1.Inflammation-related signaling pathways of differently-methylated DNA from benign and malignant ;thyroid tumors in one patient

Objective To determine the possible differences in DNA methylation of benign and malignant thyroid tumors and its role in tumorgenesis and tumor progression. Methods Infinium human methylation 450 bead chip platform was utilized to screen the whole-genome of benign and malignant thyroid tumor tissues surgically removed from a patient for the determination of their DNA methylation levels. The genes with significant differences in methylation were analyzed for their functional pathways with bioinformatics. Results There were significant differences in DNA methylation between benign and malignant thyroid tumor. The inflammation-related pathways, such as T cell, Jak-STAT and cytokine-cytokine receptor signaling pathways involved in the biological process of tumorgenesis and tumor progression. Conclusions The differences in DNA methylation between benign and malignant reveal a close relationship between inflammation and tumorgenesis and tumor progression and provide a novel method for investigation of thyroid cancer development and target drug discovery.
2.Application of Kangxiuke apozem in the treatment of severe burn shock

ObjectiveTo observe the clinical effect of Kangxiuke apozem in the treatment of severe bum shock.Methods40 patients of severe burn shock were randomly divided into a treatment group and a control group,with 20 patients in each group.All the patients were treated by the same route of counter-shock therapy,and the treatment group was additionally given Kangxiuke apozem (nasal feeding,qd,150 ml/d).Level of blood lactic acid,heart rate and urinary volume were evaluated by time.Main shock symptoms,including restlessness,hydrodipsia,perspiration and coldness,were observed.ResultsThe treatment group was significantly superior to the control group in the amelioration of blood lactic acid,heart rate and urinary volume ( t =10.485、3.219、7.429,P< 0.01 ).The symptomatic extinction of restlessness,hydrodipsia,perspiration and coldness in the extremities in the treatment group were significantly superior to those in the control group (x2=10.16、7.37、5.63、4.29,P<0.05).ConclusionAt the same time of routine counter-shock therapy,thereby prompt burn patients to smoothly go through shock period and decreas late complications.
3.Identification of Metabolized Polychlorinated Biphenyls in Fishery Products by Gas Chromatography-Tandem Mass Spectrometry

Gaschromatography-tandemmassspectrometrywasadoptedtoidentifythehydroxylatedmetabolites of polychlorinated biphenyls and analyze the target compounds in fishery products. After the silylation derivatizaiton at 60 ℃ for 40 min, the MS spectrum of derivatives was obtained by GC/MS/MS under fullscan mode, and the fragment ions with the highest relative abundance were identified as m/z 399, 399, 399, 399, 399, 448 and 469, respectively which were defined as the precursor ion for product ion scan. The MS/MS spectrum was obtained under multiple reaction mode ( MRM ) by GC/MS/MS. And the product ions corresponded with the precursor ions were m/z 364, 364, 364, 364, 364, 433 and 434, respectively. The target compounds can be identified accurately by GC/MS/MS. In the present study, we took seven hydroxylated polychlorinated biphenyls as target compounds including 3-OH-PCB101 , 4-OH-PCB106 , 4-OH-PCB112 , 4'-OH-PCB101 , 3-OH-PCB118 , 3-OH-PCB138 and 3-OH-PCB180 . According to the identification method stated as above, the MS and MS/MS spectra of seven compounds were obtained. The compounds can be qualified and quantified by the identification method. The limits of detection ranged from 0. 02 to 0. 14 μg/L, and the limits of quantification ranged from 0. 09 to 0. 48 μg/L. The method has been applied to identify the hydroxylated polychlorinated biphenyls in fishery products.
4.Protective effects of da chai hu granules (DCHKL) against alloxan (AXN)-induced rat pancreatic islets damage.

The protective effects of Da Chai Hu Granules (DCHKL) on islet cells which were incubated with 4 mmol x L(-1) alloxan (AXN) were studied. The viability of islet cells were measured with MTT. Insulin released into medium and in islets was detected by radioimmunoassay. Cell apoptosis rate was determined by flow cytometry. The expression of anti-apoptotic gene Bcl-2 and pro-apoptotic gene Bax in islet cells were measured with RT-PCR (reverse transcription polymerase chain reaction). Serum containing DCHKL can promote the activity of islet cells significantly (P < 0.01). Basal insulin secretion and high glucose-stimulated insulin secretion increased significantly (P < 0.01). Serum containing DCHKL can inhibit apoptosis of islet cells, the ratio of apoptosis was decreased. Serum containing DCHKL increased expression of Bcl-2 mRNA and decreased expression of Bax mRNA. DCHKL can significantly promote proliferation of islet cells and increase the amount of basal secretion of pancreatic islet cells and high glucose-stimulated insulin secretion. The expression of Bcl-2 increased significantly. The expression of Bax decreased significantly. DCHKL have a protective effect on the islet cells.

Objective Construct the eukaryotic expression vector of inhibitory member of the ASPP family (iASPP) and trans-fect it into colon carcinoma cell lines SW480 and Lovo by liposome .Then observe the expression of iASPP and detect the cell apop-tosis by flow cytometry .Methods The amplified PCR product was digested and inserted into pMD19-T simple vector and sub-cloned into eukaryotic expression vector pcDNA3 .1(+ ) .The recombinant eukaryotic expression plasmid pcDNA3 .1(+ )-iASPP was transfected into colon carcinoma cell lines SW480 and Lovo by liposome ,the iASPP expression was analyzed by RT-PCR .The cell apoptosis was detected by FCM .Results The eukaryotic expression plasmid pcDNA3 .1(+ )-iASPP was constructed success-fully ,the gene squence of iASPP was consistent with that reported (gi 60457962) in GenBank .The mRNA expression levels of iASPP gene of SW480 and Lovo cell lines which transfect the positive plasmid were increased ,and the cell apoptosis rates were de-creased .Conclusion We successfully constructed the recombinant expression plasmid pcDNA 3 .1(+ )-iASPP ,and the plasmid were successfully expressed in colon carcinoma cell lines SW 480 and Lovo ,the cell apoptosis rates of those cell lines were decreased .These facts indicated that reducing the high expression of iASPP may be a new strategy to renew the abilities of P 53 tumor suppressor .
6.Prognostic value of lung ground glass opacities volume ratio in patients with acute paraquat poisoning

Objective:By comparing the volume% (V% GGOs) of ground glass opacities (GGOs) in high resolution CT (HRCT) of patients with acute paraquat (PQ) poisoning at different time points, its value in the early prognosis of patients with PQ poisoning was analyzed. Methods:The data of patients with PQ poisoning admitted to Department of Respiratory and Critical Care Medicine of Chinese Armed Police Forces from June 2017 to December 2018 were prospectively analyzed. According to the follow-up results after poisoning at 90 days, the patients were divided into the survival group and death group. Three-dimensional reconstruction technology was used to calculate the change of V% GGOs on the 3rd, 5th, and 7th day after poisoning. Chi-square test and One-Way ANOVA of variance were used to compare sex, age, and time of poisoning between the two groups. The Student's t test was used to compare V% GGOs between the two groups at different time points. The receiver operating characteristic curve (ROC) was used to determine the guiding significance of the indicator on the prognosis of patients with PQ poisoning at different time points. Results:A total of 89 patients with PQ poisoning were included in the study, 49 in the survival group and 40 in the death group. There were no statistical differences between the two groups of patients in sex, age, poisoning time, oxygenation index, mean arterial pressure, total bilirubin, blood urea nitrogen, alanine aminotransferase, and aspartate aminotransferase at admission (all P>0.05). The blood PQ concentration (mg/L) in the death group was significantly higher than that in the survival group (6.35 ±0.51 vs 3.49 ±0.21, P= 0.013). On the 3rd, 5th and 7th day after admission, the V% GGOs was significantly higher than that in the survival group (3rd day: 0.062±0.020 vs 0.049±0.007, P= 0.013; 5th day: 0.292±0.130 vs 0.123±0.044, P<0.01; 7th day: 0.334±0.116 vs 0.138±0.034, P=0.019). The area under the ROC curve showed that the prognosis AUC of the 7th day V% GGOs after poisoning was 0.967, the sensitivity was 100% and the specificity was 83.33% when the threshold was 0.16, but the time point was late. On the 5th day after poisoning, the V% GGOs judged the prognosis AUC was 0.842, the sensitivity was 82.35% and the specificity was 89.47% when the threshold was 0.14. On the 3rd day after poisoning, the V% GGOs judged the prognosis AUC was 0.708, the sensitivity was 55.00% and the specificity was 78.95% when the threshold was 0.05. At this time, the sensitivity and specificity were lower than those on the 5th and 7th day. Conclusions:The proportion of ground glass opacity volume in patients with PQ poisoning can be used to evaluate their prognosis, and the best time point is the 5th day after poisoning.

8.Lymphoplasmacytic lymphoma: clinical analysis of 33 cases

Objective:To investigate the clinicopathological characteristics and survival of patients with lymphoplasmacytic lymphoma (LPL).Methods:The data of 33 newly diagnosed LPL patients in the First Affiliated Hospital of Dalian Medical University from July 2003 to May 2021 were retrospectively analyzed. The clinical characteristics, bone marrow cell morphology, immunophenotyping, chromosomal karyotype, gene mutation, treatment response and prognosis were analyzed, and Kaplan-Meier method was used to analyze the survival of patients.Results:The median age of onset of 33 patients was 66 years old (55-84 years old). There were 26 males (78.8%) and 7 females (21.2%). The common clinical manifestations were anemia (31 cases, 93.9%), enlarged lymph nodes (16 cases, 48.5%) and B symptoms (8 cases, 24.2%). All patients had bone marrow involvement and M protein, 23 of them (69.7%) were type IgM-κ, 8 cases (24.2%) were type IgM-λ, 1 case (3.0%) was type IgG-κ, and 1 case (3.0%) was type IgA-κ. Lymphocytes, lymphoplasmacytes or plasma cells was increased in bone marrow smear; 22 patients underwent immunophenotyping of bone marrow by flow cytometry, and all patients expressed B cell surface antigens (CD19 and CD20), 16 patients (72.7%) lost the expression of CD5 and CD10, 13 patients (59.1%) expressed or weakly expressed CD138 and 5 patients (22.7%) expressed CD38. Seven out of 23 cases (30.4%) who received chromosome examination had abnormal chromosomal karyotype. Fourteen out of 16 cases (87.5%) who received MYD88 L265P mutation detection harbored the mutation. Among 21 patients with evaluable efficacy, 18 patients (85.7%) responded to treatment, achieving partial remission or stable disease, but the rate of complete remission was low (14.3%, 3/21). The median follow-up time was 34 months (2-102 months), 1 case was lost. The median overall survival time was not reached, and the 3-year and 5-year overall survival rates were 79.2% and 67.9%, respectively.Conclusions:LPL is a rare indolent small B-cell lymphoma with a long course and a variety of manifestations, which is commonly seen in elderly men.Serological examination, bone marrow cell morphology and biopsy, immunophenotyping and MYD88 L265P mutation detection are important for the diagnosis and differential diagnosis.
9.Hypoprothrombinemia-lupus anticoagulant syndrome secondary to Sjogren's syndrome:A case report

Hypoprothrombinemia-lupus anticoagulant syndrome(HLAS)is a rare disease in which patients present with varying degrees of bleeding and positive lupus anticoagulant with reduced prothrombin on laboratory tests.This article reports a case of HLAS in a middle-aged woman with recurrent gingival bleeding and epistaxis as the first presentation.After admission,tests revealed prolonged prothrombin time(PT),activated partial thromboplastin time(APTT),and reduced coagulation factor Ⅱ activity,and positive lupus anticoagulant(LA).Meanwhile,the patient had symptoms of dry mouth and dry eyes for a long time,and the examination of autoantibodies,tear secretion test and salivary gland emission computed tomography(ECT)were consistent with the diagnosis of Sjogren's syndrome.The final diagnosis was HLAS secondary to Sjogren's syndrome.After treatment with methylprednisolone and cyclophosphamide,the coagulation disorder gradually improved,and no recurrent bleeding occurred.HLAS is a rare clinical case,which reminds medical staff to be alert to the possibility of HLAS when encountering patients with unexplained prolonged APTT and PT and positive lupus anticoagulant.

Objective To investigate the antimicrobial susceptibility of spectinomycin?minocycline?azithromycin and sparfloxacin to mycoplasma(Uu and Mh)and therapeutic effect of spectinomycin to my-coplasma infection in genitourinary tract.Methods①The susceptibility test:each of the4drugs was divided into two concentrations.One was at1?g/mL(sensitive concentration)and the other was at4?g/mL(resistant concentration).If mycoplasma does not grow in both concentrations,it means the drug tested is sensitive.If it grows in both concentrations,the drug tested is resistant.If mycoplasma grows in lower concentration and does not in higher concentration,it means moderate sensitive.②Treatment regimen:Spectinomycin was injected,2g/d IM,for7-10days as a course of treatmeant.Patients were followed-up7days later and2~4weeks after treatment.Results①Among1658specimens,519were found Uu positive,and61Mh positive.The resis-tance rates of Uu to4different drugs were:7.7%for minocycline,21.4%for sparfloxacin,13.9%for azithromycin and7.3%for spectinomycin.Whereas,those of Mh were:18.0%,45.9%,54.1%,and29.5%re-spectively.②The clinical effect of spectinomycin was:out of43treated patients,37(86.0%)cured,4(9.3%)markedly improved,2(4.7%)failed.Total effective rate was95.3%and so was the elimination rate of my-coplasma.Conclusion The resistant rate of mycoplasma to spectinomycin is lower than that to minocycline?azithromycin and sparfloxacin,and the former is widely used in the treatment of mycoplasma(especially Uu)infection,with a satisfactory clinical effect.
