1.Ultrasound-localized thrombin injection for the treatment of iatrogenic femoral pseudoaneurysm

Objective To evaluate the feasibility and safety of ultrasound-localized percutaneous thrombin injection (ULTI) for the treatment of iatrogenic femoral pseudoaneurysm. Methods From January 2000 through October 2001, 5 patients (3 males, 2 females, age range 38~72 years) were found to have a pseudoaneurysm confirmed by ultrasound between 1 and 3 days following femoral arterial puncture. Two patients were associated with diagnostic arteriography and 3 with stent implantation. ULTI was our first choice for the treatment of ablating femoral pseudoaneurysm. All patients following ULTI were restudied within 24 hours. Results All 5 patients were initially treated with ULTI. Thrombin was injected directly into the pseudoaneurysm with a dose of 500 units over several seconds, successful ablation was visualized immediately in 4 patients, the remaining 1 patient needed ultrasound-guided compression treatment of five minutes. Follow-up at the 24th hour showed no recurrent pseudoaneurysm after initial successful ablation in any case. No distal embolization or allergic reaction occurred. Conclusion ULTI is a safe, rapid, well-tolerated, inexpensive and effective noninvasive method for the treatment of iatrogenic femoral pseudoaneurysm and should be considered as first-line therapy.
2.Analysis on Medication Law for Differential Diagnostics of Treating Gynecological Diseases from “Chong-Ren” by CAI Lian-xiang

Objective To analyze clinical differential diagnostics and medication law of CAI Lian-xiang of treating gynecological diseases from theChong-Ren;To provide references for clinical medication. Methods Medical cases of gynecological diseases treated by differential diagnostics from“Chong-Ren”from December 2013 to January 2015 in out-patient department of Xiyuan Hospital of CACMS by CAI Lian-xiang were collected. Excel2007 was used to establish the database, and the frequency and correlation analysis were made on the differentiation, treatment principles, treatment methods, medicine and medical combination by using the large data intelligent analysis system of Chinese medicine diagnosis and treatment. The results were confirmed with CAI Lian-xiang's academic thought and clinical experience. Results Totally 103 medical cases, 277 diagnosis times, 277 prescriptions and 92 kinds of Chinese materiamedica were included. Although different types of gynecological diseases from theChong-Ren treatment of specific treatment were different, all were related to liver, spleen and kidney;prescription contained 22 high-frequency medicines (>50 times), and the first three medicines were Cuscutae Semen 178 times (64.3%), Angelicae Sinensis Radix 167 times (60.3%), Paeonize Radix 158 times (57.0%). The association analysis results showed 9 combination of Tiaochongren medicine combination group, 11 Qiangchongren meidcine combination group, 4 Zichongren medicine combination group, and 4 Guchuanrenmediicne combination group. Conclusion CAI Lian-xiang believes that gynecological diseases are closely related to “Chong-Ren”, and treatment from“Chong-Ren”focuses on the adjustment and nourishment of liver, spleen and kidney. The data miningresults of medication law is accordance with the relevant academic thoughts, which can provide references for clinic.
3.Expression of estrogen receptor alpha and beta in the uvea tissue of female rats

Objective To research the expression of the estrogen receptor alpha ( Erα) and estrogen receptor beta ( Erβ) in uvea tissues of the female rats, and to provide molecular biology data for further studies of the relation estrogen to uvea diseases. Methods Twenty-two adolescent SD female rats were selected. All rats were killed by dislocation of cervical vertebra, the eyeballs were in paraffin imbedding and made to a series of sections, using immunohistochemical method;Erα and Erβ distribution were investigated in uvea tissue of rats;and quantitied by Tanaka scores analytical method. The uteri of rats was used as positive control and PBS as negative control. The level of estradiol in serum of the rats were examined by radioimmunoassay. Results The expression level of Erβ was moderate or highter in stroma cell, anterior pigment epithelium as well as pasterior pigment epithelium of the iris, unpigmented epithelium, pigmented ciliary epithelium and vascular endocemet of the choroid layers. But Erα was not obviously expressed in uvea tissues. The expression rate of Erβ was higher than Erα in these tissues(P<0.05). Immnoreactivity positive substance was granule, which was distributed in the cytoplasm or in the nucleus. The level of estradiol in serum of the rats was (22.13±3.54)ng/L.Conclusion The expression of either Erα or Erβ in uvea tissues of rats is mainly in Erβ. The results indicate that uvea tissue is regulated directly by estrogen throught Erβ.
4.The effects of orthodontic appliance in the treatment of obstructive sleep apnea-hypopnea syndrome and hyper-tension

21 patients with obstructive sleep apnea-hypopnea syndrome(OSAHS)and hypertension were treated by orthodontic applince for 3 months.All complains of snore were alleviated or disappeared during sleep,the short of breath and drowsy in the daytime were disap-peared,AHI decreased(P<0.01)and the lowest SaO2 increased(P<0.01).The blood pressure tend to normal.The orthodontic appliance can effectively control OSAHS and hypertension.
5.Causes of early death in elderly patients undergoing maintenance hemodialysis

Objective To analyze the causes of early death in elderly patients undergoing maintenance hemodialysis ( MHD) and major risk factors.Methods A retrospective study was conducted.Fourty-three patients who had undergone MHD between January 2005 and December 2015 and died within 3 -12 months after hemodialysis in the Hemodialysis Center of China Aerospace Center Hospital were included in Group A while 106 patients who had undergone hemodialysis in the same period but survived for over 12 months were selected as control group(Group B).Results The main causes of early death of 43 elderly patients with MHD were cardiovascular ( 46.5%) and cerebrovascular diseases ( 25.6%) (CCVD), and infections(14.0%).The age,proportion of patients with diabetes and CVD,and the proportion of patients using the central venous catheter( CVC) as vascular access were higher in Group A,but the level of hemoglobin and serum albumin was lower.Indicators of cardiac structure and function showed that the left ventricular end-diastolic diameter (LVDd),left ventricular posterior wall thickness(LVPWT),and left ventricular myocardial quality index(LVMI)in Group A were well above those in Group B, but EF and E/A ratio were significantly lower(P<0.05 and P<0.01).COX regression analysis indicated that the comorbidity with CVD, a lower level of serum albumin and EF were the main risk factors for early mortality in elderly patients under MHD.Conclusion The main cause of early death in elderly MHD patients is CCVD.The comorbidity of CVD, malnutrition and decreased left ventricular systolic function might be the main risk factors for early mortality in elderly MHD patients.Early and adequate dialysis,active treatment of underlying diseases, and the correction of hypoalbuminemia can decrease complications,prolong life and improve the quality of life.
6.Curative effect observation of premature ovarian failure treated by oral taking traditional Chinese medicine with functions of tonifying kidney, nourishing blood and soothing liver and external application

Objective To evaluate the clinical efficacy of oral taking traditional Chinese medicine with functions of tonifying kidney, nourishing blood and soothing liver and external application treatment of premature ovarian failure(POF). Methods 40 patients of deficiency of liver-yin and kidney-yin type POF in Gynecology Department of Xiyuan Hospital CACMS from July 2009 to July 2013 were collected and administrated with traditional Chinese medicine with functions of tonifying kidney, nourishing blood and soothing liver and external treatment for 3 months. The changes of menstruation situation, symptoms and serum sex hormone were compared before and after the treatment. Results ① Menstruation integral were reduced after the treatment, showing a statistical significance compared with those before the treatment(t=5.694,P<0.01). ② Symptoms integral were reduced after the treatment, showing a statistical significance compared with those before the treatment(t=21.828,P<0.01). ③ Serum FSH, LH were decreased and Serum E2 were increased after the treatment,showing a statistical significance compared with those before the treatment(P<0.05). ④ The total effective rate was 92.5%(37/40). Conclusion Traditional Chinese medicine with functions of tonifying kidney, nourishing blood and soothing liver are effective for treating POF. These methods had proven to improve ovarian function by decreasing high level of FSH and LH, whereas enhancing the estrogen level, and therefore promote menstruation cycle.
7.Impact of early renal replacement therapy on the prognosis of patients with severe acute pancreatitis

Objective To explore the impact of early use of renal replacement therapy on the prognosis of severe acute pancreati-tis patients .Methods 60 cases of patients with severe acute pancreatitis were chosen between September 2015 and June 2016 in De-partment of General Surgery in our hospital ,and were randomly divided into treatment group (n=30) and control group (n=30) with the method of number table .The patients in the control group were treated with conventional therapy ,treatment group was given the early use of renal replacement therapy (within 12 h after onset) .The scores of Acute Physiology and Chronic Health E-valuation Ⅱ (APACHE Ⅱ) ,partial pressure of oxygen (PaO2 ) ,oxygenation index (PaO2/FiO2 ) ,lactate (Lac) ,C-reactive protein (CRP) ,interleukin 1 (IL-1 ) ,and interleukin-6 (IL-6) levels before treatment ,12 h ,24 h ,48 h and 72 h after treatment were com-pared between two groups ,and the incidence of abdominal compartment syndrome (ACS) was compared as well .Results The difference of the indicators before treatment between two groups was not significant different (P>0 .05);the scores of APACHEⅡ ,PaO2 ,PaO2/FiO2 and Lac levels at 12 h ,24 h ,48 h and 72 h after treatment in treatment group were all lower than those in con-trol group ,and the difference was statistically significant (P<0 .05);CRP ,IL-1 and IL-6 levels at 24 h ,48 h and 72 h after treat-ment in treatment group were lower than those in control group ,and the difference was statistically significant (P<0 .05) .In the control group ,5 cases had ACS ,the incidence rate was 16 .7% ,and there was no case having ACS in treatment group and the differ-ence was statistically significant (P<0 .05) .Conclusion The early application of renal replacement therapy can effectively improve the physiological indexes of patients with severe acute pancreatitis ,inhibit the inflammatory response rapidly ,reduce damage to the abdominal organs ,and is worthy of promotion and application in clinical practice .
8.Effect of deep brain stimulation of subthalamic nucleus on non-motor symptoms in Parkinson's disease

Objective To study the effect of deep brain stimulation of subthalamic nucleus (STN-DBS) on non-motor symptoms (NMSs) in patients with Parkinson's disease (PD).Methods Seventeen PD patients,admitted to and accepted bilateral STN-DBS in our hospital from 2012 to 2018,were chosen in our study.Unified Parkinson's Rating Scale (UPDRS) Ⅲ and Non-motor Symptoms Scale (NMSS) were used to evaluate the motor symptoms and non-motor symptoms one week before and 12 months after surgery.The correlations of motor symptoms and non-motor symptoms with age and gender were analyzed.Results The UPDRS Ⅲ total scores of 17 patients were 33.48+10.00 one week before DBS and 10.59±6.70 one year after DBS,with statistically significant difference (P<0.05).The NMSS total scores were 144.47±51.31 one week before DBS and 86.35±40.23 one year after DBS,with statistically significant difference (P<0.05).There were statistically significant differences in scores of sleep and fatigue,mental and behavioral abnormalities,sensory abnormality,cognitive impairment and constipation between one week before and one year after surgery (P<0.05).NMSS scores showed no significant correlation with gender,but showed significant correlations with age,course of disease and Hoehn-Yahr grading (P<0.05).Conclusion Bilateral STN-DBS can significantly improve some non-motor symptoms to a certain degree.
