1.Application of radiofrequency vaporization in arthroscopic treatment of knee disorders

Objective To investigate effects of radiofrequency vaporization in arthroscopic treatment of knee disorders.Methods Arthroscopic surgery was conducted in 126 consecutive cases of various knee disorders by using the Arthrocare 2000 System,includng 72 cases of meniscus injury,20 cases of synovitis,15 cases of osteoarthritis,8 cases of patella subluxation,6 cases of synovial syndrome,and 5 cases of anterior cruciate ligament rupture.Results All the patients were followed postoperatively.Knee function evaluation with the Neer Scores showed excellent outcomes in 82 cases,good in 35 cases,and fair in 9 cases,the rate of excellent-or-good outcomes being 92.9%.Conclusions Application of radiofrequency vaporization under arthroscopy for knee joint disorders has advantages of simple and accurate performance,effective hemostasis,minimal invasion,and quick recovery of joint functions.
2.Advanced research of effect of endoplasmic reticulum stress on chondrcyte

Endoplasmic reticulum stress(ERS) is a kind of subcellular pathological state,and associated with many diseases.Recently,the research of ERS on chondrocyte is at the beginning stage,and may be involved in pathogenisis of rheumatoid arthritis(RA) and osteoarthritis(OA).It has been proved that ERS can interfere the differentiation of chondrocyte,decrease the synthesis of abnormal protein,attenuate the injury of cell.But overreaction of ERS can cause chondrocyte apoptosis through an independent pathway without of Fas and NO.There are three signal transmission passages in ERS:ATF-6(activating transcription factor 6)、Ire 1(inositol-requiring 1)and PERK(PKR-like ER kinase).The three protein molecules activate apoptotic genes by TRAF-2(TNF receptor-associated factor 2)and GADD153(growth arrest and DNA-damage-inducible gene 153),initiate the chondrocyte apoptosis.Therefore,ERS may effect the pathogensis of RA and OA by modulating chondrocyte function and inducing apoptosis,but more research are needed to reveal the mechanism.
3.Prevention of early dislocation rate after total hip arthroplasty with posterior capsulotendinous repair

[Objective]To investigate the clinical results and significance and mechanism of early dislocation rate after total hip arthroplasty following the posterolateral approach with posterior capsulotendinous repaired.[Method]The incidence of early dislocation and the prosthetic range of rotation motion among 212 consecutive primary THAs with the posterior capsule and the short external rotators which had been sutured to the medius tendon was reviewed retrospectively,and was compared to the results of 486 consecutive unilateral primary THAs with a complete posterior capsulectomy.[Result]During a 6 months to 5 years(3.7 years in average)following-up,there were significant differences between two groups.In patients with posterior capsulotendinous repair,two of 224 primary total hip arthroplasty(0.9%) had an early posterior dislocation.In patients with a complete posterior capsulectomy,27 of 486 primary total hip arthroplasty(5.6%) had an early dislocation.The difference was statistically significant(x~2=8.51,P
4.A sensitive radioimmunoassav for morphine in a single hair

Objective The paper described a convenient and sensitive radioirnmunoassav method which can be used to measure the amount of morphine in a single hair. Method Succinvl-eggalbumin-morphine was used as immunogen to immunize New Zealand rabbits for obtaining high quality antiserum. The morphine- labeled with Iodine-125 was purified by HPLC. The radioimmunoassay method was established and the amount of morphine in a single hair was detected. Results The affinity constant of antibody is 3.25 ? 10~11L/M. The radiochemical purity of I-mophine is 95% and the specific radioactivity is 112?Ci/?g. The detection limit for morphine (3SD from zero-dose binding) is 0.01ng/ml. Five normai person's single hair and five addict's were detected . The length of these specimens is 9-24cm and the weight of them 0.7- 2. 1mg. The amount of morphine in the hair of normai person is 1. 75 ? 0.37ng/mg (x ? 5) and that of the addictos'is 471 ?204ng/mg (x ? s). Conclusion This method can be employed for accurate mea-surement of the amount of morphine in a single hair and the result can be adopted to indicate the probability of illicit-drug use.
5.Maintenance scheme of computer system failure of color Doppler ultrasound equipment

Objective: Through analyzing a fault case for GE P5 Color Doppler Ultrasound to find out the main performance and effect of computer failure in Medical devices fault and the solution for failure. Methods: By consulting with maintenance manuals and relevant information, and combining with the main problems of the computer equipment to make detailed analysis, and then discussed and explored the solution for resolving problem. Results: By using the elimination method of relevant calculating fault, computer system failure of Color Doppler Ultrasound can be effectively eliminated, and the normal operation of equipment can be guaranteed. Conclusion:With the rapid development of computer technology, it is applied wider and wider in the modern medical instrument and has become an important support technology of many medical equipment, and has been inseparable with medical equipment's technology development. Therefore, many computer failure of software and hardware directly become medical equipment failure, even directly affects the normal work of medical devices. Consequently, in the maintenance and repair for medical equipments, maintenance and repair of computer system becomes particularly important, and has been an important part to guarantee the normal work of medical equipment. We should pay more attention to it in work practice.
6.Inductive Effect of Ginsenosides on Differentiation of Rat Marrow Mesenchymal Stem Cells into Myocardial Cells in Vitro

[Objective] To observe the in-vitro inductive effect of ginsenosides (GS) on differentiation of rat bone marrow mesenchymal stem cells (MSC) into myocardial cells. [Methods] Marrow stromal cells were isolated from adult SD rats, and then were cultured and cloned by density gradient method and adhesive cultivation to prepare the primary MSC. The cell surface antigens of CD34 and CD44 were detected with flow cytometer to identify MSC. After subculture, the 8th generation of continuous MSC were induced by GS (250 mg/L) and 5-aza (5-azacytidine, 10 ?mol/L) and GS + 5-aza (250mg/L and 10 ?mol/L respectively) for 24, 48 and 72 hours to differentiate to myogenic cells. Meanwhile, a blank control group was set to compare the effect. After the induction, myocardial cell percentage was worked out after examining MSC count and positive cells count under the phase contrast microscope. The specific proteins of Desmin and cardiac troponin I (cTnI) in myocardial cells were detected by immunohistochemistry method, and the cardiac-specific gene expression of ?-MHC (myosin heavy chain) and ?-MHC in myocardiocytes was examined by reverse transcription-polymerase chain reaction (RT-PCR) analysis. [Results] The cultured marrow stromal cells grew well, spindle in shape, and their CD44 expression was positive, indicating that marrow stromal cells differentiated to MSC. After induction with GS and 5-aza, MSC differentiated to myocardial cells, and the myocardial cell percentage was 38% in GS group, 35% in 5-aza group and 39% in GS + 5-aza group, the difference being insignificant. The expression of Desmin, cTnI and MHC in MSC was positive, and MSC were spindle-shaped and looked like fibroblast, indicating that MSC differentiated to myocardial cells. [Conclusion] Ginsenosides can induce MSC to differentiate to myogenic cells in-vitro, and this will supply evidence for cell transplantation.
7.Effects of propofol and sevoflurane anesthesia on cognitive function and amyloid beta protein deposition in hippocampi of aged mice

Objective To evaluate the effects of propofol and sevoflurane anesthesia on cognitive function and amyloid beta protein ( Aβ) deposition in hippocampi of aged mice. Methods Thirty?six SAMP8 mice, aged 6 months, weighing 29-32 g, were randomly assigned into 4 groups ( n=9 each) using a random number table: control group ( group C ) , propofol anesthesia group ( group P ) , sevoflurane anesthesia group (group S) and propofol plus sevoflurane anesthesia group (group PS). In group P, propofol 140 mg∕kg was injected intraperitoneally, when righting reflex occurred, additional propofol 70 mg∕kg was given, and when it occurred again, additional propofol 40 mg∕kg was given. Group S continuously inhaled 1% sevoflurane for 120 min. Group PS continuously inhaled 2% sevoflurane for 120 min, and when righting reflex occurred, additional propofol 40 mg∕kg was given. Anesthesia was maintained for 120 min in P, S and PS groups. Before anesthesia and at 7, 14 and 28 days after anesthesia, Morris water maze test was performed, and the escape latency was recorded. Hippocampi were obtained to determine the expression of Aβ using immuno?histochemistry. Results Compared with group C, the escape latency was significantly prolonged at 7 days after anesthesia, and the expression of Aβwas up?regulated at 7, 14 and 28 days after anesthesia in group S, and no significant change was found in the parameters mentioned above in P and PS groups. Compared with the value at 7 days after anesthesia, the expression of Aβ was significantly down?regulated at 14 and 28 days after anesthesia in group S, and no significant change was found in the expression of Aβ at 14 and 28 days after anesthesia in C, P and PS groups. Conclusion Although sevoflurane anesthesia promotes Aβ deposition in hippocampi, it only causes short?term cognitive dysfunction, however, anesthesia with propofol or with propofol in combination with sevoflurane produces no influence in aged mice.
8.Effect of fentanyl on efficacy of low-dose ropivacaine for spinal anesthesia in patients undergoing anorectal surgery

Objective To evaluate the effect of fentanyl on the efficacy of low-dose ropivacaine for spinal anesthesia in patients undergoing anorectal surgery.Methods Forty ASA Ⅰ or Ⅱ patients,aged 20-55 yr,with body mass index 18-28 kg/m2,scheduled for anorectal surgery,were randomly divided into 2 groups (n =20 each):0.5% ropivacaine 7.5 mg group (group R) and 0.3% ropivacaine 6.0 mg+ fentanyl 10 μg group (group RF).A catheter was implanted into the subarachnoid space (L3.4 interspace) and advanced caudally until lumbar region.Group R received hyperbaric 0.5% ropivacaine 1.5 ml.Group RF received 2.0 ml mixture of hyperbaric 0.3% ropivacaine 6.0 mg and fentanyl 10μg.The onset time of sensory and motor block,upper level of sensory block,and duration of sensory and motor block were recorded.Motor block was assessed by modified Bromage scale.Results Compared with group R,the duration of sensory and motor block was significantly shortened,and modified Bromage scores were significantly decreased in group RF (P < 0.05 or 0.01),and no significant change was found in the onset time of sensory and motor block and upper level of sensory block between the two groups (P > 0.05).Conclusion 0.3 % ropivacaine 6.0 mg combined with fentanyl 10 μg provides satisfactory spinal anesthesia for anorectal surgery,with lower degree and faster recovery of motor block.
9.Anatomy of the attachment of the medial patellofemoral ligament of human knee

ObjectiveTo study the anatomic characteristic of the attachment of medial patellofemoral ligament and its function in patellar stability. MethodsThirty adult cadaver knees were used for anatomic study, and the attachments of medial patellofemoral ligament were observed and measured. Results The femoral attachment of medial patellofemoral ligament was anchored to the bone between the medial femoral epicondyle and the adductor tubercle. The fibers here were thin and narrow, and became thick and wide to the anterior. The patellar attachment was in the superior two-thirds of the medial margin of the patella. The fiber here were the thickest and the widest. Conclusion The anatomic characteristic of the attachment of medial patellofemoral ligament was revealed, providing anatomical bases for surgery.
10.Anatomical study of the femoral and tibial insertions of the anterolateral and posteromedial bundles of human posterior cruciate ligament

Objective To provide an anatomic evidence for the double-bundle posterior cruciate ligament (PCL) reconstruction, the sizes and locations of the attachments of the PCL to the tibia and the femur were measured. Methods We studied 30 cadaveric knees. PCLs were divided into anterolateral and posteromedial bundles to the insertion footprint, and those locations were measured and described. Results The distances from the center of the femoral insertions of the anterolateral and posteromedial bundles to the anterior margin of the medial femoral condyle were (8.52±1.81)mm and (11.63±1.81)mm. The vertical distances from the center of the femoral insertions of the double-bundle to the intercondylar roof were (4.67±0.55)mm and (10.32±1.23) mm. The vertical distances from the tibial insertion of the center of the double-bundle to the plane of the tibial articular surface were (8.43±1.21)mm and (14.52±2.31)mm. The distances from the medial margin of the articular cartilage of the tibial plateau to the center of the tibial insertions of double-bundle were (47.44±6.23)mm and (45.95±6.32)mm. The areas of the insertions of the anterolateral and posteromedial bundles on the femur were (107.12±15.25)mm~2 and (65.35±10.27)mm~2. The areas of the insertions of the double-bundle on the tibia were (50.07±11.33)mm~2and (51.08±10.22)mm~2. Conclusion The anatomic characteristic of the attachment of the anterolateral and posteromedial bundles was revealed, providing anatomical bases for surgery.
