1.Comparison of the Effects of Iguratimod Combined with Methotrexate and Diacerein Respectively on Relat-ed Indexes of Refractory Rheumatoid Arthritis

OBJECTIVE:To compare the effects of iguratimod combined with methotrexate and diacerein respectively on relat-ed indexes of refractory rheumatoid arthritis (RRA). METHODS:98 RRA patients were randomly divided into control group (48 cases)and observation group(50 cases). Control group received Iguratimod tablet 25 mg,twice a day+Methotrexate tablet with ini-tial dose of 10 mg,once a week,increased to 12.5 mg after 2 weeks,increased to 15 mg in the second courses,once a week. Ob-servation group received Iguratimod tablet(the same dosage and usage with control group)+Diacerein granule 50 mg,twice a day. 4-week was a course,they were treated for 6 courses. Morning stiffness time,the numbers of 28 joints tenderness and swelling,28 joint disease activity score (DAS28),erythrocyte sedimentation rate (ESR),rheumatoid factor (RF),IL-1,vascular endothelial growth factor (VEGF),tumor necrosis factor (TNF)-α,C-reaction protein (CRP),malondialdehyde (MDA),superoxide dis-mutase(SOD),total antioxidant capacity(TAOC),early peak flow(peak E),left ventricular late flow peak flow(peak A),E/A and left ventricular ejection fraction(LVEF)before and after treatment,and the incidence of adverse reactions in 2 groups were ob-served. RESULTS:Before treatment,morning stiffness time,the numbers of 28 joints tenderness and swelling,DAS28 score, ESR,RF,IL-1,TNF-α,CRP,VEGF,MDA,TAOC and peak A in 2 groups were significantly lower than before treatment,and observation group was significantly lower than control group;SOD,peak E,E/A and LVEF in 2 groups were significantly higher than before treatment,and observation group was significantly higher than control group,with statistical significances (P<0.05). And there was no significant difference in the incidence of adverse reactions (P>0.05). CONCLUSIONS:Iguratimod combined with diacerein is superior to iguratimod combined with methotrexate in improving cardiac function,oxidation-antioxidant imbalance play and reducing inflammatory reactions in the treatment of RRA,with similar safety.
2.Anticoagulation therapy after splenectomy in cirrhosis patient

Splenectomy for patients with cirrhosis has been well established.However,many issues in relation to postoperative management of these patients remain to be elucidated,mainly including reasons for portal vein thrombosis and anticoagulant use.In light of recent progresses on this topic,the current review summaries and pinpoints influencing factors of portal vein thrombosis,anticoagulant selection and corresponding rationales,monitor measures for anticoagulation,and treatment schemes for bleeding induced by anticoagulation.Our review focuses on issues associated with the selection and withdrawal time of anticoagulant.We conclude that patients undergoing splenectomy can benefit from postoperative anticoagulation,including enhanced postoperative recovery and decreasing incidence of postoperative complications.
3.Analysis on constituents of traditional Chinese medicine galamina.

OBJECTIVETo analyze constituents of traditional Chinese medicine galamina, and provide basis for the quality control of galamina.
METHODPolarization microscope and electron probe were adopted for a qualitative and quantitative analysis on microscopic appearance and micro-area constituents of galamina. Fourty-two elements contained in galamina were determined by the plasma mass-spectrometric method. Zinc oxide, the major constituent of galamina samples, was detected using the pharmacopeia method.
RESULTThe micro-area analysis on galamina samples showed that galamina is mainly constituted by various mineral particulates such as hydrozincite, smithsonite, zinc oxide and dolomite in a loose texture. It is also found that there are many counterfeit galamina sold in the market, marked by a high lead content in natural galamina samples.
CONCLUSIONGalamina is a mineral aggregation of multiple natural mineral substances, but those sold in the market has many quality problems. Therefore, a fast method shall be established to identify the authenticity of galamina and a standard shall be set to control harmful elements contained in galamina. It is suggested to conduct in-depth studies on excessive harmful elements contained in galamina medicinal materials.

4.Cancer stem-like cell-derived exosomes promotes the proliferation and invasion of human umbilical cord blood-derived mesenchymal stem cells.

OBJECTIVE:
To investigate the effect of Piwil2-induced cancer stem-like cell (Piwil2-iCSC)-derived exosomes on the proliferation,migration and invasion of human umbilical cord blood-derived mesenchymal stem cells (hucMSCs).
METHODS:
Piwil2-iCSC-derived exosomes were isolated by ultracentrifugation and identified using transmission electron microscopy,nanoparticle tracking analysis and Western blotting.Exosome uptake assay was used to identify the pathway that Piwil2-iCSCderived exosomes utilized.HucMSCs were divided into control group,PBS intervention group and exosome intervention group,and CCK-8 assay,wound healing assay,Transwell assay,Western blotting and cell karyotype analysis were used to observe the proliferation,migration,invasion,expression levels of MMP2 and MMP9 proteins,and chromosome structure of hucMSCs.
RESULTS:
The diameter of Piwil2-iCSC-derived exosomes ranged from 50 nm to 100 nm,and most of them were oval or spherical capsules rich in CD9,CD63 and Piwil2 proteins.Exosomal uptake assay showed that the exosomes executed theirs functions after entering the cells.Compared with the control cells and PBS-treated cells,hucMSCs treated with the exosomes showed significantly increased number of proliferating cells (<0.05) with accelerated healing rate (<0.05 at 24 h;<0.01 at 48 h),increased invasive cells (<0.01),enhanced protein expressions of MMP2(<0.05 PBS group;<0.01 control group) and MMP9(<0.05),but their karyotype still remained 46XY without any abnormalities.
CONCLUSIONS
Piwil2-iCSC-derived exosomes can promote the proliferation,migration and invasion but does not cause cancer-like heterogeneity changes in hucMSCs.
