Objective To explore the changes of urinary Trypsinogen Activation Peptide (TAP) levels in patients with acute pancreatitis (AP).Methods We observed the association between urinary TAP concentration and severity of acute pancreatitis with competitive enzyme-linked immunosorbent assay(ELISA). Urine samples were collected for TAP concentration at admission,24,48,and 72 h from 57 patients with AP who presented within 48 h of the onset of symptoms and from 11 control patients. Results The median urinary TAP at admission for severe acute pancreatitis(SAP) (98 nmol/L) was signficantly higher than the median for both mild pancreatitis (22 nmol/L,P0.05).The peak median urinary TAP in severe eroup was seen at admission.In the following days,TAP concentrations decreased graduallg.Conclusion Breakthrough activation of trypsinogen in pancreatic interstitial and blood may be the key event in the development of SAP.

Objective To study the application effect of feedback training mode on new neonatal recovery technology.Methods 101 midwives,obstetric doctors and pediatricians who participated the training were tested about the professional knowledge and skills of new neonatal recovery technology according to the teaching material of neonatal asphyxia recovery and neonatal resuscitation guidelines (2007 revision in Beijing).The results showed no statistical significance.And they were randomly divided into the control group (51 cases) and the observation group(50 cases).The control group adopted traditional method,that was,the teacher taught first and then the students took practice.The observation group used feedback training method.Results The theoretical and technical results of the observation group were significantly higher than those of the control group.The learning motivation,learning initiative,cooperation degree,ability of observation and solving problem and operation proficiency were also significantly higher than those of the control group.Conclusions Feedback training methods can significantly increase the learning effect of perinatal personnel,and make popularized the new neonatal recovery technology.

Objective To evaluate the relationship between cell seeding density and clinical efficacy of autologous cultured melanocyte transplantation in the treatment of vitiligo.Methods A total of 632 patients with vitiligo were enrolled in this study,and randomly classified into 4 groups to be treated with transplantation of autologous cultured melanocytes at 4 different seeding densities respectively,i.e.,(3.0-4.9)× 104/cm2 (n =201),(5.0-7.9) × 104/cm2 (n =303),(8.0-9.9) × 104/m2 (n =82),(10.0-12.0) × 104/cm2 (n =46).Epidermal sheets were obtained by suction blister biopsy from the normal skin of the vitiligo patients,and subjected to the isolation and culture of melanocytes.After 2 to 5 passages,the cultured autologous melanocytes were transplanted at different seeding densities to vitiligous lesions,which were abraded previously by ultra-pulsed CO2 laser,of these patients.All the patients were followed for 6-12 months.Results At 6 months after the transplantation,52.85％of these patients achieved more than 90％ repigmentation,and 82.28％ more than 50％ repigmentation,with no differences in the cure rate and response rate between the 4 groups (both P ＜ 0.05).The percentage of patients obtaining excellent color matching was significantly higher in the group treated with transplantation of melanocytes at a seeding density of (5.0-7.9) × 104/cm2 than in the other 3 groups at 6,12 and 24 months after treatment (all P ＜ 0.05),and higher in all the 4 groups at 12-and 24-month points compared with the 6-month point (all P ＜ 0.05),but no statistical difference was observed between the 12-and 24-month point in any of these groups (all P ＞ 0.05).Conclusions The transplantation of autologous cultured pure melanocytes is effective for the treatment of stable vitiligo with the optimal cell seeding density of melanocytes being (5.0-7.9) × 104/cm2,and the color matching appears to improve with time.

Objective To evaluate the therapeutic effect of transplantation of autologous melanocytes cultured with individualized medium in vitiligo. Methods Donor skin was obtained by suction blisters from a normally pigmented area of the abdomen of 155 patients with vitiligo. The roof of the blisters was clipped and digested with trypsin, then the suspension of epidermal cells and melanocytes were cultured in Hu16 medium.The cell division time (DOT) and melanin content of cultured melanocytes were measured followed by the adjustment of concentration of fetal calf serum, cytokines and cAMP elevating agents based on the DOT,melanin content and morphology of melanocytes for the individualized culture of melanocytes. After 2 - 5 passages, melanocytes were harvested and inoculated into ultrapluse CO2 laser-denuded lesions. All patients were followed up for at least 6 months. Results One hundred and fifty-five vitiligo patients with 204 lesions were treated with transplantation of autologous melanocytes. Of the 155 patients, 119 received 1 session of transplantation, 36 received 2 to 4 session of transplantation. Cells were expanded by 50 - 80 times in vitro after individualized culture. Repigmentation was more than 50% in 84.8% of these lesions, more than 90% in 52.94% of the lesions. A homogeneous skin color was obtained in repigmented skin, and no scarring or other side effects were observed. No influence was noted on the outcome of transplantation for sex, age, course of disease or lesion size of patients. Segmental vitiligo showed better response than vitiligo vulgaris: the effective rate and cure rate were 93.62% and 65.96% respectively for segmental vitiligo, 82.16% and 49.04% respectively for vitiligo vulgaris. Lesions located on the arms and legs (not including elbows and knees) showed the best response, with a cure rate of 73.08%, whereas acral sites were the most difficult area to repigment, with a cure rate of just 25.93%. Conclusions Individualized culture can significantly increase the success rate of melanocyte culture and expanding times of melanocytes. Transplantation of cultured autologous melanocytes is an effective modality deserving clinical application in the treatment of stable vitiligo, with the advantage of treating large depigmented area with melanocytes from a small donor site.

Objective To observe the clinical efficacy of acupoint application in canicular days plus TDP radiation in treating primary dysmenorrhea due to cold-dampness.Method Eighty-three patients were randomized into group A of 32 cases, group B of 20 cases, and group C of 31 cases. Group A was intervened by acupoint application plus TDP, group B was by acupoint application alone, while group C was by oral administration ofTong Jing Bao granules. The Visual Analogue Scale (VAS) scores of the three groups were observed before and after treatment, and the clinical efficacies were compared.Result The total effective rate was 93.1% in group A, versus 85.0% in group B and 77.4% in group C. The total effective rate of group A was significantly different from that of group B and group C (P<0.05). The total effective rate of group B was significantly different from that of group C (P<0.05). The VAS scores were changed significantly after treatment in the three groups (P<0.05). After treatment, the VAS scores of group A and B were both significantly different from that of group C (P<0.05).Conclusion Acupoint application plus TDP is an effective method in treating primary dysmenorrhea due to cold-dampness.

Salvia miltorrhiza Bge is a medicinal herb in traditional Chinese medicines. Until recently, dozens of compound ingredients with medicinal properties have been identified from S. miltorrhiza. Proteome analysis on medicinal ingredients synthesis mechanisms could provide a theoretical basis for S. miltorrhiza genetic improve-ment. In this study, a S. miltorrhiza specific protein/peptide sequence database was constructed using Illumina high-throughput transcriptome sequencing data of multiple types of S. miltorrhiza tissues. The database could act as a key component to carry out the proteome analysis in S. miltorrhiza.

Objective To investigate a simple and efficient fluorescence in situ hybridization method for prenatal diagnosis.Methods Thirty-six cases of chorionic villus samples were hybridized with probes 18/X/Y,13/21 by using traditional cuhure methods and modified three-step methods in controlled experimentation during 2012 to 2013.The hybridization rate and fluorescence signals were analyzed.Results A total of 72 hybrid zone were detected.Probe 21/13 hybridization rate and fluorescence signals of three-step modified methods were higher than that of traditional methods(99.72％ ±0.42％ vs 85.90％ ±4.15％,t =20.4,P ＜0.01; 2.58 ±0.50 vs 1.52 ± 0.55,t =7.53,P ＜0.01).Probe 18/X/Y has the same hybridization rate and fluorescence signals between three-step modified methods and traditional methods (99.57％ ±0.53％ vs99.70％ ±0.42％,t=1.30,P＞0.05; 2.22±0.42 vs2.36±0.48,t=1.57,P＞0.05).The coincidence rate of two methods was 100％ (36/36).Conclusion The modified fluorescence in situ hybridization three-step methods in the study was simple,rapid,effective and environment-friendly.At the same time,it has some defects such as the signals of 18/X/Y was not concentrated enough.Further exploration is needed.

Objective To study the effect on the environmental radioactivity level and the health of the residents in Shanghai when Qinshan Nuclear Power Station(QNPS) has been in normal operational condition for 17 years. Methods The field monitoring and the sampling analytical method were used for monitoring the radioactivity level in outdoor environment, drinking water, food and soil. Results The outdoor natural radiation level and the radioactivity level in the sample of the environmental media were on the normal background level. There was no distinct change in the environmental radiation level of Jinshan Area when QNPS was in normal operational condition. The radioactivity level in the environmental media was far lower than the limit values specified by the national standard GB. The effective dose of the outdoor natural external exposure for the residents is about 0.102 mSv/a and the effective dose of the internal exposure from the artificial radionuclides 90Sr and 137Cs in food for the residents is about 0.488 μSv/a. Conclusions There is no effect of the radioactive contamination on the environmental radioactivity level and the health of the residents in Shanghai when QNPS has been in normal operational condition for 17 years.

Objective To compare the changes of high sensitive-C reactive protein (hs-CRP) and cardiac troponin Ⅰ ( cTn Ⅰ ) levels before and after intensive therapy in patients with type 2 diabetes,and to find out the reasonable glucose-lowering rate.Methods One hundred and thirty-two cases of type 2 diabetes( T2DM group) and 135 cases of type 2 diabetes with coronary heart disease( T2DM+CHD group) received intensive therapy.After testing hs-CRP and cTn Ⅰ levels,the variations were analyzed.Results The ranges of the change in cTn Ⅰ and hs-CRP levels were different under four glucose-lowering rates in the T2DM+CHD group( P＜0.05 ).cTn Ⅰ and hs-CRP levels were higher than those before intensive therapy in the T2DM+CHD group with glucose-lowering rate greater than 4.0mmol· L-1 · d-1.The other two subgroups with glucose-lowering rate less than 4.0 mmol· L-1 · d-1 showed decreased cTn Ⅰ and hs-CRP levels.While at the end of 3 months follow-up,cTn Ⅰ and hs-CRP levels were all significantly lower than those before intensive therapy in four subgroups ( P＜0.05 ).Conclusions The increase of cardiovascular events after intensive therapy may be due to excessively fast glucose-lowering rate.The reasonable glucose-lowering rate for patients with type 2 diabetes should depend on whether there is accompanying coronary heart disease.For type 2 diabetes with coronary heart disease,excessively fast glucose-lowering rate could lead to acute rise ofcTn Ⅰ and hs-CRP levels,which causes myocardial injury.The mechanism of myocardial injury resulted from excessively fast glucose-lowering rate may be due to activation of the inflammatory pathway.In type 2 diabetes with coronary heart disease,long-term good control of blood glucose could alleviate inflammatory response and cardiac damage resulted from excessively fast glucose-lowering rate.

Objective:In this study, we established a reliable surgical procedure of lung ischaemia-reperfusion(IR) injury in rats. The research progress of different lung IR injury models and application value was also discussed.Methods:Twenty-eight adult SD rats were randomly divided into SHAM group and lung IR injury group(IR group), 14 rats in each group. In IR group, rats underwent tracheotomy under general anesthesia and received mechanical ventilation. Chest was opened in supine position, and pulmonary hilum was blocked for 30 minutes then the occlusion was removed. Samples were harvested after reperfusion for 45minutes. Rats in SHAM group received surgery and exposure of the right pulmonary artery, and experienced the same amount of time before the chest closed. Arterial blood gas was extracted postoperatively. Gross view of the lungs and pathological changes were observed, and the dry/wet ratio(W/D) was determined. Protein level of pro-inflammatory factors, markers in oxidative stress pathway, and endothelial functional markers in lung were tested by western blot analysis.Results:In IR group, there was pink foamy secretion in the airway, and the lungs exhibited signs of edema and congestion. In IR group, the alveolitis score was significantly increased, the W/D ratio was also increased, p38MAPK and NF-κB signaling pathways were activated, and the expression of TNF-α was significantly increased, while the expression of eNOS was significantly decreased.Conclusion:Left hilum clamping and bilateral reperfusion injury in lung is a practical animal model, it is a simple, low-cost and repeatable animal model for further studies. No microsurgical instruments were required during the procedure. Lung IR injury is characterized by oxidative stress response, inflammatory response and endothelial cell dysfunction.
