2.The diagnostic value of three-dimensional ultrasound for identification of benign and malignant gastric ulcer

Objective To investigate the value of three-dimension (3D) ultrasound in identification of benign or malignant gastric ulceration. Methods A total of 88 patients with gastric ulcer were collected in Tianjin Nankai Hospital from March 2014 to May 2015, including 36 malignant cases and 52 benign cases confirmed by pathological results. Results of 2D ultrasound, 3D ultrasound and gastroscope diagnosis were compared, and the diagnostic values of the three methods were evaluated by the gold standard of pathologic results. Receiver operating characteristic (ROC) curves were plotted for the main measurement indicators (ulcer surface width, ulcer surface depth and thickness of stomach wall) of 3D ultrasound. The best cut-off value was determined. Results The positive rate of malignant gastric ulcer diagnosed by 3D ultrasound was significantly higher than that of 2D ultrasound (P=0.002), but there was no significant difference between 3D ultrasound and gastroscope detection (P=0.453). The diagnostic sensitivities of malignant gastric ulcer were 86.11%, 58.33%and 91.67%for 3D ultrasound, 2D ultrasound and gastroscope detection respectively. The diagnostic specificities were 100%, 100%and 96.15% respectively. The areas under the ROC curves measured by 3D ultrasound were 0.750, 0.940 and 0.977 for ulcer surface width, ulcer surface depth and thickness of stomach wall. And the best cut-off values of the three indicators were 16.55 mm, 8.05 mm and 9.90 mm. Conclusion The 3D ultrasound can show the form and structure of gastric ulcer more clearly and intuitively, which is valuable for the differential diagnosis of benign gastric ulcer and malignant gastric ulcer, and may be used for screening malignant gastric ulcer.
3.Percutaneous permeability of lidocaine hydrochloride loaded destran-based niosomes

Objective To study the percutaneous permeability through mouse skin of lidocaine hydrochloride-loaded destran-based niosomes(LID-HLD-BNs)in vitro and in vivo. Methods HPLC was employed to exam lidocaine hydrochlo?ride. Lidocaine hydro-chloride-loaded conventional liposomes (LID-CLs) and lidocaine hydrochloride injection (LID-IJ) were used as control. Isolated mouse skin was added into Franz diffusion cell to evaluate the permeability of LID-HLD-BNs in vitro. Confocal Laser Scanning Microscopy(CLSM)was used to observe the permeation depth of mouse skin in vivo. Re?sults The permeation rate and cumulative permeation amount were significantly higher in LID-HLD-BNs group than those of LID-CLs and LID-IJ groups (P<0.05). CLSM studies also confirmed that HLD-BNs reached deeper layers of the skin. Conclusion LID-HLD-BNs has good transdermal ability.
4.Effect of Three-stage Traditional Chinese Medicine Rehabilitation on Neurological and Motor Function in Patients with Cerebral Ischemic Stroke

Objective To explore the effect of three-stage Traditional Chinese Medicine (TCM) rehabilitation on neurological function and motor function in patients with cerebral ischemic stroke. Methods 100 patients with acute cerebral ischemic stroke were randomly divided into experimental group (n=50) and control group (n=50). The three-stage TCM synthesis rehabilitation was used in the experimental group; the 2-week western medicine rehabilitation was used in the control group. The scores of National Institutes of Health Stroke Scale (NIHSS) and Fugl-Meyer assessment (FMA) were evaluated. Results There was no significance in the scores of NIHSS and FMA between both groups before and 4 weeks after treatment (P>0.05). The score of NIHSS was lower (P<0.01), and the score of FMA was higher (P<0.05) in the experimental group than in the control group in the 12th week. Conclusion Three-stage TCM synthesis rehabilitation can facilitate the recovery of neurological and motor function of patients with acute cerebral ischemic stroke.
5.Preparation of multifunctional nanoscaled red blood cells drug delivery system and its photothermal and photodynamic effects

Objective To prepare a red blood cells based multifunctional nanoscaled drug delivery system,and to study its in vitro photothermal and photodynamic effects.Methods The indocyanine green (ICG)/doxorubicin (DOX) co-loaded nanoscaled red blood cells (DIRAs) were prepared using an extrusion method.The morphology,particle size,encapsulation efficiency,and stability were determined.The heating related change of particle size was studied using a size and potential tester.The in vitro photothermal effect was studied using an infrared imaging device.The uptake of DIRAs to 4T1 cells was studied using a CLSM examination.The in vitro photodynamic effect was studied using a fluorescence probe and CLSM examination.Results DIRAs were successfully prepared with a uniform and homogeneous size which was about (97.0±20.1) nm.The Zeta potential was about-21.6 mV and the encapsulation efficiency of ICG and DOX were 93.5% and 95.2%,respectively.The DIRAs had excellent stability within 28 days.This nanoscaled drug delivery system had identical photothermal effect compared to free ICG.The cellular uptake of DOX was significantly improved after the laser irradiation and the photodynamic effect was enhanced.Conclusions The prepared DIRAs have regular shape,suitable particle size,high encapsulation efficiency and high photothermal conversion efficiency.DIRAs can improve the cellular uptake of DOX and enhance the photodynamic efficiency.This biomimetic muhifunctional nano-system could facilitate breast cancer treatment by combining PTT7PDT and chemotherapy.
6.Feasibility and safety of vaginal delivery with scarred uterus after cesarean section

Objective To explore the feasibility and safety of vaginal delivery with scarred uterus after cesarean section.Methods Sixty two parturients after cesarean section undergoing trial vaginal delivery in our department from October 2016 to October 2017 were enrolled in study (study group);60 primiparous women admitted at the same period were taken as the control group.The successful rate of vaginal delivery,the duration of labor and the incidence of postpartum complications were analyzed.The postpartum depression was assessed with Edinburgh Postpartum Depression Scale 6 weeks after delivery and compared between two groups.Results The vaginal delivery was successful in 49 parturients of each group with a successful rate of 79.0% (49/62) and 81.7% (49/60) respectively (x2=0.13,P=0.71).The first and the second stages of labor in the study group were (364± 105)min and (54±31)min,respectively,which were shorter than those of the control group [(388±93)min and (63± 18)min,t=2.325,P=0.03;t=2.145,P=0.04].The incidence rates of postpartum hemorrhage,infection and urinary retention were 3.2% (2/62) and 1.7% (1/60),1.6% (1/62) and 1.7% (1/60),4.8% (3/62) and 3.3% (2/60) in study group and control group,respectively (x2=0.309,0.001 and 0.176,P>0.05).The Apgar scores of the newborns in two groups were 8.7± 2.1 and 8.5± 1.8 (t=1.415,P=0.14) and the postpartum depression scores of the mothers were 13.7±4.3 and 12.4±3.2 (t=1.203,P=0.33),respectively.Conclusion Vaginal delivery can be chosen in parturients with scarred uterus after cesarean section,if preparations are adequate,indications of vaginal delivery are sufficient and the course of labor is closely monitored.
7.Study on the value of GBS culture combined with candida vaginalis detection for predicting preterm premature rupture of membranes

Objective To investigate the value of combined detection of group B streptococcus (GBS) and vulvovaginal candidiasis (VVC) for predicting preterm premature rupture of membranes (PPROM). Methods A total of 227 pregnant women with PPROM who were hospitalized from August 2016 to August 2017 were selected as observation group. Immediately after admission, patients were given GBS culture and detection of Candida vaginalis. 743 cases of pregnant women with term delivery in the same period were randomly selected as the control group, and were given GBS culture and detection of Candida vaginalis at 35-37 th week of pregnancy. General data such as age, gestational weeks, body mass index, pregnancy time, and parity of the two groups were recorded. The positive rates of GBS and VVC and the relationship between them were observed. Results There was no significant difference in the basic data between the two groups (P>0.05). Among the 227 pregnant women with PPROM, the positive rate of GBS was 9.69%, which was significantly higher than that of the control group (5.11%), and the difference was statistically significant (P<0.05). The positive rate of VVC in the observation group was 25.11%, which was significantly higher than that in the control group (18.71%), and the difference was statistically significant (P<0.05). The positive rate of GBS in pregnant women with positive VVC was 9.69%, higher than that in those with negative VVC, and the difference was statistically significant (P<0.05). Conclusion Positive GBS or VVC can increase the risk of PPROM. It is necessary to routinely perform GBS screening and VVC testing before 35 weeks of gestation to predict the risk of PPROM and treat the patients with positive results.

OBJECTIVETo prepare a new dental topical anesthetics, lidocaine hydrochloride loaded trans-activator of transcription peptide conjugated nano-niosome (LID-TAT-N), and to evaluate its transdermal properties and topical anesthesia effects.
METHODSLID-TAT-N was prepared using reverse-phase evaporation method, and lidocaine loaded conventional liposome (LID-CL) was prepared in the same manner as positive control. The diameter, ζ potential and encapsulation efficiency of LID-TAT-N and LID-CL were measured. The skin permeation of LID-TAT-N was examined, and compared with LID-CL and lidocaine injection (LID-IJ, as negative control), using a Franz diffusion cell mounted with depilated mouse skin in vitro for 12 hours. Each experiment was repeated six times. The anesthetic effect of the new topical anesthetic was investigated on the cornea of rabbits.
RESULTSThe mean diameter of LID-TAT-N was smaller than that of LID-CL [(152.7 ± 10.6) nm vs. (259.5 ± 15.5) nm, P < 0.01]. The 12 h cumulative permeation amount was significantly higher in LID-TAT-N group [(1 340.0 ± 97.5) µg · cm(-2)] than those of LID-CL and LID-IJ groups [(1 060.6 ± 80.2), (282.6 ± 65.1) µg · cm(-2), respectively, P < 0.05]. Rabbit corneal reflex results showed that LID-TAT-N had anesthetic effect and the duration of analgesia [(24.8 ± 2.8) min] was also longer than that of LID-IJ [(14.5 ± 2.3) min, P < 0.05].
CONCLUSIONSLID-TAT-N had good transdermal ability, and the advanced skin penetration feature can improve its tropical anesthetic effect.
