1.Allogeneic cord blood mesenchymal stem cell transplantation for treating Emery-Dreifus muscular dystrophy in one case

A male patient, aged 32 years, weighing 38 kg, was recruited at the Department of Neurology, Wuxi Second Hospital, Nanjing Medical University in February 2008. The patient had general fatigue and amyotrophy for 18 years, and recruited in the hospital 3 days following burst dizziness and vomiting. Following genetic analysis and muscle biopsy, the patient was diagnosed as having Emery-Dreifus muscular dystrophy and cerebellar infarction. HLA compatible cord blood mesenchymal stem cell (CB-MSC) transplantation was performed. The grafted CB-MSCs were from Shanghai Cord Blood Bank. The donor was HIA all-identical, Rh blood type-identical, ABO blood type-incompatible. The patient received preconditioning of busulfan + cyclophosphamide + rabbit antithymocyte globulin. The patient then received peripheral venous administration of 40 mL CB-MSCs, 10 drops/minute, with total nucleated cell number of 31.98x108. 5 weeks after transplantation, symptoms of dizziness, swallowing difficulty and regurgitation of fluid were markedly alleviated. His weight increased by 4 kg and he recovered of hand coordination to handling computer operation. 6 months following transplantation, serum creatine phosphokinase decreased from 35.79 μkat/L, before transplantation to 9.55 μkat/L; serum creatine phosphokinase isoenzyme reduced from 18.20 μkat/L before transplantation to 4.78 μkat/L. Myoglobin decreased from 413.50 μg/L to 213.20 μg/L. Functional independence measure scores increased from 71 points to 101 points. Immunological rejection or tumorigenesis was not identified. CB-MSC transplantation can evidently decrease serum creatine phosphokinase level within a short period, and improve motor function in Emery-Dreifus muscular dystrophy patient. CB-MSC transplantation is beneficial for Emery-Dreifus muscular dystrophy, and short-term safety is reliable.
3.Clinical study on plasma orexin-A levels in patients with chronic obstructive pulmonary disease.

Objective To explore the change of plasma orexin-A levels in patients with chronic obstructive pulmonary disease(COPD).Methods Lung function test and blood gas analysis were performed in 15 obese COPD patients,20 COPD patients without obesity,20 obese controls and 20 normal healthy adults(control group).In both obese COPD group and obese group,the body mass index(BMI)was higher than 25 and there was no significant difference in BMI.The plasma was deproteinized by chromatography,and the plasma orexin-A levels were determined by radioimmunoassay.Results The plasma orexin-A levels in the obese COPD group[(8.82?1.90)ng/L]and the non-obese COPD group[(8.69?1.84)ng/L]were significantly higher than those in the obese group[(7.18?1.45)ng/L(P 0.05).Plasma orexin-A levels in patients with COPD correlated negatively with partial pressure of oxygen(PaO_2)(r = -0.527 ,P
4.To Explore the Doctor-patient Relationship During the Process of Teaching in Clinical Practice

During the process of teaching in clinical practice, there objectively exist the contradiction between the obligation of teaching and the invasion of the right to informed consent, and the contradiction between the growth of medical students and the invasion of patients' right to privacy. Health reforms, such as the regulation of "patient selecting doctor", bring some side effects to the teaching in clinical practice. In order to maintain the right to informed consent and privacy, to benefit the teaching in clinical practice, to develop the doctor-patient relationship, and to decrease medical disputes, the art to deal with the doctor-patient relationship must be explored. The laws and regulations of the teaching in clinical practice and of the right of doctors and patients must be developed too.
5.Investigation and Analysis of Dose-Related Items in Drug Package Inserts

OBJECTIVE:To provide reference for standardization of drug package.METHODS:5 dose-related items stated in a total of 426 package inserts of chemical drugs collected from the drug storeroom in our hospital were investigated and analyzed.RESULTS:Only 32.39% of the package inserts were with complete labeling.The labeling rate for special group was low and only 40% labeled the maximal dose.Less than 2% of child dose stated the doses according to body surface area,and the problem of over-dose was common.Only 27.46% had the item of "under supervision of a physician".CONCLUSION:It is necessary to strengthen supervision on drug package inserts and improve cognition on the importance of drug package inserts.

Interstitial cystitis (IC) is a chronic condition characterized by pain of the bladder and pelvic region, urgent micturition, frequent micturition and urine increased. Despite intense investigation to our knowledge, the etiology of this disease was unknown, and treatment is empirical. Recrudesce of IC was inextricable and hard to radical cure. This paper reviewed the literature published in the last years.
8. Influences of 4-amino-2-methy cantharidinmide on epileptiform electroencephalogram, GABA and GABAB receptor in convulsive rats

OBJECTIVE: To investigate the anticonvulsion effect of 4-amino-2-methyl cantharidinmide (AMC) and its influences on epileptiform electroencephalogram (EcoG), contents of γ-aminobutyric acid (GABA) and GABAB receptor. METHODS: Rat model of penicillin-induced-convulsion (PIC) was established by intracortical (ic) penicillin (PNC) injection in rat motor cortex. Valproate (VPA) was used as the positive control drug. Convulsion seizure latency and racine behavior study graduations were used as indexes to evaluate the efficacy. RM6240C multi-channel biological signal collection-processing system synchronously recorded EcoG of convulsive rats after intragastric (ig) administration of AMC (25.0, 100.0 mg · kg-1). The effects on convulsion and epileptiform discharge were analyzed. The contents of GABA and the expression of GABAB receptor in the cortex and hippocampus regions of rats were determined by immunohistochemistry technique. RESULTS: AMC at the two doses and VPA could reduce epileptiform activities and discharge and prolong the latencies of epilepsy seizure, compared with the PIC group (P < 0.01). In the rats treated with AMC at low dosage, the GABA contents in cortical and hippocampal regions were higher than those in normal control and model groups, but with no statistics significance (P > 0.05); compared with model control, the expression quantity of hippocampal GABAB receptor protein was significantly increased (P < 0.05), while the increase in cortex was not significantly different (P > 0.05). GABA expression quantity in groups of larger dosage AMC and VPA were respectively higher than that in the normal group and the model group, and the GABAB receptor protein expression quantity also significantly increased (P < 0.05). CONCLUSION AMC can antagonize the convulsion seizure and inhibit the epileptiform discharge induced by penicillin in rats. The anti-convulsion mechanism of AMC is possibly related with increasing GABA content and GABAB receptor expression in cortex and the hippocampus. 9.Study on Antioxidant Activity of Extract of Cassia Obtusifolia

Objective To study antioxidant activity of extract of Cassia obtusifolia. Method The free radical-scavenging effect of the extract was evaluated in the flavins system; erythrocyte membrane was exposed to three radicals generation systems, the inhibition of the extract on lipid peroxidation was determined. Results The extract showed strong scavenging effect on superoxide radical and dose- dependently inhibited the increases of lipid peroxidation content in the membrane exposed to xanthine-xanthine oxidase system, H2O2 or UV light. Conclusion Antioxidant activity of the extract was obvious.
