated the area under the curve (AUC) and Youden index.Results Among 98 cases,16 cases occurred Ⅵ-B district lymph node metastasis (16.33%).Single factor analysis results showed that lymph node metastasis in Ⅵ-B area of PTC patients were related to the tumor size (x2 =12.864,P <0.001),tumor capsular invasion (x2 =16.354,P < 0.001),the right neck Ⅵ-A area lymph node metastasis (x2 =16.065,P < 0.001),tumor number (x2 =15.593,P < 0.001) and neck lymph node metastasis (x2 =21.098,P <0.001),but they were not related to the patients' gender,age and lesion location (all P > 0.05).Lymph node metastasis in Ⅵ-B area of PTC patients were related to the number of right neck Ⅵ-A area lymph node metastasis.When the number of right neck Ⅵ-A metastatic lymph nodes was 2.5,the sensitivity and specificity were 70.60% and 70.00% respectively,AUC was 0.754,and Youden index was 0.406.Conclusion For patients with PTC,primary tumor diameter > 1 cm,tumor extracapsular invasion,Ⅵ-A area lymph node metastasis,multiple tumor and lateral cervical lymph node metastasis were the predictive factors for the lymph node metastasis in Ⅵ-B area.When the number of right neck Ⅵ-A area metastatic lymph nodes was greater than 3,we should dissect Ⅵ-B area.
3.Effect of different intervention plans on the expression of adhesion related cytokines and clinical outcomes after uterine adhesions separation surgery

Objective:To observe the effects of different intervention schemes on the expression of adhesion-related cytokines, menstrual recovery and clinical outcome of patients after transcervical resection of adhesion (TCRA) .Methods:180 patients received TCRA in our hospital from Feb. 2022 to Feb. 2023 were divided into group A, group B and group C according to different post-operative intervention programs, with 60 patients in each group. Patients in group A were treated with artificial cycle of estrogen and progesterone after surgery. On this basis, patients in group B were placed with a uterine birth control ring, and patients in group C were injected with sodium hyaluronate gel into the uterine cavity. The grade of uterine cavity adhesion, improvement rate of menstruation and pregnancy outcome at 2 months after operation and pregnancy outcome within 1 year after surgery were compared between the three groups at 2 months after operation. The relative mRNA expression of endometrial tissue transforming growth factor β1 (TGF-β1), platelet-derived growth factor BB (PDGF-BB), tissue inhibitor of matrix metalloproteinase-1 (TIMP-1) and basic fibroblast growth factor (bFGF) at uterine cavity adhesion in each group were detected and compared.Results:At 2 months after surgery, the uterine adhesion rates in group A, group B, and group C were 43.33%, 15.00%, and 11.67%, respectively. There was no significant difference in the uterine adhesion rates between group B and group C ( P>0.05), but they were significantly lower than those in group A ( P<0.05) ; Meanwhile, the degree of intrauterine adhesions in group B and group C was significantly milder than that in group A ( P<0.05). The menstrual improvement rates of group A, group B, and group C at 2 months after surgery were 76.67%, 93.33%, and 96.67%, respectively. There was no significant difference between group B and group C ( P>0.05), but they were all significantly higher than group A ( P<0.05). At 2 months post surgery, the relative expression levels of TGF-β1, PDGF-BB, TIMP-1, and bFGF mRNA in the endometrial tissue at the site of uterine adhesions in group A were 0.77±0.26, 0.58±0.27, 0.54±0.15, and 0.62±0.14, respectively. In group B, they were 0.37±0.16, 0.37±0.14, 0.26±0.11, and 0.29±0.10, respectively. In group C, they were 0.32±0.16, 0.21±0.09, 0.27±0.08, and 0.34±0.18, respectively. The relative expression levels of cytokines in each group were significantly lower than during surgery ( P<0.05). There was no significant difference in the relative expression levels of various cytokines mRNA between group B and group C at 2 months after surgery ( P>0.05), but both were significantly lower than group A ( P<0.05). The pregnancy success rates within 1 year after surgery in group A, group B, and group C were 40.00%, 55.00%, and 58.33%, respectively. The pregnancy success rate in group C was significantly higher than that in group A ( P<0.05) . Conclusions:The application of metauterine contraceptive ring or sodium hyaluronate gel on the basis of estrogen and progesterone treatment after TCRA can effectively prevent postoperative re-adhesion of patients with intrauterine adhesions, improve clinical symptoms, and reduce the expression level of adhesion cytokines. The effects of the two schemes are equivalent.
4.Applying Rasch analysis to transforming scores on the activities and participation component of the ICF Rehabilitation Set: A multicenter study

Objective:To explore the suitability of using Rasch analysis to transform scores on the Activities and Participation component of the ICF Rehabilitation Set.Methods:Between January and May 2017, five hundred and fifteen post-acute rehabilitation patients were recruited in 13 research centers through quota sampling. Specially-trained health professionals used the Activities and Participation component of the ICF Rehabilitation Set to collect data. Rasch analysis was performed using RUMM 2030 software to test for goodness of fit, threshold estimates, and local dependency.Results:Of the five hundred and fifteen participants, 279 (54.2%) had a neurological condition, 119 (23.1%) a cardiopulmonary condition, 49 (10.0%) a musculoskeletal condition, and 68 (13.2%) some other condition. After Rasch analysis, the Activities and Participant component of the ICF Rehabilitation Set was divided into two domains: Activities, and Participation with 9 categories under activities and five under participation.Conclusions:After rescoring the response options, deleting four poorly-fitting categories and generating one testlet, it was found that the activities and participation component the ICF Rehabilitation Set can be divided into two outcome measures to be applied in clinical practice.
5.Video-electroencephalogram follow-up in internal nuclear inclusion disease:a case report

An adult female patient of internal nuclear inclusion disease(NIID)with a major clinical manifestation of decreased intelligence and recurrent disturbance of consciousness was followed up for nearly 4 years.In particular,the evolution of the video-electroencephalogram(VEEG),the auxiliary diagnosis of VEEG,and the prediction value of VEEG for clinical outcome were summarized.We found that(1)the background rhythm in NIID patients evolved with the progression of the disease;(2)electroencephalogram reactivity could predict the outcome of NIID coma;(3)VEEG helped to determine whether the consciousness disturbance caused by NIID was due to non-convulsive status epilepticus,This case provides a new idea to explore the application of VEEG in NIID.
6.Dawn of CAR-T cell therapy in autoimmune diseases

Chimeric antigen receptor (CAR)-T cell therapy has achieved remarkable success in the treatment of hematological malignancies. Based on the immunomodulatory capability of CAR-T cells, efforts have turned toward exploring their potential in treating autoimmune diseases. Bibliometric analysis of 210 records from 128 academic journals published by 372 institutions in 40 countries/regions indicates a growing number of publications on CAR-T therapy for autoimmune diseases, covering a range of subtypes such as systemic lupus erythematosus, multiple sclerosis, among others. CAR-T therapy holds promise in mitigating several shortcomings, including the indiscriminate suppression of the immune system by traditional immunosuppressants, and non-sustaining therapeutic levels of monoclonal antibodies due to inherent pharmacokinetic constraints. By persisting and proliferating in vivo, CAR-T cells can offer a tailored and precise therapeutics. This paper reviewed preclinical experiments and clinical trials involving CAR-T and CAR-related therapies in various autoimmune diseases, incorporating innovations well-studied in the field of hematological tumors, aiming to explore a safe and effective therapeutic option for relapsed/refractory autoimmune diseases.
