Objective To evaluate the efficacy of ultrasound-guided sacral block in the obese patients by comparing it with the traditional positioning method. Methods Sixty obese patients, aged 25-50 yr,weighing 56-80 kg,with body mass index ≥ 30 kg/m2,scheduled for elective anorectal surgery, were randomly divided into 2 groups (n =30 each) using a random number table:ultrasound-guided sacral block group (group U) and traditional positioning method group (group T).The sacral canal puncture point was located via either ultrasonic or traditional positioning method. After successful puncture,1% lidocaine 20 ml was injected. The number of patients in whom puncture was successful at first attempt and the number of patients in whom the time for puncture ≤ 1 min were recorded. The adverse reactions were observed. The efficacy of sacral block was evaluated. Results Compared with group T,the success rate of puncture at first attempt was significantly increased (P < 0.05),and no significant changes were found in the number of patients in whom the time for puncture ≤ 1 min or efffcacy of sacral block in group U (P > 0.05).No adverse reactions were found in the two groups. Conclusion Ultrasound-guided sacral block can raise the success rate of puncture at first attempt and provide similar efficacy of block in the obese patients when compared with the traditional positioning method, and thus has significant clinical value.

OBJECTIVE:To conduct supplementary study for quality standard for Compound vaccariae tablets. METHODS:TLC was used for the qualitative identification of Vaccariae;HPLC was used to determine the contents of vaccarin and anthranilic acid. The column was YMC-Pack Pro C18 with mobile phase of methanol- 0.3% phosphpric acid(gradient elution)at the flow rate of 1.0 ml/min,the detection wavelength was 280 nm,column temperature was 35 ℃ and volume size was 10 μl. RESULTS:The chromatographic spots were clear and well-separated without interference from negative control. The linear range was 0.055 7-2.228 7μg(r=0.999 9)for vaccarin and 2.011 4-40.227 9 μg(r=0.999 9)for anthranilic acid;RSDs of precision, stability and reproduc-ibility tests were no more than 1.01%;average recoveries were respectively 99.69%(RSD=1.11%,n=6) and 99.84%(RSD=1.18%,n=6). CONCLUSIONS:The method is simple, rapid and accurate with good reproducibility, and can be used for quality control of Compound vaccariae tablets.

Chinese Journal of Medical Instrumentation 2018;42(3):180-181
A wireless blood pressure measurement system was designed to facilitate the measurement of the patient's blood pressure and to transmit the measured data safely and reliably. Through PDA, radio frequency identification technology (RFID) and bluetooth technology, the function of reading patients' information and statistics and analysis of blood pressure, heart rate data was realized. The IDEA and RSA joint algorithms were used to encrypt the patients' data and the key of the IDEA algorithm to ensure the security of the patient' data. The test results showed that the system had high accuracy of measurement data, safe and reliable transmission, and improved the nurses' work efficiency.

OBJECTIVEFactor VIII( FVIII) gene knockout mouse model was established for further study on the treatment of hemophilia A.
METHODSExons 16-19 of the mouse FVIII gene were knocked out by ET clone, ES homologous recombination and tetraploid embryo compensation technology. PCR, reverse transcriptase-PCR(RT-PCR) and immunohistochemistry were used to detect the transcription and translation pattern of FVIII. The phenotype of the knockout mice was analyzed by examining the activated partial thromboplastin time (APTT) and FVIII activity (FVIII:C).
RESULTSPCR, RT-PCR and immunohistochemistry confirmed that FVIII was deficient in the FVIII gene knockout mouse. The APTT results showed that FVIII-deficient mouse plasma had a prolonged clotting time compared to normal mouse plasma. The FVIII:C in heterozygous, hemizygous and homozygous mice was 80%, 8% and 10% of that in normal mice, respectively.
CONCLUSIONThe phenotype of the FVIII gene knockout mouse appears grossly similar to that of human with hemophilia A. Establishment of this model may promote the development of new technologies of treatment to hemophilia A.

Chinese Journal of Cardiology 2015;43(2):134-140
OBJECTIVETo investigate the relation of epicardial adipose tissue volume (EATV) determined by dual-source CT (DSCT) cardiac angiography and EAT-derived inflammatory factors to coronary heart disease (CHD) and vulnerable plaque.
METHODSA total of 260 patients underwent cardiac computed tomography to evaluate stenosis of coronary artery, and blood samples were obtained from each patient. CHD was confirmed in 180 patients by DSA and CHD was excluded in the remaining 80 patients (NCHD). Vascular remodeling index and plaque vulnerability parameters (fatty volume, fibrous volume and calcification volume and fiber volume) were measured in CHD patients and correlation with EATV was analyzed. Epicardial adipose tissue (EAT) and intrathoracic adipose tissue (TAT) were collected from 40 CHD patients undergoing CABG surgery, and, mRNA and protein expressions of leptin and MMP9 were detected by RT-PCR and Western blot analysis.
RESULTS(1) The EATV was significantly higher in the CHD group than in NCHD group ((121.2 ± 40.6) mm³ vs. (74.7 ± 18.1) mm³, P = 0.01). (2) Subgroup analysis of the CHD patients demonstrated that EATV was significantly higher in patients with positive remodeling than in patients without positive remodeling ((97.6 ± 42.0) cm³ vs. (75.5 ± 25.4) cm³, P = 0.01). Lipid plaque volume was positively correlated with EATV (r = 0.34, P = 0.002); however, fiber plaque volume was negatively correlated with EATV (r = -0.30, P = 0.008). (3) Logistic regression analysis indicated that EATV was an independent risk factor for positive vascular remodeling (OR = 2.01, 95% CI: 1.30-2.32, P = 0.01). (4) mRNA and protein expression of leptin and MMP9 in EAT was significantly upregulated in 40 CHD patients who received CABG surgery compared to 40 NCHD patients (P < 0.01). However, there was no significant difference (P > 0.05) in mRNA and protein expression of leptin and MMP9 from the SAT between CHD and NCHD patients. (5) In the CHD group, leptin and MMP9 levels in EAT and EATV were positively correlated with lipid plaque volume and fibrous plaque volume (P < 0.05).
CONCLUSIONSEATV is an independent risk factors of coronary heart disease and plaque vulnerability; EAT secretion of inflammatory cytokines from CHD patients is significant increased compared to NCHD patients, EAT secretion of inflammatory cytokines are positively correlated with EATV, both of which are determinants affecting vascular remodeling. Reducing EATV might help to attenuate inflammation and plaque vulnerability and reduce the risk of coronary heart disease.

Chinese Journal of Hepatology 2015;23(2):130-133
OBJECTIVETo investigate the effects of activation of the GLP-1 receptor on the p38MAPK signaling pathway in hepatic stellate cells (HSCs).
METHODSHSCs were isolated and identified according to morphological features; the levels of GLP-1R protein were determined by western blotting. The HSCs were randomly divided into a control grouP (normal saline treatment) and experimental grouP(liraglutide treatment); after 120 hours, the expression of p38MAPK mRNA was examined by RT-PCR and of phosphorylated (p)-p38MAPK protein was detected by western blotting.
RESULTSGLP-1R proteins were detected in the HSCs. Compared with the control group, the experimental group showed significantly decreased p38MAPK mRNA and p-p38MAPK protein (both P < 0.01).
CONCLUSIONThe p38MAPK signaling pathway could be down-regulated when GLP-1R is activated in HSCs.
