1.MRI of the Knee in Rheumatoid Arthritis:Comparison with Radiography

Objective To evaluate the value of magnetic resonance imaging in rheumatoid arthritis(RA) of the knee and compare with radiography.Methods 34 cases of RA of the knee were performed MRI and radiography.Enhanced MRI scans were obtained in 13 knees.Results On MRI there were bone erosion in 34 knees,subchondral sclerosis in 21 knees,meniscus destruction in 22 knees,cartilage damage in 15 knees,tibia movement in 10 knees,posterior cruciate ligament dragged in 8 knees.Pannus adhesion,proliferation synovium and joint effusion were showed on enhanced MRI in all of the 13 knees.On radiography bone erosion was showed in 1 knee,subchondral sclerosis in 11 knees,joint space stricture in 16 knees,tibia movement in 8 knees.Conclusion MRI is much better than radiolgraphy for diagnosis of RA of the knee.
2.Modified associating liver partition and portal vein ligation for staged hepatectomy: a systematic review

Objective To evaluate the feasibility,safety and effectiveness of modified associating liver partition and portal vein ligation for staged hepatectomy (ALPPS).Methods The published literatures associated with modified ALPPS were pooled from Embase,Pubmed,Medline,Google Scholar databases.The studies were included or excluded depends on our predetermined criteria.We selected data and performd descriptive analysis from the included studies.Results Five articles were included and reviewed.A total of 62 patients underwent five modified procedures,including monosegment ALPPS (m-ALPPS),anterior approach ALPPS,partial-ALPPS,radiofrequency-assisted liver partition with portal vein ligation (RALPP) and associating liver tourniquet and portal ligation for staged hepatectomy (ALTPS).There were 50 (80.6%) patients diagnosed liver metastatic colorectal cancer.The average operation interval of modified ALPPS was between 8 ~ 22 days and growth rate of future liver remnant (FLR) ranged from 48.7% to 62.3%,the feasibility to perform ALPPS stage 2 was 98.4%.The incidence of severe postoperative complications were between 11.8% ~33.3%.The 90-day mortality for monosegment ALPPS,partial-ALPPS and RALPP was 0,while the figure was 8.3% in ALTPS.The in-hospital morbidities were 5.9% and 8.3% for anterior approach ALPPS and ALTPS,respectively,which were 0 in the other three modified groups.Clinical response evaluation,including R0 resection rate,overall survival rate,disease-free and recurrence rates were merely presented 83.3%,80%,50%,50% in m-ALPPS group,while 100%,100%,95%,5% in modified ALTPS group.Conclusion Modified ALPPS with improved safety is feasible in clinical practice.However,the effectiveness still needs further studies.
3.Expression of survivin and its significance in suprarenal epithelioma

Objective To detect the expression of survivin in suprarenal epithelioma(SRE) and elucidate its function in suprarenal epithelioma.Methods A total of 53 SRE specimens and 8 nomal nephridial tissues were obtained.Immunohistochemistry method was used to test protein expression of survivin.Semi-quantitative RT-PCR was used to measure survivin mRNA levels.Results Survivin protein were detected in 33 of 53(62.3%) SRE tissues.In contrast,no expression of survivin in nomal nephridial tissues was detected.The expression rate of survivin mRNA in tumor tissues(56.25%,18/32) was significantly higher than that in normal tissues(0,0/8).Conclusion Over expression of survivin is common in suprarenal epithelioma.Survivin may play an important role in SRE and is associated with prognosis.

5.Factors related to choice of community health service centers in standardized training general practitioners in Shanghai

Objective To investigate the factors related to choice of community health service centers (CHSCs) in standardized training general practitioners (GPs) in Shanghai.Methods Totally 152 GPs,(including those of Traditional Chinese Medicine) who completed the residency training in 2013 and signed employment contract with community in Shanghai were surveyed with a self-designed questionnaire.Results Among all the participants,113 (74.3%) attached the greatest importance to the salary level when choosing CHSC,58 (38.2%) expected annual salary more than 120 000 Yuan during the first 3 years.GPs tended to work in the city center(40.1%) or CHSCs near to their home(38.2%),mainly because of the job commuting (59.9%).Considering career development,the CHSCs with high level comprehensive service (51.3%) and with large population (5.0%) were more attractive.GPs paid more attention to the national demonstration community (57.2%) and CHSCs with member ofShanghai outstanding young physician training program(51.7%).In this survey,household register was the main influencing factor for choosing CHSCs,Shanghai natives paid more attention to the location of the community than those not Shanghai natives (61.8% vs.34.2%),more inclined to CHSCs that close to their house (55.3% vs.21.1%),non-Shanghai natives preferred authorized strength (51.3% vs.28.9%).GPs of Western medicine expected higher salary than GPs of TCM (x2 =13.648,P < 0.05),Males expected higher salary than females (x2 =7.799,P < 0.05),Conclusions GPs pay the most attention on salaries and tend to work in the city central area,prefer CHSC with high level comprehensive service and large served population.The main factor influencing GPs to chose CHSCs is the household register,the category of Gps and gender.
6.Efficacy of intravenous thrombolysis for the treatment of wake-up ischemic stroke under the guidance of multimode CT

Objective To observe the efficacy and safety of recombinant tissue type plasminogen activa-tor (rt-PA)for the treatment of the patients with wake-up ischemic stroke (WUS)under the guidance of multimode CT. Methods Eighteen patients with WUS (a thrombolytic group)suitable for intravenous thrombolysis after multimode CT imaging screen at the Department of Neurology,Shiyan Hospital of Integrated Traditional and Western Medicine,Hubei Province from October 2012 to October 2014 were enrolled retrospectively. Twenty patients with WUS (a control group)who underwent multimode CT imaging screen were suitable for intravenous thrombolysis,but because of exceeding time window or rejecting thrombolysis and other reasons without having intravenous thrombolysis from February 2012 to February 2014 were enrolled retrospectively. The control group was treated with conventional therapy and the thrombolytic group was treated with rt-PA (0. 9 mg/kg)intravenous thrombolytic therapy. The indicators including fibrinogen (Fib),coagulation function (prothrombin time [PT ]),activated partial thromboplastin time (APTT ), platelet (PLT ),high-sensitivity C-reactive protein (hs-CRP ),National Institute of Health Stroke Scale (NIHSS )scores,and activities of daily living scores (Barthel index)at before treatment and 24 h,7 and 14 days after treatment were observed respectively. The adverse events and complications were documented and compared with the control group. Results There were no significant differences in Fib,PT,APTT, PLT,hs-CRP,NIHSS score and Barthel index before treatment between the thrombolytic group and the con-trol group (all P>0. 05);at day 7 and 14 after treatment in the thrombolytic group,compared with before treatment,Fib (14 d after treatment),PLT,and hs-CRP were decreased,PT and APTT were prolonged,the NIHSS scores were decreased,and Barthel indexes were increased. There were significant differences (all P<0. 05). At day 14 after treatment,there were significant differences in Fib,PT,APTT,hs-CRP,NIHSS scores,and Barthel indexes (Fib:3. 25 ± 0. 38 g/L vs. 3. 55 ± 0. 28 g/L;PT:15. 7 ± 3. 2 s vs. 12. 9 ± 2. 5 s;APTT:42. 7 ± 3. 5 s vs. 38. 7 ± 2. 6 s;PLT:[189 ± 26]× 109/L vs. [201 ± 23]× 109/L;hs-CRP:5. 7 ± 0. 6 mg/L vs. 11. 3 ± 2. 2 mg/L;NIHSS scores:5. 6 ± 2. 4 vs. 9. 2 ± 4. 5;and Barthel indexes:68 ± 15 vs. 47 ± 5)between the two groups (all P <0. 05). Except 1 patient occurred symptomatic intracerebral hemorrhage after thrombolysis,no other serious complications were observed in the thrombolytic group. One patient in the control group had stress gastric ulcer and bleeding,no symptomatic intracerebral hemorrhage occurred. Conclusion Multimode CT guidance can be used as a reliable imaging evidence for patients with WUS expanding intravenous thrombolytic time window. Under the multimode CT guidance, using rt-PA for intravenous thrombolytic therapy has a certain efficacy.

Objective To compare the pathological and serological difference of rheumatoid arthritis (RA) models in severe combined immunodeficiency (SCID) mice transplanted with synovial tissues from patients with rheumatoid arthritis (SCID-HuRAg mice) established either by renal capsule or subcutaneous back heterotopic transplantation. Methods RA synovium and normal human cartilage were co-implanted subcutaneously into the backs or under the renal capsule of 15 SCID mice. Engrafted tissues and serum were taken at the 4th and 8th week after transplantation. Histopathology and ELISA were performed to compare their histological and serological differences with RA. Results The morbidity and taken rate were significantly increased in the subcutaneous back of the mice group than the renal capsule group. The degree of cartilage erosion as well as the titers of serum IgM type rheumatoid factor suggested no significant difference between the two groups of SCID-HuRAg model devel oped by different engraft methods. Conclusion Back subcutaneous transplantation SCID-HuRAg model can be an ideal and stable animal model for studies on the pathogenesis and biotherapy of RA.
8.Analysis on CTPA Image Quality by Using High and Low Concentration Contrast Agent with Various Injected Flow Velocities

Purpose To compare the CTPA image quality by using contrast agent with different concentration at different injection rate so as to provide suitable contrast agent injection for patients.Materials and Methods A total of 346 patients with suspected acute pulmonary embolism who required to undergo CTPA examination were randomly assigned to high (370 mgI/ml) and low (320 mgI/ml) concentration groups,and each group was further divided into six subgroups with different velocity (3.0,3.2,3.4,3.6,3.8 and 4.0 ml/s).The CT value of the main pulmonary artery,right pulmonary upper lobe artery and right lung under leaf posterior basal segmental artery was measured.Results In the high concentration group,there were no significant differences in pulmonary artery average CT value,noise,single to noise ratio (SNR) and contrast to noise ratio (CNR) among the subgroups with different velocity (P>0.05).In the low concentration group,the difference was not statistically significant in pulmonary artery average CT value (P>0.05) among the subgroups with different velocity;however,the noise,SNR and CNR of 3.0 ml/s subgroup had significant differences compared with other subgroups (P<0.05).There was no significant difference in average CT value of pulmonary artery between the subgroups with the same velocity in the two concentration groups (P>0.05).In addition,except that the noise,SNR and CNR of 3.0 ml/s subgroup showed significant differences with other subgroups either in high concentration group or in low concentration group (P<0.05),there were no significant differences in the above-mentioned parameters among other subgroups with the same velocity in both groups (P>0.05).Conclusion Compared with high concentration contrast agent,the image obtained by using low concentration contrast agent shows no difference in pulmonary artery average CT value but with low iodine flow and iodine flow rate,which can reduce the risks of contrast media induced nephropathy (CIN) and contrast agent extravasation.
9.Diagnostic efficacy of prostate imaging reporting and data system version 2.1 based on multiparametric MRI in detection of clinically significant prostate cancer

Objective:To evaluate the diagnostic performance of the prostate imaging reporting and data system version 2.1 (PI-RADS v2.1) based on multiparametric MRI (mpMRI) in the detection of clinically significant prostate cancer (csPCa).Methods:A total of 561 patients who underwent prostate mpMRI in the First Affiliated Hospital of Guangxi Medical University from June 2015 to December 2020 due to elevated prostate specific antigen were collected ambispectively. The patients were divided into csPCa group (276 cases) and non-csPCa group (285 cases) according to pathological findings. Prostate were scored according to the PI-RADS v2.1 scoring standard by a junior and a senior radiologist. The prostate volume was measured and the prostate specific antigen density (PSAD) was calculated. The diffusion-weighted imaging and dynamic contrast-enhanced MRI images were processed to measure the quantitative parameters of the index lesion, including apparent diffusion coefficient (ADC), volume transfer constant (K trans) and rate constant (K ep) values. The Mann-Whitney U test was used to compare the difference in parameters between the two groups. The predictors of csPCa were screened by logistic regression analysis. Predictive model of multi-parameter was established. The receiver operator characteristic curves were used to evaluate the efficacy of PI-RADS v2.1 and the model in diagnosing csPCa, and the comparisons of area under the curve (AUC) were conducted by DeLong test. Results:Compared with non-csPCa group, the patients in csPCa group had higher PI-RADS score of senior physician, PSAD, K trans and K ep value, lower ADC value ( Z=-16.69, -12.49, -3.43, -4.67, 13.91, all P<0.001). The PI-RADS scores of senior physician (OR=3.064, 95%CI 2.428-3.866, P<0.001), PSAD (OR=1.554, 95%CI 1.170-2.064, P=0.002) and ADC value (OR=0.095, 95%CI 0.032-0.288, P<0.001) were the predictors of csPCa. The AUC of junior, senior physician PI-RADS and combined prediction model were 0.861 (95%CI 0.830-0.892), 0.895 (95%CI 0.868-0.922) and 0.923 (95%CI 0.898-0.944). The pairwise difference was statistically significant (the PI-RADS score between the junior and senior physicians Z=3.24, P=0.001, the difference between the PI-RADS score of junior physician and prediction model Z=5.54, P<0.001, the difference between the PI-RADS score of senior physician and prediction model Z=4.20, P<0.001). Conclusion:Based on mpMRI, the application of PI-RADS v2.1 by junior and senior radiologists has the high diagnostic efficacy for csPCa, and the multi-parameter model has the best diagnostic efficacy for csPCa.
