1.NEUROLOGIC MANIFESTATIONS IN PATIENTS WITH PULMONARY CARCINOMA

The authors report 105 cases of neurologic manifestation ia 822 patients with pulmonary carcinoma, with an incidence of neurological complication of 19.5%. The neurologic manifestations were the first symptom in 64 cases (61%), which showed orderly as headache, hemi-plegia, seizure, paraplegia, mental sympioms, backache, vertigo, sciatica, facial palsy, etc. In these 105 cases there were 93 suffered metastasis, which involved cerebrum, spinal cord, meninges, cerebellum etc. Twelve patients were nonmetastatic, among whom, carcinomaious encephalopathy, myopathy, dermtomyositis etc were observed
3.The role of four criteria in assessment of the severity and prognosis of hyperlipidemic acute pancreatitis

Objective To explore the four criteria,including bedside index for severity in acute pancreatitis(BISAP),Ranson score,modified CT severity index(MCTSI) and acute physiology and chronic health evaluation scoring system Ⅱ (APACHE Ⅱ) in assessment of severity and prognosis of hyperlipidemic acute pancreatitis.Methods A total of 326 patients with hyperlipidemic acute pancreatitis were studied retrospectively from August 2006 to July 2015.The discrepancy of the four criteria in assessment of severity and prognosis of hyperlipidemic acute pancreatitis was compared with chi-square test and receiver operating characteristic curve.Results The incidences of moderately severe acute pancreatitis and severe acute pancreatitis,local complications and mortality of patients with BISAP score ≥3,Ranson score ≥3,APACHE Ⅱ score≥8 and MCTSI score≥4 were significantly higher than BISAP score < 3,Ranson score < 3,APACHE Ⅱ score < 8 and MCTSI < 4 respectively (all P < 0.05).As far as severity was concerned,the sensitivity and AUC of APACHE Ⅱ were 57% and 0.814,which were higher than the other systems.The second most sensitive criterion was BISAP.In assessment of local complications,the sensitivity and AUC of MCTSI were 68% and 0.791,which were higher than the other three.The most sensitive criterion to predict mortality was BISAP with sensitivity 89% and AUC 0.867,which was followed by APACHE Ⅱ.Conclusions All four criteria can be used to determine the severity,local complications and mortality.Generally,BISAP is simple and easy to practice,and better than the other three.
4.Blockage reasons of temporary catheter in femoral vein and internal jugular vein for hemodialysis patients

Objective To discuss the blockage reasons and countermeasures of temporary catheter in central venous including femoral vein and internal jugular vein for hemodialysis patients.Methods The reasons of blockage of temporary catheter in central venous among 60 hemodialysis patients from January 2014 to September 2015 in Hemodialysis Room of the First People's Hospital of Longquanyi district of Chengdu were reviewed.There were 32 cases in femoral vein and 28 cases in the right internal jugular vein.The incidence of blockage of the two temporary catheter in the first two weeks,the incidence of blockage caused by mechanical factors and blood clots or fibrous sheath and the incidence of blockage which needed to change the catheter in the two groups patients within the first two months were analyzed.Results The first rate of defective function in the first two weeks of femoral vein group was 53.13%(17/32),and 7.14%(2/28) of the right internal jugular vein group,there were significant difference between the two groups (x2=13.061,P<0.001).The incidence of catheter blockage due to mechanical factors of femoral vein group was 96.88% (31/32),and 53.57% (15/28) of the right internal jugular vein,there were significant difference between the two groups (x2 =15.654,P<0.001).The incidence of blockage caused by blood clots or fibrous sheath and the incidence of blockage which needed to change the catheter in the first two months were no significant differences between the two groups(P=0.061,0.096).Conclusion The cases of temporary catheter in femoral vein have blockage earlier,and they have a higher risk of catheter jam than the cases of temporary catheter in right internal jugular vein.
5.Reconstruction of neuron synapses is involved in the function reestablishment of rat neuroimmunoreglation network

Objective To reestablish neuroimmunoregulation network and its function.Methods Whole-some gene array was used to analyze the gene expression involved in neuroimmunoregulation function of the lateral hypothalamic(LH) in rats at different immunizational time.The function trees of different genes were analysed with the PathWay Miner public database.Results 632 genes were differentially expressed: including 374 of 2-day immunized group,62 of 4-day immunized group and 196 of 6-day immunized group.Function signal pathway analysis for 398 up-regulated genes showed 27 genes were involved in 31 cell functional signal conduction pathways,including the well-known signal conduction pathways of synaptic reconstruction.Conclusion Synapses reconstruction appears to be the important pattern of functional reestablishment of neuroimmunoregulation network.
6.Antiplatelet and Anti-thrombotic Effects of ?-cyclodextrin Inclusion of Dragon's Blood

Objective To investigate antithrombotic and antiplatelet effects of Dragon's Blood,?-cyclodextrin inclusion of Dragon's Blood.Methods The methods of injection of adenosin diphosphate(ADP) into mouse tail vein,electrically stimulated arterial thrombosis in rats were used to evaluate the anti-thrombotic effects of Dragon's Blood and?-cyclodextrin inclusion of Dragon's Blood,respectively.Platelet aggregation(PRP or washed platelets) was tested according to Born's method.Results ?CYDB significantly protected against thrombosis caused by ADP on mice,and electrical stimulation on rats.In vitro?-cyclodextrin inclusion of Dragon's Blood inhibited AA,ADP,PAF-induced platelet aggregation.Conclusions The results suggest that compared with Dragon's Blood,?-cyclodextrin inclusion of Dragon's Blood has more potent anti-thrombotic effects on the thrombotic models,its mechanisms may be closely related to the inhibition of anti-platelet aggregation.?-cyclodextrin inclusion of Dragon's Blood can improve the bioavailability of Dragon's Blood.
7.Clinical research of dialyzer reprocessing machine W-F168-B.

This is a research project aimed at investigating the clinical efficiency of W-F168-B, a dialyzer reprocessing machine made by Weilisheng Biological Material Company in Chengdu, evaluating its effects on total cell volume (TCV) and dialysis sufficiency, and assessing its disinfection efficacy. All subjects were randomly divided into two groups, whose dialyzers were reprocessed by W-F168-B (made in Chengdu) and RENATRON II (made by MINNTECH Company in the United States) respectively. We detected TCV of the dialyzer before and after reusing and calculated their diversity. We also calculated all subjects' Kt/V. The values of TCV diversities of two groups were 5.5 +/- 4.15 (experimental group) and 4.5 +/- 2.56 (control group) respectively, P > 0.05. The levels of Kt/V of experimental group and control group were 1.25 +/- 0.26 and 1.24 +/- 0.19, P > 0.05. T-test showed that there was no statistically significance difference between the two groups. The result of every hemoculture is negative. In conclusion, these two dialyzer reprocessing machines are similar in the effect of the dialyzer's performance and the subject's dialysis sufficiency. The reused dialyzers reprocessed by these two machines are safe for use.

The current document is based on a consensus reached by a panel of experts from the Chinese Society of Allergy and the Chinese Society of Otorhinolaryngology-Head and Neck Surgery, Rhinology Group. Chronic rhinosinusitis (CRS) affects approximately 8% of Chinese adults. The inflammatory and remodeling mechanisms of CRS in the Chinese population differ from those observed in the populations of European descent. Recently, precision medicine has been used to treat inflammation by targeting key biomarkers that are involved in the process. However, there are no CRS guidelines or a consensus available from China that can be shared with the international academia. The guidelines presented in this paper cover the epidemiology, economic burden, genetics and epigenetics, mechanisms, phenotypes and endotypes, diagnosis and differential diagnosis, management, and the current status of CRS in China. These guidelines—with a focus on China—will improve the abilities of clinical and medical staff during the treatment of CRS. Additionally, they will help international agencies in improving the verification of CRS endotypes, mapping of eosinophilic shifts, the identification of suitable biomarkers for endotyping, and predicting responses to therapies. In conclusion, these guidelines will help select therapies, such as pharmacotherapy, surgical approaches and innovative biotherapeutics, which are tailored to each of the individual CRS endotypes.

9.Value of diffusion-weighted imaging in distinguishing pancreatic carcinoma from mass-forming chronic pancreatitis: a meta-analysis.

BACKGROUNDSeveral previous studies have shown that diffusion-weighted imaging (DWI) can provide additional information for focal pancreatic lesions by demonstrating more restricted diffusion in solid malignant tumors than in chronic pancreatitis, which can be indicated by a decreased apparent diffusion coefficient (ADC). However, these studies have a modest sample size and convey inconclusive results. The aim of this study was to determine, in a meta-analysis, the diagnostic performance of quantitative diffusion-weighted magnetic resonance imaging in distinguishing pancreatic carcinoma from mass-forming chronic pancreatitis.
METHODSWe determined the sensitivities and specificities across studies. A summary receiver operator characteristic (sROC) curve was constructed to calculate the area under the curve (AUC).
RESULTSThe pooled sensitivity of DWI was 0.86 (95% CI: 0.80-0.91) and the pooled specificity was 0.82 (95% CI: 0.72-0.89). The AUC of the sROC was 0.91 (95% CI: 0.88-0.93).
CONCLUSIONSDWI may be a potentially technically feasible tool for differentiating pancreatic carcinoma from mass-forming chronic pancreatitis. However, large-scale randomized control trials are necessary to assess its clinical value.
