1.Hemodynamics simulation of internal carotid artery siphon and relevant influential factors

BACKGROUND:The siphon of internal carotid artery is a complex of bending and surrounding structure, which has become a research hot in the field of medical imaging and regional anatomy. There is little research on the hemodynamics of internal carotid artery. Finite element analysis provides the basis for the related dynamics research. OBJECTIVE:To explicit the hemodynamic characteristics of the normal and stenosis internal carotid artery, and to explore the relevant influential factors. METHODS:Finite element model of the siphon of internal carotid artery was built based on 64-slice spiral CT data, and then, three-dimensional models were constructed using Mimics 10.01 software. There were simulations of the normal and post-stenosis blood flows with ANSYS 13.0 CFX software, to observe the changes of hemodynamics, and to analyze their characteristics and differences. RESULTS AND CONCLUSION:The normal blood flow at the siphon of internal carotid artery was in a laminar state, but rotation and turbulent flow formed at the two angle regions. The velocity of external bending zone was slower than that of the internal. The slower was the velocity, the more obvious was the turbulence. The wal shear force decreased at the angle regions, and the wal shear force of external bending zone was smal er than that of the internal. At the region of arterial stenosis, the blood flow was sped, and at the downstream, turbulent flow and low zone of wal shear force were formed. With the increase of stenosis severity, the turbulent flow and low wal shear force area were expanded. Central stenosis showed more obvious effects than the eccentric one. The degree of stenosis and bending at the siphon of internal carotid artery can directly influence the formation of turbulent flow and low wal shear force area, which are more obvious at the external bending zone and central stenosis.
2.Isolation and Identification of Platelet by Stepped Centrifugal Method

Objective To establish a method for the isolation and identification of platelets.Methods 10 healthy volunteers were selected to collect the EDTA anticoagulant venous blood of 3 tubes,each tube was 2 ml,which was divided into the whole blood cell tube,platelet rich plasma (control group),and stepped centrifugal platelet extract (experiment group).Platelet was isolated by simple centrifugation method(PRP) and stepped centrifugal method.The two groups were full blood count and analyzed by microscopic morphology and platelet activity test.Leukocyte specific HGB gene and platelet mitochondrial ND1 gene content was analyzed by real time PCR.Results Platelets were extracted and detected in control group and experimental group.Platelets were found and white blood cells and red blood cells were not remained in experimental group.Platelets and sporadic white blood cells were found in control group.The platelet pick up rate of experiment group was significantly higher than control group,the difference was statistically significant.Experimental gene content HGB of experiment group was significantly lower than control group,the difference was statistically significant (t=-3.281,-2.865,P<0.05).ND1 gene content of experiment group higher than the control group,the difference was not statistically significant.There was no significant difference for platelet activity test between experimental group and control group (t=-0.046,-0.799,P> 0.05).Conclusion A isolation and identification method of stepped centrifugal platelet was established.The method can be used for the study of platelet gene and the functional analysis of platelets.
3.Research on the geometric difference between different ITV generation methods in non-small cell lung cancer treated with stereotactic radiotherapy

Objective To study the geometric difference between six different ITV generation methods from 4DCT for patients with non?small cell lung cancer ( NSCLC) treated with stereotactic ablative radiotherapy technique ( SABR) . Methods Between Dec. 2013 and Mar. 2014,16 patients were enrolled in this retrospective study. All patients underwent imaging with 4DCT scans. The MI and DI index were evaluated between six ITV generation methods:combining GTV from all 10 respiratory phases ( ITV10 );combining GTV from four respiratory phases, including two extreme phases ( 0% and 50%) plus two intermediate phases ( 20% and 70%) ( ITVYeo ) which was proposed by Seung?Gu Yeo of Soonchunhyang University;combining GTV from two extreme phases ( ITVEI+EE ) . And combining GTV from five odd phases (10%,30%,50%,70%,90%)(ITVodd).Accordingly the ITVEVEN which was combined from the remaining five even phases (20%,40%,60%,80%,0%),and ITVAVG,ITVMIP were contoured from two reconstructed 4DCT sequences,finally,a method which was not sensitive to the tumor volume and motion characteristic was selected for clinical use. Data were compared using a variance analysis followed by Student?Newman?Keulsa test both in same group or between groups. At the same time, the volume and the three dimensional movements of the tumor, the relativity of MI and DI were analyzed by Multi?parameter regression analysis. Results The mean (range) tumor motion (RLR,RAP,RCC,and R3D) are 3. 5 mm (1. 4?8. 4 mm),4. 5 mm (1. 1?8. 6 mm),9. 5 mm (0?10 mm),12. 3 mm (2. 5?55. 3 mm) respectively. The IGTVx volume are Underestimated by 25. 7%,35. 6%,17. 9%,12. 8%,3. 6%,4. 8%( P=0. 000) respectively. The MI index comparisons between six ITV generation methods and ITV10 showed statistical significance:0. 69,0. 62,0. 80, 0. 86,0. 93,0. 91 ( P=0. 006 ) . The DI index showed no statistical significance:0. 98, 0. 98, 0. 97, 0. 97, 0. 99,0. 98(P=0. 130).The tumor size and motion amplitude were certified not the independent factors for the MI index of ITVodd and ITVEVEN . Conclusions IGTVODD/EVEN based on odd or even 4DCT phases is not sensitive to the tumor size or motion characteristic and is proved to have a good marching with ITV10 meanwhile maintaining a reasonable contouring efficiency,it can be recommend to the institutions which was not equipped with the deformable registration systems.
4.Dexmedetomidine inhibits 5-HT-induced intrapulmonary artery vasoconstriction.

OBJECTIVETo investigate the effect of dexmedetomidine on 5-HT-induced constrictions of isolated human intrapulmonary arteries and explore the mechanisms.
METHODSLung tissue was obtained from patients undergoing surgery for lung carcinoma. Intrapulmonary arteries were dissected and cut into rings, which were mounted in a Multi Myograph system to determine the effect of dexmedetomidine (0.3-3 nmol/L) on 5-HT-induced vasoconstrictions. The influences of the endothelium removal and various drugs including L-NAME, yohimbine and indomethacin were tested on the effects of dexmedetomidine.
RESULTSDexmedetomidine (0.1-100 nmol/L) did not obviously affect the resting tension of endothelium-intact human intrapulmonary arteries. 5-HT induced concentration-dependent contraction in endothelium-intact intrapulmonary arteries [pD2: 6.11∓0.05, Emax: (102.10∓1.96)%]. In the rings with intact endothelium, dexmedetomidine (0.3-3 nmol/L) significantly attenuated the Emax and pD2 of 5-HT-induced vasoconstriction [pD2: 5.94∓0.03, Emax: (79.96∓1.31)%]. 5-HT also induced concentration-dependent contraction in endothelium-denuded intrapulmonary arteries [pD2: 6.10∓0.07, Emax: (107.40∓3.20)%]. Dexmedetomidine produced no significant effects on the rings with denuded endothelium. The effects of dexmedetomidine on 5-HT-induced vasoconstriction was suppressed by L-NAME and yohimbine, but not by indomethacin.
CONCLUSIONDexmedetomidine can inhibit 5-HT-induced vasoconstriction of isolated human intrapulmonary arteries probably through α2-adrenergic acceptor and NO released from the endothelium.

Despite the complex vascular effects of dexmedetomidine (DEX), its actions on human pulmonary resistance arteries remain unknown. The present study tested the hypothesis that DEX inhibits vascular tension in human pulmonary arteries through the endothelial nitric oxide synthase (eNOS) mediated production of nitric oxide (NO). Pulmonary artery segments were obtained from 62 patients who underwent lung resection. The direct effects of DEX on human pulmonary artery tension and changes in vascular tension were determined by isometric force measurements recorded on a myograph. Arterial contractions caused by increasing concentrations of serotonin with DEX in the presence or absence of L-NAME (endothelial nitric oxide synthase inhibitor), yohimbine (α2-adrenoceptor antagonist) and indomethacin (cyclooxygenase inhibitor) as antagonists were also measured. DEX had no effect on endothelium-intact pulmonary arteries, whereas at concentrations of 10⁻⁸~10⁻⁶ mol/L, it elicited contractions in endothelium-denuded pulmonary arteries. DEX (0.3, 1, or 3×10⁻⁹ mmol/L) inhibited serotonin-induced contraction in arteries with intact endothelium in a dose-dependent manner. L-NAME and yohimbine abolished DEX-induced inhibition, whereas indomethacin had no effect. No inhibitory effect was observed in endothelium-denuded pulmonary arteries. DEX-induced inhibition of vasoconstriction in human pulmonary arteries is mediated by NO production induced by the activation of endothelial α₂-adrenoceptor and nitric oxide synthase.

6.Epidemiology and trend forecast of newly diagnosed pulmonary tuberculosis patients in Meishan City , 2014-2019

Objective To understand the epidemiological characteristics of the newly diagnosed pulmonary tuberculosis patients in Meishan City from 2014 to 2019, and to carry out trend prediction and analysis, so as to provide theoretical basis for the formulation of prevention and control strategies. Methods Raw data of newly diagnosed tuberculosis patients in Meishan City from 2014 to 2019 were collected from Meishan Statistical Yearbook. And Excel was used to manage the raw data. The epidemiological characteristics of newly diagnosed pulmonary tuberculosis patients in Meishan City from 2014 to 2019 were analyzed, and the epidemic trend of pulmonary tuberculosis in Meishan City in the next two years was predicted by grey GM(1,1) model. Results From 2014 to 2019, a total of 1 832 smear positive pulmonary tuberculosis cases were reported in Meishan City. The annual average reported incidence rate of smear positive pulmonary tuberculosis was 26.95/100 000. The overall incidence rate of smear positive pulmonary tuberculosis showed a downward trend. Among the 1 832 newly diagnosed tuberculosis cases, the majority were male, aged 31~40 years old, spring onset, township residence and rural residence, farmers and students. The predicted incidence of pulmonary tuberculosis in Meishan City from 2014 to 2019 based on grey GM(1,1) model is basically consistent with the actual incidence, and the predicted incidence of pulmonary tuberculosis in 2020 and 2021 are 27.68/100 000 and 30.25/100 000 respectively. Conclusion The incidence of newly diagnosed pulmonary tuberculosis in Meishan City from 2014 to 2019 has a downward trend in general, while it may increase in the next two years, so the prevention and control measures for high-risk groups, regions and seasons should be strengthened.
7. Correlation between HLA-B~* 13:01 allele and liver dysfunction in patients with occupational medicamentosa-like dermatitis due to trichloroethylene

OBJECTIVE: To explore the correlation between human leukocyte antigen( HLA)-B~* 13 : 01 allele and liver dysfunction in patients with occupational medicamentosa-like dermatitis due to trichloroethylene( OMDT). METHODS: Twenty patients with OMDT were chosen as study subjects by using a convenient sampling method. The sequence-based genotyping method was used for detecting HLA-B~* 13 : 01 allele in the DNA samples from peripheral blood of all study subjects. The serum levels of total protein,albumin,total bilirubin,direct bilirubin and alanine aminotransferase,aspartate aminotransferase and alkaline phosphatase activities in patients were examined. The correlation between the number of HLA-B~* 13 : 01 alleles and the liver function indices was also analyzed. RESULTS: There were 16 patients carrying HLA-B~* 13: 01 allele. The serum total protein in the HLA-B~* 13: 01 carriers was higher than that of non-carriers( P < 0. 05). The serum total protein was positively correlated with the number of patients carrying HLA-B~* 13: 01 alleles( P < 0. 05). CONCLUSION: The degree of liver function damage in OMDT patients may be related to carrying the HLA-B~* 13: 01 allele.
