1.Clinical value of three-dimensional ultrasound in the diagnosis of female pelvic floor dysfunction disease

Objective To analyze the clinical value of three-dimensional ultrasound in the diagnosis of pelvic floor dysfunction disease.Methods From June 2016 to December 2017,95 cases of female pelvic floor dysfunction disease in the People's Hospital of Dongyang were selected as observation group.And 90 healthy women received physical examination during the same period were selected as control group.Both two groups were subjected to three-dimensional ultrasonography,and the results were analyzed.Results In the observation group,the anteroposterior diameter and area in resting period,anteroposterior diameter and area in tension period,anteroposterior diameter and area in anus period of the anal pelvic diaphragmatic hiatus were (65.58 ± 6.38)cm,(20.01 ± 3.98) cm2,(68.40 ± 5.54) cm,(22.50 ± 3.12) cm2,(60.17 ± 5.55) cm and (19.28 ± 2.52) cm2,respectively,which were significantly higher than those in the control group [(51.84 ± 3.46) cm,(13.93 ± 2.09) cm2,(53.65 ± 4.23) cm,(14.60 ± 2.95) cm2,(48.95 ± 3.48) cm,(13.45 ± 1.76) cm2] (t =18.066,12.903,20.272,13.200,16.369,18.150,P =0.000,0.000,0.000,0.000,0.000,and 0.000).But the pubic rectal muscle thickness of resting period,tension period,anal contraction period in the observation group were (10.28 ± 1.96) cm,(10.63 ± 1.57) cm,and (10.01 ± 1.13) cm,respectively,which were lower than those in the control group [(10.28 ± 1.96)cm,(10.63 ±1.57)cm,(10.35 ±1.49)cm] (t =1.015,1.940,1.741,P =0.311,0.053,0.083).The observation group was asymmetrical,asymmetrical on both sides and interrupted continuously.In the control group,the echo was uniform,bilateral symmetry,without interruption.Conclusion The use of three-dimensional ultrasound in the diagnosis of female pelvic floor dysfunction disease has good clinical value.
2.Characteristics of clinical Shigella isolates por ducing the extended -spectrum and AmpCβ-lactamases

Objective To investigate the phenotypes and genotypes of extended -spectrum β-lacta-mases (ESBLs) and AmpC β-lactamases produced by cefoxitin-resistant Shigella strains isolated from Zhe-jiang province and the virulence of those strains .Methods Kirby-Bauer antibiotic testing was performed to screen cefoxitin-resistant strains from 356 Shigella isolates.The serotypes of the cefoxitin-resistant strains were detected .The phenotypes of ESBLs and AmpC β-lactamases from cefoxitin-resistant strains were detec-ted by ESBLs confirmatory test and AmpC-three-dimensional test ,respectively .The genotypes of ESBLs and AmpC β-lactamases were analyzed by PCR and sequence analysis .The virulence genes ( virA, ial, iapH, set1A, set1B and sen) in the cefoxitin-resistant strains were screened by PCR .Results Eight cefoxitin-re-sistant strains were identified from 356 Shigella isolates.Among them,six strains were identified as Shigella flexneri (S.fleaneri) strain (four F2a,one F2b and one F2c),and the rest were identified as Shigella sonnie (S.sonnei) strain.Among all eight cefoxitin-resistant strains,five strains showed positive results for ESBLs confirmatory test and two strains showed positive results for AmpC-three-dimensional test .Seven out of the eight strains harbor ESBLs genes (CTX-M-14,15,57 and/or OXA-30),two of which were positive for AmpC genes (DHA-1 and CMY-2).Five out of the eight cefoxitin-resistant strains carried all of the six tested viru-lence genes,while the other three strains possessed four virulence genes except for set1A and set1B.The two strains producing both ESBLs and AmpC β-lactamases were susceptible only to imipenem .Conclusion ESBLs positive isolates are prevalent strains of cefoxitin-resistant Shigella with potential high virulence .Some of them also produce AmpC β-lactamases ,and the DHA-1 type of AmpCβ-lactamase is identified for the first time in China .

4.Safety and efficacy of picosecond Alexandrite laser with a diffractive lens array for treatment of facial photoaging

Objective To evaluate the efficacy and safety of a 755 nm picosecond Alexandrite la-ser with a diffractive lens array in the treatment of facial photoaged skin .Methods Twenty-six pa-tients with facial photoaging were recruited and received 3 treatments at 4-week intervals .Laser energy was applied over the entire face at a fixed spot size of 6 mm ,with a fluence of 0 .71 J/cm2 and 5Hz . Blinded clinical assessment was performed by 2 independent dermatologists on a 5-point global pho-toaging scale (GPS) .Patients were also questioned on the extent of improvement of rhytides ,skin tightening ,and complexion with a 4-point global aesthetic improvement scale (GAIS) and satisfaction . Adverse events were also evaluated .Results Twenty-six patients completed the treatment .Compared with the baseline ,there was a significant improvement in facial photoaged skin after 3 treatments ,and these positive outcomes were maintained up to the 3-month follow-up ,according to the GPS and GAIS scores .Moderate pain and transient erythema were observed as the two main discomforts associated with the treatment .Most patients were satisfied with the treatment .Conclusions This 755 nm pico-second Alexandrite laser with a diffractive lens array optic is effective in the treatment offacial pho -toaged skin ,and the therapy also seems safe and well tolerated .
5.Predictive value of serum histone deacetylase 3 on major adverse cardiovascular events in patients with stable coronary artery disease after percutaneous coronary intervention

Objective:To investigate the predictive value of serum histone deacetylase 3(HDAC3) on major adverse cardiovascular events (MACE) in patients with stable coronary artery disease after percutaneous coronary intervention (PCI).Methods:Eighty patients with stable coronary artery disease who underwent PCI in Jinshan Branch of Shanghai Sixth People′s Hospital from October 2017 to October 2018 were selected as the research subjects, which were divided into the MACE group (26 cases) and non-MACE group (54 cases) according to the occurrence of MACE. The clinical data, blood lipid index, HDAC3, C-reactive protein(CRP) and other indicators of the patients between two groups were compared, independent risk factors for MACE after PCI were analyzed by Logistic regression analysis, and the predictive value of various indicators on MACE were analyzed by receiver operating characteristic(ROC) curve.Results:There were no statistical differences between the two groups in general information such as gender, age, body mass index (BMI), smoking history, drinking history, history of heart failure, hypertension, hyperlipidemia, diabetes and vascular lesion count ( P>0.05). The Gensini scores in the MACE group was significantly higher than that in the non-MACE group [(18.47 ± 3.23) vs. (14.46 ± 3.62)]( P<0.05). There were no statistical differences in total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterin(LDL-C), high density lipoprotein cholesterin (HDL-C) levels between the two groups ( P>0.05). The levels of HDAC3 and CRP in the MACE group were significantly higher than those in the non-MACE group [(3.93 ± 0.50) mg/L vs. (2.92 ± 0.56) mg/L, (22.06 ± 4.56) mg/L vs. (17.56 ± 3.28) mg/L] ( P<0.05). Logistic multivariate regression analysis showed that HDAC3, CRP and Gensini scores were independent risk factors for MACE in patients with stable coronary artery disease after PCI ( P<0.05). ROC curve analysis showed that the area under the curve (AUC) of serum HDAC3,CRP and Gensini scores predicted MACE in patient with stable coronary artery disease after PCI were 0.924, 0.785, 0.803. The diagnostic efficacy of serum HDAC3 was significantly higher than that of CRP and Gensini scores ( Z=2.019, 2.147, P<0.05). Conclusions:The up-regulation of serum HDAC3 expression has correlation with MACE in patients with stable coronary artery disease after PCI, which can be used as a predictor of MACE.
6. Clinical features and diagnosis of childhood leukoencephalopathy with cerebral calcifications and cysts in four cases

Objective:
To investigate the clinical features and diagnostic bases of childhood leukoencephalopathy with cerebral calcifications and cysts (LCC).
Methods:
The clinical data involving manifestations and laboratory examinations of 4 children with LCC admitted to Beijing Children's Hospital Affiliated to Capital Medical University from 2012 to 2017 were retrospectively summarized. Each patient had a follow-up visit ranging from 4 months to 5 years and 9 months after initial examination.
Results:
Patients consisted of 2 males and 2 females, whose age of onset was respectively 2 years and 9 months, 6 years and 2 months, 7 years and 10 months, and 5 years and 1 month. The main clinical symptoms of these cases included headache, dizziness, partial seizure and claudication, and two of these cases had insidious onset. Cerebral calcifications and cysts with leukoencephalopathy were detected by neuroimaging in all patients. In addition, multifocal microhemorrhages and calcifications were observed by magnetic susceptibility-weighted imaging (SWI) series in 3 patients. Brain biopsy performed on 1 case disclosed a neuronal reduction in the cerebral cortex, loosening of focal white matter, multifocal lymphocyte infiltration, fresh hemorrhages, and gliosis, as well as angiomatous changes of blood vessels with hyalinized thicken-wall, stenotic or occlusive lumina and calcification deposits. The compound heterozygous mutations of n.*10G>A and n.82A>G in SNORD118 were identified in 1 case by target-capture next-generation sequencing. Sanger sequencing verified that the variant n.*10G>A was a novel mutation and it was of paternal-origin, while the variant n.82A>G was of maternal-origin, which had already been reported to be pathogenic to LCC. Follow-up study had shown continued partial seizure in 1 case and remissive claudication in another, while the remaining 2 cases had a relatively favorable outcome without obvious neurological symptoms at present time.
Conclusions
The clinical manifestations of LCC are nonspecific, and the onset of the disease tends to be insidious. The triad neuroimaging findings of cerebral calcifications, cysts and leukoencephalopathy are essential to the diagnosis of the disease, and the signals of microhemorrhages revealed by SWI series provide another eloquent reference for the diagnosis. As biopsy is invasive and usually unavailable in the early stage, gene assessment, instead of pathological data, should be the gold standard in the diagnosis of LCC.
7.Establishment and evaluation of an artificial intelligence model for the diagnosis of facial vitiligo

Objective:To construct an artificial intelligence model for the diagnosis of facial vitiligo, so as to realize artificial intelligence-assisted diagnosis of facial vitiligo.Methods:Based on digital single-lens reflex (SLR) camera images of vitiligo skin lesions and YOLO (You Only Look Once) v3 algorithm, a skin lesion detection model Vit3 was established, and its performance was evaluated by comparing its detection results and labeling results of dermatologists. On the basis of the Vit3 model, both optical and ultraviolet images of vitiligo and non-vitiligo skin lesions were taken by using an artificial intelligence-based facial skin image collector, and the gray values of vitiligo and non-vitiligo skin lesion areas on the ultraviolet images were measured by using an image processing technique. According to the gray-value threshold between vitiligo and non-vitiligo skin lesions, a facial vitiligo diagnosis model Vit4 was established. Cochran′s Q test was used to compare the diagnostic results of the Vit4 model and dermatologists, and the diagnostic performance of the Vit4 model was evaluated. Results:For 100 SLR camera images of vitiligo skin lesions (167 lesional sites) and 100 SLR camera images of normal skin, the diagnostic sensitivity of the Vit3 model was 92.81% (155/167) . For 97 pairs of facial skin images (including 50 vitiligo lesions, 30 pityriasis alba lesions, 7 amelanotic nevus leisons, and 10 normal skin tissues) , the diagnostic accuracy rate, sensitivity and specificity of the Vit4 model were 88.66% (86/97) , 88.00% (44/50) and 89.36% (42/47) respectively, and there was no significant difference in the diagnostic accuracy rate between the Vit4 model and dermatologists (92.78%[90/97], χ2=2.323, P > 0.05) . Conclusion:The artificial intelligence model Vit4 was established for the diagnosis of facial vitiligo with favorable diagnostic performance, and could serve as an objective and convenient method for the auxiliary diagnosis of facial vitiligo.
8.Effects of weekend admission on total hospitalization expenses for elderly patients with hip fracture under the geriatric orthopedic co-management

Objective:To explore the impact of weekend hospitalization on total hospitalization expenses for elderly patients with hip fracture under the geriatric orthopedic co-management.Methods:A retrospective analysis was conducted to analyze the clinical data of elderly patients with hip fracture who had been hospitalized for surgical treatment at Beijing Jishuitan Hospital from May 2015 to December 2020. They were divided into 2 groups based on their admission date. Group A was admitted from Monday to Thursday while Group B from Friday to Sunday. The general demographic data, diagnostic information, comorbidities, hospitalization expenses of the patients were collected. The differences in total hospitalization expenses, hospitalization time, rate of surgery within 48 hours and rate of hospital mortality between the 2 groups were analyzed by rank sum test, chi square test, correlation analysis, and multiple linear regression.Results:A total of 6,075 patients with hip fracture were included in this study, including 1,675 males and 4,400 females with a median age of 80 (74, 85) years. There were 3,935 ones in group A and 2,140 ones in group B. The total hospitalization expenses for group A was 58,160.52 (49,215.45, 72,748.94) yuan, insignificantly lower than those for Group B [58,412.90 (49,163.58, 72,712.61) yuan] ( P>0.05). The rate of surgery within 48 hours for group A was 75.8% (2,984/3,935), significantly higher than that for group B [49.3% (1,054/2,140)]. The hospitalization time for group A was 5 (4, 7) days, significantly less than that for group B [5 (4, 7) days] ( P<0.05). There was no significant difference in the rate of hospital mortality between the 2 groups ( P>0.05). Multiple linear regression analysis showed that total hospitalization expenses were significantly higher for patients admitted on weekends, hospitalization time was positively correlated with total hospitalization expenses, and total hospitalization expenses were significantly lower for the patients undergoing surgery within 48 hours ( P<0.05). Conclusion:Admission on weekends can increase total hospitalization expenses, prolong hospitalization time, and reduce rate of surgery within 48 hours for elderly patients with hip fracture.
9.Significance of carbonic anhydraseⅨprotein in fibrotic foci of mammary invasive ductal carcinomas

Objective To study the relationship between fibrotic focus ( FF ) and carbonic anhydrase ( CA) Ⅸin invasive ductal carcinomas ( IDC) of the breast.Methods In 167 cases of IDC, the FF was assessed morphologically, and expression of ER, PR and CA Ⅸ was evaluated using MaxVision immunohistochemistry.Results The expression of CAⅨin IDC with and without FF was 56.3%(45/80) and 28.7%(25/87) respectively, with significant difference (P=0.001).In IDC with FF, the CA Ⅸexpression of tumor cells in tumors with CAⅨ-positive fibroblasts (35/40, 87.5%) was significantly (P<0.001) higher than that in tumors with CAⅨ-negative fibroblasts (10/40, 25.0%).In IDC with FF, the CAⅨexpression of fibroblasts of FF in grade 3 IDC ( 23/33, 69.7%) was significantly ( P =0.006 ) higher than that in grade 1 +2 tumors (17/47, 36.2%).The ER and PR expression of tumor cells in tumors containing CA Ⅸ-negative fibroblasts was 72.5% ( 29/40 ) and 65.0% ( 26/40 ) respectively, whereas the ER and PR expression of tumor cells in tumors containing CAⅨ-positive fibroblasts was 50.0%(20/40) and 42.5% (17/40) respectively; the difference was statistically significant (for both ER and PR, P=0.04 ) .The age of patients with tumors containing CA Ⅸ-negative fibroblasts was significantly ( P=0.002) older than those containing CA Ⅸ-positive fibroblasts.The FF diameter/tumor diameter in tumors containing CA Ⅸ-positive fibroblasts was significantly larger than those containing CA Ⅸ-negative fibroblasts.(3) For the groups of tumor size≤2 cm and tumor size between 2 cm to 5 cm, the diameter of the fibrotic focus was significantly (P<0.01) smaller than the fibrotic focus size of tumors >5 cm in size. Conclusions CA IX expression is correlated with FF, and that in fibroblasts of FF correlated with patients′age, tumor grade, hormone receptors and FF diameter/tumor diameter.CAⅨexpression in FF might be a marker for poor prognosis in patients with breast cancer.

The ER and PR expression of tumor cells in tumors containing CA IX-negative fibroblasts was 72.5% (29/40) and 65.0% (26/40) respectively, whereas the ER and PR expression of tumor cells in tumors containing CA IX-positive fibroblasts was 50.0% (20/40) and 42.5% (17/40) respectively; the difference was statistically significant (for both ER and PR, P=0.04). The age of patients with tumors containing CA IX-negative fibroblasts was significantly (P=0.002) older than those containing CA IX-positive fibroblasts. The FF diameter/tumor diameter in tumors containing CA IX-positive fibroblasts was significantly larger than those containing CA IX-negative fibroblasts. (3) For the groups of tumor size≤2 cm and tumor size between 2 cm to 5 cm, the diameter of the fibrotic focus was significantly (P<0.01) smaller than the fibrotic focus size of tumors>5 cm in size.
CONCLUSIONSCA IX expression is correlated with FF, and that in fibroblasts of FF correlated with patients' age, tumor grade, hormone receptors and FF diameter/tumor diameter. CA IX expression in FF might be a marker for poor prognosis in patients with breast cancer.
