1.Effect of shenmai injection on endothelial function during myocardial ischemia/reperfusion injury in rabbits

BACKGROUND: With the extensive development of reperfusion treatment on acute myocardial infarction, myocardial ischemia/reperfusion (I/R) injury cause clinical concern intensively. In recent years, nitric oxide (NO) and endothelin (ET)are hot point of researches. Myocardial protective role of Chinese traditional medicine is highly regarded.OBJECTIVE: To study the change of endothelial function during myocardial I/R injury of rabbits, the effect of shenmai injection (SMI) and mechanism.DESIGN: Randomized controlled animal study.SETTING: The First Hospital Affiliated to China Medical University.MATERIALS: The experiment was carried out in Laboratory Animal Center, Department of Biochemistry and the Second Electron Microscope Room of China Medical University from September 2002 to January 2003. Twenty-one healthy female Japanese white rabbits weighing 1.5-2.5 kg were in this study. SMI was produced by the Three Nine Medicine Industry Co., Ltd., Yaan (batch number: 010110).METHODS: ① Grouping: The animals were randomly divided into three groups, 7 cases of each group. Sham-operation group: Rabbits were gone through thread only and were not ligated coronary artery; I/R group and I/R + SMI group:Coronary artery occlusion was maintained 40 minutes at the ligature was released and reperfusion continued for 40 minutes. ② Administration: Rabbits in I/R group were injected 20 mL saline through ear margin vein (injected one third of saline at 10 minutes before ligature, two thirds of saline when unloosing the ligature). Rabbits in I/R + SMI group were injected with SMI which was diluted with saline to 20 mL (1.5 mL/kg). The method of injection was as same as I/R group.③ Index observation: 4 mL venous blood samples were obtained before ligature, at 40 minutes of ischemia and 40minutes of reperfusion. Half of the blood samples were separated serum to detect NO production (NOP) by nitric acid deacidizing enzyme process; another were separated plasma to detect ET by radiation immunity process. In the experiment ends, NOP and ET of myocardial tissue were detected; total of superoxide dismutase (T-SOD) of myocardial tissue were detected by sulphur purine oxidase process and malondialdehyde (MDA) were detected by thiobarbituric acid development process. Myocardial ultrastructure was observed with transmission electro microscope.MAIN OUTCOME MEASURES: ① Contents of serum NO and plasma ET in each.group before ligature, at 40 minutes of ischemia and 40 minutes of reperfusion; ② contents of NOP, ET, T-SOD and MDA of myocardial tissue; ③ Myocardial ultrastructure.RESULTS: All 21 rabbits were involved in the final analysis. ① Content of serum NOP: Content of serum NOP was lower in I/R group than that in sham-operation group at 40-minute ischemia and 40-minute perfusion (P < 0.01), but was higher in I/R +SMI group than that in I/R group (P< 0.01). ② Content of plasma ET: Content of plasma ET was higher in I/R group than that in sham-operation group at 40-minute ischemia and 40-minute perfusion (P < 0.01), but was lower in I/R +SMI group than that in I/R group (P < 0.01). ③ Content of NOP was negative correlation with that of ET 40-minute ischemia and 40-minute perfusion (P < 0.05, P < 0.01). ④ Content of NOP and activity of T-SOD: Those were lower in I/R group than those in sham-operation group at 40-minute perfusion (P < 0.01), but were higher in I/R +SMI group than those in I/R group (P< 0.01). ⑤ Contents of ET and MDA: Those were higher in I/R group than those in sham-operation group (P < 0.01), but were lower in I/R +SMI group than those in I/R group (P < 0.01). ⑥ Content of NOP was positive correlation with T-SOD activity (P < 0.05), and negative correlation with content of MDA (P < 0.05). In addition, content of ET was negative correlation with T-SOD activity (P < 0.05), and positive correlation with content of MDA (P < 0.05).CONCLUSION: Myocardial I/R leads to abnormality of vascular endothelial function, and SMI can improve endotheliual function and relieve myocardial I/R injury.
2.Effects of ShenFu injection on endothelial function of rabbits with myocardial ischemia-reperfusion injury

Objective To investigate the changes of endothelial function during myocardial ischemia and reperfusion (I/R) injury of rabbits, and the protection of ShenFu injection (SFI) for the injury, as well as the related mechanism. Methods Twenty one Japanese white rabbits were randomly divided into 3 groups (7 each): sham operated group (A), I/R group (B) and SFI-treated I/R group (C). The activity of serum endothelial nitric oxide synthase(eNOS), the concentrations of serum nitric oxide products (NOP) and plasma endothelin (ET) before ligation of coronary artery, 40min after ischemia and 40min after reperfusion were determined. The concentration of total superoxide dismutase (T-SOD) and malonyl-dialdehyde (MDA) in myocardial tissue were determined. Ultrastructure of myocardial cell was observed by electro microscope. Results 1) Serum eNOS and NOP decreased and plasma ET increased ingroup B than that ingroup A after 40min of ischemia (P<0.01), even more significant after 40min of reperfusion. NOP was correlated positively with ETafter 40min of ischemia (P<0. 05) and after 40min of reperfusion ( P<0. 01). In 40min after reperfusion, T-SOD was decreased and MDA was increased ingroup B than that in group A, significantly (P<0. 01). NOP were correlated positively with T-SOD, while were correlated negatively with MDA (P<0.05). ET was correlated positively with MDA, while was correlated negatively with T-SOD (P<0.05). Ultrastructure of myocardial cell changed abnormally. 2) SFI can increase eNOS and NOP after 40min ischemia and 40min reperfusion, and decrease ET after 40min reperfusion, significantly (P<0. 01). SFI can increase T-SOD and decrease MDA significantly (P<0. 01), the abnormality of myocardial cells can be relieved obviously. Conclusion SFI can improve the endothelial function in which free radicals are involved, and prevent myocardial tissue from I/R injury.

Objective Using Hitachi 7170 external quality assessment return target value to evaluate the accuracy of 10 items after Regression calibration of the Vistros 350 dry-type Biochemical Analyzer.Methods The same quality control samples were separately tested on two instruments,and results were reported to the clinical National Center for Clinical Laborato-ries.Substituted the external quality assessment return target value result from the National Center for Clinical Laboratories by using Vitros350 into regression calibration equation,then the getting data were compared with the external quality assess-ment return target value obtained from Hitachi 7170,and the deviation analysis was processed.The total error range from the America Clinical Laboratory Amended Bill was used as the standard.For the results within the reference range,error less than 1/2CLIA’88 total error,taken as the comparable judging standard,as it satisfied the requirement.For the results out off the reference range,error less than CLIA’88 total error,those still satisfied the requirement.For those items not meet the requirements,it must to do the regression calibration for Vitros350,using Hitachi 7170 as the standard instrument.Results The deviations of 7 items were all less than 1/2CLIA’88 allowed total error,with LDH was 0.16~-9.89,CK 2.92~6.25, ALT -4.64~-8.07,TBIL 0.08~2.67,TP -0.37~4.41,ALB 2.74~4.77 and URIC 1.04~3.0 respectively,and did not need re-calibration.For GLU and CREA,only one out of the reference range sample,the error range was >1/2CLIA’ 88,but
5.Application of the Requirement of Biological Variation Determination in the Quality Management of Blood Cell Analyzer

Objective To study application of the quality management of Blood Cell Analyzer according to the requirement of biological variation determination.Methods Collected the indoor imprecision value(CV%) from 8 items detected by blood cell analyzer during from April to Nov.of 2016,and the bias (Bias%) of 8 items of two EQA (external quality assessment) from the ministry of health in 2016.Then according to the 3 levels of the minium,appropriate and optimal quality specifications derived from the biological variability the rates of imprecision and bias were culculated.The pass rate of the imprecision and bias was calculated.By using mean bias and mean imprecision and biological variation 3 levels of total error (TEa) crite rion,and to calculate the corresponding σ and QGI value,so as to evaluate the performance of whole blood cell analyzer.Then improved the quality.Results For the imprecision value of 8 items,except the MCHC average value,all others were all 100 % meeting the appropriate level of quality requirements.For the bias value (Bias %) from 8 items,except MCH,all others were over 80 % meeting the appropriate level of quality requirements.While for the calculated σ value,based on the best level of quality requirements,except the σ value of WBC was 4.6,the σ value of all other items were all<3.Based on the appropriate level of quality requirements,except the σvalue of MCHC was 1.9,the value of σ of all other items were all> 3,and based on the minimal requirements,the σ value of all 8 items were all >3.After analysis,this blood cell analyzer,except that MCHC should use the minimal quality standard requirements,all other examination items could used the proper quality standard requirements,and the calculated QGI were all <0.8.Conclusion Based on the biological variation determination requirement and calculated σ and QGI value,this method could be used to more accurate quality evaluation of blood cell ana lyzer.Which is a higher levelof quality management,will be more conducive to quality improvement and better serve the clinical.
6.Preliminary Application of DSA Parametric Imaging in Evaluating the Hemodynamics of Brain after Interventional Therapy for Stenosis of Carotid Artery

Objective To evaluate the changes of cerebral perfusion and hemodynamics in the patients with mono-carotid artery stenosis after stenting with the technique of DSA parametric imaging. Methods 15 patients with mono-carotid artery stenosis(the stenosis>75%) undergone carotid stenting were choosed. Digital subtraction imagines of diplo-carotid arteries were acquired before and after operation, then the imagines were processed by special soft ware in personal computer. The region of interest (ROI)were selected in the brain,internal carotid artery and superior sagittal sinus separately, the time-gray scale curves of the ROIs were drawn with the soft ware, from which,then acquired the following parameters from the time-gray scale curves,the largest gray values of brain in disordered side pre-and post-operation, and the parameters including peak value (PV), mean transit time (MTT) ,time to peak (TP), time of appearance to the peak , the max slope rate of the curve and relative time of cerebral circulation were also evalua-ted, respectively. The imaging speed was 7.5 pictures per second. The results were statistically evaluated by using matched-t test. Results Before the stenting, the values of the parameters peak value, the max slope rate of the curve,TP,MTT,relative time of cer-ebral circulation were 108.20±5.58 , 1.23±0.37 , (4.78±0.24)s , (8.20±0.42)s and(4.92±1.03)s , respectively; after the stenting , the values of the parameters above-mentioned were 114.20±7.58, 2.01±0.36, (4.14±0.40)s, (3.55±0.56)sand(4.18±0.89) s , respectively, the difference of the parame-ters above-mentioned pre-and post-operation were statisti-cally significant (t=5.97 , 8.00 , 0.21 , 10.21 , 10.12 and 4.14,P<0.05). Before and after operation, the values of time of appearance to the peak were (5.39±0.24) s and)(5.37±0.78) s , respectively , there was no statistical significance (t=0.21, P> 0.05). Conclusion DSA parametric imaging can be used to evaluate the changes of cerebral perfusion and hemodynamics before and after arterial stenting.
7.Clinical observation on effect of yishen jianpi huayu decoction in treating chronic renal insufficiency.

OBJECTIVETo observe the therapeutic effect of Yishen Jianpi Huayu Decoction (YJHD) in treating chronic renal insufficiency (CRI).
METHODSForty-three patients with CRI were selected and randomly assigned into two groups, the 24 patients orally administered with YJHD in the treated group and the 19 administered with coated aldehyde oxystarch in the control group, the therapeutic course was 2 months. The symptom, physical sign, kidney function, blood lipids of patients were observed before and after treatment and the comprehensive clinical efficacy of the treatment was evaluated.
RESULTSThe total effective rate was 87.5% and 52.6% in the two groups respectively, it was significantly higher in the treated group than that in the control group (P < 0.05). The improvement of kidney function, blood lipids, and hemorrheologic parameters in the treated group were superior to those in the control group (P < 0.05).
CONCLUSIONYJHD is effective in treating chronic renal insufficiency.

8.Investigation on the status of traditional Chinese medicine nursing knowledge-attitude-practice of newly recruited nurses in traditional Chinese medicine hospital

Objective:To investigate the current status of the traditional Chinese medicine (TCM) nursing knowledge-attitude-practice of newly recruited nurses in TCM hospitals, so as to provide evidence for improving the standardized training of TCM nursing among newly-recruited nurses.Methods:From March to April in 2020, a total of 800 newly-recruited nurses from 6 TCM hospitals above the municipal level in Hunan Province were selected as research objects by convenient sampling method. The investigation was conducted by TCM Nursing Knowledge-Attitude-Practice Questionnaire. A total of 800 questionnaires were distributed and 768 valid questionnaires were collected, with an effective recovery rate of 96%.Results:The total score of the Knowledge-Attitude-Practice Questionnaire of the newly recruited nurses of the TCM hospital was (191.61±41.80) ; the scores of knowledge and attitude dimension were (73.93±17.21) and (78.17±15.81) , at a middle level; the score of practice dimension was (39.51±8.78) , at a low level. There were significant differences in the total score of the questionnaire among the new nurses with different hospital levels, working years, whether they graduated from a college of TCM and whether they had received the training of TCM nursing knowledge-attitude-practice ( P<0.01) . Conclusions:The newly recruited nurses in TCM hospitals have a high level of TCM nursing knowledge and a positive attitude towards TCM nursing. However, in clinical work, TCM nursing has poor application ability and insufficient use of clinical skills. In-depth study of TCM nursing knowledge should be strengthened, focusing on the cultivation of TCM nursing thinking. It is also important to improve laws, regulations and related rules, and to conduct training according to the different characteristics of new nurses.
9.A qualitative study on the status quo and dilemma of cultural care practice in diabetes specialist nurses

Objective:To explore the status quo of diabetes specialist nurses' practice of cultural care and the cultural care dilemma they encountered.Methods:Using descriptive research method, from March to August 2022, 12 diabetes specialist nurses from Class Ⅲ Grade A hospitals in Guangzhou were selected by objective sampling to conduct a one-on-one structured in-depth interview. Colaizzi 7-step analysis method was used to analyze and extract interview data.Results:A total of 3 themes and 10 sub-themes were extracted, including perception of cultural care (lack of understanding of the concept of cultural care), the main contents of cultural care practice (respecting the religious beliefs and customs of diabetes patients, providing language consistency communication for diabetes patients, offering personalized health guidance for diabetes patients, changing diabetes patients' bad health concepts to maintain patients' health, and seeking support systems for diabetes patients), obstacles to practicing cultural care (self-factors, patient factors, environmental factors, organizational management factors) .Conclusions:The diabetes specialist nurses lack the understanding of the concept and content of cultural care, and their cultural care ability is insufficient, but they have implemented some cultural care for diabetes patients in their nursing practice. In view of the cultural care dilemma encountered by this group in clinical work, efforts should be made to solve it through diabetes specialist nurses, relevant experts, hospitals, departments and so on.
10.Role of innate lymphoid cells in oral squamous cell carcinoma microenvironment

Oral squamous cell carcinoma (OSCC) is the most common oral malignancy. It has a high incidence, strong invasion ability, easy metastasis, poor curative effect, and poor prognosis. Innate lymphoid cells (ILCs) are an important part of immune cells located in the mucosal barrier, which play an important role in the occurrence, development and outcome of tumors. ILCs are the key cells for decoding the regulatory mechanism of tumor microenvironment and the signatures for tumor progression. This paper reviewed the latest progress on ILCs, summarized the possible characteristics and functions of ILCs in the microenvironment of OSCC, and explored the relationship between ILCs and the occurrence, development and immunotherapy of OSCC.
