1.Selective Intrahepatic Cholangiography via Choledochofiberscope and Litholytic Therapy

This paper reported twenty-two cases of residual biliary duct stones.Because of biliary duct stenosis of stone impaction,the intrahepatic duct could not show by routine cholangiography through T-tube. We inserted a nylon cathetr lmm in diamenter into the selective biliary duct,and per- formed cholangiography through the catheter.The results were satisfactory in 20 of the 22 pa- tients. In the patients with large stones in the intrahepatic duct,the stone-dissolving liquid was instilled through a catheter which was kept in the selective duct.Finally,the stones fragmentated and were removed out successfully.
2.Prevention of Complications Caused by Removal of Residual Stones Through Choledochoscope After Operations

It is reported here that removal of residual stones and choledochofiberscopy through T-tube fistula were peformed on 512 patients. The removal procdeure followed by complica- tions, including fever (19 cases), hemorrhage (11 cases), fistulation between T-tube fistula and duodenum (6 cases), false passage formed by damage to the fistula (4 cases) and gallstones falling into false passage and being difficult to remove (2 cases) . The compli- cations with exception of the gallstones falling into false passage were adjusted after antibiotic treatment, coagulant drip by inserting a catheter and false passage drainge, respectively. The opening of T-tube fistula did not heal following remval of residual stones after Roux-en-y in two patients whose wounds only healed after suturing the intestinal wall and abdominal wall by reoperation. The causes, prevention and treatment of complications Were discussed in this papar.
3.Simultaneous determination of tetrandrine,fangchinoline and berberine hydrochloride in Fengtongan Capsule by RP-HPLC

AIM:To develop a method for simultaneously determining tetrandrine,fangchinoline and berberine hydrochloride in Fengtongan Capsule(Radix Stephaniae tetrandrae,Medulla Tetrapanacis,Fructus Chaenomelis,Cortex Phellodendri Chinensis,etc).METHODS:RP-HPLC was used with Phenomenex Gemini C_ 18 column.The mobile phase was a mixture of methanol-acetonitrile-phosphate buffer solution(0.01 mol/L potassium dihydrogen phosphate and 0.01 mol/L sodium heptanesulfonate were mixed equally,which contained 0.2% triethylamine,then was adjusted to pH 3.0 by phosphoric acid)(25∶20∶55).The detection wavelength was at 230 nm.RESULTS:There was a good linear relationship in tetrandrine content of 0.051 52-0.824 3 ?g(r= 0.999 9),fangchinoline content of 0.035 20-0.563 2 ?g(r= 0.999 9),and berberine hydrochloride content of 0.031 36-0.501 8 ?g(r= 0.999 7).The average recovery of tetrandrine was 97.23%(RSD=1.3%,n=9),99.56%(RSD= 1.6%,n=9)for fangchinoline and 98.22%(RSD=1.5%,n=9)for berberine hydrochloride.CONCLUSION:The method is simple,selective and reproducible.It can be applied to the quality control of Fengtongan Capsule.

5.Clinical analysis of 740 cases of laparoscopic hysterectomy

Objective To evaluate the clinical efficacy of four different methods of laparoscopic hysterectomy: laparoscopic supracervical hysterectomy(LSH),classic intrafascial supracervical hysterectomy(CISH),laparoscopic-assisted vaginal hysterectomy(LAVH),and total laparoscopic hysterectomy(TLH). Methods A retrospective analysis was carried out on 740 cases of laparoscopic hysterectomy in this hospital from January 1999 to December 2004 in respect of operating time,intraoperative blood loss,weight of removed uterus,postoperative recovery,time to normal sexual life,sexual satisfaction,and incidence of complications. Results The operations were accomplished under laparoscope in all the 740 cases.The operating time of LSH,CISH,TLH,and LAVH were 95.3?32.4 min,84.5?31.7 min,105.3?34.5 min,and 169.4?37.4 min,respectively.The time to normal sexual life of LSH,CISH,TLH,and LAVH were 30?5 d,50?9 d,35?7 d,and 54?11 d,respectively.The incidences of complications of LSH,CISH,TLH,and LAVH were 1.7%(2/120),1.3%(4/310),2.8%(7/250),and 1.7%(1/60),respectively. Conclusions Laparoscopic hysterectomy is safe and reliable.Of four methods of laparoscopic hysterectomy,each has its advantages and disadvantages,and method selection should be based on patient's own conditions.

9.Surgical approach to intrahepatic bile ducts: anatomical study and clinical application

Objective To explore a new surgical approach to intrahepatic segmental bile ducts.MethodSurgical anatomic relationships between intrahepatic bile ducts and blood vessels in 30 adult liver specimens were studied.Results Left and right hepatic bile duct lie to the superior anterior board of the left and right trunk of the portal vein; left medial and right anterior segmental duct lie to the anterior medial edge of corresponding portal vein branches. Right posterior segmental duct lies on the visceral side of right anterior portal branches in 73%(22/30). and on the visceral side of posterior right branches in 80%(24/30). Left lateral segmental duct lies in the deep visceral side of sagittal portion of left portal vein in 93%(28/30).A new combined operational routes getting to segmental ducts from both visceral and diaphragmatic faces were designed to treat 38 patients with multiple intrahepatic calculis.Conclusion Combined operational routes from both visceral and diaphragmatic facies can easily expose and cut open intra and extra hepatic ducts and the strictures, removing calculi.
10.Qualitative research of career development needs among of the first session specialist nurses of intensive care unit in Jiangxi province

Objective To understand the career needs of the first session specialist nurses of intensive care unit (ICU) in Jiangxi province and to provide reference for their career management.Methods A qualitative research based on phenomenology was adopted.The first session specialist nurses of ICU in Jiangxi province were enrolled and received in-depth interviews.Results Three themes were found,including career identity needs,career development needs and career management needs.Conclusions Nursing managers should understand the ICU specialist nurses' real feelings and developing career management strategies.
