1.Determination of the Main Component of Captopril Tablets by Derivative Spectrophotometry

OBJECTIVE:To establish derivative spectrophotometry for the determination of the main component of Captopril tablets.METHODS:Under the alkaline condition of sodium hydroxide,2,4-dinitrofluorobenzene was used as derivatization reagent to react with captopril at 40 ℃ in the water bath.Then UV spectrophotometry was adopted to detect the content of captopril with detection wavelength set at 342 nm.RESULTS:The linear range of captopril was 1.0~14.0 ?g?mL-1(r=0.999 9) with an average recovery of 99.61%(RSD=1.27%,n=9).The RSD of intra-day and inter-day both were less than 2%.CONCLUSION:This method is stable,reliable,rapid and simple for the determination of the main components of Captopril tablets.
2.Clinical study on combination of multiple regimens in treatment of osteoporosis in perimenopause and postmenopausal women

Objective To evaluate clinical efficacy of multiple regimen combination in treatment of osteoporosis of perimenopausal or postmenopausal women. Methods From Jul. 2008 to Dec. 2009, 109 women with low bone mineral density (BMD) or osteoporosis treated in Department of Obstetrics and Gynecology, Affiliated Second Hospital, Wenzhou Medical College were enrolled randomly into 3 group,including 36 women in Group A managed by osteoform 1000 mg/d + alfacalcidol 0. 25 μg/bid orally, 40 women in group B managed by osteoform 1000 mg/d + alfacalcidol 0. 25 μg/bid + tibolone 1.25 mg/d orally and 33 women in group C managed by ostcoform 1000 mg/d + alfacalcidol 0. 25 μg/bid +bisphosphonates 70 mg/w orally. After 48 weeks BMD on lumbar 1 -4 (L1-4) and left femur were detected by X-ray. Bone alkaline phosphatase(BALP) ,cross linked clelopeptide of type Ⅰ collagen(CTX) and 25-hydroxychole calciferol [25 (OH) D3] was measured by enzyme linked immunosorbent assay (ELISA).Result Seven women (6. 4%, 7/109) were withdrawed form this study, including 2 cases losing follow up in group A, 3 cases stopping treatment in group B, 2 cases giving up treatment due to severe adverse effect (burning in upper abdomen) in group C. (1) Pain relieve: after 48 weeks treatment, women in 3 groups improved symptom of pain significantly, the rates of pain relieve were 85% (29/34)in group A, 92% (34/37) in group B and 94% (29/31) in group C. (2) BMD: BMD was improved significantly in women in 3 groups after treatment. BMD of L1-4 were (0.88±0.15) g/cm2 in group A,(0.89±0.18) g/cm2 in group B and (0.87±0.10) g/cm2 in group C before treatment, and converted to (0.90±0.01) g/cm2 in group A, (0.93±0.09) g/cm2 in group B and (0.91±0.11) g/cm2 in group C after treatment. BMD of left femur were (0.87±0.07) g/cm2 in group A, (0.87±0.07) g/cm2 in group B and (0.85±0. 12) g/cm2 in group C before treatment and converted to (0.90± 0.03) g/cm2 in group A, (0.91±0.08) g/cm2 in group B and (0.89 ±0.12) g/cm2 in group C after treatment. It was shown significantly different BMD between group B or C and group A (P < 0. 01), however, there was no significant different BMD between group B and C (P >0. 05). (3) Index of bone metabolism: BALP were (26±6) μg/L in group A, (26±9) μg/L in group B and (28±7) μg/L in group C before treatment and converted to (22±5) μg/L in group A, (20±9)μg/L in group B and (22±8)μg/L in group C after treatment, which showed statistical difference (P < 0.05). CTX were (0.85±0.20) ng/L in group A, (0.84±0.47) ng/L in group B, and (0. 88 ±0. 11) ng/L in group C before treatment and converted to (0. 81 ±0. 19) ng/L in group A, (0. 77±0.33) ng/L in group B, and (0.82 ±0. 14) ng/L in group C after treatment, which showed statistical difference (P < 0. 05). Conclusions Those 3 regimens combination could be used in treatment of osteoporosis by decreasing bone conversion, increasing bone density, decreasing bone absorption. Regimen A was only suitable for basic therapy,the other two regimens could provide better treatment.
3.Diagnostic and Therapeutic Value of Single-balloon Enteroscopy in Suspected Small Intestinal Diseases

Background:Single-balloon enteroscopy(SBE)is a new method for the examination of small intestine,its clinical value in suspected small intestinal diseases need to be further studied. Aims:To evaluate the diagnostic and therapeutic value of SBE in suspected small intestinal diseases. Methods:A total of 73 suspected small intestinal diseases patients who had undergone 81 SBE examinations from July 2011 to October 2013 at Chengdu Military General Hospital were retrospectively examined,indications,diagnostic and therapeutic value of SBE in suspected small intestinal diseases were analyzed. Results:Of all the 81 examinations,33(40. 7% )were obscure gastrointestinal bleeding,29(35. 8% ) incomplete intestinal obstruction,and 19(23. 5% )chronic abdominal pain or diarrhea. The intubation depth was 230 cm for the oral approach,and 100 cm for the anal approach. The diagnostic yield of SBE was 67. 9% ,the main lesions were small intestinal ulcer,small intestinal inflammation,small intestinal tumor,small intestinal polyp. A total of 8 patients underwent endoscopic therapy,of whom 5 underwent endoscopic hemostatic therapy and 3 underwent resection of polyp. No serious complications were found. Conclusions:SBE is a safe and reliable diagnostic and therapeutic method for suspected small intestinal diseases,and its main indications are obscure gastrointestinal bleeding and incomplete intestinal obstruction.

Objective To detect the serum antibody to HPV6 in patients with condylomata acuminata (CA) by use of human papilloma virus(HPV)6bL 1 virus-like particles (VLPS), and to assess the significance of detecting HPV infection. Methods HPV6bL 1 VLPS were produced by a HPV6bL 1 recombinant baculovirus in insect cells,purified by CsCl density gradients centrifugation,and used for ELISA to screen 136 human serum samples in three groups. Results The prevalence of antibody to HPV6bL 1 VLPs was much higher in patients with codylomata acuminata (75%Ab+;mean abs 0.111?0.094) than that in patients with cervical cancer (14.3%Ab+;mean Abs 0.029?0.022)and that in control groups (2.9%Ab+;mean Abs 0.012?0.024). 6.Studies on Separation and Purification of Total Flavones from Licorice by Macroreticular Adsorptive Resin

OBJECTIVE: To establish a technical process for the separation and purification of total flavones from Licorice. METHODS: The static absorption capacity of macroreticular adsorptive resins D101、Hz-806、AB-83 for total flavones from licorice were compared. The macroreticular adsorptive resin columns on the Licorice extractives were eluted respectively with different concentrations of ethanol, then the content, the weight of residue and purity coefficient of flavones from Licorice in the eluant were detected. RESULTS: The optimal technological conditions were found in AB-8 as follows: flow rate=3ml/min, sample concentration=1.5 mg/ml, pH=5 and 80% ethanol was used as eluting agent. CONCLUSIONS: AB-8 macroreticular adsorptive resin can effectively separate and purify total flavones from licorice, the purity coefficient thus obtained being over 50%, which meets the requirement of the study of effective components of herbal medicine.
7.Preparation of Levofloxacin and Triamcinolone Acetonide Double-loaded Ophthalmic Gel

OBJECTIVE:To prepare Levofloxacin and triamcinolone acetonide double-loaded ophthalmic gel. METHODS:Us-ing levofloxacin hydrochloride and triamcinolone acetonide as main components,carbopol-940P as base material,HPMC K4M as tackifier,Levofloxacin and triamcinolone acetonide double-loaded ophthalmic gel was prepared. Using dissolution time as index, the contents of carbopol-940P and HPMC K4M were determined by single factor test,and dissolution time,viscosity and the con-tents of 2 main components were also determined. RESULTS:The concentrations of carbopol-940P and HPMC K4M were 0.4%and 1.2%,separately. The dissolution time was more than 24 h and viscosity was 1 142.67 Pa·s. The content of levofloxacin hydro-chloride was 97.3% of labelled amount (RSD=0.84%,n=3),and that of triamcinolone acetonide was 92.97% of labelled amount(RSD=1.32%,n=3). CONCLUSIONS:Levofloxacin and triamcinolone acetonide double-loaded ophthalmic gel has been prepared successfully.
8.Development and application of an ICU new nurses orientation program based on the Miller pyramid competency model

Objective To develop and apply an ICU orientation program based on the Miller pyramid competency model. Methods 45 new ICU nurses in 2013 and 2014 were selected as the experimental group. The experimental group was oriented by the new program,including the orientation of preceptors, workshop-centered orientation courses, checklists-precepting and comprehensive competency evaluation methods. 39 new ICU nurses in 2011 and 2012 were selected as the control group. The control group was oriented by the conventional program,including focused theory classes and traditional precepting.New nurses′ satisfaction with the orientation and the competency were analyzed before and after intervention in two groups. Results Satisfaction with the orientation, self-evaluated competency and peer-evaluated competency in the experimental group were(16.71±1.19) scores, (126.52±3.31) scores and (90.00±2.68) scores respectively,which were much better than the control group and had statistically significant difference in two groups. Conclusions The ICU new nurse orientation program based on the Miller pyramid competency model can improve the new nurses′ satisfaction with the orientation and the competency after the orientation.

Objective To investigate the diagnostic value of Bcl-2, miR-451 and Th17 cells in esophageal cancer and their relation with recurrence. Methods We selected 101 patients with esophageal cancer as the experimental group and 95 healthy patients as the control group. 10.Effect of tetrandrine on nitroglycerin induced activation of satellite cells in trigeminal ganglia.

OBJECTIVETo evaluate the effect of tetrandrine (Tet) on nitroglycerin(GTN)-induced activation of the satellite cells released inflammatory cytokines and to explore its mechanism.
METHODNeonatal rat satellite cells of trigeminal ganglia were cultured and separated into three groups. Group CON: the cells were normal cultured; Group TGN: the cells were cultured with 0.55 mmol x L(-1) GTN; Group Tet: the cells were treated with 0.55 mmol x L(-1) GTN and 1 x 10(-7) mol x L(-1) Tet respectively. Cell viability after GTN and Tet was detected by AlamarBlue assay. The concentration change of intracellular Ca2+ ([Ca2+]i) in single satellite cell loaded with Fluo-3/AM was determined by laser scanning confocal microscopy. NF-kappaB and IL-1beta mRNA levels were determined by FQ-PCR. Through double-immunofluorescent staining identifies satellite cells and determines the expression of NF-kappaB protein.
RESULTSatellite cells activities decreased with GTN stimulating, but according to the viability and modality of the cells, 1 x 10(-7) mol x L(-1) Tet was the suitable prophylaxis. Tet can inhibit the elevation of cytosolic free calcium of rat satellite cell and decrease the mRNA and protein levels of NF-kappaB and the mRNA levels of IL-1beta.
CONCLUSIONVia preventing Ca2+ influxion, Tet inhibited NF-kappaB activation of satellite cell which decreased IL-1beta expression.
