1.The effect of MPCNL in treatmentof complex proximal ureteral calculi

Objective To explore the effect of MPCNL in the treatment of complex proximal ureteral calculi.Methods 90 cases were involved in treatment of complex proximal ureteral calculi in our hospital.MPCNL group were 45 cases and U RL group surgery were 45 cases.Results The operation time,bleeding,single surgery success rate,stone-free rate after surgery success for URL and MPCNL were(34.8 ± 10.5) minutes and ( 112.5 ± 12.4) minutes,(12.5 ± 4.6)ml and (51.3 ±3.7)ml,71.1% and 95.6%,91.1% and 97.8% respectively(P < 0.05).Conclusions The operntive time of URL is shorter,blood loss of URL is less than those of MPCNL,but the success rate of surgery and stone-free rate of URL is not higher than those of MPCNL.Both treatment methods are safe and reliable without serious complications.
2.Analysis of 375 Drug Package Inserts on "Drug Use for Pregnant Women"

OBJECTIVE:To investigate the "drug use for pregnant women" in drug package inserts and to provide reference in standarding the drug package inserts.METHODS:375 package inserts of common drugs collected in our hospital were analyzed statistically in terms of the item of "drug use for pregnant women".RESULTS:Among the 375 drug package insert,134 stated "forbidden or contraindicated for pregnant women";51 stated "to be used with caution for pregnant women",40 had indefinite explanation about the drug use for pregnant women,and 23 had no explanation for drug use for pregnant women.CONCLUSIONS:It is advisable for the related department in our country tighten control on the safety of drug package inserts by organizing experts to conduct a regular post-marketing reevaluation on the drugs,urging drug manufactures to timely update the drug package inserts,setting and improving the standard format of drug package inserts to ensure the safety of clinical drug use.
3.Changes and significance of ubiquitin binding protein in cardiomyocyte hypertrophy

Objective To establishment the model of angiotensin Ⅱ (AngⅡ) inducinged cardiomyocyte hypertrophy in H9c2 myocardial cells, and observinge the the changes of ubiquitin binding protein leveljunction protein ubiquitination level. Methods Myocardial cell isolated and subcultured H9c2 were cultured in vitro andmyocardial cell, they were divided into three groups in different AngⅡ concentration (10-8, 10-7, 10-6 mol/L), with in different hours (0, 6, 12, 24, 48, 72 h), and there is also a control group; measured the total protein contentlevel were measured useby BCA method , measured the number of cardiomyocytes in the same area were measured, and the relative surface area of myocardial cells were calculated. The expression of ubiquitin binding protein cardiomyocyte hypertrophy was time- and concentration-dependently induced by AngⅡ, wheremanifested, as total cellular protein content,relative cell surface area and the expression of ubiquitin binding proteinubiquitinated desmin significantly increasing compared with the control group (P < 0.05), which were most significant in 10-7 mol/L AngⅡtreated group for 48 h. Conclusions Cardiomyocyte hypertrophy model induced by 10-7 mol/L AngⅡ for 48 h , was successfully established , where H9c2 cardiomyocyte hypertrophy was induced by 10-7 mol/L AngⅡ in 48 h, and ubiquitin binding proteinubiquitinated desmin significantly increased in cardiomyocyte hypertrophy.
4.The design and implementation of real-time transmission system for pre-hospital emergency ECG

Objective:To design real-time transmission system for pre-hospital emergency electrocardiogram(ECG) so as to provide specialty and accurate diagnostic analysis about ECG before patient achieved at hospital and to shorten the waiting time for diagnosis and treatment after they achieved at hospital.Methods: 4G network and virtual private dial-up network(VPDN) were used to real-time transmit ECG data of patient from ambulance to hospital so as to doctors of ECG department and emergency room can analyze the detecting reporter, choose plan of emergency guidance and prepare operation and treatment for patientsaccording to the received ECG data in time.Results: The situations of emergency patients have been grasped and been primarily confirmed before they achieved hospital, therefore, the doctors could made relative preparation for fast and specially treating patients after they once achieved hospital.Conclusion: The real-time transmission system of pre-hospital emergency ECG can remedy the shortage of technical strength of pre-hospital emergency team, and shorten the treating time of emergency patients, and increase the success rate of rescue.
5.Advances in the treatment of West syndrome with hormonal therapies

West syndrome is one of the most common refractory epileptic syndromes with onsets mostly in infancy.The prognosis is generally poor.The morbidity rate reported in the literature is 0.2‰ to 0.5‰.Hormonal therapies(Adrenocorticotropic hormone and glucocorticosteroid)are recommended as preferred treatment options for West syndrome.It can effectively control spasms, ameliorate recognition impairment, and improve developmental outcomes.The incidence of side effects after hormonal therapy are high, and irritation, hypertension and infection are frequently reported.In this paper, advances in therapeutic mechanism, usage and dosage, short-term effectiveness, long-term effectiveness, prognosis and adverse effects of hormonal therapies are reviewed.

7.Effect of Capsule- deficient strains CAP64 on apoptosis of mice microglia cell-line (N9) cell in vitro

Objective To study the effect of capsule deficient strains CAP64 on apoptosis of mice microglia cell line (N9) cell in vitro . Methods N9 cells were co cultured with CAP64, the flow cytometry was employed to detect the N9 cellular apoptosis on different phase. Results The average apoptotic index of N9 after cultured for 2、4、8、16、24 hours with CAP64 were 9 33%、12 93%、15 37%、16 30% and 20 5%, respectively. There were significant differences between experimental group and control group. Conclusion Capsule deficient strains CAP64 can induce microglia cell line (N9) apoptosis
8.Factors affecting the recovery of spinal functions in patients with spinal metastatic carcinoma

BACKGROUND: Tumor spinal metastasis is often complicated with pain,limitations in movement, paralysis and poor prognosis.OBJECTIVE: To investigate the factors affecting the recovery of the spinal functions in patients with spinal metastatic carcinoma and its therapeutic methods so as to improve the living quality of the patients with a terminal cancer.DESIGN: A case-controlled observation.SETTING: The Department of Tumor and Department of Radiotherapy of the Affiliated Hospital of Guangdong Medical College.PARTICIPANTS: We selected 162 patients with spinal metastatic carcinoma with different degrees of paralysis who received treatment at the Department of Oncology and the Department of Radiotherapy of the Affiliated Hospital of Guangdong Medical College between March 1992 and June 2001. They participated in this experiment voluntarily.METHODS:①Radiotherapy and chemotherapy; All the patients were given symptomatic treatment with simple radiotherapy or a combined therapy of simple radiotherapy and chemotherapy, in which the dose of chemotherapy was 2 Gy/time,5 times a week,3 weeks in total. A combined method was used for chemotherapy according to the nature of the different primary tumors. The chemotherapy was conducted for 3 therapeutic courses in average ranging from 2 to 6 courses of treatment with 21 days for each course. ② Physical rehabilitation therapy included exercise therapy, massage (twice/day), functional electrical stimulation (once a day),bladder training and toilet training (once or twice every day), 10 days as a therapeutic course, 3 successive therapeutic courses in total. After the treatment, analgesic pain, muscle power of the limbs and the recovery of defecation function were observed.MAIN OUTCOME MEASURES: Pain-alleviating, muscle power of the limbs and the recovery of defecation function after the operation. RESULTS: Totally 162 patients with spinal metastasis carcinoma were recruited, and all the patients entered the stage of result analysis at the end of therapy with no loss in the midway. ① Pain complete alleviating was in 97 cases (59.88%); no alleveclting in 0 case. ② The condition was remarkably improved in 127 cases(78.40%), and it was not improved in 35 cases (21.60%). The improved rate of muscle power of the patients with one focus and 2 adjacent focuses was 92.98%. The improved rate of the patients with multiple foci was 64.59%. The muscle power of the patients with a disease history of less than 2 weeks was improved remarkably, but there was no change in muscle power in patients with a disease history of more than 2 months. CT or MRI showed that spinal cord at the affected segment of the 21 patients suffered severely compression before the treatment, and only 2 cases were improved in muscle power, with no other changes.③The defecation function of 162 patients all became normal.CONCLUSION: Patients with spinal metastatic carcinoma complicated with paralysis have different degrees of relief and amelioration in pain,muscle power of the limbs and defecation function after radiotherapy,chemotherapy and synthetic rehabilitation therapy. Factors affecting the recovery of spinal function mainly included the length of disease course,compressed degree of the spinal cord, the quantity of metastatic focus, the health status of the patients and therapeutic time and the quality of primary rehabilitation and so on. Mental help to the patients should be paid attention to so as to improve the quality of life at the same time.
9.Three Vessels and Trachea View in Prenatal Diagnosis of Fetal Aortic Arch Abnormalities

Purpose To explore the value of three vessels and trachea view in rapid screening of aortic arch malformations. Materials and Methods 51 fetal subjects who were prenatally diagnosed with aortic arch abnormalities were enrolled, and their diagnoses were confirmed by postnatal echocardiography or autopsy results after induction. Targeted fetal cardiac examinations were conducted under obstetric ultrasound examination conditions, ultrasound images were restored and compared with sonographic features of normal aorta. Results Among the 51 fetus cases with aortic arch anomalies, right-oriented aortic arch with left subclavian artery vagus was shown in 17 cases, left-oriented aortic arch with right subclavian artery vagus in 2 cases, double aortic arch in 2 cases, aortic arch disarticulation in 6 cases, and aortic arch coarctation in 24 cases. Diagnostic accuracy of ultrasound was 94.1%. Sonographic changes were displayed in three-vessel or three vessels and trachea view in all cases. Conclusion Three vessels and trachea view plays an important prompt role in ultrasound diagnosis for fetal aortic arch abnormalities. But when it is manifested as the“1oo”sign, it will be rather difficult for the differential diagnosis of aortic arch disarticulation and severe aortic arch coarctation.
10.Clinical analysis of twenty-eight patients with alveolar soft part sarcoma

Objective To study the clinical features of alveolar soft part sarcoma (ASPS) and assess its prognostic factors.Methods The clinical data of 28 ASPS patients from January 2006 to January 2009 were analyzed retropectively,and were followed up to get their prognosis.The survival analysis and risk factors were analyzed and compared.Results Two cases with soft tissue primary ASPS got recurrence after the first resection,the recurrence rate was 8.0% (2/25).After re-resection,no postoperative recurrence occurred.Three cases with bone primary ASPS did not have postoperative local tumor recurrence,but 2 cases appeared tumor progression and got pulmonary metastasis.All the patients had poor overall prognosis,11 cases survived without tumor,8 cases survived with tumor,9 cases died.The median survival time was 38.5 months,5-year survival rate was 46.4% (13/28),5-yeax survival rate of male patients was 50.0% (5/10),female patients was 44.4%(8/18),5-year survival rate between male and female patients had no statistical significance (P =0.260).The 5-year survival rate of patients without distant metastasis was 81.8% (9/11),while the rate of patients with distant metastasis (pulmonary,brain or bone metastasis) was only 23.5%(4/17),and the rates had significant difference (P =0.036).Age,gender had no significant relation with the prognosis,while tumor size,location and distant metastasis were the important factors affecting the prognosis.Conclusions ASPS commonly occurs in young persons.The hips and thighs are the most common sites ofsofte tissue primary ASPS.Prognosis has relation with distant metastasis,tumor size and part or thorough resection.The risk of recurrency and metastasis can be decreased through assessing disease condition as soon as possible,developing a reasonable treatment plan and wide resection of the primary tumor.
